Introduction

In the evolving landscape of healthcare, nursing stands as a cornerstone, blending compassion with innovation. Colonel Binu Sharma, Senior Director of Nursing at Max Healthcare, shared her inspiring journey in a recent InnoHealth Magazine podcast. With over 45 years in nursing, she reflected on the profession’s challenges, the importance of leadership, and how technology is shaping modern nursing.

A Journey Rooted in Passion and Perseverance

Born in Mahipalpur, a small village in Delhi, Colonel Sharma’s path to nursing was unconventional. Initially excelling in mathematics, she pursued a career in statistics but soon realized her true calling lay in healthcare. She joined the Military Nursing Services and later the Indian Navy, embarking on a lifelong journey dedicated to patient care and medical leadership.

“My mind was on mathematics, but my heart and soul did not belong there. I didn’t choose nursing; nursing chose me,” she reflected.

Overcoming societal resistance, she defied expectations to become a leading force in healthcare. Her story serves as a testament to the resilience and determination required to break barriers and excel in the nursing profession.

The Role of Compassion and Critical Thinking in Nursing

Nursing is more than a profession—it is a unique combination of art and science. Colonel Sharma emphasized that while knowledge and technical skills are essential, a nurse must also embody empathy, patience, and adaptability.

“Nurses are critical thinkers, decision-makers, and caregivers. They need to understand medical technology, patient conditions, and rapidly changing environments,” she stated.

From handling gunshot wounds in emergency rooms to assisting in childbirth and critical care, a nurse’s role is multifaceted. Nurses must be physically fit, mentally resilient, and equipped to navigate high-stress environments while maintaining emotional balance.

Technology in Nursing: A Game Changer

The integration of technology has revolutionized nursing, improving efficiency, patient monitoring, and medical procedures. Colonel Sharma highlighted how advancements such as AI-driven patient monitoring systems, robotic-assisted procedures, and digital health records are enhancing patient care.

“Nursing digitalization is progressing by leaps and bounds. Today, nurses use apps, tablets, and AI to track patient data and improve clinical efficiency,” she noted.

However, technology alone cannot replace the human touch. The key lies in blending innovation with the fundamental values of care and empathy that define nursing.

Coping with Emotional and Professional Demands

Nursing is emotionally and physically demanding. Young nurses, particularly those new to the field, often experience burnout due to long hours, high-stress situations, and the emotional weight of patient care.

To support mental health and well-being, Colonel Sharma emphasized the need for mentorship programs, stress management strategies, and a focus on work-life balance. Hospitals and healthcare institutions must recognize the importance of providing psychological support to nurses, especially in the aftermath of the COVID-19 pandemic, which tested the endurance and resilience of healthcare workers worldwide.

Recognition and the Future of Nursing

Despite their indispensable role, nurses often struggle for recognition and fair compensation. Colonel Sharma believes that recent global events, including the COVID-19 pandemic, have amplified the visibility and importance of the nursing profession.

“Nurses are no longer just blind followers of orders; they are decision-makers, researchers, and leaders. We now have nursing leaders in boardrooms, influencing healthcare policies and decision-making,” she said.

With increasing empowerment, the future of nursing looks promising. More nurses are pursuing advanced degrees, specializing in critical areas such as oncology and ICU care, and playing a vital role in shaping healthcare innovation.

Conclusion

Colonel Binu Sharma’s journey underscores the essence of nursing—where compassion meets technology to drive impactful healthcare outcomes. As the profession continues to evolve, the fusion of skill, dedication, and digital innovation will ensure that nurses remain at the heart of patient care.

Through continuous learning, technological adaptation, and unwavering commitment, the next generation of nurses can look forward to a future where they are recognized not only as caregivers but also as pioneers in modern healthcare.

At the InnoHEALTH Conference 2024, we had the privilege of sitting down with Professor Lavanya Nandan, Principal of Nightingale Institute of Nursing, Noida. Our conversation revolved around how technology and artificial intelligence (AI) are transforming the nursing profession.

