<p class="wp-block-paragraph">The discussion moved beyond innovation buzzwords to examine the real-world impact of genomics, artificial intelligence, standardization, ethics, and most importantly patient trust.</p>



<h3 class="wp-block-heading"><strong>Diagnostics Moves From the Background to the Frontline</strong></h3>



<p class="wp-block-paragraph">For decades, diagnostics functioned quietly in the background of healthcare. Patients rarely interacted with diagnosticians, and lab reports were often viewed as secondary confirmations of a clinician’s judgment. According to Dr. Mahajan, that perception has fundamentally changed.</p>



<p class="wp-block-paragraph"><strong><em><mark style="background-color:rgba(0, 0, 0, 0);color:#09599a" class="has-inline-color">“Diagnostics is no longer about confirming illness. It is about predicting disease, preventing it, and managing a person’s entire health journey.”</mark></em></strong></p>

<p class="wp-block-paragraph">This shift reflects a broader transformation in healthcare itself. Preventive screenings, wellness monitoring, and early risk assessment now rely heavily on diagnostics. Individuals today engage with testing not only when they are sick, but also when they want to stay healthy making diagnostics a part of everyday life rather than episodic care.</p>



<h3 class="wp-block-heading"><strong>Molecular Diagnostics and the Democratization of Precision</strong></h3>



<p class="wp-block-paragraph">One of the most powerful drivers of this change has been molecular diagnostics. What was once considered niche or highly specialized is now increasingly accessible to routine patients. Dr. Mahajan highlighted how PCR and genomic testing have become household terms, especially following the COVID-19 pandemic.</p>



<p class="wp-block-paragraph"><strong><em><mark style="background-color:rgba(0, 0, 0, 0);color:#09599a" class="has-inline-color">“Today, we can predict susceptibility to disease by testing DNA from a simple blood or saliva sample and that has completely changed how early we can intervene.”</mark></em></strong></p>



<p class="wp-block-paragraph">Molecular diagnostics has revolutionized disease detection across oncology, infectious diseases, and autoimmune disorders. The ability to diagnose earlier, predict risk, and tailor treatment plans has pushed healthcare closer to the ideal of personalized medicine—where therapy is shaped around the individual rather than the average patient.</p>

<h3 class="wp-block-heading"><strong>Artificial Intelligence: Acceleration With Accountability</strong></h3>



<p class="wp-block-paragraph">Artificial intelligence has emerged as a powerful force within diagnostics, improving efficiency, speed, and accuracy in radiology and pathology. AI-powered tools can now screen pathology slides, assist radiologists in detecting early cancers, and reduce diagnostic delays—particularly in regions with limited access to specialists.</p>



<p class="wp-block-paragraph">However, Dr. Mahajan was unequivocal in her caution: AI is a tool, not a decision-maker.</p>



<p class="wp-block-paragraph"><strong><em><mark style="background-color:rgba(0, 0, 0, 0);color:#09599a" class="has-inline-color">“AI can screen faster and reduce errors, but it cannot replace clinical judgment. Without human oversight, even the smartest algorithm can misdiagnose.”</mark></em></strong></p>



<p class="wp-block-paragraph">She emphasized that AI systems are only as good as the data on which they are trained. A lack of diversity or quality in training datasets can introduce bias, leading to skewed results that directly impact patient care. Continuous validation, transparency, and regulatory oversight are therefore critical to safe AI adoption.</p>



<h3 class="wp-block-heading"><strong>Standardization: The Backbone of Credibility</strong></h3>



<p class="wp-block-paragraph">One of the most pressing concerns raised in the conversation was the lack of uniformity across diagnostic laboratories. Variations in reference ranges, methodologies, and reporting standards often leave patients confused and clinicians uncertain.</p>



<p class="wp-block-paragraph">Dr. Mahajan stressed that accreditation, adherence to international quality standards, regular audits, calibrated equipment, and continuous staff training are not optional—they are fundamental. Without standardization, diagnostics risks becoming volume-driven rather than value-driven, leading to over-testing, patient anxiety, and unnecessary healthcare costs.</p>



