<p style="text-align: justify !important;">Dozee is a patented contact-free health monitor, invented by Mudit Dandwate and Gaurav Parchani, graduates from IIT. It is a thin sensor sheet which goes below the mattress and monitors heart health, respiration, sleep quality and stress levels with 98.4% accuracy as compared to medical devices. It’s AI-powered technology not only flags cases of health deterioration before they become serious but also provides personalized recommendations to improve health.</p>
<p style="text-align: justify !important;">Dozee has saved 36 lives so far by alerting users about health deterioration and helping them detect Tuberculosis, Arrhythmia, Pneumonia, Pulmonary Edema, Cardiac arrest and underdiagnosed conditions such as Sleep Apnea.</p>

	<p style="text-align: justify !important;"><em><strong>Lt General (Dr.) Rajesh Pant</strong></em> is an internationally recognized Cyber Security expert, presently tenanting the prestigious appointment of National Cyber Security Coordinator at the Prime Minister’s Office, India. General Pant brings to the table an interesting mix of military operations, academic excellence, corporate governance, and cybersecurity wisdom. Prior to this, he was the Head of the Army’s Cyber Training establishment for three years. He served in the Army Signals Corps for 41 years wherein he was awarded three times by the President of India for distinguished service of the highest order. 	<p><strong>Q. On behalf of InnoHEALTH Magazine, we congratulate you on your new assignment. For our readers, we would like you to share your short-term and long-term vision for Cybersecurity from national</strong><strong> security perspective</strong>.<br />
Short-term vision is to issue National Cyber Security Strategy 2020-25 early next year. Task force is working overtime on this by consulting all stakeholders. Long-term vision is to create an all-encompassing cyber vertical at the national level, to handle incident response, cybercrimes, legal issues and capacity building.</p>

<strong>Q. We know that there are some fundamental technological shifts waiting to happen like 5G, and along-with it massive (Internet of Things) IoT deployments and especially use cases of connected healthcare. Can you share your views on the cybersecurity implications of the connected devices?</strong><br />
<a href="https://innohealthmagazine.comtheme/iot-can-truly-transform-rural-healthcare-india/">IoT security</a> is a priority topic world over and this is because the limited security capabilities of these devices are also an afterthought. We need to work on a framework, to bring baselinesecurity through the manufacturers and developers of these devices. These devices are omnipresent in our lives, we find them in our home environment to industrial environments including hospitals. We have seen attacks in the past, where such devices are compromised to launch massive denial of service attacks to manipulate the workings of critical infrastructure.<br />
Also, the issue of IoT security is multidimensional, from <a href="https://innohealthmagazine.comissues/patients-sensitive-health-data/">data security</a>, privacy to device security. As we discuss this, there are multiple acts and bills pending in the Parliament on these topics. While the bills and acts will provide a framework, we need to also create awareness on both sides, supplier and consumer on the possible risks and mitigation strategies.</p>

<strong>Q. What steps can be taken to improve the security in such <a href="https://innohealthmagazine.comtrends/medical-iot-future-of-connected-health/">connected devices</a>?</strong><br />
When I say baseline security framework, it can be achieved in multiple ways.<br />
As of today, most devices that we use including mobile phones, do not have a security testing certification. So, we can agree with the industry and look at important test cases and if they can do self-certification on such test cases.<br />
<em>For </em>example: the device should not have weak default login credentials, it is sending data to a remote server and can be operated remotely. So, we can come up like a 5-star rating framework like that of the energy consumption but for the security of IoT devices basis what kind of tests they clear.<br />
Industry bodies can agree on various levels of security and what it takes to achieve that level. Such a framework, when implemented, can provide confidence to consumers and users on the kind of device they are using vis-a-visthe use case they have at hand. So, they might use a higher security rating device in a use case where the stakes are high.<br />
The other approach is to get the security testing done with notified agencies. Department of Telecom for example has announced mandatory security testing of network elements for telecom given telecom is a part of the critical infrastructure and security issuescannot be taken lightly.<br />
Also, some of the emerging concepts in connected devices are missing in the various governing acts of the industrial connected devices. So, we also need to update our legal frameworks to cover software-based tempering of such devices and make the manufacturers and service providers accountable and proactive towards the security of the systems they provide.</p>

<strong>Q. What are the threats that you foresee for the health sector? </strong><br />
There are three areas we see where health sector can be impacted:<br />
First is the data breaches and <a href="https://innohealthmagazine.comissues/ransomware-epidemic/">ransomware attacks</a> on healthcare data. As we know, among all the data, healthcare is the most sensitive and sought after by malicious actors. Outside of India, we have seen umpteen cases where ransomware has crippled the health system and it is only after paying the ransom the hospitals can start operation again. Timely backups and encryption of healthcare data during storage is a preventive measure that clinical establishments can take to mitigate the breach and ransomware attacks.<br />
Second is the manipulation of connected devices. The topic of IoT and connected devices security, as discussed in the above sections, directly apply to the medical devices. Healthcare is a domain where attacks on such devices can be life threatening, especially when there are implantable devices. As we have the new Medical Device Regulation Act in India since 2018, we should also consider cyber security aspect in the devices which have a communication interface. For example, a pacemaker which has a communication interface can be manipulated remotely and the patient’s life is at risk.<br />
Third is the manipulation of health system including the building management. We are probably not very far from the days when sophisticated attacks, as we see in the movies, on high security establishments by manipulating the building controls. The building management systems are very weak when it comes to security. Every hospital is a building and imagine what a false fire alarm would mean to patients in Intensive Care Unit. Or even loss of air conditioning or sudden spikes in electrical power.<br />
There is a proposed act <a href="https://innohealthmagazine.cominnovatiocuris/disha-act/">DISHA</a>, Digital Information Security Healthcare Act, which might address some of the legal aspects of security in the healthcare setting. A lot needs to be done in this area, and we are on our way.</p>

