<p>In the latest episode of the InnoHealth Magazine podcast, host Dr. Soumya Singh sat down with Mr. Sanjay Jain, Founder and CEO of Akhil Systems Pvt. Ltd., to explore the evolving landscape of healthcare IT in India. With over three decades of experience in healthcare digitization, Mr. Jain offered rich insights into the journey of Akhil Systems, the digital transformation of hospitals, and the challenges and opportunities that lie ahead.</p>



<h3 class="wp-block-heading">A Three-Decade-Long Journey</h3>



<p>Founded in 1994, Akhil Systems emerged out of a vision to fill the glaring gaps in healthcare automation. Mr. Jain recalled joining the company as a software developer, eventually becoming its co-leader with Rekha Murthy, his business and life partner. Their commitment to improving healthcare delivery through technology has fueled the company’s success across India and in more than 10 countries globally.</p>



<p>The company’s first major milestone came in 1995 with Mulchand Hospital in North India. This was followed by key partnerships with hospitals like Yashoda and Teerath Ram. An international breakthrough came in 2001 with an implementation project in Saudi Arabia—an experience that exposed Akhil Systems to sophisticated insurance and electronic medical record (EMR) requirements, far ahead of India’s at the time.</p>



<h3 class="wp-block-heading">Turning Points in Health IT</h3>



<p>A critical turning point arrived in 2004 with an ambitious project for STS Apollo Hospital in Dhaka, Bangladesh (now Evercare). The goal was to build a paperless hospital on par with European standards. Mr. Jain proudly shared that for the last 20 years, patients at that facility haven’t carried a single paper prescription—proof of the system’s enduring success.</p>



<h3 class="wp-block-heading">Challenges of EMR Adoption</h3>



<p>Despite technological advancements, EMR adoption remains inconsistent in India. Mr. Jain emphasized that the barrier is rarely technological—it’s often behavioral. Even when hospitals are equipped with capable software, implementation falters due to inadequate planning, insufficient training, and resistance to change among medical staff. He stressed the importance of top-down commitment and continuous support to ensure successful adoption.</p>



<p>“One of the biggest lessons,” Mr. Jain said, “is that mindset matters more than the system. The same EMR works wonders in one hospital and is barely used in another.”</p>



<h3 class="wp-block-heading">Key Features for Successful Implementation</h3>



<p>Akhil Systems developed unique solutions like single-window billing and multilingual pharmacy packaging to streamline patient experiences. These innovations, supported by user-friendly apps and doctor-focused tools, highlight the importance of customizing IT systems to suit local workflows and user needs.</p>



<p>Mr. Jain offered practical advice to hospitals considering a Health Information System (HIS) purchase:</p>

<h3 class="wp-block-heading">Data Security: A Growing Concern</h3>



<p>With increasing digitization, cybersecurity has become a critical issue. Mr. Jain admitted that this area has often been neglected but is now gaining attention following recent ransomware attacks. He stressed the importance of data encryption, secure communication protocols, regular backups, and robust firewall protections—whether data is stored on-premise or in the cloud.</p>



<p>“Security should be baked into the infrastructure and application from the start,” he warned, highlighting the role of standards like HIPAA, GDPR, and India’s upcoming DPDP Act.</p>



<h3 class="wp-block-heading">The Future: AI, IoT, and Digital Ecosystems</h3>



<p>Looking ahead, Mr. Jain is optimistic. He believes the next five years will be transformative for Indian healthcare, driven by:</p>

<li>Wider EMR adoption under national initiatives like ABDM<br></li>



<li>Remote patient monitoring using IoT and wearable devices<br></li>



<li>AI-powered diagnostics that assist clinicians with early detection<br></li>

<p>His company is preparing for this future with “Agastam,” a new cloud-native, microservices-based platform aimed at overcoming traditional EMR adoption challenges.</p>

<p>Mr. Jain’s journey is a testament to how passion, persistence, and purpose can drive meaningful change. From a developer to a leader in digital healthcare, his story mirrors India’s own transition from paper-based systems to intelligent, connected healthcare networks.</p>



<p>As he aptly summarized: “Digital transformation in healthcare is no longer optional. It is a necessity—and the time to act is now.”</p>

<p>At the InnoHEALTH Conference 2024, I had the privilege of speaking with Dr. Ravi Gaur, a renowned onco-pathologist with four decades of experience in diagnostics and precision medicine. Our discussion covered the impact of artificial intelligence (AI), healthcare accessibility, and the future of medical innovation in India.</p>



