<p><strong>India Science Wire:</strong> Bio-printing is a method of tissue replication that temporarily or permanently supports and nurtures living cells. This technique is a potential alternative to organ transplantation, which could be useful in manufacturing functional human tissues such as skin by using specifically engineered biomaterials or bio-inks to print artificial living tissues.</p>



<p>An indigenous state-of-the-art 3D Bio-Printer ‘Mito Plus’ launched by Indian Tech Startup Avay Biosciences has been found to be helpful in printing human tissues. Mito Plus was launched at Bengaluru Tech Summit held between 16th and 18th November 2022. The prototype of Mito Plus was installed at the Indian Institute of Science (IISc), Bangalore, the top-ranked science research institute by NIRF Rankings.</p>



<p>Mito Plus is an advanced version of 3D bio-printer developed with inputs on the prototype from the research lab of Dr Bikramjit Basu at IISc, and developed by Avay which an IIT Madras alumnus co-founded. It is one of the advanced 3D bio-printers in India. Avay Biosciences provides wholly indigenous software and hardware development for end-to-end Bio 3D printing solutions in India.</p>



<p>Manish Amin, Chief Executive Officer, Avay Biosciences, said,<em> </em>“MITO plus is an advanced bio-printer at its price range which can be used to print a wide range of biomaterials. This printer comes up with an inbuilt UV curing option, HEPA filter and effective temperature control features. In this the print-head and the print-bed can be cooled up to four degrees celsius and heated to 80 degrees celsius. MITO plus can be used for pharmaceutical drug discovery and drug testing applications, it can also be used in cancer biology and cosmetology applications.”</p>

<p>Bio-printers work in almost the same way as other 3D printers do, with one major difference i.e instead of delivering materials such as plastic, metal or powders, bio-printers deposit layer of biomaterials, that may include living cells, to build complex structures like skin tissue, liver tissue, etc. 3D bio-printing is a unique gift to humanity through science and technology. However, many challenges are yet to be solved. ‘’There is still a long way to go before we can create fully functioning and viable organs for human transplant,” Amin explained.</p>



<p>“We are working on having our printers develop skin &#8211; the most common type of layered tissue that could help victims of severe burns. These tissues can also be used for toxicology screens and other testing mechanisms,” said Suhridh Sundaram, Chief Operating Officer, Avay Biosciences.</p>



<p>Typically bio-printing uses various polymers which attempt to recreate the extracellular matrix (ECM) native to the specific cell. The availability of cost-effective bio-printers is essential in developing artificial organs, since all future research depends on this infrastructure. If animal cells are used, bioprinting can also be utilized to create artificial meat, a space-age food dream.&nbsp;</p>

<p>The medical devices segment is diverse, creative, and dynamic sector that includes a wide range of products such as implants, surgical instruments, medical equipment, In-vitro diagnostic reagents, and consumables. It is an essential part of the healthcare system for diagnosis, treatment, and prevention of health issues. The Indian medical devices market is currently estimated to be $11 bn, with global market share of nearly 1.5% . India is Asia’s 4<sup>th</sup> biggest market and ranks in top 20 global medical devices markets. It is reported that the Indian medical devices sector is at its nascent stage and import centric, around 80% of medical devices are imported to meet the domestic requirements, majority of them come from the US, China, and Germany. Despite high import dependency, the Indian medical devices sector is expected to grow at 7 % CAGR during 2021 to 2025 and is projected to observe the rapid growth in the Asia Pacific market. </p>



<p>The accelerated growth is anticipated due to concurrent reasons such as rising demand of devices because of global pandemic, rising prevalence of chronic diseases, and elderly population which is expected to grow by 41% in 2031 compared to 2021. To reduce the import dependency and satisfy the rising demand of medical devices, a comprehensive, and carefully devised strategy is required, which shall assist in achieving the fundamental objectives such as accessibility, affordability, quality, in-house manufacturing and skilled manpower.</p>

<h2 class="wp-block-heading" style="font-size:22px">The various government initiatives</h2>



<p>Although 100 percent FDI through automatic route in medical devices was allowed in 2014, tangible attention of policy makers and bureaucrats appeared only in the last two or three years during and post-pandemic. To strengthen the medical devices sector, the government provided the financial support of 25 crore to Andhra Pradesh MedTech Zone (AMTZ) for a common facility centre. Subsequently, to improve manufacturing in cancer/radiotherapy, imaging devices, renal and cardio-respiratory devices and implants sectors, the production linked incentives scheme for medical devices was initiated in 2020, with an amount of 3,420 crore. Further, the medical devices parks were envisaged by allocating 400 crores for developing the medical devices, research facilities and skilled manpower. By introduction of the medical devices rule 2017, which categories the medical devices in four classes, the quality and efficacy was inducted in this sector. </p>