<strong>Q. Our readership consists of health experts all over the world. Any message for them?</strong><br />
We are at the cusp of a new age where we look to take advantage of <a href="https://innohealthmagazine.comexpert-opinion/ai-iot-healthcare-need-future/">Artificial Intelligence</a> to Internet of Things. For such a knowledge economy to take off, health sector is at the center of it and health experts need to pay attention on what they are buying and how such systems are managed and operated. Through intervention of Ministry of Health &amp; Family Welfare and responsible bodies such as National Accreditation Board of Hospitals &amp; Healthcare Providers (NABH) of Quality Council of India, we plan to recommend a cyber audit and increased awareness of information security.<br />
We would not want our hospitals and clinical establishments to be a prey for malicious actors. Rather we would want our experts to leverage technology to take the country to the next level in providing care to a wider population at a lower cost and of the highest quality.</p>

<p style="text-align: justify !important;">I am very pleased and excited by the global acceptance of the SDGs across governments, businesses, and civil society. Yet, actual implementation lags far behind our aspirations and needs. In other words, the big challenge is aligning our plans and actions with the goals. For this we need planning, partnerships, and financial resources in budgets and business investments.</p>

	<p><strong>Q. As the world’s most renowned economist, what do you think should be India’s road map to achieve Sustainable Development Goals?</strong></p>
<p style="text-align: justify !important;">India should pursue the SDGs through six main transformations. First, quality education for all, based on expanded budgets for education at the Union and State levels. Second, quality healthcare for all, similarly based on expanded healthcare budgets at the Union and State levels. Third, decarbonisation of energy, by eliminating the use of coal, oil, and natural gas, and shifting to solar, wind, hydro, and geothermal energy. This will not only contribute to ending global warming but will also clean the air and save millions of lives. Fourth, sustainable land use by restoring degraded lands, protecting endangered species, and ending deforestation. Fifth, investing in sustainable cities, through all-electric vehicles, green spaces, walking areas, and affordable housing. Sixth, being the world leader in using Information and Communications Technologies (the digital revolution) for sustainable development.</p>

	<p><strong>Q. Countries have adopted various healthcare financing models as per their needs like Beveridge, Bismarck, and National Health Insurance Model, but every system has its limitations. Do you have any recommendations or hybrid model which India should adopt to serve its healthcare needs for varied segments of its population?</strong></p>
<p style="text-align: justify !important;">The best healthcare financing is a single-payer approach by government (shared between the Union and State levels). The US model of private health insurance is unjust, costly, and unfair, based on a powerful private &#8211; sector lobby that makes exorbitant profits at the expense of society, especially at the expense of the poor. India continues to under-invest in healthcare. It should be spending at least 4 percent of GDP on publicly financed healthcare, with the share rising gradually overtime.Until recently, India has been spending little more than 1 percent of GDP, a shockingly small amount, and perhaps the lowest public-sector outlay on health of any major country. Of course, good health comes not just through a high-quality and equitable health system, but also through healthy life practices. India should avoid the American fast-food industry and the accompanying obesity syndrome, the heavy addiction to cigarettes, and the massive air and water pollution that claim so many lives. Quality education for all, and gender equality will also strengthen health outcomes.</p>

<p style="text-align: justify !important;">The surprising point about India is how the government chronically under-invests in healthcare. In this case, India is following the US model, of relying on privately provided healthcare. But that only works for the upper class, not the poor.</p>

	<p><strong>Q. How should Indian healthcare policies be structured to integrate the technological innovation to achieve its healthcare goals?</strong></p>
<p style="text-align: justify !important;">Digital technologies enable low-cost, high-quality, and innovative delivery of healthcare. Smartphones can play a vital role: to connect households with health providers, for remote sensing of patient data, for guiding community health workers (such as <a href="https://innohealthmagazine.comwell-being/malaria-control-eradication-program/">ASHAs</a>), for telemedicine and distance diagnostics, for collecting and reporting data, etc. More generally, we are in an era of rapid technological breakthroughs in diagnostics, big data collection and processing, remote monitoring, new medicines, genomics, social medicine (including community-based mental health services), and environmental determinants of health. India should turn to its key institutions such as the Public Health Foundation of India to help set roadmaps for universal health coverage based on innovative technologies. And as with the US National Institutes of Health and the UK’s Wellcome Trust, India should explore an innovation ecosystem built on public financing, private foundations, and private-sector financing for cutting-edge R&amp;D. A single-payer system should reimburse private providers at fixed public rates to give them the incentives to reduce costs and raise the quality through innovations. In other words, a single-payer system can still induce innovation by private providers, but within the context of universal coverage and without the excessive lobbying power of private health providers. European models will be helpful here.</p>