<h3 class="wp-block-heading"><strong>Over-Testing and the Need for Value-Based Diagnostics</strong></h3>



<p class="wp-block-paragraph">While access to diagnostics has improved, it has also created a paradox: more testing does not always mean better care. Over-testing can result in false positives, misdiagnosis, and emotional distress for patients.</p>



<p class="wp-block-paragraph">Dr. Mahajan advocated for stronger collaboration between clinicians and diagnosticians, evidence-based testing protocols, and better patient education. The goal, she explained, is <strong>smarter diagnostics</strong>, not more diagnostics.</p>

<p class="wp-block-paragraph">Addressing recent controversies around lab practices and data integrity, Dr. Mahajan underscored that trust is fragile—and once lost, difficult to rebuild. Transparency, she argued, is the most powerful tool laboratories have.</p>



<p class="wp-block-paragraph">She noted a positive cultural shift: patients now actively want to speak with diagnosticians, discuss reports, and understand their health data. This growing engagement reflects a new era where patients expect not just accuracy, but accountability and empathy.</p>



<h3 class="wp-block-heading"><strong>The Road Ahead: Technology With a Human Core</strong></h3>



<p class="wp-block-paragraph">Looking forward, Dr. Mahajan expressed optimism about emerging breakthroughs such as multi-omics diagnostics, AI-assisted tools for rural healthcare, and continuous monitoring through wearables. These innovations promise earlier detection, ultra-personalized treatment, and improved access.</p>



<p class="wp-block-paragraph">Yet the podcast made one truth abundantly clear: technology alone cannot define the future of diagnostics.</p>



<p class="wp-block-paragraph">The future will belong to systems that combine scientific excellence, ethical practice, rigorous quality standards, and human judgment—where every report carries not just data, but responsibility.</p>

<h3 class="wp-block-heading">A Serendipitous Introduction</h3>

<p class="wp-block-paragraph">While visiting the Bay Area, I reached out to my classmate Saurabh Gupta, a professor of biomedical engineering, who suggested I meet Professor Anurag Mairal. During my visit to his lab, we were joined by a young medical student who had recently dropped out of his program. Professor Mairal invited me to participate in a coaching conversation he was having with this student about healthcare in general.</p>



<p class="wp-block-paragraph">Our discussion touched on a fundamental critique of modern medicine—that our current system operates as &#8220;sick care&#8221; rather than true healthcare. While I had encountered this concept before, our conversation highlighted how difficult it is to overcome the inertia of existing systems and transition toward genuine preventive care.</p>



<p class="wp-block-paragraph">We also explored a particularly striking insight about healthcare economics in developed nations: as populations live longer, healthcare costs rise exponentially. I positioned this as a &#8220;Malthusian Trap&#8221; (borrowing from Paul Lillrank&#8217;s work), where health systems cannot simultaneously control costs and pursue longer lifespans for their populations. The Finnish healthcare system exemplifies this challenge—only 5% of an individual&#8217;s lifetime healthcare expenses occur before age 65, with the bulk concentrated in their final years.</p>



<p class="wp-block-paragraph">Following our discussion, Professor Mairal graciously presented me with his book, inscribed with the thoughtful note that &#8220;we should reimagine healthcare together.&#8221;</p>



<h3 class="wp-block-heading">The Book: A Chronicle of Healthcare Innovation</h3>



<p class="wp-block-paragraph">Reimagining Health: Inspiring Stories of Innovators serves as a compelling chronicle of the struggles, setbacks, and ultimate successes of individuals who create solutions for problems they deeply care about. Each story is both touching and meaningful, offering unique insights into the innovation process.</p>



<p class="wp-block-paragraph">Mairal&#8217;s perspective as an integral part of Stanford&#8217;s biodesign ecosystem provides him with a valuable vantage point. Being immersed in an environment of driven individuals and enabling resources gives the author firsthand understanding of how innovation can become an everyday pursuit, while also revealing the inevitable hurdles and challenges that accompany it. Similar to Mairal, the co-author, Sonali also comes from an ecosystem of health systems. Hence, both are present to the whole lifecycle of technology led innovations in healthcare.</p>