<h3 class="wp-block-heading"><strong>AI in Healthcare: Friend or Foe?</strong></h3>



<p>With the rise of AI, many people worry that it might replace human intelligence. However, Dr. Gaur had a refreshing take on this.</p>



<p><strong><em>&#8220;AI will not replace human intelligence; it will enhance it. It’s not artificial intelligence—it’s augmented intelligence. AI, when used correctly, can amplify human skills, improve precision, and drive innovation in healthcare.&#8221;</em></strong></p>



<p>While AI is transforming personalized medicine, diagnostics, and treatment plans, he emphasized that the key is collaboration between AI and human expertise.</p>

<p>Dr. Gaur, who has worked closely with both the private sector and government, explained the challenge:</p>



<p><strong><em>&#8220;Technology evolves rapidly—think about how your smartphone gets updates every few weeks. But government regulations are reviewed every 3–5 years. By the time policies are updated, the technology they were meant for is already outdated.&#8221;</em></strong></p>



<p>That said, he highlighted positive progress.</p>



<p><strong><em>&#8220;The government is increasingly open to collaboration with private industries and researchers. The COVID-19 pandemic proved that when urgency is high, solutions can be found quickly. Now, we need to apply that same speed to AI and healthcare policies.&#8221;</em></strong></p>



<h3 class="wp-block-heading"><strong>India vs. Developed Countries: Where Can We Improve?</strong></h3>

<p><strong><em>&#8220;We have world-class hospitals in metro cities, but step outside the urban bubble—just 150 kilometers away—and the infrastructure starts to weaken. There’s a major imbalance in healthcare distribution.&#8221;</em></strong></p>



<p>One of the biggest challenges is ensuring that quality healthcare reaches rural and remote areas.</p>



<p><strong><em>&#8220;Health is a fundamental right. Someone in Delhi should have access to the same healthcare as someone in Mizoram or Ladakh. Right now, that’s not the case. But AI, telemedicine, and digital diagnostics can help bridge that gap.&#8221;</em></strong></p>



<p>He believes that startups and disruptive innovations will play a crucial role in transforming India’s healthcare system.</p>



<p><strong><em>&#8220;Yes, many healthcare startups fail, but failure leads to success. We need more startups, more ideas, and most importantly, faster adoption of these innovations by the government.&#8221;</em></strong></p>

<p>“AI has helped in overcoming accessibility barriers, especially in rural areas. It allows us to maintain better records, track patient progress, and reach a wider audience at a lower cost,” he explained.</p>



<p>AI has also revolutionized capacity building in public health, particularly during and after the COVID-19 pandemic. With restrictions limiting physical interactions, AI-driven virtual training sessions enabled healthcare professionals to continue learning and enhancing their skills remotely. According to Dr. Kumar, AI has not only reduced training costs but also expanded the reach of educational programs to previously underserved areas.</p>

<p>“To run any program effectively, funding is necessary. However, leveraging technology allows us to reach a larger audience with minimal expenditure. Sustainability lies in creating value-driven models that maximize impact with limited resources,” he explained.</p>



<p>Dr. Kumar emphasized the need for financially sustainable training programs where participants contribute a small fee, reducing dependency on external funding. One successful approach has been the use of technology-driven remote training, which enables physicians and healthcare workers to receive quality education without incurring travel costs.</p>



<h3 class="wp-block-heading">Making Healthcare More Accessible and Affordable</h3>



<p>&nbsp;Dr. Kumar stressed that affordability should not be a barrier to quality healthcare. By adopting innovative models and utilizing existing resources efficiently, healthcare professionals can create a more inclusive system that caters to underserved populations.</p>



<p>He drew attention to the small, everyday expenditures that individuals make—such as transportation or discretionary spending on non-essential items—and suggested that redirecting even a fraction of these resources toward healthcare could make a significant difference.</p>



<p>“If we save small amounts from our daily expenses and invest in healthcare, we can create sustainable solutions that benefit a larger population,” he noted.</p>

From a system heavily reliant on paper-based documentation to one now integrating <strong>artificial intelligence (AI) and digital health</strong>, the landscape has changed significantly. <strong>Dr. Sanjeev Kumar</strong>, a seasoned medical professional with vast experience in <strong>public health and global healthcare systems</strong>, shared his insights at the <strong>InnoHealth Conference 2024</strong>.</p>