	<p><strong>Q. How globalization can help developing countries in improving healthcare? Is it a win-win situation for both developing and developed countries?</strong></p>
<p style="text-align: justify !important;">SDG 3 sets a shared global set of health objectives &#8211; “<a href="https://innohealthmagazine.comnewscope/healthy-lives-everyone-everywhere/">healthy lives for all at all ages</a>”. In other words, the SDGs can build a shared global perspective and strategy. Of course, the gains are win-win. If all countries coordinate on decarbonizing the energy system, all will gain in improved health (fewer extreme heatwaves and climate-related disasters, reduced air pollution). If they cooperate on improved delivery of primary healthcare, there will be fewer uncontrolled epidemics of emerging diseases. If they cooperate on regulations on antibiotic use, there will be much less onset of antibiotic resistance. If all countries cooperate on sustainable farm practices, there will be healthier diets and less irresponsible use of antibiotics for farm animals. In other words, there are huge gains to global cooperation. Moreover, the poor countries depend on financial support from the rich countries like &#8211; to finance the control of epidemic diseases, the shift to clean energy, and the implementation of universal health coverage. Crucial institutions like the Global Fund to Fight AIDS, TB, and Malaria, need much more global financing. The world’s billionaires should be doing much more than most of them are doing now to put their vast wealth at the service of humanity. No country should say, “America first,” or “India first,” or “China first.” All countries should say “The World First,” and we will all come out ahead.</p>

	<p><strong>Q. What are the updates for 2019 regarding Indo-Danish relationship in Healthcare?</strong></p>
<p style="text-align: justify !important;">The relationship between India and Denmark is becoming better each day. In December 2018, the Foreign Ministers of both the countries met in New Delhi and relaunched the Indo-Danish Joint Commission, which provides the framework for all strategic sectors’ cooperation including healthcare. This breakthrough paved the way for the Danish Prime Minister’s visit to India in January this year. Among other things, he participated in Vibrant Gujarat spearheading a Danish business delegation, where he also met with Prime Minister Modi. The purpose of these visits was to strengthen political and economic ties between India and Denmark because both countries will benefit tremendously from increased interaction and business collaboration. In this regard, healthcare is a key focus area for 2019. This decision is based on the success of two large projects in 2018; one is focused on non-communicable diseases and the other on attaining the SDGs within healthcare. We were able to develop concrete grounds for working in a focused manner in this field and assist Danish companies looking to gain a footing in India. Several Danish companies are already established in the Indian market including Novo Nordisk, Widex, Coloplast, Fertin, Falck and many more are expressing their interest.</p>

	<p style="text-align: justify !important;"><strong>Q. The Danish Health System is incorporating changes for good in recent past with new hospitals and other delivery mechanisms, can you share your viewpoint on the relevance of them to India?</strong></p>
<p style="text-align: justify !important;">The Danish health system is a publicly funded scheme, which covers all Danish citizens. There is a lot Denmark can contribute to India in terms of know-how and sharing of best practices. Areas where Denmark can assist India to include:</p>
<ul>
<li>Digitalization of some solutions to address the challenges that exist in the current healthcare system. Denmark is a leader when it comes to digital workflows and working with electronic health records. Like in India, there are multiple stakeholders within the healthcare system (doctors, patients, nurses, local and state authorities, pharmacies, etc.) that are bound to work together but at the same time operate as individual entities as well. Denmark managed to achieve a high level of efficiency by digitalizing processes and thus, improve efficiency.</li>
</ul>
<ul>
<li>Digitalization has helped Denmark save both time and money. These learnings can be adopted in India to bring down costs in public as well as private set-ups.</li>
</ul>
<ul>
<li>Empowered its patients allowing them to access all relevant information about their health and well-being. I believe that the Indian government’s launch of <a href="https://innohealthmagazine.comnewscope/ayushman-bharat/">Ayushman Bharat Scheme</a> is also a move in a similar direction and with the similar intent of providing better healthcare access to all. Therefore, I see a great scope of collaboration and partnership between the two countries.</li>
</ul>
<ul>
<li>Other areas where Denmark has done exceptionally well is the use of Telemedicine for connecting patients in remote locations and reduce time at the hospitals.</li>
</ul>
<ul>
<li>The Danish hospital system is undergoing unprecedented expansion and restructuring. Denmark has spent years of research and gathered experts from all over the world to develop 5 new national super hospitals and renovating 11 existing hospitals. India can leverage this knowledge while it is at a stage where there are still new hospitals getting on.</li>
</ul>

