Ms. Ishi Khosla is a practicing clinical nutritionist, columnist, author, a researcher and an entrepreneur. She is actively involved in clinical practice at the Centre For Dietary Counselling in Delhi and has spearheaded the launch of the first of its kind health food company “Whole Foods India”. She’s authored several books, including Eating at Work, The Diet Doctor, Is Wheat Killing You? and The Cholesterol Facts. She has been nominated as a member of the Programme Advisory Committee, Doordarshan Kendra; is a member of the Governing Body of Lady Irwin College, Delhi University; and is a consultant to several corporate and multinational organizations.

She has been felicitated by the President of India for work in the field of nutrition. She has also been listed among the 25 most powerful women in the country by the India Today Group.

Having spent the last 30 years focusing on the functional aspects of food in treating chronic health problems that we have faced, I have to say that it is for the first time in my professional experience that the word ‘I’, yes, the Almighty immunity has been spoken of at such a global scale. Immunity is being sold” by every brand, every food label and even toilet cleaners are being tagged with immune benefits …..

This was possibly inevitable & fast coming- the aggressive pace at which our food production & consumption have grown & changed. It is only natural that in the times of a health crisis, the entire world is pausing to, in fact, reflecting on what we are actually putting “inside our bodies”

Pick up any prescription of a patient fighting COVID-19 today and you will see what I am talking about in bold ink – Vitamins D, C + Zinc and Magnesium even probiotics – some of these elements have become the salient factors in fighting the pandemic across the world. A review & meta-analysis published in the Frontier of public health this year showed an exceptionally high mortality among patients in ICU who have Vitamin D levels less than 20 ng/ ml . Evidence indicates that vitamin D helps by preventing the cytokine storm and subsequent acute respiratory distress syndrome (ARDS ) which is commonly the cause of mortality. In addition a low Vitamin-D level was associated with an 80 percent higher risk of COVID-19 infection compared to those with normal levels.

If we zone in to the latest research we need to recognise the fact that our gut has a mind of its own with the underlying microbiome.

It was already estimated in the year 2011, that around two billion people in the world are deficient in one or more micronutrients. A review & meta analysis done in Singapore published in Nutrients in May 2021 clearly states that absence of micronutrient deficiency is strongly associated with lowered risk of infection & better outcomes among COVID-19 patients & that it offers a cost effective preventive & therapeutic approach for the pandemic. Several programmes have been launched over the years in India to improve nutrition and health status of the population; however, a large portion of the population is still affected by micronutrient deficiency. When linked with food allergies, the connection becomes even clearer – to give you all an example from my area of expertise – wheat-related disorders like Celiac Disease which have gone up by Five Fold since 1974 (that’s roughly in the last 50 years) (Annals of Medicine) such diseases are associated with multiple micronutrient deficiencies due to malabsorption, high inflammation and lower immunity; most common nutritional deficiencies include iron, calcium, folate, Vitamin B-12. Ample research has also shown that iron deficiency anaemia can affect our immune system – the body’s natural defence system. This increases our vulnerability to infection. But it has taken a global health catastrophe to shine light on this underlying issue that is affecting us in more ways than we know and there’s never been a better time in history to establish the importance of food and nutrition in bridging the gap when it comes to being future-ready as a country to fight for better health.

If we zone in to the latest research we need to recognise the fact that our gut has a mind of its own with the underlying microbiome. More than 70% of our immune system resides in our gut, so it is obvious that for maintaining a resilient immune system, we need to have the right gut composition of pro and prebiotics. The focus has been to constantly reduce systemic chronic & acute inflammation through anti-inflammatory agents like Ginger, Turmeric, Giloy, Tulsi, Honey and other Phytonutrients available in brightly coloured fruits and vegetables and healthy fats like Omega-3 and medium chain triglycerides in virgin coconut oil. By simply cutting down pro-inflammatory agents from our diets like chemical additives, trans fats and allergens plus reducing sugars and high glycemic carbohydrates ( most cereals & refined carbohydrates ) we can really make a lot of difference in fighting health issues.

Truth of the matter remains that prescriptive advice only makes sense in times of adversity when it’s often too late, when the fight is already underway and a lot has to be done.

