Panel Discussion Session Report

The session “Challenge to Change: Reframing Clinical Realities in Nursing” brought nursing to the center of healthcare transformation, highlighting both the pressing challenges faced by the profession and the pathways toward sustainable change. Moderated by Prof. Dr. Raminder Kalra, Principal, Holy Family College of Nursing, New Delhi, the session featured a distinguished panel representing academia, government services, corporate healthcare, and the non-governmental sector.

The discussion opened with an overview of the State of the World’s Nursing 2025 (WHO) report, emphasizing global and national workforce realities. While the global nursing workforce has grown to nearly 30 million, inequitable distribution remains a major concern. India continues to face an acute shortage, with a nurse-to-population ratio of 1.9 per 1,000, significantly below the WHO recommendation of 3 per 1,000. This shortage, coupled with urban–rural imbalance, heavy workloads, and migration, has placed immense strain on the healthcare system.

Key challenges identified during the session included staff shortages, burnout, inadequate compensation, lack of recognition, safety concerns, and mental health stressors. Panelists highlighted that nurses often work extended hours with minimal rest, face workplace violence, and experience emotional exhaustion due to continuous exposure to illness, suffering, and death. Despite being the backbone of healthcare delivery, their contributions are frequently undervalued.

A critical concern discussed was attrition and competency gaps among newly graduated nurses. Corporate sector insights emphasized the difficulty of integrating novice nurses into high-acuity clinical environments without structured mentorship and skill reinforcement. The absence of adequate clinical exposure in some nursing institutions, especially those existing only on paper, further exacerbates the issue. This has led to increased dependency on induction programs, simulation-based learning, and on-the-job training within hospitals.

From the government sector perspective, panelists underscored the non-implementation of minimum wage recommendations and staffing norms, despite legal and policy-level interventions. Migration trends were identified as a growing threat, with a large proportion of qualified Indian nurses seeking better pay, working conditions, and professional growth abroad—raising concerns about long-term workforce sustainability within India.

Technology and digital health emerged as promising enablers of change. Examples included electronic medical records, acuity-based staffing models, barcode medication administration, and digital duty rosters, all of which help reduce documentation burden and improve patient safety. However, panelists emphasized that technology must be paired with adequate training and organizational support to be effective.

The role of NGOs and public-private partnerships was highlighted as crucial in addressing continuous professional development and competency building. Innovative digital platforms offering free Continuing Nursing Education (CNE) credits and competency-based learning were presented as scalable solutions to support working nurses who struggle with time, financial, and resource constraints.

The session concluded with a strong consensus: retaining nurses requires more than recruitment. It demands fair compensation, respectful work environments, structured mentorship, mental health support, career progression pathways, and recognition of nursing leadership. Reframing clinical realities is not optional—it is essential for the future of healthcare.

From Digital Promise to Systemic Impact

Public health leadership in the coming decade will be defined not by technology adoption alone, but by the ability to translate innovation into usable, scalable, and equitable systems. India’s COVID-19 response demonstrated how digital platforms—capable of tracking every vaccine dose from supply to beneficiary—can strengthen transparency, governance, and service delivery at population scale. The real lesson, however, lies in execution rather than technology.

Innovation Exists—Translation Lags

Healthcare innovation today is abundant, especially in AI-driven tools and data platforms. Yet the biggest challenge remains the translational phase—the difficult journey from proof-of-concept to real-world validation and adoption. This “valley of death” persists due to fragmented funding, regulatory complexity, and lack of interdisciplinary teams. Leadership must therefore focus on building ecosystems that support innovators beyond ideas and into implementation.

Digital-First Systems: Risks to Manage

As health systems become digital-first, three risks stand out. First, digital exclusion—access to mobile phones does not guarantee digital literacy or trust. Second, user fatigue, especially among frontline workers managing multiple non-integrated applications alongside manual reporting. Third, fragmentation, where systems across hospitals, labs, and programs fail to communicate, undermining care continuity and decision-making.
Future leaders must prioritize interoperability over replacement, enabling existing systems to speak to each other and generate unified, reliable data.