<p style="text-align: justify !important;">One of the common areas of work between Denmark and India is dealing with NCDs. It is a global challenge. Denmark is currently investing a lot in developing solutions to address this global multifaceted issue. Our research environments, universities, and companies are trying to create products that could be suitable not only for Denmark but across the globe. One of the very specific things Denmark could explore is:</p>
<ul>
<li>India has a lot of data available and Denmark could collaborate with India to develop solutions and therapies.</li>
</ul>
<p style="text-align: justify !important;">India is a bigger country with a much bigger amount of data; this can be utilized to further make the Danish healthcare sector more effective.</p>
<p style="text-align: justify !important;">Furthermore, private players dominate the Indian healthcare sector, which has created different incentives and quicker implementation time. Both learnings from private players and data from India can be beneficial to the Danish healthcare sector.</p>

	<p style="text-align: justify !important;">In a recent healthcare conference, the people were unaware when I spoke about how on October 21, 2013, the former US Vice President Dick Cheney’s doctors disabled his pacemaker’s wireless capabilities to thwart possible assassination attempts. To this, everyone had an issue on accountability. It remains unanswered that who would be liable, the doctor, hospital, company which provided pacemaker or the insurance company. Amidst all this, nobody was concerned about the patient as none present there questioned it. Maybe we are awaiting a major catastrophe to happen to put our minds to work. It is our endeavor to equip the healthcare community to this aspect of digital health.</p>

The health records, data, hospital information system and individual medical devices are all targets. The vulnerability is in technology and lack of awareness of staff. There are many examples to quote wherein hospitals have become the victim of attacks. A recent audit of a corporate hospital in Delhi by our team revealed that 80% of equipment is vulnerable to cyber attack, thus, risking the patients. Healthcare providers are not sensitized to issue as required hence this magazine is dedicated to Cybersecurity. We have named it <a href="http://innovatiocuris.com/training-and-consulting/cyber4healthcare/">Cyber4Healthcare</a> and even launched a training program in the healthcare domain to update knowledge of this extremely important topic. There is an immediate necessity that we invest in staff and equipment to make our systems robust enough to protect them from cyber-attacks.</p>

	<p><strong>Q. What is the Swedish interest in India and your priority topics for the Indian health sector?</strong></p>
<p style="text-align: justify !important;">We have a very fruitful bilateral collaboration with India in the health sector, beginning with our development cooperation in areas such as mother- and child health, midwifery, and work against communicable diseases. Cooperation has now increased to mutually beneficial partnerships. For the past ten years, we have had a Memorandum of Understanding on Health between our countries and next year we are going to celebrate this with an India-Swedish Health Year.</p>
<p style="text-align: justify !important;">Many collaborations of mutual interests are ongoing and being planned that focus in a wide range of areas like antimicrobial resistance, elderly care, pharmaceuticals, non-communicable diseases and digital innovations including artificial intelligence, where the emphasis will be on knowledge transfer, capacity building and skills training.</p>

<p style="text-align: justify !important;">We are in the process of putting out calls and setting up incubators which will encourage close collaboration between the innovators in Sweden and India. Solutions are only sustainable if both sides contribute and share a strong sense of ownership. <p style="text-align: justify !important;">Any suggestions on possible partnerships? It is not so much what the Indian healthcare system can gain, but rather, how can we join forces to overcome common challenges. Swedish businesses have been in India for many years. Within healthcare, we have AstraZeneca, who recently celebrated 40 years in India, and Getinge who has been investing in the Indian ecosystem for many years. Their focus, like many others, has been in building the capacity of their employees and the users of their products to increase efficiency. The businesses also bring with them lessons from Sweden so that we can help India leapfrog outdated technology. Sweden also ranks high in most innovation indexes – it can be argued that our relatively small size makes collaborations outside traditional silos necessary in order to succeed. Public actors, private companies, and institutions often work together in finding innovative solutions to serious healthcare challenges. We would like to work with India to build spaces for similar ecosystems across India.</p>

	<p><strong>Q. What are the common areas for healthcare research and ongoing partnerships for both sides to collaborate?</strong></p>
<p style="text-align: justify !important;">Under the MoU, there are extensive collaborations between public authorities and universities from both countries. For instance, the Swedish Public Health Agency, Medical Product Agency and Forte (the Swedish Research Council on Health, Working Life and Welfare) have an ongoing collaboration with their Indian counterparts. There are ongoing discussions on extending collaborations with new players like our National Board of Health and Welfare and E-Health Authority as well as between our medical universities such as Karolinska Institute, Uppsala University and the different AIIMS.</p>
<p style="text-align: justify !important;">Through these collaborations, we hope to demonstrate best practices and technologies. We have research calls in different areas for example with EU in big data and artificial intelligence for monitoring health status and quality of life after cancer treatment, under JPIAMR call on diagnostics and surveillance 2019.</p>
<p style="text-align: justify !important;">Innovation-the UNNATI way is an India-Sweden Innovators for strategic business development to address global challenges. It is an industry-supported initiative coordinated by Business Sweden in collaboration with AGNIi-Invest India and ATAL Innovation Mission, NITI Aayog.</p>

Additionally, the Indian Ministry of Health has proposed that Sweden collaborate closely with a new-AIIMS and its associated Health and Wellness Centres. AIIMS-Jodhpur has been suggested to us and we are currently considering the scope of such collaboration.</p>