Truth of the matter remains that prescriptive advice only makes sense in times of adversity when it’s often too late, when the fight is already underway and a lot has to be done. Preventative health continues to be a buzz word in a country like ours where lifestyle related disorders are growing at a super high pace and are expected to cross 1 billion cases by 2022.

It has to start much earlier, this entire process of awareness and the responsibility lies equally in all our worlds – right from agricultural practices, soil quality to fortification, biodiversity, introduction of alternate grains & millets , manufacturing practices , waste management, positive labelling to disseminating the right information. There are so many glorious examples of best practices we can look upto to address some of the issues we are facing today. Addressing Vitamin D deficiency through food fortification in the West (and in India), supplying probiotic shots of fermented milk to citizens of Japan to fight gut infections at scale under Dr Minoru Shirota since 1935 , the innovation of FOSHU and FFC ( foods & functional claims ) system of labelling health claims in Japan , elimination of pro-inflammatory trans fatty acids started in Denmark, Canada and New York.

I, in my own small way, have taken the opportunity in the past to not only preach but showcase a model to manufacture and retail safe foods, high on nutrients and functional foods free of undesirable constituents like chemical additives, trans fats using healthy ingredients, apart from this the allergen free foods including gluten and dairy free, and more. Among the first of its kind food company Wholefoods started as a home kitchen outfit, expanded to have presence in some of the leading hospitals across the NCR region. It clearly established to me and my colleagues that there is an appetite for health if delivered in a sound holistic manner.

It is time for collective action, integrated thinking and innovative solutions to find ways that can actually work for the people and the planet. Coming together of all stakeholders with a shared vision will ensure that the ultimate goal of ending all forms of malnutrition and nutritional security is achieved and we need not have to wait for a pandemic to build a robust immune system. It is the only sound investment to build a healthy population that will eventually pave the way for a healthy nation.

Imagen’s product ‘OsteoDetect’, an Artificial Intelligence-based diagnostic tool that can quickly detect distal radius wrist fractures is one of the latest products to get US FDA marketing clearance. Its machine learning algorithm studies 2 dimensional x-rays or radiographs for signs of fracture by analysing the posterior-anterior and medial-lateral x-ray images of adult wrists. Studies conducted on comparing fracture detection aided by OsteoDetect and certified orthopaedic surgeons showed that the readers’ ability to detect wrist fractures was improved by using the software as evident by greater sensitivity, specificity, and positive and negative predictive values, compared with unaided standard clinical practice.

Arnav Kapoor, an Indian-origin researcher at MIT, has developed a device named ALTEREGO that lets others hear words you are thinking without your making a sound. The product looks like a curved bone hooked to one ear that touches the jaw on the chin and under the lower lip which works by using ‘subvocalisations’ which are the tiny, imperceptible movements in the jaw each time you say a word in your mind. There are four sensors which pick up the signals and transmit them to a computer that has been trained to recognize and convert them back to words which are conveyed to the listener’s mind through bone conduction in the listener’s jawbone, instead of words being openly spoken and conveyed. With the use of Alterego’s technology, one could ask questions soundlessly and get a voice reply only in your ears. Presently the product has an accuracy of 92% which claims to keep improving with the exposure to more types of vibrations and words. Times of India

A study undertaken by Dr. Satish Dubey at IIT Delhi has revealed a new portable diagnostic device which is developed by Inito, Bengaluru-based company that can do a dozen tests with the comfort of a person being at home. It has the advantage of reading dozens of diagnostic test strips with a 99.12% correlation when compared to lab grade scanners irrespective of the mobile phone used. This makes it a very reliable tool for portable diagnostics. The device uses patent-pending imaging technology called flat lens that allows a single portable device paired with a smartphone to conduct dozens of diagnostic tests with the same device. The first test that the company has launched is a fertility monitor targeted at couples trying to have a baby. Soon tests for Thyroid, Diabetes and Vitamin D will be added to the device. To add more tests to the device, you just need to update the App and order the strips for the user. Advantages of the device are that it is easy to use, portable, small size, affordable and also third-party strips can be added to the Inito platform by just an App upgrade. The device has won CII Design Award, India Design Mark and Japanese Good Design Award. The device is priced at INR 3195 and is available on Inito’s official website and Amazon. Soon the device will be made available at pharmacies. HT Team