<p>With experience spanning roles in the <strong>Indian Army, UNICEF, the National Health System Resource Center (NHSRC), and global organizations like WHO and the UN</strong>, Dr. Kumar has witnessed first-hand how public health has evolved both in <strong>India and internationally</strong>.</p>

<li><strong>Introduction of Accredited Social Health Activists (ASHA)</strong> – A new <strong>community health workforce</strong> emerged to support rural healthcare delivery.</li>

<li><strong>Growing Public Awareness</strong> – Citizens are now more <strong>informed about public health</strong>, especially after <strong>COVID-19</strong>, shifting focus from <strong>only clinicians to public health professionals</strong>.</li>



<li><strong>Increase in Life Expectancy</strong> – From an <strong>average lifespan of 32 years</strong> when he was born, life expectancy in India has now reached <strong>71 years</strong>, marking a major public health achievement.</li>

<h3 class="wp-block-heading"><strong>The Role of Technology in Healthcare</strong></h3>



<p>One of the most <strong>exciting changes</strong> Dr. Kumar highlighted is the <strong>integration of technology and AI</strong> into healthcare.</p>



<p>“Artificial intelligence didn’t start for medicine, but now it’s revolutionizing healthcare,” he emphasized.</p>



<p>Dr. Kumar envisions <strong>AI playing a crucial role not just in patient care but in preventive healthcare</strong>. He categorizes healthcare needs into three groups:</p>



<ol class="wp-block-list">
<li><strong>Healthy Individuals</strong> – Using <strong>AI-powered apps</strong> to track <strong>diet, exercise, and lifestyle choices</strong> to prevent future illnesses.</li>



<li><strong>At-Risk Individuals</strong> – AI can help monitor those with <strong>a family history of diseases like diabetes</strong> and recommend <strong>personalized health interventions</strong>.</li>



<li><strong>Patients with Chronic Diseases</strong> – Many people with conditions like <strong>hypertension and diabetes remain undiagnosed</strong> or do not adhere to treatment. AI-based <strong>diagnostic tools and remote monitoring</strong> can improve compliance and early detection.</li>
</ol>



<p>&#8220;In India, half of hypertensive patients <strong>don’t even know they have hypertension</strong>, and among those diagnosed, many don’t take medications properly.&#8221;</p>



<p>Emerging <strong>non-invasive health tech</strong> could soon allow <strong>simple blood pressure or hemoglobin checks</strong> by merely <strong>touching a patient’s finger</strong>, similar to how plant health is assessed using chlorophyll sensors.</p>

<p>“We need to take care of those who are healthy, or hospitals will be flooded with cases, and the community will experience more stress.”</p>

<li><strong>Aging Population</strong> – Over <strong>110 million Indians</strong> are now above <strong>60 years old</strong>, many suffering from chronic illnesses.</li>



<li><strong>Longer Life Expectancy, But Not Healthy Years</strong> – Due to medical advancements, people with <strong>diabetes and heart disease live longer</strong>, but their <strong>quality of life</strong> is often compromised.</li>



<li><strong>Need for Better Compliance in Treatment</strong> – AI-based reminders and <strong>community health interventions</strong> can ensure better <strong>medication adherence</strong>.</li>

<p>At <strong>InnoHEALTH Conference 2024</strong>, we had the privilege of speaking with <strong>Dr. Shirshendu Mukherji</strong>, an esteemed <strong>microbiologist and medical microbiologist</strong> with over <strong>30 years of experience</strong> in healthcare innovation. His work has been instrumental in <strong>supporting innovative and affordable solutions</strong> in India and beyond. Dr. Mukherji has also played a crucial role in leading initiatives like <strong>Grand Challenges India</strong> and <strong>Mission COVID Suraksha</strong>, both of which have aimed to develop impactful healthcare solutions for pressing global health challenges.</p>



<h3 class="wp-block-heading"><strong>The Power of Innovation in Healthcare</strong></h3>



<p>Reflecting on his session at the conference, Dr. Mukherji shared insights on the <strong>importance of impactful healthcare innovations</strong>. He paid tribute to <strong>Rear Admiral surgeon Dr. V.K. Singh</strong>, a mentor and a pioneer in fostering innovation within the medical community. Singh’s contributions extended beyond his service in the armed forces—he guided <strong>young innovators on grant writing and identifying unmet healthcare needs</strong>, helping them transform ideas into practical solutions.</p>