I know India is well ahead in AI technology while Sweden has more long-term experience in extracting relevant information from vast amounts of data. Our patient registries have enabled us to have one of the highest cancer survival rates in the world and that is only one example. I believe we will see some collaborations in the coming year, especially as you are arranging a delegation to Sweden addressing this exact topic in May next year. I will follow this progress closely.</p>

We have completed three years and this year has been very eventful; we have launched <a href="http://icfhe.in/">IC Foundation of Healthcare and Excellence (ICFHE)</a>, a not-for-profit organization for social interventions and are now connected to the community of 40,000 like-minded people. We have put this issue on the theme of &#8220;<a href="http://innohealth.in/">Innovation in Diabetes-Unmet needs</a>&#8220;given India has 73 million population suffering from Diabetes and 50% of people are not even diagnosed properly which means 7% of the Indian population is diabetic. World’s 18% diabetics are in India, second largest in the world. At present, the world has 425 million diabetics which would become 629 million in the year 2045.</p>
<p style="text-align: justify !important;">We intend to handle this menace by conducting awareness programs, find innovative methods to bring diagnostic tools and treatments at a lower cost with quality &amp; behavior modification programs for preventive and curative measures.</p>
<p style="text-align: justify !important;">The lifestyle changes are the key to the treatment of lifestyle diseases like hypertension, diabetes, obesity and select cancers. They cause complications for other parts of the body such as eyes, heart, kidney, and foot. People must be told of the importance of diet, exercise, and meditation in order to have a healthy lifestyle. We have taken this initiative and created a website to raise awareness of the latest scientific interventions <a href="http://www.experimentswithsugar.in">www.experimentswithsugar.in</a></p>
<p style="text-align: justify !important;">We intend to publish articles regularly of common interest and would also encourage the professionals in this field to share their experiences through our magazine. Our team participated in ESICON 2018, to learn latest on the diabetes treatment from the top experts. We have also surveyed the clinicians for their unmet needs and will soon be releasing the findings.</p>
<p style="text-align: justify !important;">We plan to have the next issue on the theme Cyber4Health: security and privacy issues linked with the health sector and their solutions. Healthcare providers are welcome to share their articles and help us increase awareness in this new arena. Our cybersecurity training programs for senior officers of the Armed Forces and various healthcare organizations were very well received.</p>

	<p style="text-align: justify !important;">For young people, less heard ailment Ankylosing Spondylitis (AS) is an emerging disturbance, but that is a stark reality: initially, victims develop symptoms such as <strong><a href="https://innohealthmagazine.comresearch/low-back-pain-disability/">low back pain</a></strong> and stiffness that lasts for more than 30 minutes and worsens in the morning or after prolonged inactivity.</p>
<p style="text-align: justify !important;">Panacea lies in <strong><a href="https://innohealthmagazine.comtrends/exercise-in-big-parks-can-reduce-depression/">regular exercise</a></strong> before AS grips individual like a wasp with stiffened ligaments and muscles. Dr. Uma Kumar, states that such cases were burgeoning, and timely treatment can only stave off complications.</p>
<p style="text-align: justify !important;">Patients with AS must exercise regularly because it helps to limit spinal deformity and maintain their spinal movement and flexibility, while also relieving the back pain. Swimming and deep breathing are the best exercises. In addition, a hot shower in the morning can help to ease the pain and stiffness. Patients should avoid smoking, eat a <strong><a href="https://innohealthmagazine.comwomen-corner/healthy-diet-tips-for-moms/">healthy diet</a></strong> and maintain a <strong><a href="https://innohealthmagazine.comtrends/overstitch-weight-loss/">healthy weight</a></strong>.</p>
<p style="text-align: justify !important;">AS is not curable, but it is completely treatable if detected early. Only 10% of patients have a severely disabling disease. With the help of the correct <strong><a href="https://innohealthmagazine.comtrends/pill-to-determine-medication-ingestion/">medication</a></strong> and lifestyle modification, approximately 80% people with AS remain completely independent or just minimally disabled in the long-term.</p>