An innovation by an Indian research scholar, Ramya Veerubhotla at IIT Kharagpur, is the development of a disposable and flexible battery made from paper that could generate power from the bacteria present in sewage water. The battery is made using air cathode and the anode can be made from any carbon-based material. The product is unique because it is very light in weight and the power generation starts within ten seconds, unlike other usual microbial fuel cells which take a couple of days to start power production as the bacteria need time to adjust to the environment. The capacity of power generated from one cell is in the range of a few microwatts. For more power production, one can stack and fold these cells together giving it a compact shape. This research recently received the First prize and a cash award of INR 10 lacs at KPIT Sparkle 2018. www.news18.com

The product Naked 3D Fitness Tracker is a health and fitness tool for people to track changes in their measurements, body fat percentage, lean mass, fat mass, circumference and overall shape. The start-up has recently raised a $14 million Series A led by Founders Fund. The device has two parts, first is a scale that has sensors and a computer and second being a weight scale that spins a person around so that the stationary mirror can grab the body scan of the user in about 15 seconds. Naked uses infrared light to capture your 3D body model which then gets visualised on the Naked App and alerts the user when there is progress being made by the user in terms of their fitness and also where there is potential for more work to be done. The device has an Intel x86 processor, RAM, 4GB DDR4 RAM and a 64GB SSD which makes it easy to beam the image directly from the device to the user’s phone. The depth data of the users’ body is not uploaded to the cloud instead is being handled on the device itself and also the resultant image is more of a TSA body scan image rather than a 3D avatar. A person can permanently delete the data from his phone if he/ she wishes to. Presently the device is priced at US$ 1395. www.techcrunch.com

A group of researchers under the mentorship of Scientist Alberto Salleo at Stanford University has created a stretchy patch which when applied directly to the skin, takes up sweat and assesses the amount of cortisol the person is producing. This stretchy, rectangular sensor around a membrane/specifically binds only to cortisol but also detects charged ions like sodium or potassium present in our sweat. This patch has a waterproof layer that protects it from contamination. So far, a wearable prototype version has been made but if this is converted into reality it would be very easy to monitor salt imbalances in our bodies and reveal the emotional state of people who are under immense stress and do not open about their situation. The present methods for measuring cortisol levels requires several days of waiting for the lab results. This device seems to solve the waiting problem for the results and is user-friendly. All the user needs to do is to sweat enough to glisten the patch and connect it to a device for analysis which gives results in seconds. The researchers are still working on its reusability though the prototype seems to work multiple times so long as it is not saturated with sweat. A paper co-authored by Scott Keene and Andrew Marais of Stanford University and colleagues at Ecole Nationale Superieure des Mines in France about this wearable sensor was published in Science Advances on 20 July 2018. https://news.stanford. edu/2018/07/20/wearabledevice- measures -cortisol-sweat

Iris is the part of the eye that is colored around the pupil and controls the amount of light entering the eye. It is found to be damaged in conditions like Aniridia in which people are born with a partial or missing iris or it could happen later in life due to traumatic injury or surgical removal due to melanoma. With patients of eye damage, the surgically implanted device known as the CustomFlex Artificial Iris is a boon. Recently this prosthetic iris has got the US FDA approval. The CustomFlex Iris is surgically inserted and held in place either by sutures or eye’s anatomical structures. This iris is made of thin, foldable medical grade silicon and is custom-fitted and colored for each patient. A study of nearly 400 adults and children with Aniridia or other iris defects found that the device was safe and effective, with 70% of participants reporting significant decreases in light sensitivity and glare, as well as improvement in health-related quality of life. The study also found low rates of adverse side effects. As per FDA, the artificial iris is contradicted in patients who have uncontrolled or severe chronic inflammation, abnormally small size of eyes, untreated chronic glaucoma, cataract caused by the rubella virus, abnormal blood vessels on the iris, inner eye infections and certain kind of damaged blood vessels in the retina. Also, pregnant women cannot be its takers.