<p>Dr. Mukherji emphasized that <strong>true innovation must reach the common man</strong>—it should be affordable, accessible, and of high quality. He pointed out that while research and discoveries often happen in laboratories, their true value lies in <strong>real-world application</strong>. Innovation should not just exist in research papers or on shelves; it needs to be implemented in a way that <strong>impacts lives at scale</strong>.</p>



<h3 class="wp-block-heading"><strong>Overcoming Challenges in Healthcare Innovation</strong></h3>



<p>When asked about the challenges in healthcare innovation, Dr. Mukherji acknowledged that <strong>obstacles are inevitable</strong>. He quoted <strong>Swami Vivekananda</strong>, saying, <em>“The day you do not come across a problem or obstacle, take it for granted you are trotting on the wrong path.”</em></p>

<p>He stressed that innovation in healthcare <strong>is not a solo endeavor</strong>—it requires a <strong>collaborative, multidisciplinary approach</strong>. A successful innovation team should include:</p>

<p>Each of these roles plays a <strong>critical part in bringing innovation from concept to reality</strong>. Without a well-rounded team, even the best scientific ideas <strong>might not reach the people who need them most</strong>.</p>



<h3 class="wp-block-heading"><strong>The Evolution of Healthcare Innovation in India</strong></h3>



<p>Dr. Mukherji noted that in the <strong>last decade, India’s innovation ecosystem has grown tremendously</strong>. A significant factor behind this growth is the <strong>increased government support for healthcare innovation</strong>. Unlike before, today’s innovators <strong>no longer struggle with a lack of funding</strong>—there are multiple funding opportunities available for promising projects.</p>



<p>However, securing funding is not just about having a great idea. According to Dr. Mukherji, funding agencies look for:</p>

Dr. Mukherji advocates for a <strong>reverse approach to innovation</strong>—instead of starting with technology and finding its use, innovators should first <strong>identify a critical gap in the community</strong> and then work backward to find <strong>scientific and technological solutions</strong>.</p>

Today, AI is set to revolutionize healthcare and diagnostics in a similarly profound way. Through AI-powered medical diagnostic devices and wearables, the technology is becoming an integral part of people&#8217;s lives, allowing for earlier detection of potential health risks and empowering individuals to take preventive actions.</p>



<p>For tech-savvy generations like Millennials and Gen Z, AI has become an essential tool in health management. These generations are highly dependent on technology-driven devices and wearables that provide faster, more efficient, and round-the-clock access to health screenings. Wearables equipped with AI-driven diagnostic imaging, predictive analytics, and digital health records are creating a comprehensive ecosystem for disease prevention.</p>



<p>Beyond smartwatches and fitness trackers, smartphones are also playing a key role in health monitoring. These devices and apps allow users to track their physical activity, vital signs, and overall health in real-time. Studies show that one in four adults in the U.S. own a smartwatch, and more than two-thirds of Gen Z and Millennials in the U.S. use wearables to monitor their health. This widespread adoption of AI-powered devices underscores the strong connection between technology and health-conscious consumers in advanced economies.</p>



<p>The market for wearable medical devices is also experiencing rapid growth. What sets AI-driven wearables apart is their ability to offer personalized alerts and proactive recommendations, enabling users to seek medical care promptly in emergencies. Additionally, these devices have become a fashionable statement. In India, the wearable medical devices market, valued at $1.3 billion in 2022, is expected to reach $5.7 billion by 2027, with the global market projected to hit $14.5 billion.</p>

AI algorithms analyze vast amounts of data—such as medical images, bio-signals, and patient history—to identify potential health risks early, enabling preventative interventions and reducing the likelihood of chronic diseases.</p>



<p>One of the key advantages of AI-based diagnostic devices is their ability to offer highly personalized, private, and preventive care. As data privacy and security are top concerns for Gen Z, these AI-driven tools cater to their desire for confidential and customized healthcare solutions. As digital-first generations increasingly prioritize privacy and tailored services, AI and machine learning technologies will continue to meet their expectations for advanced, preventive health management.</p>

<p><mark style="background-color:rgba(0, 0, 0, 0);color:#a03622" class="has-inline-color">Dr. Bilal Thangal is the Medical Lead at NURA, where he standardizes screening protocols, updates guidelines, and enhances medical procedures. He also trains doctors and staff in medical ethics and patientcentered care. Before joining NURA, Dr. Bilal was a Senior Resident in Critical Care in Kerala for six years, specializing in ICU management.</mark></p>