A Platform for Nursing to Shine

Professor Nandan began by expressing her gratitude for the opportunity to discuss nursing on such a prominent platform. She emphasized how crucial it is for nurses to be recognized in the broader spectrum of healthcare, alongside management and IT professionals.

“It’s wonderful to see nursing being acknowledged as an evolving profession. This platform allows us to address challenges, inclusivity, and progress in the field,” she said.

AI & Technology: Challenge or Opportunity?

When asked about the challenges nurses face with AI and rapidly changing technology, Professor Nandan had an interesting perspective.

“I wouldn’t call it a challenge,” she explained. “Rather, it’s an opportunity we have yet to fully explore. AI, virtual reality, and simulation-based learning are tools that can enhance our profession. We’ve already started integrating them into our curriculum and professional training.”

She pointed out that many corporate hospitals have already embraced digital documentation, replacing traditional paper records. This shift makes nurses more technologically competent and improves patient care.

“In the next five years, AI will be an integral part of nursing. We’re already conducting virtual seminars and on-site workshops to train nurses in these advancements. For example, I recently attended a one-day digital health workshop in Mumbai specifically for nurses. The industry is actively preparing for this transition.”

How is Nursing Adapting to AI?

The conversation then turned to how the nursing industry is implementing AI and tech training.

Professor Nandan reassured that change is already underway. Training programs are now embedded in both educational institutions and healthcare facilities.

“A great initiative has come from the Indian Nursing Council in collaboration with IIT Raipur. They’ve introduced a six-month certification program where IT professionals train nurses in healthcare technology at a very affordable cost. This kind of partnership is exactly what we need to move forward.”

Additionally, telehealth, robotic-assisted surgeries, wearable health gadgets, and digital data collection are already becoming part of nursing practice. Even medical equipment is increasingly embedded with AI, making it essential for nurses to understand and operate them effectively.

Simulation Labs: The Future of Nursing Education

One of the most exciting developments is the rise of simulation-based learning.

“In India, we’re seeing the growth of simulation labs where nurses can practice on AI-powered mannequins. This allows them to refine their skills in a safe, controlled environment before working with real patients,” she shared.

Final Thoughts

As we wrapped up our conversation, Professor Nandan expressed her confidence in nurses adapting to the digital revolution.

“I don’t see this as a challenge but as an innovation. With continuous training, collaboration, and willingness to learn, we can enhance the quality of nursing and patient care.”

It was truly inspiring to hear her insights. As the healthcare landscape evolves, one thing is certain: nurses are not just keeping up with technology – they are leading the way.

Healthcare is an ever-evolving field, constantly shaped by technological advancements, innovative approaches, and sustainability measures. At InnoHealth 2024, Major General Dr. Jagtar Singh (recipient of the Vishisht Seva Medal) shared valuable insights on the progress, challenges, and opportunities in the healthcare sector. In a conversation with Anjali, host of the InnoHealth Magazine podcast, he provided a detailed perspective on sustainability, innovations, and international best practices that can be adapted in India.

Sustainability in Healthcare: Challenges and Progress

When asked about sustainability in healthcare, Dr. Singh acknowledged the significant strides made but emphasized that there is still much work to be done. He pointed out that sustainability varies depending on the context—what works in a metropolitan setting differs from what is required in resource-constrained environments, such as those encountered in the armed forces. Both scenarios present distinct challenges, yet progress has been made in both civilian and military healthcare systems.

Innovations in Healthcare: A Dual Perspective

Dr. Jagtar categorized healthcare innovations into two broad areas:

Hospital- and Administration-Facing Innovations – These focus on improving operational efficiency, enhancing clinical accuracy, and streamlining administrative workflows.

Patient-Facing Innovations – These address challenges patients face daily, such as appointment scheduling, medication guidance, follow-ups, and discharge procedures.