Digitization Is a Tool, Not Care Itself

Digitization improves efficiency, not empathy. Data improves outcomes only when leaders embed it into daily clinical and administrative decision-making. Poorly designed systems risk increasing documentation burden and reducing time for patient care. Technology must therefore support—not substitute—human judgment and compassion.

Clinicians at the Center

Clinicians often experience digital transformation as overwhelming rather than enabling, largely due to inadequate training and time constraints. Integrating digital health into medical education, simplifying workflows, and automating data capture are essential to restoring clinicians’ focus on care.

Building for the Future

As India rapidly expands medical colleges and district-level healthcare infrastructure, digital planning must begin at the design stage. Looking ahead, AI will shape near-term public health systems, while quantum computing holds long-term promise. Both will require sustained investment, strong validation, and clinician trust.

Conclusion

The next decade of public health leadership demands systems thinking, not just technological ambition. Innovation must cross the valley of death, digital platforms must be interoperable, and care must remain central. Technology is the tool—health equity and patient care are the purpose.

Exploring How AI Can Transform India’s Healthcare System

The integration of Artificial Intelligence (AI) into healthcare systems marks a seminal shift in the way medical services are delivered globally. In India, where barriers to access, affordability, and quality of healthcare are prevalent, AI emerges as a beacon of hope. This was the focus of a riveting fireside chat featuring Dr. Krithika Rangarajan, Associate Professor at the Department of Diagnostic & Interventional Oncoradiology and Principal Investigator at the Centre of Excellence for AI in Health, AIIMS Delhi, moderated by Mr. Sachin Gaur, Director of Operations at InnovatioCuris.

Unveiling the Journey into AI

Dr. Rangarajan opened the discussion by sharing her unique journey into the world of AI. “I got into AI when it was not fashionable,” she reminisced. With a background in medicine and a passion for technology, she found her niche in radiology, an area synonymous with technological progress. Dr. Rangarajan emphasized that the convergence of her interests led her to explore AI, allowing her to contribute to a field that directly benefits people.

Her story resonates with aspiring professionals in the audience, showcasing that pursuing one’s passion can lead to groundbreaking innovations. “The other disciplines are not that difficult; if you want it, you can access them,” she urged, motivating the younger generation to embrace interdisciplinary approaches.

The Centre of Excellence for AI in Health

The conversation shifted to the Centre of Excellence for AI in Health, designated by the Ministry of Health and Family Welfare. Dr. Rangarajan explained the center’s mission: to address the challenges of deploying AI solutions in healthcare. “We realized that while we talk about AI in theoretical terms, very little has actually translated into practical applications,” she noted.

The center aims to align AI initiatives with national healthcare programs, ensuring that solutions are not just innovative but also accessible and scalable. “We start from the problem and then move to the solution,” she articulated, emphasizing the importance of addressing real healthcare challenges.

Trust in AI: Navigating Challenges

The discussion then turned to the pressing issue of trust in AI. Both speakers agreed that building trustworthy AI systems is paramount, especially in healthcare where lives are at stake. Dr. Rangarajan shared her concerns about the disparity between AI’s potential and its actual deployment. “How do we ensure that all levels of AI are trustworthy?” she questioned, prompting a critical dialogue on the ethical considerations of AI in healthcare.

Mr. Gaur added that the responsibility lies not only with developers but also with healthcare professionals who must communicate effectively with patients regarding the use of AI technologies. “We need to ensure that patients do not blindly trust AI,” he emphasized, advocating for transparency in AI applications.

Data Quality: The Backbone of AI

Dr. Rangarajan highlighted the significance of high-quality data in creating effective AI models. “Eighty to ninety percent of the problem is about data,” she stated, stressing that without accurate and diverse datasets, AI solutions would falter. She urged healthcare professionals to contribute data, especially from underserved populations, to rectify biases in AI training.

In a powerful analogy, she referenced the challenges faced by AI in recruitment, where models may inadvertently favor certain demographics over others. “This is the kind of invisible bias that can undermine our efforts,” she warned.