	<p><strong>Question: Can you please explain in detail AS? </strong></p>
<p style="text-align: justify !important;">AS is a chronic inflammatory type of spondyloarthritis (a broad term for all types of arthritis that affects the spine) that is further classified as an ‘axial spondyloarthritis’ because it affects the joints of the spine and pelvis (where the sacral bone attaches to the iliac bone on either side of the body). When spinal ligaments get inflamed at the point where they attach to the vertebrae, the ‘bone-making cells’ of the body get stimulated and start to grow bone within the ligaments, which results in the formation of bony bridges between adjoining vertebrae – this is what leads to a stiff back.</p>
<p><strong>Question: Does it affect other parts of the body as well?</strong></p>
<p style="text-align: justify !important;">Sometimes, other joints of the body such as the ankle, knee, and hip may also be affected. AS is more common in young men between 20 and 30 years of age than women. It can occur in children as well. The prevalence of AS ranges from 0.8% to 1.8% in the general population.</p>
<p><strong>Question: Are genetic and environmental factors linked to AS? </strong></p>
<p style="text-align: justify !important;">The exact cause of AS is unknown. However, there is a strong genetic predisposition and most patients with AS have the HLA B27 gene. About 6% to 8% of the Indian population has the HLA B27 gene and amongst them, those with a family history of AS in a first-degree relative have a 30% chance of developing the disease. However, only 2% of individuals with this gene actually develop the disease. It is hypothesized that some stimuli (e.g. environmental factors, gut microbiome) trigger AS development in genetically predisposed individuals. Moreover, AS can also develop in individuals without HLA B27. It is likely that there are some undiscovered genetic factors that could also be involved in its etiology.</p>
<p><strong>Question: Whether Initial symptoms itself are alarming or not? </strong></p>
<p style="text-align: justify !important;">Initial symptoms may not appear alarming, but they can be serious. Patients with AS initially develop symptoms such as low back pain and stiffness that lasts for more than 30 minutes and worsens in the morning or after prolonged inactivity. They often find it difficult to turn on the bed during the latter part of the night and the pain sometimes wakes them up from sleep. Typically, the back pain improves with activity. Some of the tendons and ligaments of the body may also get inflamed (e.g. costochondritis &amp; Achilles tendinitis).</p>
<p><strong>Question: What are the other symptoms?</strong></p>
<p style="text-align: justify !important;">Other symptoms of AS are chest stiffness; pain in the neck, hip, shoulder, glutes (buttock) and heel; as well as arthritis of the knee, ankle, toes or fingers. About 30% of the patients may develop uveitis (painful red eye) and a similar number of patient’s long-standing disease may develop osteoporosis (thin weak bones). The lungs, heart (valves) and kidneys may also get affected.</p>
<p><strong>Question: How can it be diagnosed accurately? </strong></p>
<p style="text-align: justify !important;">We do clinical diagnosis with imaging and genetic tests. AS is diagnosed based on a patient’s clinical profile and radiological (X-ray) investigations. Blood tests include genetic tests to detect the HLA B27 gene; and tests that detect raised erythrocyte sedimentation rates (ESR) and increased C-reactive protein (CRP) levels to help support the diagnosis. X-ray imaging cannot detect bone changes during the initial stages of the disease making early diagnosis difficult. However, MRI scans of the sacroiliac joints can be used to diagnose AS in the early stages, when the X-ray of this region still appears ‘normal’.</p>
<p><strong>Question: Can treatment help to slow down or stop the progression of AS?</strong></p>
<p style="text-align: justify !important;">The aim of treatment is to relieve pain; slow down or stop disease progression, and maintain mobility of the spine. Medicines for AS include non-steroidal anti-inflammatory drugs (NSAIDs), analgesics, disease modifying anti-rheumatic drugs (DMARDs) and biologics (targeted therapy). NSAIDs provide pain relief and also retard the progression of AS and patients prescribed these medicines must take them regularly and NOT ‘as and when required’. DMARDs like methotrexate, sulphasalazine, and leflunomide are prescribed for patients with peripheral arthritis. Patients who do not respond well to these drugs may be prescribed biologics such as anti-TNF alpha agents and anti-IL-17 inhibitors, as long as they have not had TB or a similar significant illness in the past.</p>
<p style="text-align: justify !important;">These biologics and other tools that help us assess the disease have made it possible for patients to lead an almost normal life. Oral or parenteral glucocorticoids have no role in the management of AS though intraarticular steroid injections may sometimes be needed.</p>
<p><strong>Question: Is surgical intervention a solution? </strong></p>
<p style="text-align: justify !important;">Surgery is rarely required for correcting spinal deformity, though a hip or knee replacement surgery may be needed if there is significant joint damage.</p>
<p><strong>Question: What should be a frequency for consulting doctors?</strong></p>
<p style="text-align: justify !important;">Regular follow-up with the doctor is extremely important to adjust the dose and to detect any drug adverse effects or any complication or comorbidity at the earliest.</p>

The most common form of arthritis in the United States is osteoarthritis, followed by gout and rheumatoid arthritis. Symptoms include pain, aching, stiffness, and swelling in or around the joints. Arthritis affects people of all ages, including children. Although the risk of developing arthritis increases with age, more than half of adults with arthritis are younger than 65. About 1 in 4 adults has arthritis in US.</p>
<p style="text-align: justify !important;">According to Centers for Disease Control And Prevention in US, Arthritis affects about 1 in 4 adults in the United States; That’s 54 million men and women; As the US population ages &amp; obesity increases, the number of adults with arthritis is expected to increase to 78 million by 2040; one-third of adults living in rural areas have Arthritis; over 1 in 2 adults with Arthritis in rural areas are limited by it; Arthritis is a leading cause of disability in the US. Twenty-four million adults report limitations due to Arthritis. And, the most common form of Arthritis in US is osteoarthritis.</p>
<p style="text-align: justify !important;">People with arthritis can manage symptoms &amp; reduce pain by learning self-management strategies and being physically active.</p>
<p style="text-align: justify !important;">Prevention tips &#8211; Early diagnosis and appropriate management of arthritis, including self-management activities, can help people with the condition live well without pain. Everyone should exercise regularly to stay healthy, including people with Arthritis. Physical activity has been proven to reduce the pain and restore function. There are proven exercise programmes that can help people with arthritis increase their physical activity safely and comfortably. Maintaining a healthy weight has shown to decrease the risk of developing Osteoarthritis and gout and may decrease disease progression and arthritis-related activity limitations.</p>

