<p><strong>Artificial intelligence</strong> (AI) has tremendous potential and offers numerous benefits, but it also comes with several challenges and threats. AI being a new field, government agencies and organizations need to take a leap in understanding its pitfalls to ensure it is developed and used responsibly. </p>



<p class="has-text-color" style="color:#610fae;font-size:25px"><strong>Common pitfalls of artificial intelligence are:</strong></p>



<p>AI systems rely on huge amounts of data, therefore, raise data privacy concerns. The sensitive data can be potentially misused or can be accessed by unauthorized entities. AI systems are also vulnerable to hacking attacks to either access confidential data or to manipulate data to deceive the AI model. There are also ethical concerns, such as AI can be used for surveillance, spreading misinformation, and scams. AI generated images can be one such example, where the images can be used to scam people. AI models can inherit biases which are present in their training data leading to biased predictions and outcomes. Also, it can be difficult for AI to adjust to challenging situations where it may have to generalize new data, therefore, questioning its robustness. The lack of transparency in AI models can be challenging in understanding how these models were trained and arrive at their predictions. </p>

<p>Apart from these concerns, there also lies legal and regulatory challenges. AI usage picked up like wildfire but we are way behind in enacting regulation. Developing regulations can be challenging as various countries may have different approaches. There are also questions of data ownership &#8211; who owns and controls the data? Who should be liable in case of misuse? So, there is an urgent need to evolve with the changing technology landscape for AI to be used ethically. Training large AI models can have significant energy consumption, raising environmental concerns. </p>



<p>Over reliance on AI models can lead to job displacement and reduced human oversight in the processes. It is important to anticipate the ways AI can be misused and address these pitfalls by collaborating with various stakeholders as we do not know what the landscape would be after 5-10 years. Ensuring AI systems work reliably across diverse situations should be our next step forward by prioritizing ethical considerations, transparency, fairness, and accountability.</p>

<p><strong>Climate change</strong> is a global phenomenon that arises from the increased concentration of greenhouse gases in the Earth&#8217;s atmosphere, primarily due to human activities such as burning fossil fuels and deforestation. This phenomenon has far-reaching implications, including significant effects on human health. The complex interplay between changing climate patterns and health outcomes highlights the urgent need for comprehensive strategies to mitigate and adapt to these impacts. The recent floods are caused due to warming of planet/ climatic changes. Dr Pachauri, Nobel Laureate, mentioned that glacier melting would be due to climatic changes and it would create havoc was criticized by many experts then, but it is true today. Many islands would submerge due to climatic changes and it would cause many socio-economic problems to many countries like Maldives. The following health issues would come up due to climatic changes.</p>



<p class="has-text-color" style="color:#076040;font-size:25px"><strong>Heat-Related Illnesses</strong></p>



<p>Rising global temperatures are associated with an increased frequency and intensity of heatwaves. Prolonged exposure to extreme heat can lead to heat-related illnesses, such as heat exhaustion and heatstroke. Vulnerable populations, including the elderly, children, and individuals with pre-existing health conditions, are at a higher risk. Heat stress can worsen cardiovascular and respiratory conditions and even lead to fatalities. Europe and the USA have recently gone through&nbsp; heat related problems due to increased temperature.</p>

A lady created a virtual husband of her choice by using Replika, which is again built on a neural network machine learning model similar to ChatGPT. A research scholar producing a research paper every day, is it acceptable? Are we losing human ingenuity and touch? Is it taking over human beings? The Danish Prime Minister delivered a speech in parliament partly using ChatGPT to highlight a point that is fascinating and terrifying. Can ChatGPT take over all functions of leadership whose role would become limited.  It is a hot topic of the globe today and it should be regulated before it becomes too powerful. Geoffrey Hinton, a pioneer of AI resigned from his position from Google citing growing concerns about ethical implications of the technology he helped create.</p>

<p>Of late there are many buzzwords being adopted into the health sector, is it inroad of IT into healthcare? It is very common to use these buzzwords by professionals without even understanding their meaning like AI, machine learning, ChatGPT and blockchain etc. I was invited by one of the IITs to give a talk on innovations in healthcare and to my surprise none could tell me the difference between innovation and invention.&nbsp; It is strongly desired that the meaning of each word being used should be understood.&nbsp; Technology should be to assist healthcare providers and not to take over their role. One of such buzzwords which has made inroads recently is ChatGPT, and I have asked many but none could tell me that it is an acronym for Chat Generative Pre-trained Transformer.&nbsp;</p>