While significant progress has been made in clinical and administrative innovations, he stressed the need to focus on patient-centered innovations. The core question, he noted, is whether the healthcare system is genuinely addressing patients’ everyday challenges and making the healthcare journey more accessible and efficient.

International Healthcare Innovations: Lessons for India

Drawing from his experience on a study mission to the United States, he shared key takeaways from visiting award-winning hospitals recognized for healthcare excellence. One common theme among these institutions was their unwavering focus on streamlining healthcare delivery through well-defined processes.

A notable example was MD Anderson Cancer Center in Texas, where patient care for women with cancer was meticulously structured. Everything from specialist consultations to diagnostic services and allied medical support was centralized on a single floor, ensuring seamless healthcare delivery. This level of organization, he suggested, is something India must work toward—shifting the focus from fragmented structures to optimized processes that enhance the patient experience.

The Need for Process-Oriented Healthcare

Dr. Jagtar referenced the Donabedian model of healthcare quality, which consists of three key elements: structure, process, and outcomes. While hospitals often emphasize infrastructure (structure) and performance metrics (outcomes), he pointed out that the real challenge lies in refining hospital operations (processes). By concentrating on processes, hospitals can significantly improve efficiency, reduce patient wait times, and enhance overall healthcare quality.

One critical issue he highlighted is the tendency of healthcare institutions to design systems based on administrative convenience rather than patient needs. He urged a reorientation toward patient-centric healthcare, ensuring that every operational change considers its impact on those receiving care.

Final Thoughts

Dr. Jagtar’s insights shed light on the crucial aspects of innovation and sustainability in healthcare. From recognizing the importance of patient-centric solutions to adapting global best practices, his perspectives underscored the need for a more streamlined and process-driven approach in India’s healthcare system.

InnoHealth 2024 provided a platform for meaningful dialogues like this, paving the way for a future where healthcare in India becomes more innovative, sustainable, and patient-focused.

Introduction

At the InnoHEALTH Conference 2024, Dr. Urmila Bhardwaj, a distinguished nursing and healthcare professional, shared her insights on the evolving role of artificial intelligence (AI), sustainability, and community health nursing. In an engaging discussion with host Mercilina, Dr. Bhardwaj shed light on how these factors are reshaping the healthcare landscape.

Dr. Bhardwaj’s Journey: From Clinical Practice to Academia

Dr. Bhardwaj’s career trajectory is an inspiring story of dedication and growth. Beginning with a diploma in nursing, she pursued higher education while working, eventually obtaining her MSc in Community Health Nursing from PGI Chandigarh and a PhD. Over the years, she contributed immensely to academia, serving as a professor at Jamia Hamdard and later as the Dean of Nursing at Sharda University. Her expertise spans bedside nursing, research, and healthcare education.

Reflecting on her experiences, Dr. Bhardwaj credited Holy Family Hospital for instilling in her the fundamentals of patient care, Jamia Hamdard for nurturing her academic and research skills, and Sharda University for exposing her to the advancements in AI and technology-driven healthcare.

Artificial Intelligence: A Double-Edged Sword in Nursing

When asked about the role of AI in nursing, Dr. Bhardwaj acknowledged its tremendous potential in academics, particularly in enhancing learning and research. However, she expressed reservations about its clinical application, emphasizing the irreplaceable value of personalized care.

“Every patient is unique,” she noted. “AI functions based on standardized protocols, but human differences, emotions, and responses cannot be programmed.”

She highlighted the importance of human touch in nursing, explaining that even unconscious patients respond positively to physical touch, which has been scientifically proven to accelerate recovery. While AI can be a valuable tool for data management and diagnostics, it cannot substitute the compassion and individualized attention that nurses provide.

Sustainability in Healthcare: The Nurse’s Role

Sustainability has become a global priority, and Dr. Bhardwaj emphasized the significant role nurses play in fostering a sustainable healthcare ecosystem. Hospitals are high-energy consumption environments with significant waste production. Nurses, she suggested, should collaborate with biomedical engineers and healthcare professionals to implement eco-friendly solutions and reduce hospital pollution.

