Alarming! 26.7 Crore Tobacco Users in India

“As per the Global Adult Tobacco Survey 2017, there are 26.7 crore users of tobacco in India. 5,500 children initiate tobacco use every day. One-third of these tobacco users will die prematurely. Lacs and lacs of Indian families are devastated with tobacco deaths, which are preventable. “Tobacco is the single most preventable cause of death.” Sambandh Health Foundation (SHF) Trustee and Head of Tobacco Control, Sanjay Seth said at a function in Chandigarh recently.

As the adage goes, “Prevention is better than cure”, tobacco is the single most preventable cause of death. With the aim of having a tobacco-free society, doctors of PGIMER, Chandigarh got together for a meeting to address the tobacco epidemic.
Organizing and leading the doctors to meet in PGI Chandigarh, Dr. Anand Mishra, Additional Professor, Department of Cardiothoracic Surgery said, “tobacco is an epidemic killing 10 lac Indians every year. It is the need of the hour that this issue is addressed at all levels in the Government as well as the community.”
An official statement after the meet said, “If we talk about the healthcare costs, the estimated economic cost of tobacco has been estimated at Rs. 1,04,500 crores (US$ 22.4 billion) which is 1.16 percent of the GDP and 12 percent more than the combined State and Central Government expenditures on health (2011- 12). The financial burden due to direct and indirect costs of tobacco-attributable diseases will continue to push families of millions of tobacco users towards poverty and stealthily decelerate the economic growth of the country.”
The Indian Council Medical Research (ICMR) estimates India will have more than 17.3 lakh new cancer cases and more than 8.8 lakh deaths due to cancer by 2020. 90% of oral cancers and 50% of all cancers are caused due to tobacco. This will not only affect the individual’s health and the families of the cancer patients but also adversely affect the economy of the country as a whole, the statement informed.
Dr. Vedant Kabra, Head of Department Surgical Oncology, Manipal Hospitals, and Voice of Tobacco Victims (VoTV) patron, said, “tobacco not only destroys physical health but also is a cause of extreme emotional trauma for the cancer patient as well as the family apart from draining them financially.”

India is a country of diversity with 1.3 billion population of which 70 per cent resides in villages and have access to 30 per cent of medical assets of the country. Poverty is a significant issue of the country, despite having one of the fastest-growing economies in the world, clocked at an economic growth of 7.6 per cent in 2015. It is estimated that 23.6 per cent of Indian population, or about 276 million people, live below $1.25 per day.

As Health is a state subject-there is lot of mismatch in states like Kerala and Punjab have best health indicators, while Uttar Pradesh is the poorest as per the NITI Aayog report. Indian government spends approx. 1.5 per cent of its GDP on health sector and mplans to make it 2.5 per cent which is much less than many developing countries while the USA has dedicated 16 per cent of its GDP. India is riddled with very basic public health issues leading to disease burden. Keeping this in mind, Prime Minister Narendra Modi has launched six initiatives: Open Defecation free country by 2019, Swachh Bharat Mission, National Health Policy 2017, Digital India with e-health, medical device manufacturing and door-to-door screening of chronic diseases.

The healthcare needs holistic approach which depends on multiple factors. The present government has taken many positive steps including launching of National Health Policy 2017 after the gap of 12 years. It has announced many initiatives like health insurance of people who cannot afford basic healthcare and upgrading of health infrastructure.

The regulation of medical devices have been brought out and is applicable w.e.f January 1, 2018 ending uncertainty of medical device manufacturers having global market of 220 billion US dollar. Challenges bring opportunities such as Indian healthcare market is around US$ 100 billion while it is expected to grow US$ 280 billion by 2020. The healthcare IT market is US$ one billion and is expected to grow 1.5 times by 2020. There is requirement of 7 lakh hospital beds which need investment opportunities of 25-30 billion US dollar. We need to bring innovations in hospital planning,devices, diagnostics, drugs and use of technology to reduce healthcare delivery cost and yet quality.

We failed targets of Health for All by 2000, National Rural Health Mission, and Millennium Development Goals and now launched Universal Health Coverage; its success would depend on providing healthcare facilities and strict accountability. We need to focus on primary health care and customise our healthcare delivery system by learning from experiences of other countries.