<h3 class="wp-block-heading">The Power of Purpose-Driven Innovation</h3>



<p class="wp-block-paragraph">A central theme emerges throughout the book: most successful innovators experience some trigger or defining moment that sets them on their problem-solving journey. The book clearly articulates to readers that &#8220;if you deeply care about a problem, you can overcome the hurdles and make an impact.&#8221;</p>



<p class="wp-block-paragraph">We need more such books to inspire people to take the leap into innovation. As authors notes toward the end, some of the fish who make the jump transform into mighty dragons, (reference to a mythological story) —a beautiful metaphor for the potential that lies within purpose-driven individuals.</p>



<h3 class="wp-block-heading">Looking Forward: Prevention vs. Treatment</h3>



<p class="wp-block-paragraph">While the authors acknowledge alternative medicine and preventive health as crucial future directions, many of the technological interventions profiled still focus primarily on treating illness rather than preventing it. I hope (authors) they will consider a future edition that profiles individuals who approach healthcare specifically through the lens of disease prevention.</p>



<h3 class="wp-block-heading">Recommendation</h3>



<p class="wp-block-paragraph">I highly recommend this book for anyone seeking inspiration and courage to solve problems they deeply care about. It demonstrates that meaningful innovation often begins with personal passion and perseverance. A Delightful Coincidence.</p>



<p class="wp-block-paragraph">As a postscript, I must share a remarkable coincidence: while reading this book in the Delhi Airport lounge (traveling from Norway to Bengaluru), I reached page 35, which mentioned Professor Balram Bhargava. Looking up, I realized Professor Bhargava was sitting directly infront of me in the lounge! It was a pleasure to bring the book and his mention in it to his attention. Sometimes the world feels remarkably small, or perhaps what we read influences what we notice around us.</p>

Estrogen’s role in protecting brain cells from oxidative stress and inflammation is only beginning to be fully understood, but its implications highlight the importance of proactive cognitive health strategies during midlife.</p>



<h3 class="wp-block-heading"><strong>A Missed Public Health Opportunity</strong></h3>



<p class="wp-block-paragraph">Despite the established link between menopause and the increased risk for several NCDs—including cardiovascular disease, diabetes, osteoporosis, and mental health disorders—this life stage remains largely absent from mainstream public health programming. Menopause is rarely addressed as a distinct health milestone requiring preventive care, leaving many women unaware of the risks and without access to timely interventions.</p>



<p class="wp-block-paragraph">This gap reflects broader gender disparities in healthcare and reveals a pressing need for systemic change. Integrating menopause into the public health narrative offers a pivotal opportunity to reduce the long-term burden of chronic illness and promote healthy aging.</p>



<h3 class="wp-block-heading"><strong>Towards a Gender-Responsive Health Agenda</strong></h3>



<p class="wp-block-paragraph">Public health strategies must begin to see menopause not as a private medical issue but as a societal health turning point. This requires:<br>&#8211; Risk-based screenings for hypertension, diabetes, dyslipidemia, and bone density<br>&#8211; Culturally appropriate counselling on lifestyle changes, physical activity, and nutrition<br>&#8211; Access to mental health services, tailored to midlife emotional and cognitive challenges<br>&#8211; Educational initiatives that empower women with knowledge about their changing bodies and health risks</p>



<p class="wp-block-paragraph">A lifespan approach to women’s health, beyond the reproductive years, can help build more equitable and effective health systems.</p>



<p class="wp-block-paragraph">Menopause is far more than the end of menstruation—it is a critical inflection point in a woman’s health journey. By recognizing the strong link between menopause and non-communicable diseases, we can shift from reactive treatment to proactive prevention. It’s time we brought menopause to the forefront of public health discourse and created supportive ecosystems that enable women to thrive—not just survive—through this transition and beyond.</p>

He proposed an AI-based model to support and improve triage decisions, developed using a dataset of 1,200 patients. The AI algorithm demonstrated superior performance compared to other models, with heart rate emerging as the most significant predictor of triage category. Dr. Ankit highlighted the importance of feature analysis in the model and discussed its limitations, including the small sample size and the need for more robust objective parameters like mortality.</p>