<p>Moreover, the national pharmaceutical pricing authority swung in action for price capping of knee implant, stents, oxygen concentrator and point of care devices (POC) to reduce the dealer’s margin. To address the rising price of drugs, the government opened 8604 affordable outlets (Jan Aushadhi Kendras) which offer 1451 drugs and 240 types of surgical supplies. Another step towards Universal Health Coverage (UHC), The initiative of the Ayushman Bharat-Pradhan Mantri Jan Arogya Yojana (AB-PMJAY) was pioneered by providing the health insurance of ₹ 5 lakhs per family per year, which covers approximately 50 crore Indian poor and vulnerable families. The forenamed initiatives deserve the praise for notable work by the government, but there is still a necessity to advance medical devices sectors to achieve the vision of affordable and quality care to every stratum of the society.</p>



<h2 class="wp-block-heading" style="font-size:22px">Areas of improvement and suggestions</h2>



<p>Despite various schemes initiated by the government, there is a necessity for a comprehensive and clear roadmap for accelerated and continuous growth in the sector. The urge for improvements in the sector is further stressed by the diversity in the medical devices product segment which demands the wide range of interventions in different areas as underscored below.&nbsp;</p>



<p><strong><em>Medical devices regulation:</em></strong> The single window online portal for efficient management of information associated with medical devices is essential. The online portal should allow ease in medical device registration, obtaining import and manufacturing licensing, clearance for clinical investigation to all the stakeholders like Co-WIN portal used for vaccination drive.</p>



<p><strong><em>Research and Development (R &amp; D) funding</em></strong>: The allocation of dedicated funds to encourage joint research with industry, academic and start-ups is much entailed. The fund should be disbursed for building contemporary laboratories for various evaluations, implant and instruments testing, animal studies, equipment testing etc. for R &amp; D activities. These well-equipped laboratories could lead to enhanced research-oriented collaboration, partnerships and innovation in the domestic medical devices sector.&nbsp;</p>



<p><strong><em>Price regulation of medical devices: </em></strong>The projected Indian annual per capita income in 2022 is ₹ 1,78,944 ($2,357.643) which translates ₹14,912 per month. The per month income for quality health services to common people is surely less, thus price regulation of medical devices and hospital charges could play an important role in offering affordable medical devices to all. However, careful understanding of medical devices and well devised strategy for price regulation will be required to maintain stability in the medical devices segment.</p>

<p><strong><em>Healthcare Infrastructure:</em></strong> The highly infectious COVID-19 virus has created panic in the healthcare community and its deleterious episodes are continuing. On other hand it has emphasised the need for infrastructure requirements. The various media reports stressed the shortage of beds in health care centres, which left patients unattended for a long time. This insinuated the need for a good physical foundation for health services and healthcare research for better management and treatment of diseases. Thus, the need for contemporary healthcare infrastructure is essential, which may also attract the domestic manufactures to invest in medical devices sectors.</p>



<p><strong><em>Promoting the collaboration:</em></strong> The idea of industrial cluster near to all academic Institutions, could provide the essential environment for industrial collaboration, The global giant in medical devices technology such as Medtronic, Stryker, GE Healthcare, Philips, and Medtronic shall be encircled in this type of collaboration, it can be easily done since most of them already have centres in India. The triple helix model, commonly used in western countries, shall be tried in this collaboration initiative, where government, industry and Institutes would interact for improvement in the domestic medical devices segment.</p>



<p><strong><em>Innovation parks: </em></strong>The promotion of the ecosystem that houses the network of institutions, start-ups, clinical setting, funding agencies is much needed. This type of ecosystem is called innovation parks; the medical devices parks should be initiated to provide the impetus to scale up the production, encourage the research to market translation and quality medical devices. These parks shall further have strong association with the Ministry of Human Resource Development to make necessary corrections in National Education Policy 2022, by designing proper course work to meet the domestic medical device industry requirements.</p>



<p><strong><em>Creating centres of excellence: </em></strong>The further boost in creating centres of excellence at reputed institutes would attract world class faculties to engage them in medical devices which are fit for India. These centres shall be further strengthened to drive the role in product development, validation, certification, promote the safety and efficacy and use of latest technology such as Internet of Things (IoT), Robotics, Telemedicine, Artificial Intelligence (AI) etc in the medical devices sector.</p>



<p><strong><em>Trained Human Resource:</em></strong> As pre survey from AMTZ the half of the workforce in the medical devices sector are unskilled, signaling the need for skill development programs in this sector. The National Institutes for medical devices like IITs, NITs and IISc could be envisaged. These institutes shall provide training, internship, and hands -on experience in medical devices. Further, association with private players shall be encouraged for research and training funding. The course curriculum in these Institutes shall be drafted such that it brings doctors, technicians, service engineering, scientists, and engineers together like Clinical Engineering, a joint initiative by three Institutions namely IIT Madras, SCTIMST and CMC Vellore to improve the skills in the domain.</p>