The concept of Diagnosis Related Group (DRG) making financial package for group of diseases which is known to patients, providers and third party payors should be considered by improving deficiency found in its execution by the USA.

The various schemes of present government are appreciable but success would depend on strict monitoring, corporate hospitals have high cost and many unethical practices are reported every day in media. Public hospitals to National Rural Health Mission have not delivered as required because of corruption, lack of resources and application of management practices. The need is not old wine in new bottle but strict control in implementation of various schemes launched.

Menace of Non-Communicable diseases: A far reaching impact

Non-communicable diseases (NCDs) represent a major peril to the economic growth and development and human health of any nation. The economic loss to India is pegged at $4.58 trillion before 2030 due to NCDs and mental health. The vicious cycle of high out-of-pocket expenditure on health, poor economic growth, workforce productivity loss and rising disease burden are intricately linked to the rising menace of NCDs.

The South Asian region─with a population of more than 1.7 billion─is experiencing a marked demographic transition─characterised by declining birth and death rates and an increasingly aging population. This increase in longevity has also been accompanied by a rise in the prevalence of NCDslike diabetes (78 million cases in 2015). India has the second highest number of diabetic patients in the world, at about 69 million, after China, and this figure is likely to reach 140 million by 2040 and almost half of them remain un-diagnosed. A recent study showed that seven out of ten diabetics in India – especially those in the 20-24 age group – do not take enough steps to control their blood sugar level – even after diagnosis.

The Economic Impact of Diabetes, As per a study conducted by the Indian Institute of Public Health, the number of diabetes patients in the country is likely to go up to 120 million in next 20 years as against the current 70 million. On an average a diabetic patient spends about INR 25,000 annually to manage the disease and its associated complications. Beyond placing a financial strain on the patient due to treatment expenditures, poor management of diabetes significantly impacts the patients’ quality of life: it may lead to loss of productivity due to multiple hospital visits, elevating rates of absenteeism, diminishing the energy and focus of a productive workforce, and depleting critical workplace skills ultimately leading to reduced mobility and span of life. Diabetes related complications like kidney failure, nerve damage along with obesity, dyslipidemia and hypertension make diabetes management quite complex for clinicians.

In this context, it becomes important that healthcare professionals have access to more educational resources to remain updated about how to tackle the diabetes epidemic. But due to scarcity of time and high patient load in South Asian healthcare facilities, medical practitioners find it very difficult to update their knowledge. Remaining updated can help clinicians offer better diabetes management options to their patients.

A Possible Solution: E-Learning Courses e-Learning programs can help clinicians in acquiring skills required to address the growing complex management of diseases. E-learning courses can be tailored to fit the clinicians schedule, delivering content right at the moment when it is demanded by the doctor. Interactive e-learning programs also offer peer to peer learning, thus enabling clinicians to discuss learning from key cases. Those e-learning programs which also offer the opportunity of contact classes, offer a richer and immersive learning experience.

An effective e-learning program will incorporate guidelines for determining the correct and appropriate blend of instructional strategies, including on-line learning, face-to-face instruction, and skill demonstrations, thus facilitating the learning of a clinician far beyond than traditional learning materials. Unlike traditional CMEs, e-learning can be designed to be self-paced, and research has indicated that e-learning reduces the learning time by at least 25-60% when compared to traditional learning, which can be an added advantage for time-constrained clinicians.

Intelligent e-learning management systems also adapt and learn from the learners’ aptitude and change and deliver a personalized learning experience. The right mix of video, audio, and text content formats help to ensure that delivery of content is mapped to the medium most appropriate & relevant to concept development.

Keeping in mind these perspectives, a Six Months Certification Course in Diabetes in partnership with Fortis C-DOC Hospital was launched by BMJ in South Asia. This course is endorsed by the Royal College of Physicians. This is a six months online learning course offered in two formats: with hands-on training and online-only – to cater to the learning needs of clinicians in South Asia.

Courses such as these will go a long way in knowledge & skill upgradation for clinicians in areas which the country faces a high disease burden. Such courses are clinician-centric and learner-led, and have been developed keeping in mind the real and on-ground challenges faced by doctors in South Asia.