<p class="wp-block-paragraph">In the past decade of my clinical practice, I have encountered numerous patients brought to the hospital in life-threatening conditions. Among them were several young individuals who, despite having no prior medical history, required ICU admission due to major health issues such as heart attacks, strokes, kidney or liver dysfunction, and other serious conditions. During the assessment of their medical histories, no significant health issues were identified before hospitalization. Unfortunately, some of these young patients succumbed to their ailments. When questioned by the patients&#8217; relatives about the preventability of these clinical conditions, doctors responded affirmatively, emphasizing that prior health check-ups might have detected abnormal parameters earlier.</p>



<p class="wp-block-paragraph">The modern lifestyle, characterized by altered food habits and disrupted sleep patterns, underscores the importance of identifying minor health issues in their early stages to prevent major catastrophic events. While individuals invest significantly in health insurance to mitigate the financial impact of hospitalization, they often neglect preventive measures. Many diseases are preventable, and the progression of these conditions can be controlled or reduced with timely interventions. Therefore, preventive medicine warrants equal importance alongside therapeutic management. As the saying goes, it is better to build dams than to wait for the flood.</p>



<p class="wp-block-paragraph">Preventive medicine involves promoting health measures to avert disease, a form of prophylaxis that reduces the risks of diseases, disabilities, and death. There are four levels of prevention:</p>



<ol class="wp-block-list">
<li><strong>Primordial prevention: </strong>Preventing the occurrence or development of risk factors.</li>
</ol>



<ol start="2" class="wp-block-list">
<li><strong>Primary prevention: </strong>Preventing disease when a risk factor is present.</li>
</ol>



<ol start="3" class="wp-block-list">
<li><strong>Secondary prevention: </strong>Preventing the complications of an existing disease.</li>
</ol>



<ol start="4" class="wp-block-list">
<li><strong>Tertiary prevention: </strong>Preventing disability or death from an existing disease.</li>
</ol>



<p class="wp-block-paragraph">By the time individuals seek medical consultation, they often already have established diseases or risk factors. While doctors can provide medications for secondary or tertiary prevention, primordial and primary prevention are equally vital, emphasizing the proverb that prevention is better than cure.</p>



<p class="wp-block-paragraph">Consider, for example, a young overweight individual presenting with persistent headaches and elevated blood pressure. Treating the high blood pressure with medications constitutes <strong>secondary prevention</strong>. However, had the same individual consulted a doctor a few years earlier when noticing weight gain, the doctor might have advised lifestyle modifications to control body weight, representing <strong>primary prevention</strong>. If attention had been given to healthy food, nutrition, and exercise before weight gain occurred, this health education would be considered <strong>primordial prevention</strong>.</p>



<p class="wp-block-paragraph">Physicians can guide individuals in adopting preventive health measures, emphasizing that preventive measures extend beyond medications to include simple lifestyle modifications, exercises, nutrition, and health awareness. While abundant information on healthy living is available on the internet, individuals often struggle to discern when to consult a doctor in the absence of active health issues.</p>

<p class="wp-block-paragraph">Even in the absence of symptoms, it is advisable for individuals to undergo routine health check-ups annually. Early detection of abnormal parameters is fundamental to preventive medicine. These check-ups should include blood investigations (<strong>complete blood picture, renal function test, liver function test, thyroid profile, random blood sugar, and lipid profile), ECG, 2D-ECHO</strong>, and <strong>abdominal ultrasonography</strong>. If all results fall within normal limits, primordial prevention—incorporating healthy food, nutrition, exercises, and discouraging alcohol, smoking, and drug use—is sufficient.</p>



<p class="wp-block-paragraph">For individuals with deranged lipid profiles and elevated cholesterol levels, dietary and lifestyle modifications are recommended as cholesterol is a risk factor for sudden heart attacks. Similarly, individuals with higher blood sugar levels can implement lifestyle changes, exercise, and dietary modifications to maintain HbA1c below 5.7%, avoiding the need for medications.</p>