Encouraging Innovation Through Collaboration

The conversation also explored the potential for collaborative innovation in India. Dr. Rangarajan announced that the Centre is collaborating with the Gates Foundation to conduct hackathons aimed at fostering innovative solutions in healthcare. “We want to encourage young entrepreneurs and innovators to participate and contribute,” she said, highlighting the center’s commitment to open innovation.

Mr. Gaur echoed this sentiment, emphasizing that successful AI applications must stem from a deep understanding of clinical needs rather than merely relying on data. “Approach your doctor to participate in building solutions,” he advised the audience of budding entrepreneurs.

Advice for Future Innovators

As the dialogue neared its conclusion, Dr. Rangarajan offered words of wisdom for graduates entering the healthcare field. “Technology and medicine are intertwined; use every new tool that comes your way,” she encouraged. She also underscored the importance of pursuing one’s interests genuinely, rather than following trends. “Do what truly interests you, even if it’s not fashionable,” she concluded.

Conclusion: A Vision for the Future

The fireside chat underscored the transformative potential of AI in India’s healthcare landscape. With leaders like Dr. Rangarajan at the helm of research and development, the future looks promising. By fostering collaboration, ensuring data quality, and building trust in AI, India can pave the way for innovations that enhance healthcare access, affordability, and quality.

As the session wrapped up, it was clear that the journey towards integrating AI into healthcare is not just about technology but also about people—healthcare

At the InnoHealth Conference 2024, host Mercilina sat down with Mr. Arun Agarwal, the founder of Janitri, a Bangalore-based health tech company revolutionizing maternal and newborn care. With a focus on critical monitoring during pregnancy, labor, and the neonatal period, Janitri is working to prevent maternal and infant mortality through wearable and AI-enabled technology.

A Mission Rooted in Compassion

“Janitri is a Sanskrit word for ‘mother’,” Arun explained. “Our vision is a world where no mother or baby dies during pregnancy, labor, or the newborn phase.” The startup, now eight years into its journey, has partnered with over 600 hospitals and has already impacted the lives of more than 8,000 mothers, monitoring over 150,000 pregnancies.

Janitri focuses on the critical “first thousand days” — from conception to two years after birth — by enabling real-time monitoring of vital signs through wearable devices. These solutions are designed to assist in both hospital and home settings, ensuring that early decisions can be made based on accurate data.

The Ground Reality: Urban vs. Rural Disparities

When asked about the current state of maternal and infant health in India, Arun highlighted a pressing concern. “India sees about 3 crore births annually, but we have only around 50,000 gynecologists,” he said. This stark mismatch in resources creates a heavy reliance on frontline workers like staff nurses and ASHA workers, especially in rural regions.

Statistically, India’s maternal mortality rate stands at approximately 97 per 100,000 live births, while the infant mortality rate is around 29–30 per 1,000 live births. However, these figures vary widely across states and are notably higher in rural areas, where access to emergency interventions like C-sections is limited. “In urban areas, C-sections are often used to prevent complications. But rural settings don’t have that luxury,” Arun noted.

Bridging the Gap with Technology

To address this disparity, Arun believes technology is the most viable and scalable solution. “We cannot bridge the gap with manpower alone, but we can with tech,” he emphasized. Janitri’s wearable devices monitor vital signs during prenatal, labor, and postpartum phases and assist healthcare workers in making timely decisions.

The Indian government is also stepping in with supportive policies. Programs like the Pradhan Mantri Surakshit Matritva Abhiyan and the integration of tech into public healthcare systems (including RCH, previously known as the Mother-Child Tracking System or MCTS) show promise. Arun expressed optimism about India’s growing focus on medtech innovation, which is being bolstered by favorable regulatory policies and investment in medical device manufacturing.

Looking Ahead

Janitri’s approach is not just about building products — it’s about building trust and empowering frontline workers with tools that enable better outcomes. Arun’s grassroots research, which included visits to over 100 hospitals and consultations with healthcare providers and families, shaped the company’s deep understanding of on-the-ground challenges.

As the conversation concluded, Arun reiterated Janitri’s core belief: “Many of these maternal and neonatal deaths are preventable. It all comes down to timely monitoring and intervention — and that’s exactly what we’re here to enable.”