Until I was diagnosed with high blood sugar and the week immediately after with a <a href="https://innohealthmagazine.comnewscope/national-medical-commission-bill/">medical emergency</a>, which has no direct link with my diabetic condition, but the treatment is such that I need to have a strict regime of a brisk walk, frequent meals, and schedule of oral drugs and insulin. As I am writing this I have spent 6 days in a hospital bed (which was the first time since I was born) and 14 days out of it. The condition I had, essentially, I felt reborn but this time it seemed with a greater purpose. So, I am writing this article &#8211; not as 35 years old but a 20-day infant.</p>
<p style="text-align: justify !important;">It seems the drugs that I am taking have made me as curious as a child &#8211; nothing seems impossible. I was able to synthesize a few lines on the hospital bed, which summarize my state of the mind. For English readers, this poem essentially means that I want to win my diet, in my mind and the exercise that this body can do. I need a resolve, determination, and courage from the almighty combined with the blessings of my elders and love of dear ones to defeat the voices of the world.</p>
<p style="text-align: justify !important;">With this thought process in my mind, the project “Experiments with Sugar” (<a href="http://www.experimentswithsugar.in/">www.ExperimentsWithSugar.in</a>) was born. With a team of 11 &#8211; from different walks of life: medical experts, researchers, a filmmaker, and more joining every day; we aim to enable 1 million people living with diabetes either cure or better manage their diabetes by the end of September 2021. I will be reporting the progress to you for the next three years via the InnoHEALTH magazine. This is the first social cause project we have undertaken at “<a href="http://www.icfhe.in">InnovatioCuris Foundation for Healthcare and Excellence” (</a>ICFHE), a not-for-profit organization as a mission mode project.</p>
<p style="text-align: justify !important;">With the support of our experts, I managed to meet with some of the top experts and we are getting a great response. To name a few: Dr. Rakesh Sahay, Head of Department at Osmania Medical College; Joseph Duffy, Executive Director, Head of A Neo Diabetic’s Tryst with Destiny &amp; Birth of Innovative Enterprise to Combat Challenge External Discovery Chemistry – Kenilworth, Merck &amp; Co.</p>
<p style="text-align: justify !important;">We plan to release a few episodes every month as a web series to provide scientifically correct information but in an accessible way to the common man. So, as a reader do guide us and send good suggestions our way. By the year-end, we also aim to produce a very high-quality report on the unmet needs of the stakeholders involved in the life of a person living with diabetes. This will then feed into the Smart India Hackathon planned in February 2019 as a diabetes track to support innovation in the country. We need suggestions for a planned roundtable in December 2018 in Hyderabad to validate our findings and further record the proceedings on the best scientific knowledge on diabetes.</p>

	<p style="text-align: justify !important;">Have you ever heard someone say, “I’m exhausted”, or “I’m dead tired”? This is often how people unknowingly express the sensation of burnout. It is widely known that burnout is closely linked with <a href="https://innohealthmagazine.comwomen-corner/stressful-can-events-increase-womens-obesity/">stress</a>, but it is much more than simply being tired, and it is often the case that the seriousness of burnout is overlooked.</p>
<p style="text-align: justify !important;">Let us then ask the questions that need to be asked when talking about the subject of burnout. Is it possible to prevent burnout? How do we know when we have been exposed to either too much &#8211; positive or negative stress? How can we change our <a href="https://innohealthmagazine.comwell-being/contracting-lifestyle-disease-adulthood/">lifestyles</a> accordingly to make the meaningful and sustained change to make ourselves more immune to burnout?</p>
<p><strong>Stress is perception based</strong></p>
<p style="text-align: justify !important;">It is primarily our perception or evaluation of something that determines whether or not we become stressed. If we perceive an object to be too challenging or potentially dangerous in some way, the “flight” type of emotional and mental reactions surface, eventually influencing the body – this is the route of negative stress. If something is challenging enough and we deem the odds of success leading to a potential opportunity in our favour, the “fight” type of emotional and mental reactions occur, and we experience positive stress.</p>
<p style="text-align: justify !important;">Regardless of the polarity of stress (positive or negative), biochemically your body reacts in the same way. Whenever our subconscious autopilot system detects a stressor, adrenaline and cortisol are released from our adrenal glands located on top of the kidneys. Our blood pressure rises, our muscles tense, heart rate elevates, and digestion is inhibited, all preparing us to either fight or take flight. We all have experienced this feeling, like a sudden rush. However, being constantly exposed to prolonged episodes of stress impacts the health of our body and reinforces unhealthy patterns mentally as well as emotionally. It can weaken the immune system, cause more regular mood swings, and can manifest as if being ‘on the ashes’, ultimately resulting in burnout.</p>