Beyond environmental sustainability, she also stressed financial sustainability. Sharing her research on tetanus vaccinations, she explained how cost-effective preventive care can save lives and reduce healthcare expenses. A simple, low-cost tetanus shot for expecting mothers can prevent neonatal tetanus, eliminating the need for costly medical interventions.

“Sustainability isn’t just about the environment,” she explained. “It’s about ensuring efficient resource utilization and promoting preventive healthcare.”

Community Health Nursing: A Transformative Force

Dr. Bhardwaj passionately spoke about the vital role of community health nurses in both rural and urban settings. Unlike hospital nurses, community health nurses have greater autonomy and the ability to directly influence public health.

“Community health is not limited to rural populations,” she emphasized. “Even urban elites can benefit from preventive healthcare measures.”

She highlighted the concept of primordial prevention—educating young individuals before unhealthy habits develop. For instance, informing school children about the dangers of smoking can prevent tobacco addiction before it even begins.

From prenatal care to elderly support, community health nurses work across all life stages, making them essential contributors to a healthier society.

A Message for Future Nurses

Concluding the discussion, Dr. Bhardwaj left listeners with an inspiring message:

“Serve with sincerity. See God in every individual. If nurses embrace spirituality—treating every patient with the same care and respect they would offer to the divine—the healthcare system will become a place of true compassion and healing.”

Final Thoughts

Dr. Urmila Bhardwaj’s insights provided valuable perspectives on the intersection of AI, sustainability, and community health nursing. While technological advancements continue to shape the future of healthcare, her emphasis on the human touch remains a crucial reminder of what truly defines nursing.

1. Introduction

The profession of nursing is not just about the delivery of healthcare services; it’s about leading change and advocating for the welfare of patients. In this dynamic and challenging field, leadership skills are critical. Recognizing the necessity of proactive, communicative, and transformative leadership in nursing, an online discussion was convened to delve into this topic more deeply.

A key aspect of transformative leadership is the ability to bring about significant change within an organization. In nursing, such change might involve implementing new patient care techniques, introducing new technology, or working towards more effective collaboration among team members. The goal is to improve patient outcomes and increase overall efficiency in healthcare delivery.

The online discussion attracted an impressive roster of speakers and participants, creating an ideal environment for sharing ideas and experiences. The conversation was enriched by the diverse perspectives of the participants, including frontline nurses, healthcare administrators, and academics. One of the prominent speakers, Merlin Gayathri. G, shared insights that underscored the importance of transformational leadership in nursing. The ability to lead with vision, inspire others, and create an environment that encourages growth and innovation was emphasized.

Other key contributors to the dialogue were Dr Urmila Bhardwaj, Prof. Lavanya Nandan, Captain Sunita Fernandes, all of whom have extensive experience and knowledge in the field of nursing leadership. Their contributions highlighted the evolving challenges in healthcare and the role of effective leadership in addressing these issues.

The aim of the discussion was not just to shed light on the importance of leadership in nursing, but also to inspire and empower participants to take on leadership roles within their own spheres of influence. It was an opportunity to learn from seasoned leaders, understand the various dimensions of nursing leadership, and explore ways to cultivate these skills. The discourse proved to be an enlightening exploration of the complexities, challenges, and opportunities of leadership in nursing.

2. Key Insights and Contributions

A significant portion of the discussion was dedicated to unpacking the multifaceted role of leadership within the nursing profession. Merlin Gayathri. G, a key speaker, initiated the dialogue by highlighting the importance of transformational leadership in navigating the complex landscape of healthcare. Merlin’s perspective was supported by a common understanding that leadership goes beyond positional authority and involves a deeper engagement with the core values and principles that drive effective healthcare delivery.

A unique insight during the discussion was the concept of gaining an “edge over adults“. This referred to the need for leaders in the nursing field to harness their experience, wisdom, and insight to overcome challenges and enhance their leadership skills. The concept implies that with maturity and experience, leaders can develop a unique perspective that can help them make more informed decisions.