Sonskusale has recently developed a first of its kind ‘sense-then-respond’ smart bandage which uses sensors to detect subtle biomarkers that signal wound healing. The device has a microprocessor to read the data captured by the sensors, communicate with the mobile and also can direct the bandage to release medication if it is needed. It takes into account things like sensing whether the wound is getting enough oxygen, right pH level is there, what is the temperature near the wound and is there any inflammation? All this information is communicated to a central processor where the doctor already has programmed drug release for antibiotic or growth factor to improve healing. Such a bandage has applications in bed sores, burns and surgical wounds with the advantage of reducing complications from infections and reducing the number of amputations. The key is the timely application of the bandage to the wound site to fasten healing. https://www.digitaltrends.com/cooltech/ smart-bandage

It used to be a routine practice in India to massage infants and give them a sunbath. But changing lifestyles in recent decades have confined kids to indoors. This has led to the emergence of Vitamin D deficiency as a major problem among infants.

A study from Delhi has suggested an estimate for the minimum duration for which infants need to be exposed to sunlight every day to achieve sufficient levels of vitamin-D in their body.

Doctors at the University College of Medical Sciences, New Delhi enrolled 100 infants and asked their mothers to record the time, duration, and the body area exposed during sunbath every day starting 6 weeks of age.

The study found that an exposure of as low as about 30 minutes of sunlight per week with about 40% of the child’s body exposed to the sunlight can help achieve adequate vitamin-D status at 6 months of age. The researchers also found that the ideal time to give the sunbath is between 10 AM and 3 PM.

“This small intervention can help improve levels of vitamin D in infants without the need for supplements or formula feed. And can possibly benefit more than 16 million babies born in the country every year,” Dheeraj Shah, professor of pediatrics at the University College of Medicine Sciences, New Delhi, who is a co-author of the study.

The study took into account factors like the skin color of infants and season of sunbath. About 90% of the mothers in the study were themselves vitamin D deficient. Mothers who were on vitamin D supplements were excluded so that the study results remained unaffected. The journal Indian Pediatrics publishes the study.

“We are planning further studies with a larger group of infants to test it as an intervention therapy”, said Piyush Gupta, corresponding author of the paper. The skin synthesis with Vitamin D when ultraviolet rays from the sun convert a molecule,7-dehydrocholesterol, on the skin into an inactive form of the vitamin. This enters the blood circulation and converts to its active form in the liver and kidney.

Several studies over the years have shown that more than 50% of the Indian population is vitamin D deficient. This is a matter of concern, especially for infants. Because Vitamin D is important for calcium absorption, normal growth and development, and maintenance of bone health and hair follicles. Insufficient vitamin D levels cause lethargy, irritability, and a predisposition to infections. Extreme Deficiency of vitamin D can cause rickets which results in malformation of leg bones.

Umesh Kapil, professor of Gastroenterology at the AIIMS, New Delhi, felt that the proposed strategy may not be feasible in winters when the temperature remains low. While agreeing with Kapil, Shah said, “We have reported that for the winter months. If the child is fully clothed with only face and hands are exposed, the required sunbath is calculated to be two hours per week or approximately 17 minutes per day.”

Ramesh Agarwal, professor of Pediatrics AIIMS New Delhi, suggested that randomized control trials must be done to test this intervention as a therapy. He also highlights the need to evaluate the safety of sun exposure in relation to hyperthermia, burns and skin cancer in infants.

Between cravings and morning sickness, eating well during pregnancy may not always be a piece of cake. However, prenatal nutrition is a serious business. It is necessary for you and your baby’s growth, brain development, reduced risk of birth defects and a strong immune system. Since it is not how much, but what you eat that makes the difference, here is how the healthy pregnancy diet can help. During pregnancy, the body requires extra calorie, protein, vitamins, mineral in order to support the baby’s growth and to allow for changes in mothers body.

Important considerations in pregnancy

The extra food you eat should not be just empty calories, it should provide the nutrients your growing baby needs. For example, calcium help makes bones and teeth strong, while you are pregnant; you still need calcium for your body plus extra calcium for the developing baby. Similarity, you require the all-essential nutrients than you did before become pregnant.

Here are some of the most common nutrients you need and the foods that contain it:

Food craving during pregnancy

Food craving during pregnancy is normal. Although there is no widely accepted explanation for food cravings, almost two thirds of all pregnant women have it. If you develop sudden urge for a certain food, go and indulge your craving, it provides energy or an essential nutrient. However, if your craving persists and prevents you form getting other essential nutrients in your diet; try to create more of a balance in your daily diet.