<h2 class="Body" style="text-align: justify; text-justify: inter-ideograph; color:#18509a; font-size: 22px; line-height: 1.7;"><strong><em>Patenting is the formal way to protect innovative ideas.</em></strong></h2>



<p><strong><em>IP (intellectual property): </em></strong>Patenting<strong><em> </em></strong>is the formal way to protect innovative ideas. Intellectual property right offers various advantages such as, competitive advantages against the established competitors, prevents ideas from theft, and signals to investors about the new technology. Patenting in the Indian medical devices sector shall be encouraged for continuous and stable growth in the sector.</p>



<p><strong><em>Custom Duty:</em></strong> It is a long pending request from the Indian Medical devices industry to increase the custom duty on imported medical devices. The demand is to increase the custom duty from 0 to 7.5 % to 15% and reduce the GST from 18% to 12%. The call on the request shall be considered after proper evaluations, since on one side it will ease the entry of local medical device manufacturers in the market but on other side it will lead to deficiency of quality and innovative product in market.</p>



<p><strong><em>Acceptance of personalized treatment</em></strong>: Personalized care often called precision medicine is tailor-made treatment to patients based on their molecular profiling. It is an emerging and exciting treatment approach which envisages an important role in preventing and predicting diseases and educates patients to get the right treatment. There shall be a policy to promote such patient centric treatment initiatives within the country, which will contribute to the growth in the medical devices segment since personalized medicine has over-reliance on diagnostic devices.</p>



<p>The aforementioned suggestions are specific to build a strong foundation for the Indian medical devices sector. These are a few steps to reduce import dependency and realize the self-reliant India. If these areas are stressed for the next decade, India could become the global supplier in medical devices from the status of importer.</p>

The vaccine, Rotavac, is currently licensed in two countries, while the grant of license is under process in 30 other countries. It has also been prequalified by the World Health Organisation (WHO), making it eligible for procurement by UN and other humanitarian organizations for their <a href="https://innohealthmagazine.commagazine/public-health-biotech/">public health</a> vaccine programmes.</p>

	<p style="text-align: justify !important;">Agappe Diagnostics Ltd. has won the <a href="https://innohealthmagazine.cominnohealth-conference/young-innovators-award-2018/">award</a> for commercializing an automated cartridge-based specific protein analyzer. The instrument has become highly popular as it is smaller and cheaper compared to similar machines available from multinational companies.</p>
<p>The President presented the awards to mark the 20th National <a href="https://innohealthmagazine.comissues/technology-to-the-rescue/">Technology</a> Day.</p>
<p style="text-align: justify !important;">He also gave away national awards to five medium and small-scale enterprises for commercializing indigenous technologies. The winners are Synkromax Biotech, ANTS Ceramics, 3B Blackbio Biotech, Envision Scientific and Hind High Vacuum.</p>
<p style="text-align: justify !important;">Synkromax got the award for marketing a life-saving implant developed by IIT Madras, while ANTS Ceramics got the award for commercializing high-end zirconia ceramic products and carbon sulphur analysis crucibles and 3B National Technology Awards presented Blackbio <a href="https://innohealthmagazine.comnewscope/aspirin-to-aspiration/">Biotech</a> for working on a kit for diagnosis and prognosis of chronic myeloid leukemia.</p>
<p style="text-align: justify !important;">Envision Scientific got the award for commercializing an indigenously developed drug eluting stent for diabetic patients and Hind High Vacuum got for commercializing thin film metallized alumina circuit for space applications.</p>
<p style="text-align: justify !important;">Three start-ups got award for developing technologies that have commercial potentials &#8211; Astrome Technologies at the Indian Institute of Science, Bengaluru, for its work on GigaMesh, which is said to be the first point to multi-point millimeter wave wireless communication solution in the world; CyCa Onco Solutions at Kalinga Institute of Industrial Technology, Bhubaneswar, for developing two drug delivery devices for delivery of anticancer drug, Crisplatin; and Xcellence in Bio Innovation and Technologies at BITS Pilani Hyderabad campus, for developing a point of care device for testing antibiotic sensitive of pathogens causing urinary tract infections.</p>
<p style="text-align: justify !important;">Speaking on the occasion, the President noted that the National Technology Day was landmark day for the country as it was on this day two decades ago that the Pokhran nuclear tests took place and demonstrated India’s capacity as a nuclear weapons state as well as a mature and responsible technology power, capable of harnessing sensitive knowledge.</p>
<p style="text-align: justify !important;">He said technology was a destiny for the country as all national programmes required a force multiplier in the form of technology. But, at the same time, it must also be about equity. “Its (technology’s) fruits must be accessible to all. Finance and resources should be available to all technologists who may wish to turn entrepreneurs – and to migrate from the lab room to the shop floor.” Above all, the President said, gender equity must be integrated with technology production and technology sharing.</p>