The discussion also revolved around the concept of influence, particularly in the context of leaders shaping the mindsets of their juniors, employees, and students. The speakers underscored the critical role of leaders in mentoring and inspiring their juniors. They discussed how leaders could model positive behaviors, motivate team members, and facilitate their professional growth, ultimately influencing them to emulate similar values and principles in their practices.

The notion of legacy also emerged as an important theme during the conversation. The speakers expressed the belief that successful leaders would leave a lasting impact that could shape the future generations of nurses. This legacy would not only be defined by their professional accomplishments, but also their contribution to nurturing a new generation of transformational leaders in the nursing profession.

Overall, the dialogue offered profound insights into the intricate relationship between leadership and the nursing profession. The speakers painted a vivid picture of how transformative leadership could foster a nurturing and productive environment, enabling the nursing profession to navigate challenges and adapt to the ever-changing landscape of healthcare.

3. Platform Sharing and Acknowledgements

Platform sharing and collaboration were integral to the discussion on transformational leadership in nursing. There was a strong emphasis on shared knowledge and the benefits of learning from each other’s experiences and perspectives. One of the significant moments of the session was when the speakers acknowledged the key personalities who shared the platform. The invaluable contributions of each were recognized and praised, fostering a supportive and collaborative atmosphere.

Merlin, also shared her experiences and insights on leadership, contributing significantly to the enriching discussion. Her successful leadership style was lauded and held up as a model for others in the nursing profession to emulate. Alongside these commendations, the panel also thanked Professor Lavanya, Surgeon R Admiral Dr V K Singh. These individuals, although not elaborated upon in detail in the initial summary, must have made significant contributions to the discourse, directly or indirectly, considering the depth of appreciation expressed for them.

Dr V K Singh, the Managing Director of InnovatioCuris Private Limited, was particularly highlighted for facilitating the dialogue. His role in providing a platform for such a crucial conversation was greatly acknowledged. InnovatioCuris Private Limited, by hosting this event, underlined its commitment to fostering leadership in nursing and contributing to the healthcare field’s development.

In conclusion, the platform sharing and acknowledgments portion of the discussion served to underline the collaborative nature of such discourses. By providing recognition and expressing gratitude, the session promoted a culture of mutual respect and shared knowledge, crucial for growth and progress in any professional field, including nursing.

4. Role of Leadership in Addressing Challenges

Throughout the discussion, it was highlighted that leadership, particularly in the nursing field, is not solely about directing, but also about fostering a sense of collective responsibility. The leaders, be they nurse managers or lead practitioners, must understand the dynamics of their team, the workplace, and the wider healthcare landscape to ensure their team’s effectiveness.

This perspective grows from the understanding that the field of nursing is paved with challenges, from high-stress environments and workload management to dealing with diverse patient populations. Such challenges require innovative solutions, and here’s where the role of leadership becomes even more pronounced.

Participants in the discussion agreed that leadership should be a collective endeavor, meaning that all members of the healthcare team should work together towards common goals, rather than following a single person. This collaborative effort can address the various challenges that the nursing field faces.

One key point that arose from the discussion is the role of leaders in influencing and guiding juniors. Leaders must focus not only on their own professional development but also on the growth of their subordinates. By doing so, they foster an environment that nurtures the individual growth of nurses and helps them feel valued. This in turn enhances their productivity and commitment to their work.

Moreover, they emphasized that leaders should actively motivate and reward their staff, as this has been shown to have a positive impact on job satisfaction and retention rates. Positive reinforcement can take many forms, from formal recognitions and promotions to simple words of encouragement.

The conversation underlined that transformational leadership can result in job satisfaction, lower burnout rates, and higher quality of care for patients. Leaders who demonstrate this style can stimulate and inspire their followers to exceed expected performance, through their continuous support and development.

In conclusion, the session illuminated that a comprehensive approach to leadership, which includes shared decision-making, regular feedback, and recognition of the unique strengths of each team member, can help navigate the various challenges encountered in nursing, and ensure the delivery of optimal patient care.