<p>We would be discussing one such concept in every issue of our magazine InnoHEALTH. The present issue would discuss ChatGPT which too has its positive and negative sides like any other concept.&nbsp; It is requested to share your views in magazines to learn from each other’s point of view. Would like to invite articles on your experience in the health sector on these newer thinking. ChatGPT can be used to communicate with patients to answer their queries, provide medication schedules and appointment times etc. While there are many benefits it also has its limitations. I leave it to readers to judge its potentials and pen down learning.</p>

<p>Healthcare institutions can be planned as structures can be completed when required and equipment can be made operational as it is available in the market but the concern lies about human resource, it takes many years to develop experienced medical staff for a teaching hospital. In India we have 612 medical colleges out of which 322 are in the government sector and 290 are in the private sector, there are 163 nursing colleges in the govt. while 1833 were in the private sector. Many of these private medical and nursing colleges have been raised for commercial consideration and quality of education is far more than desired. Many of these colleges do not have dedicated hospitals but affiliations.</p>



<p>We need to emphasise on quality medical/nursing colleges rather than quantity as it ultimately affects healthcare of the community. There are approximately eight lakh nurses shortage in India. In the budget of 2023 the govt. has announced 157 new nursing colleges to overcome shortages of nurses, a welcome step but there are many factors of shortage and opening of colleges only would not solve the problem. The working conditions of nurses in India are far more than desired, they have long working hours, meagerly paid and have higher risk while working in hospitals. The society also does not recognize their work as desired. There are better alternate jobs available now like the IT sector which is more lucrative. It has five days a week working , where they often work from home and have no risk like working in hospitals. In fact, many nursing colleges are closing as there are no takers for nursing seats. </p>

<p>This scenario is seen in many countries of the west, research gives insights that the British Indian Nurses Association is planning to take 5000 nurses every year to the United Kingdom, there are nearly 30,000 nurses working alone in the UK. Would we be able to meet our requirement of nursing personnel if we do not look up their issues? We need to take a holistic view of total human resource for healthcare to improve healthcare delivery. </p>



<p>We need to develop Allied health Professionals on priority as it is still not properly regulated though one of the most important part of healthcare. The govt. has brought act but its progress is far more than desired.&nbsp;</p>



<p>The COVID-19 has shown the way that a lot more is required to be done for a healthy India. We need to act in mission mode and complete various initiatives launched recently in time bound fashion.&nbsp; Initiatives launched like digital health missions should not end up like “Health for All” by 2000. Health is a very important part as a healthy population makes a healthy nation.</p>

<p>The year 2022 has been enormously challenging for business, health of people and it has badly affected all the systems. The year had a surge of COVID-19 pandemic, earthquakes, flood, cyclone and in the end the dispute between Russia-Ukraine leading to a war where so many innocent lives had to be perished since they inhabit the war affected areas,bringing down economy and surging miseries to world order. Hope was there for recovery in 2023 but another impediment occurred when a new variant of COVID-19 was reported in China that has affected millions of people. Will there be a repercussion in India is yet to be known? We are definitely better prepared as a country than the last epidemic. Healthcare neglected by successive governments is receiving its due attention after losing thousands of people during the pandemic. Though we have forged ahead in a&nbsp; better way than many developed countries, the whole health system has to gear up and fix accountability for neglect.</p>



<p>We, at IC, had our first physical conference on “Digital Health” and more than 300 people attended the same on&nbsp; Dec. 9 &amp; 10 at IIIT, Delhi. First time nurses were made part of the conference. Young innovator award was given to four participants out of 102 participants. The idea was to deliberate on health issues to find possible solutions and bring to notice of appropriate authorities.&nbsp;</p>

<p>The next conference would be on October 13 &amp; 14&nbsp; at IIIT, Delhi on the theme of “Clinical Validation” and registration is already open. Seeing enthusiastic students, we have introduced the best three papers presented for award in the session of students category. These would also be published in our bi monthly magazine received by 97,000 professionals across the globe.</p>



<p>We, InnovatioCuris (IC) and InnovatioCuris Foundation Healthcare Excellence (ICFHE) team wish everyone associated with various activities of IC &amp; ICFHE including various not for profit initiatives a very happy and prosperous new year 2023.&nbsp;</p>

<p>Health sector has drawn everyone’s attention because of high mortality and morbidity during COVID 19 pandemic. Health was earlier having low priority and has now drawn attention of all stakeholders. Many initiatives have been taken to improve healthcare. The use of IT has been recognized by using telemedicine, healthcare apps and information technology. The flip has been provided by increased coverage of optical fiber and launch of 5G services. In BharatNet project of India 5.6 lakh kms of optical fiber has been laid and all gram panchayats have been connected by optimal mix of media. The National Digital Health Mission is a good initiative in this direction however it should be implemented in project mode at a faster pace with no slippages.</p>