	<p style="text-align: justify !important;">A team of Indian researchers have discovered that carriers of a set of genetic variants in the Chromogranin A (CHGA) gene called‘CHGA promoter haplotype2’ may be at higher risk for cardiovascular and metabolic disorders. An estimated 35 to 40 percent of Indian population may carry this genetic variant.</p>
<p style="text-align: justify !important;">The study is based on analysis of genomic DNA samples from over 750 individuals from Indian population. The research findings have been published in the Journal of Biological Chemistry.</p>

	<p style="text-align: justify !important;">Though the CHGA promoter haplotypeis present in other ethnic populations, it occurs more frequently in population of South Asian ancestry. “It is a protein of neuroendocrine origin and is secreted along with hormones like catecholamines. Earlier studies had suggest edits role in regulation of cardiovascular and metabolic diseases but there was no data aboutit in South Asian population,” said Lakshmi Subramanian, first author of this paper.</p>

	<p style="text-align: justify !important;">“Westudied genomic DNA of Indians and discovered a specific set of changes in the CHGA genesequence called Haplotype2 which contributed to increased CHGA gene expression, and ultimately increased CHGA protein levels in plasma. When the clinical parameters of those in the study were compared, Haplotype2 carriers displayed higher levels of metabolic and cardiovascular traits like plasma glucose, blood pressure and body mass index,” explained Dr. Nitish R. Mahapatra, Professor at IIT Madras.</p>

	<p style="text-align: justify !important;">However, he said, these results need further validation in animal models as well as large scale studies in individuals with metabolic syndrome. “We hope these findings would help unravel biological pathways and mechanisms underlying these complex diseases and would help in the development of therapeutic as well as preventive strategies,” said Dr. Mahapatra.</p>
<p style="text-align: justify !important;">“Basic research in cardiovascular biology is in a very nascent stage in India with only a handful of researchers working in this field. This new study significantly contributes towards understanding molecular basis of cardiovascular and metabolic diseases. This is a fast emerging area with tremendous therapeutic and diagnostic potential,” commented Dr Shyamal K. Goswami, a professor at School of Life Science, JNU, who is not connected with the study.</p>

Costing alone isn’t the only factor in making healthcare affordable; as what is affordable to some may not be the same yard stick of affordability for others.</p>
<p style="text-align: justify !important;">Intelligently moderated by the guest from IIT Madras Eur Ing Muthu Singaram, with intense participation by the audience and focused response by the panelists, the session highlighted standard problems that are universal in nature and are a concern for industry representatives from the emerging markets and the industry leaders as well.</p>

	<p style="text-align: justify !important;">Highlighting his concern, Biten Kathrani of Boston Scientific voiced his opinion saying, “Affordability today is very important not just for emerging markets but for the global health care environment. But in the guise of affordability and much talked about affordability, one of the critical things that we have forgotten to do in health care is to figure out challenges associated with awareness, access and adoption. So today we must look beyond affordability and not ignore awareness, access and adoption.”</p>
<p style="text-align: justify !important;">Addressing the flip side of the subject of beating around about affordability, Dr. Karthik Anantharaman of BPL Medical Technologies highlighted, “Traditionally developing medical devices is very capital intensive with years of R&amp;D that adds to the cost and makes it very unviable for the product to be of any commercial viability. The best way forward is to produce products that are of need by the doctors and not indulge in ambitious research programs that can take as long as 4-5 years of development time and just the research alone can cost a couple of millions of dollars.”</p>

	<p style="text-align: justify !important;">Highlighting the concern of the market towards the newly-launched products and services, another panelist Stephen Victor said, “Life cycle of a diagnostic device is not predictable as some competition may arrive in the market within six months of its launch, with additional facilities, extra benefits and probably at a cheaper cost. And there are other factors that can contribute to failure of the product despite it being very innovative and technically superior. For this, you need Curators for startups or need big corporates to invest in service and maintenance for new innovative products.”</p>
<p style="text-align: justify !important;">With an emphatic conclusion about the worries of many of the startup aspirants, M.V. Amaresh Kumar, who works for Abdul Kalam Institute in Andhra Pradesh, a quasi-government establishment, emphasized, “Cost of innovation is always high as it involves immense research and investment in resources with no guarantee of market share. Government establishments like the Andhra Metric Zone and Abdul Kalam Institute have come to the rescue of such innovators who need hand-holding by someone for making new products commercially viable and relieving the inventor of such mental burden. Only such a marriage between innovators and non-profit establishments can ensure innovations to be introduced at reasonable price – not necessarily at a cheap rate, but affordable.”</p>