5. Transformational Leadership and its Impact

The crux of the conversation delved into the concept of transformational leadership and its substantial impact in a professional context, especially in nursing. As highlighted in the discussion, transformational leadership is not solely about a position of authority, but more importantly about the action and change that it prompts.

In this model of leadership, leaders inspire their team to work towards a shared vision, encouraging collaboration and fostering a conducive working environment. They work on building strong interpersonal relationships, provide continuous feedback, and invest in their employees’ growth and development. They also encourage a culture of innovation and creativity, enabling their team members to develop new strategies and solutions.

A critical point of discussion was the phenomenon of employee turnover due to a toxic work environment. A startling statistic raised was that 58% of employees in India reportedly leave their jobs because of a toxic boss. This statistic underscored the grave need for transformational leaders who can reverse this trend. Toxic leadership tends to stifle creativity, hinder growth, and lead to an overall demoralizing work environment, resulting in high employee attrition rates.

Transformational leaders, on the other hand, play an essential role in creating an environment that encourages open communication, collaboration, and respect. They act as role models, inspire their team members, stimulate their intellect, and consider individual needs, thereby reducing the likelihood of creating a toxic environment.

The discussion asserted that transformational leadership has the potential to turn around these challenging scenarios in nursing. By adopting a transformational leadership approach, leaders can significantly improve job satisfaction, productivity, and overall team morale. This approach can also have a ripple effect on patient care, as a content and motivated nursing workforce can provide better patient outcomes.

The consensus from the conversation was that transformational leadership in nursing is not only about the development of individual nurses but also about shaping the future of the nursing profession itself. This change is seen as an essential step towards addressing various systemic challenges in nursing and ensuring that nurses feel valued, respected, and motivated in their roles.

6. The Role of the Moderator and Vote of Thanks

The discussion was moderated by Captain Sunita, who skillfully facilitated the session, ensuring a smooth flow of ideas and active participation from all the speakers. Her expertise in guiding the conversation allowed for a comprehensive exploration of the topic, ensuring that all key aspects were covered.

During the vote of thanks, Prof Lavanya expressed gratitude to Captain Sunita for her exceptional moderation skills and her ability to maintain the engagement of the participants throughout the session. The moderator’s role was instrumental in creating an inclusive and collaborative environment where ideas could be freely shared and discussed.

Prof Lavanya also extended heartfelt appreciation to Merlin Gayathri, for her valuable contribution to the discussion. Merlin’s insights, experiences, and storytelling added depth and authenticity to the conversation. Her firsthand accounts of leadership in the organization resonated with the audience, providing practical examples and inspiring others to adopt transformational leadership approaches.

Dr. Urmila Bharadwaj, another esteemed speaker, was acknowledged for sharing her extensive knowledge and rich experience in the nursing profession. Her unique perspectives and profound understanding of the subject matter enriched the discussion, providing valuable insights for the participants.

Furthermore, the participants expressed their gratitude to Professor Lavanya, Surgeon R Admiral Dr V K Singh for their presence and contributions to the conversation. Their valuable inputs and perspectives contributed to the comprehensive understanding of transformational leadership in nursing.

The participants also extended their thanks to all the attendees, forum members, and guests who actively participated in the discussion, making it a truly collaborative and enlightening experience. The collective engagement and diverse viewpoints enriched the conversation and encouraged a deeper exploration of transformational leadership in nursing.

In conclusion, the vote of thanks highlighted the significant contributions of the moderator, speakers, attendees, and organizers in creating an engaging and thought-provoking session. The collective efforts and shared commitment to transformational leadership in nursing fostered an atmosphere of learning and growth, paving the way for a brighter future in the field.

7. Conclusion

The discussion on transformational leadership in nursing provided a comprehensive understanding of the significance and impact of effective leadership in the healthcare setting. It served as a platform for shared learning, collaboration, and reflection on the challenges and opportunities faced by leaders in nursing.