<p>An approval to 157 new medical colleges were given out of which 63 are already functional. It would create extra beds and human resources to take care of the health of the community. Quality of care, accessibility and affordability of healthcare would still remain a concern for years to come. Health is state subject hence status is different from state to state,70% population is rural based and has only 30% national healthcare assets. The new poverty line, which is USD 2.15 per day, draws a deep insight about how much one can afford in this amount of necessities of life forgetting about quality of life. The wasteful expenditures and extravaganza need to be curtailed by everyone to bare minimum to  take care of healthcare.</p>

<p>There is always a solution to every problem however a will is required to resolve, there may be few hard decisions to take which may hurt some people. Streamlining and improving any system would hurt the interest of some people hence would be resented. I have no political affiliation but in my personal opinion the present government is trying to solve problems though there is always scope to improve. We need to create a sense of responsibility in the staff to tackle corruption, audit of health workers’ performance, materials and clinical outcomes. The quackery and negligence to patients should be handled with heavy hands. I had done a project to streamline the system of a state government funded by foreign agency which was abandoned because of many pressures, a lesson to be learnt by all.</p>



<p>Maslow’s hierarchical triangle of needs is very relevant, need to take care of the physiological needs of the community first before affiliation and security needs. A healthy mind needs a healthy body. Any expenditure on health is a good investment of the nation. Let all of us strive to get positive holistic health to create a strong nation.</p>

<p>WHO has defined Health as “state of complete physical, mental and social well-being and not merely the absence of disease or infirmity” if we take this definition most of us are not healthy? There is a necessity to explain this definition to every fellow citizen and relevant functionaries. We as individuals should strive to achieve positive health, which is the duty of both state and individual. Health is a multisectoral approach and to achieve quality of life it is important that every department should plug in to be an integral part of a healthcare provider.</p>



<p>In our personal life we give more importance to money than health. Nation is healthy if individuals are healthy, but how much of the budget is allocated for health by governments in the last few decades. Pandemic has taught us lessons after we lost many precious lives. There is a lack of health infrastructure, manpower and rampant corruption was reported during the pandemic. </p>

<p>Things are improving but too late and too little. There are many initiatives the government has taken to create devices, increase the number of medical colleges/AIIMS to self-sustain health services as per norms of WHO but it would take a few years before it fructifies. It has also created the National Health Authority and digital health mission to bring more efficiency. It would provide healthcare id to all citizens, healthcare providers and hospitals/nursing homes and clinics. Pharmacies and chemists would also be roped in. It is a herculean task to give ids to such a big population. This platform once completed should culminate into better control and qualitative health care at optimum cost.</p>



<p>We citizens also have a duty to take care of our health and not only criticize the government, with improving literacy we would understand healthcare better and contribute towards positive health of the community. We at InnovatioCuris strive to create qualitative healthcare at optimum cost by various initiatives. There are new issues created by use of technology such as Cyber attacks etc. It also opens new doors for legal and ethical issues. It increases efficiency by use of technology like Artificial Intelligence and our team continuously conducts training of healthcare providers to make them aware of these newer issues. We want active participation of all to take healthcare to greater heights. </p>



<p>We have Yoga day today and for better health take care of these national initiatives which have now become global. The benefits of Yoga are scientifically proven and now adopted as “International Yoga Day”. Let us make India a healthy nation.</p>

<p>We are signatory like many countries of “Health for All by 2000 “but we never achieved it, we also signed an initiative of WHO called Sustainable Development Goals (SDGs), which was also not being achieved as required. There have been many committee reports like Bhore Committee (1943), Mudaliar committee and many others but what have we learnt or achieved by these reports? We had launched many missions like National Rural Health Mission (NRHM), Urban Health Mission and National Health Mission but we never achieved our goal of Universal health coverage. We kept on shifting our health systems like old wine in  a new bottle with no accountability and rampant corruption which also revealed in handling the present pandemic. The present COVID 19 has revealed all the weaknesses of our health systems, the ill planned health system caused a lot of cost in terms of human loss and financials. We need to take care of the bottom of the pyramid- two third of people in India live in poverty: 68.8% of the Indian population lives on less than $2 a day, over 30% even have less than $1.25 per day considered extremely poor. How does this segment of the population take care of basic necessities of life forget about healthcare with such a meager amount? It is incumbent on the government that public hospitals provide qualitative healthcare as good as private hospitals. </p>



<p>Hospitals need to be audited, accredited and follow evidence-based medicine and not allowed to do unnecessary diagnostic tests and treatment to inflate bills. Everyone knows about challenges in the public health system but there isn&#8217;t any attention paid by successive govts. for the last 75 years to improve the situation. </p>