	<p style="text-align: justify !important;">However, whether it is a one-time major life event (think of a tsunami), or the constant day-to-day sub-threshold sensation of being ‘<a href="https://innohealthmagazine.comwomen-corner/stressful-can-events-increase-womens-obesity/">stressed</a>’ (think minor but regular waves), at one point you’ll reach your breaking point where the stress reaction gives way to exhaustion, resulting in the sensations of feeling burnt out.</p>
<p style="text-align: justify !important;"><strong>I can handle everything &#8211; Always!</strong> Burnout usually works in either of the two ways, more frequently as a manifestation of chronic stress, or the result of one sudden life-changing event. Because burnout is closely linked with increasing stress, you may already assume that this isn’t a result of one single event but is rather a question of <a href="https://innohealthmagazine.comtrends/indoor-lifestyle/">lifestyle choices</a> and skills to handle the inner turmoil that gets triggered by certain events.</p>
<p style="text-align: justify !important;">Burnout can happen to anyone and everyone, however, there are certain types of people more prone than others. Those who are success or achievement oriented, so-called “career” people, managers and entrepreneurs tend to be more susceptible. They are the people who seem capable of anything. These sorts of people live for their work, strive for success and never switch off.</p>
<p style="text-align: justify !important;">They are burdened with numerous to-do lists and they check their work emails on a Sunday. They are always on the go and find it difficult taking time out. They validate themselves based on their achievements and getting things done. Even during their vacation, they feel the need to be on the go, filling their days with planned activities and engagements. The bigger picture is very often completely lost; they just don’t seem to be able to stop the lifestyle of ‘busyness’.</p>

However, their lives are filled with dangerous amounts of positive stress or eustress. Of course, this positive stress feels good in the beginning and is indeed useful because it helps to get things done and achieve goals. However, in positive stress ‘mobilizing us for action’ means the constant release of adrenaline and cortisol from the adrenal glands – the stress hormones.</p>

	<p style="text-align: justify !important;">From the perspective of burnout, it is irrelevant whether the events that lead to the need to mobilize are “positive” or “negative” and if this lifestyle becomes habitual, it can quickly lead to near addiction because one adrenaline rush primes the craving for another. Negative stress is not a problem because everyone already avoids it due to its association with negative feelings. Most of the cases of burnout today are caused by people “riding the wave” of positive stress because people do not expect it to be dangerous – on the contrary, it is almost actively encouraged and often glorified.</p>

	<p><strong>Lifestyle Changes &#8211; No Easy Way Out </strong></p>
<p style="text-align: justify !important;">It is very much the common and accepted prevailing attitude of our modern working world that it&#8217;s perfectly normal to be “stressed”. It is no wonder then that more and more of us are facing burnout.</p>
<p style="text-align: justify !important;">In order to prevent or recover from burnout, we must adjust our whole lifestyle. Already the term “lifestyle” suggests that this is not just a minor phenomenon that can be simply fixed with a few good night’s sleep or a vacation. This is not a bacterial infection that can be cured by taking a few pills and then rushing back to our “normal” life where stress reaction is considered inevitable. It is this attitude that is what really needs changing leading to skills that help in handling reactions.</p>
<p style="text-align: justify !important;">One must consider both the physical and psychological sides, understanding that psychological arousal leads to a stress reaction in the physical body. It’s not just about reassessing values, setting priorities or time management. The most crucial elements to address are the chronic emotional and mental reactions.</p>
<p style="text-align: justify !important;">What can be done to prevent burnout from taking hold? By educating ourselves on how we function internally will help us further understand the problems associated with burnout. This will help you make better and more informed choices for yourself.</p>
<p style="text-align: justify !important;">Learning relaxation exercises, in particular stress relieving breathing exercises will also help, but in essence it’s already alleviating a consequence caused by a reaction. It is important to take time out during our busy days to just sit and simply be. It seems obvious, but many of us feel guilty for investing in sufficient leisure time. Make it a point to put aside a certain time each week to do that thing you have been meaning to do, make good on that promise to meet a friend, do something with your loved ones, or do something entirely for yourself. Just make sure you do it. Making sure we are getting 7-9 hours per night of continuous quality sleep can make all the difference in how we think and feel.</p>
<p style="text-align: justify !important;">Finally, try consulting a competent specialist, psychiatrist, psychologist, nutrition therapist, fitness trainer, or a yoga teacher to guide you in all directions – physical, emotional, mental, conscious and the self. Seek a solution that enables you to release automatic emotional reactions. You have found an effective method when the same situations do not trigger the reaction that was previously the problem.</p>
<p><strong>CONCLUSION </strong></p>
<p style="text-align: justify !important;">Burnout should not be underestimated or downplayed as a mere side effect of modern day living. In understanding the physiological effects, we see just how dangerous burnout can be in the long-term. By implementing a lifestyle adjustment will ensure that burnout is prevented, however learning to discontinue emotional reactions that lead to stimulation of the adrenal glands is the key. Learning intra-personal skills and training awareness will naturally expand your understanding in many other areas of life, helping to transition from dealing with problematic consequences to skillfully creating your own destiny.</p>