The discourse highlighted that transformational leadership is not just a theoretical concept but a practical approach that can bring about positive change. It was underscored that leaders in nursing need to be proactive, adaptive, and visionary, fostering a culture of trust, engagement, and continuous improvement.

Participants of the discussion were inspired to reassess their roles as leaders and the strategies they employ. They recognized the need to embrace a more inclusive and transformative leadership style to address the complex and evolving challenges in healthcare. The importance of developing strong communication skills, empowering team members, and recognizing and rewarding their contributions were emphasized as key elements of transformational leadership.

Furthermore, the discussion shed light on the significance of fostering a positive work environment and ensuring the well-being of nurses. The role of leaders in promoting work-life balance, offering support and resources, and actively addressing burnout and stress among healthcare professionals was acknowledged as essential.

The overall sentiment expressed by the participants was one of optimism and determination. The collective belief was that through collaborative efforts, the nursing profession can advance and achieve greater heights. The power of transformational leadership to inspire, motivate, and guide individuals and teams towards excellence in patient care was recognized as a catalyst for positive change.

In conclusion, the discussion on transformational leadership in nursing provided valuable insights, perspectives, and practical approaches to enhance leadership effectiveness in the field. It emphasized the need for leaders to adapt, innovate, and embrace a transformational mindset to meet the evolving demands of the healthcare landscape. By nurturing a culture of empowerment, compassion, and continuous learning, nursing leaders can drive positive change and elevate the profession to new heights, ultimately benefiting patients, healthcare organizations, and the broader healthcare ecosystem.

This year the International Council for Nurses selected an important theme for International Nurses’ Day, “Our Nurses’, our future”. It’s a critical time of reflection for nursing as a profession and we move forward which led me to the title of my editorial. What did the leaders of yesteryear do, differently? The leadership at the time amongst women and in nursing was about courage, empowerment and elevation of the status of women and nurses, albeit that it was not as inclusive as it should be.

Nursing as a profession at the time, was fairly subservient, with nurses taking instructions from the doctor and having very little autonomy. Clinical practice was excellent with nurses giving the best care to their patients. It may not have been evidenced based but they delivered to the best of their ability under the circumstances. Matrons were formidable people who had so much power and were autocratic at so many levels.

The discipline in nursing was paramount to remaining in the profession. Uniforms were white and starched and nursing caps were worn to keep the hair in order. These later changed to become more ornamental and much smaller. The working hours were long and many senior nurses would share that they worked 12 days before they got a weekend off.

Training was strict. There were penalties at so many levels. Many senior nurses recall the preliminary exams after being in training for 3 months. If you failed, you were asked to leave. It was their way of weeding out the bad apples early in training. So while much of this sounds archaic and foreign to many of you, it built a solid foundation for the nursing profession. Nursing was a disciplined profession where the patient came first. The important thing to remember about yesteryear was nurses were known for their caring ability. Care and compassion is what drove many nurses to remain in the profession. It is no wonder that they called them the ladies with the lamp.

So, that brings me to nursing today.

Women are emancipated and are occupying senior roles and there are changing family dynamics with the breadwinner not necessarily the man of the family. But have they become totally accepted? Do we truly have gender equity and how does this play out in the various spaces, at home, in the boardroom, on the golf course to mention a few? People and communities want value for their money. They want the best possible care and will express their dissatisfaction publicly. Against this landscape, nursing is also facing serious challenges today.

We have great nurse leaders today as well. The principles that guide nursing leadership today are innovation, globalization, research, and change. We are beginning to see more nurse leaders in executive roles and taking on senior roles at universities. We have had and continue to have deans, deputy vice chancellors and vice chancellors who started out as nurses.

But I also ask myself, what would the leaders of yesteryear do differently around the changes in nursing education. How can we collectively manage the current situation that seems to be exacerbating the shortage of nurses in most countries. What would we have to do differently, in order to make nursing a disciplined profession focussed on the best patient outcomes. Perhaps, some food for thought for us.