<p class="wp-block-paragraph">Awareness about health-related issues can aid in identifying major health concerns early. By cultivating the habit of periodic health status checks, many diseases can be prevented. Even healthy individuals should consult a doctor annually to discuss health check- up reports, allowing for preventive measures. Further investigations may be necessary if preliminary reports are abnormal, requiring regular follow-up with a doctor.</p>



<p class="wp-block-paragraph"><strong>Common preventive measures advised by doctors to healthy individuals include:</strong></p>



<ol class="wp-block-list">
<li>Periodic measurement of blood pressure.</li>
</ol>



<ol start="2" class="wp-block-list">
<li>Annual blood tests to identify diabetes, thyroid disorders, and dyslipidaemia.</li>
</ol>



<ol start="3" class="wp-block-list">
<li>Breast cancer screening for women over 40 years and colon cancer screening for men aged 50 to 75 years.</li>



<li>Smoking, alcohol, and drug cessation.</li>
</ol>



<ol start="5" class="wp-block-list">
<li>Avoidance of a sedentary lifestyle and maintenance of a healthy body weight.</li>
</ol>



<ol start="6" class="wp-block-list">
<li>Regular vaccination and immunization.</li>
</ol>



<ol start="7" class="wp-block-list">
<li>Hand and oral hygiene.</li>
</ol>



<p class="wp-block-paragraph">Preventive care investigations or tests may vary by age group, with cancer screening and bone density screening becoming more relevant to elderly people. Despite the absence of apparent health issues, everyone should prioritize preventive healthcare for several reasons:</p>

<p class="wp-block-paragraph">In today&#8217;s rapidly evolving world, the intersection of healthcare and innovation has become a focal point for many discussions. Healthcare is often misunderstood as being synonymous with hospitals, surgeries, and medical technologies. However, it is an umbrella term encompassing a wide array of elements, including policy, climate change, and even the everyday practices that contribute to maintaining health.</p>

<p class="wp-block-paragraph">Innovation in healthcare is not confined to groundbreaking technologies or advanced medical devices. It includes any new method, idea, or product that improves health outcomes or makes healthcare delivery more efficient. Consider the example of the nanoparticle mask developed during a hackathon, which helped protect against COVID-19. This is a prime example of how innovative thinking can address specific health challenges.</p>



<p class="wp-block-paragraph">Another example of innovation is the use of biodegradable plastics, which addressed the environmental health issues. By reducing plastic waste, we can mitigate the adverse effects of microplastics on human health. For instance, research has shown that microplastics are now prevalent in breast milk and even in human reproductive organs, highlighting the importance of finding sustainable alternatives to conventional plastics.</p>



<p class="wp-block-paragraph">Similarly, the development of apps and software to monitor health metrics, such as heart rate or glucose levels, exemplifies innovation that empowers individuals to take charge of their health.</p>

This meant food had to be consumed quickly to avoid spoilage, leading to frequent trips to gather fresh supplies. As time progressed, people began using simple methods to extend the shelf life of their food, such as using mud pots to keep food cool. These pots maintained a lower temperature than the surrounding air, helping preserve food for longer periods. The invention of the refrigerator revolutionized food storage, with modern refrigerators using advanced cooling technologies. However, traditional refrigerators had drawbacks, including the release of harmful CFCs and high energy consumption. Innovations have led to eco-friendly refrigerators that use alternative refrigerants and are more energy-efficient, reducing their environmental impact and contributing to better public health.</p>



<h3 class="wp-block-heading">The Role of Perspective</h3>

<p class="wp-block-paragraph">It&#8217;s not like we don&#8217;t know any of this; we all know it and continue to do it even though we have the choice to change. We have become complacent, living by what is going on without questioning it. Many have even stopped being curious. Curiosity leads to questions, questions lead to discussions, discussions generate ideas, ideas bring together like-minded individuals willing to work on them, and when people work on these ideas, there is always improvement. And that&#8217;s what we call &#8220;innovation&#8221; in high terms. It is as simple as that.</p>



<p class="wp-block-paragraph">Innovation in healthcare is a multifaceted concept that goes beyond technology. It involves rethinking how we approach health and well-being, finding new ways to solve problems, and considering the broader implications of our actions. By expanding our understanding of what healthcare and innovation mean, we can create a healthier world for everyone.</p>