During pregnancy your tastes for certain foods may change, you may suddenly dislike foods you were fond of before your pregnancy. In addition, during pregnancy, some women feel strong urges to eat non-food items such as ice, laundry starch, dirt, chalks, ashes, paint chips, clay.

This is called pica and may be associated with a mineral deficiency like anaemia. Do not give in to theses non-food cravings; they can be harmful to both you and your baby.

• Never eat non-food items like clay , starch, coffee grounds
• Avoid alcohol during pregnancy; alcohol has been linked to premature delivery, mental retardation, birth defects and low birth weight babies
• Do not eat shark, swordfish, king mackerel, or tile fish (also called white snapper), because they contain high levels of mercury.

Unveiling myths with reality

Pregnancy produces a host of physiological and emotional changes in women. Advice seems to pour in from all sources, well-meaning friends, family member, and colleague at work and even perfect strangers tend to comment on a pregnant woman’s health. In the face of often-contradictory advice, especially concerning ones diet, it may be hard for a mum to achieve a comfortable and healthy balance. Eating a healthy diet is one of the most important things any pregnant women can do to enhance her health as well as the health of her baby. Here are some nutrition myths followed by information to get you and your baby off to a good start.

Myth#1: You are eating for two

In reality, you are eating for one plus one very small being. Most women only need to consume an extra 300 calories per day. Far from being a tome to overindulge, pregnancy is a time to eat sensibly and healthily. The suggested weight gain during the pregnancy is approximately 24-35 pounds (10-15 kg). However, this weight may vary depending upon each mother-to-be, pregnancy weight and the number of babies she is carrying. An under weight woman may gain up to 40 pounds, while an overweight woman may be asked to gain just 15 pounds, your doctor will specify an appropriate weight gain for your height and body build.

Myth#2: Eat low carbohydrate, high protein diet

In reality, weight loss diet, high protein – low carbohydrate, is not recommended during pregnancy. According to the daily food guide pyramid, grains, fruits and vegetables should compromise more of your diet than the meat groups. In fact, 2-3 servings of protein are required each day for pregnant women. This is easily achieved by consuming an egg/ 2-3 oz of poultry or meat/ ½ cup of dried beans or peas in the course of one day. Extra calories consumed during pregnancy should be evenly distributed among five major food groups.

Myth#3: Pregnancy induced high blood pressure caused by too much salt

In reality, pregnancy induced high blood pressure is due to a variety of physiological changes in the body during pregnancy and is therefore not treated the same way as hypertension in non pregnant adults. Moderation is the key. While overindulging in processed foods, typically containing too much salt would be unhealthy, eliminating salt and consuming no salt or low sodium specific food will be inappropriate. Aim to get 6 grams of sodium as a table salt in your foods every day.

Myth#5: It is better to rely on vitamins rather than ones diet during pregnancy

In reality, Vitamin should never be a substitute for a healthy diet, while most obstetrician and midwives will prescribe a multivitamin for their patients, these vitamins are intended to supplement and not replace a sensible diet. The best sources of vitamin and minerals may be found in their naturally state as they are better absorbed and are accompanied by their other nutrients such as protein or fiber.

Myth#4: Now that you are pregnant, it is a good time to start thinking about folic acid and B complex vitamins

In reality, Folic acid is particularly important in the first days and weeks of fatal formation, for this reason it is recommended that women take multivitamin before becoming pregnant to ensure they have adequate folic acid during the early development of the baby’s brain and spina bifida. Optimal folic acid consumption should continue through out pregnancy and in every woman’s diet during the childbearing years. Beside vitamins, excellent sources of B complex vitamin include orange juice, beans, citrus fruit, dark green leafy vegetables, nuts and whole grains.

Repeatedly, it has been emphasized that reduced nutritional intake has a depressing effect on physical development. Malnutrition during the period of rapid brain growth will lead to some neurology impairment. Past studies of nutrition in women during pregnancy have shown a definite relationship of the diet of the mother and the condition of baby at birth.

To protect your self and your precious baby from any possible complications review your lifestyle and try to change any unpreferable behaviour related to your diet toward the better.