<p>The present government is doing something but it is too little and too late. We need to build up healthcare infrastructure at breakneck speed since it deals with human life, death and most importantly the health of the nation to secure a population bereft of diseases. We have enough expertise and repository of wisdom to make our health system versatile and best in the region we can create with the intervention of political forces. Health care is a troubled industry all over the world as Sir Nigel Crisp states, “ The challenges for both rich and poor countries are similar; how to shift focus  from cure to prevention; how to integrate various technologies and care pathways; how to maximise the gains from science and technology; how to fund health systems that can provide equitable access to healthcare for whole population”</p>

<p>In India, there are millions of people with poor or no health services at all. In the past, these people succumbed to their fate or insurgencies. Now mass media or mobile phones provide alternate solutions. Entrepreneurs and NGOs suggest what can be done with what is available. Even the government has realized that the traditional path through full blown medical schools and highly educated doctors at super specialty hospitals will not solve India’s health problem any time soon. There is a need to strengthen primary health care, public participation and awareness is required. COVID 19 has taught us many lessons to improve healthcare systems, telehealth and teleconsultations have found its roots which was not happening for many years. It has happened due to compulsions to adopt telemedicine by healthcare providers and patients. We need to analyse the best practices in India and abroad by a group of experts with inputs from healthcare providers and receivers to produce workable documents in a time bound manner for implementation, however there is a need to monitor vigorously any health care delivery system to make it more responsive and robust.</p>

<p>COVID-19 has given us many lessons by handling miseries and challenges given to mankind. It has forced us to innovate due to compulsions. The production of vaccines is one such big success story for India lauded by the world community. The impetus of the government encouraged the community to make devices by health startups and be “Atmanirbhar”. Many items we were importing are gone for export. Annual budget of 2022 has also encouraged innovators by incentives. The Prime Minister often speaks about innovations and participates in Hackathons and Ideathons conducted in the past by the Ministry of Education. The funding and prizes given to young innovators to motivate is the new norm of today. We also encourage such initiatives by publicizing through our magazine. </p>

<p>InnoHEALTH has a circulation of 93,000 readers. With multiple sources the funding in the present situation has become more convenient which was earlier a tough task. The behavior of the community has changed drastically that they are now transacting business with so ease in virtual/online mode which was not done in the past. We have entered the digital world by compulsion to organize events in virtual modes, teaching in class to election rallies are accepted online. While we lost something but also gained by not running organisations in physical mode. </p>

<p>We have progressed and brought innovations in fast track. India is home to 81 Unicorn with a total valuation of USD 279 billion of these 43 with USD 89 billion emerged last year. USA is top in number of Unicorn while India is fourth after China and UK. Media is reporting how we are added to every month&#8217;s list of Unicorn and soon we will surpass 100 Unicorns.</p>



<h2 class="Body" style="text-align: justify; text-justify: inter-ideograph; color: #a5a5a5; font-size: 22px; line-height: 1.7;"><strong><em>Let us innovate to improve quality, reduce cost and import. It would generate more employment and create a better economy.</em></strong></h2>



<p>In Jan 2022 there are 46 companies in the world having decacorn status out of which India has four: Flipkart, BYJU’s, Paytm and Swiggy. Let us not count on rosy pictures but to be successful analyse our failures too. IBM institute study finds that 90% of Indian startups fail within the first five years of inception. Innovation has become a buzzword lately but what we need to ensure the reduction in failure rate of innovations. Let us innovate to improve quality, reduce cost and import. It would generate more employment and create a better economy. As quoted in my book: Alexander Blass, CEO of Innovation Institute of America, Inc. and winner of Top Innovator of the Year award said “It is no secret that many of the world’s top innovators come from India. Within the past few decades, India has embarked upon an incredible transformation from an agrarian-based society to a knowledge-based economy. Along with the population growth came survival instincts and the need to be different, better and unique. One can see innovation everywhere in India, whether in large game changing innovations that garner lots of publicity, or in less obvious yet important incremental fashions”</p>



<p>We are celebrating 75 years of independence but simplified govt policies in the last few years and recent challenges have encouraged our younger generation of 11 to 12 years of age to innovate who have been awarded by the President/Prime minister every year. We also publicise their good work through our innovators’ club meetings and magazine to let the ecosystem know and motivate others. The Prime Minister always talks of “Naya Bharat”, it is our duty as citizens to make our country of 1.3 billion population the best in the world. It should not be a country known for snake charmers and beggars but rather a country of innovators showing way to others.</p>