In terms of our nursing practice, we have made strides in moving into evidence based practice and critical thinking. The notion of do because I say so, does not work any longer. The nurse of today is questioning protocols and doctor’s orders. They want to be part of the multidisciplinary team and make a contribution to the plan of care.

But we also see many more adverse events. Perhaps, we can take comfort and say it is because we have improved measures and records today. But clinical competence of nursing practitioners are often questioned and we must take responsibility for ensuring the delivery of safe patient care. The caring aspect of nursing is being monitored, measured and critiqued on a daily basis. Unfortunately, most often our report card does not look as great as we would like. What will we need to do differently to change our scores?

The education of nurses in nursing colleges has been with us forever and perhaps, based on the numbers, it was necessary to adopt this mode of provider and delivery. However, this also leads to isolation of the education of nurses. Unlike their university counterparts, nurses trained at colleges are not exposed to other students or disciplines. This may not augur well in terms of interprofessional education, which is being discussed in more health related education circles.

So, a snapshot of nursing today indicates that we have made strides in nursing leadership, evidence based practice, research and nursing education with more nurses acquiring masters and doctoral degrees. But it is going to take collective leadership and focus for us to manage the change in qualifications, regulation and improved practice in nursing. We are the generation that have always been hard on ourselves and had difficulty celebrating our achievements. That takes me to the future of nursing. The generation who will review our legacy.

Once again, it is going to be influenced by the political landscape. Better informed communities making informed choices and demands. People who will not accept the status quo and will want change. Women will continue to move up the ladder and take on more roles. The challenge will be whether they will come into their own. Will women be supportive of other women in the workplace? There will be new issues on the table then with the future generation demanding work life balance, flexible working conditions and corporates moving into virtual offices.

Leadership in nursing will also take on a new look and feel. The future generation of nurse leaders are going to be critical of their predecessors and would expect change to happen more expediently. Young nurses are going to speak out and may be less inclined to throw caution to the wind.

Hospital and community settings will also be different. People are already becoming better informed and making their own diagnoses prior to consulting their doctors. Dr Google is always the first point of consultation and if there is any concern, they will proceed to the doctor. Technology is going to be a major disruptor. Access to information and services is going to be so much easier. Online check in will progress from airports to hospitals, and clinics. Electronic health care records are going to allow patients to have access to their own history and move with their records to any practitioner.

Patients will have choice and Quality will be the only differentiator. Hospitals and clinics will need to publish their clinical and quality outcomes, and patients will choose the facility they wish to make use of. Everyone is looking for the ideal experience, so measurements of patient/customer experience will continue to increase and healthcare will become more competitive. Patients will shop around for quality and price in the future before they choose a healthcare provider.

With changes in society and healthcare delivery systems, education of nurses will also need to change. Interprofessional education will become more necessary, as health professionals will work in teams. Students will have access to the latest devices and books may become a thing of the past. No lugging of big bags with books and notes, just one device with everything on it. The concept of travel light comes to mind. I believe nurses will be better educated and will demand higher salaries with improved education, so there will be an introduction of healthcare technicians to take over the administrative and non-clinical tasks .

Clinical competence will be critical for nurses and healthcare professionals with increase in litigation. Patients know their rights and will be well versed with the responsibilities of nurses and other healthcare professionals. The nurse patient relationship will change with a demand for more patient information and follow up care. Families will also become very involved in the care of their loved ones, and home care will be in demand. Institutional care will be limited to acute care and with reduced length of stay. The future sounds like an adventure and I believe it is going to make a difference to healthcare delivery globally.

In conclusion, each period comes with its own achievements and challenges. Yester year was about courage, growth and empowerment of women and elevated education of nurses. Today, it is about innovation, career progression for women, internationalization, evidenced based practice, and research. Tomorrow is going to be about increased technology, casualization of formal employment, work life balance, transformed healthcare and empowered patients and communities.
