<p>हमसे पहले की नर्सिंग नेतृत्व की महिलाएँ साहस, सशक्तिकरण और स्त्रियों एवं नर्सों की स्थिति को ऊँचा उठाने पर केंद्रित थीं। हालांकि, यह पूरी तरह समावेशी नहीं था। उस समय नर्सिंग एक अधीनस्थ पेशा था – नर्सें डॉक्टरों के निर्देशों का पालन करती थीं और उनके पास बहुत कम स्वायत्तता थी।</p>



<p>फिर भी, नैदानिक कार्य उत्कृष्ट था। भले ही यह साक्ष्य-आधारित न हो, नर्सें अपनी परिस्थितियों में अपनी पूरी क्षमता के साथ रोगियों की देखभाल करती थीं। मैट्रन बहुत ही शक्तिशाली और कई स्तरों पर अधिनायकवादी होती थीं। अनुशासन नर्सिंग पेशे में बने रहने की मूल शर्त था।</p>

<li>केवल 3 महीने की ट्रेनिंग के बाद प्रारंभिक परीक्षा होती थी। असफल होने वालों को तुरंत निकाल दिया जाता था। यह “खराब सेबों” को जल्दी पहचानने का तरीका था।</li>
</ul>



<p>यह सब भले ही आज पुराना या सख्त लगे, लेकिन इसने नर्सिंग पेशे की मजबूत नींव रखी। नर्सें अपनी देखभाल और करुणा के लिए जानी जाती थीं। यही वजह है कि उन्हें <em>“लेडीज़ विद द लैम्प”</em> कहा गया।</p>



<h3 class="wp-block-heading">नर्सिंग : आज</h3>



<p>आज महिलाएँ स्वतंत्र हैं, वरिष्ठ भूमिकाओं में हैं और नेतृत्व कर रही हैं। लेकिन समाज में क्या उन्हें पूरी तरह स्वीकार किया गया है? क्या हमें वास्तव में लैंगिक समानता मिली है?</p>



<p>आज का परिदृश्य बदल चुका है:</p>



<ul class="wp-block-list">
<li>लोग और समुदाय मूल्य और गुणवत्ता की अपेक्षा करते हैं और असंतोष सार्वजनिक रूप से व्यक्त करते हैं।</li>



<li>नर्सिंग गंभीर चुनौतियों का सामना कर रही है।</li>

<li>महिलाएँ और ऊँचे पदों पर जाएँगी, लेकिन प्रश्न रहेगा – क्या महिलाएँ कार्यस्थल पर एक-दूसरे का समर्थन करेंगी?</li>



<li>आने वाली पीढ़ी कार्य-जीवन संतुलन और लचीली कार्य परिस्थितियाँ माँगेगी।</li>



<li>वर्चुअल ऑफिस और डिजिटल प्रणालियाँ सामान्य होंगी।</li>



<li>नर्सिंग नेतृत्व की शैली और अपेक्षाएँ बदलेंगी। युवा नर्सें अधिक मुखर होंगी और परिवर्तन की गति तेज चाहेंगी।</li>

<li>रोगी-नर्स संबंध बदलेंगे, रोगी अधिक जानकारी और फॉलो-अप की माँग करेंगे।</li>



<li>परिवार देखभाल में अधिक शामिल होंगे।</li>



<li>संस्थागत देखभाल केवल तीव्र देखभाल तक सीमित होगी, और मरीजों की रुकने की अवधि घटेगी।</li>



<li><strong>होम केयर</strong> की माँग बढ़ेगी।</li>
</ul>

<p>Most cases of astigmatism, which is blurred vision caused by an irregularly shaped cornea and near sightedness, can be corrected with laser eye surgery. The procedure done here requires removal of some of the corneal tissue, often leaving patients with lingering dry eyes. So STAAR Surgical which is headquartered in Monrovia, California, USA and is a leading manufacturer of ophthalmic surgical products wherein designs, develops and sells implantable eye lenses, took a note of this scenario and recently launched EVO visian implantable collamer lenses. </p>

<p>These EVO intraocular lenses provide an alternative with a minimally invasive new way to correct or reduce both conditions of nearsightedness and astigmatism. The procedure takes almost an hour in which a flexible collagen-containing lens is implanted between the iris and natural lens. Though the implant can sit in the eye permanently but still can be plucked out by an ophthalmologist if needed. The USP of this product is that it combines the perks of contacts and laser surgery.</p>



<p>As per the published results of the clinical trials, nearly 88% of patients reported 20/20 or better and almost all patients achieved 20/32 or better distance vision after six months. An added feature of the product is also that the lenses block some ultraviolet rays for added protection.</p>

<p>Hot flash due to menopause is a sudden feeling of warmth, usually very intense over face, neck and chest, and profuse sweating. More than 50% of menopausal women experience hot flashes which are very uncomfortable and can last for years with an average of seven years. Though Hormone Therapy is available for treating it and is also effective and safe when used appropriately but it does involve risk and also all patients are not suitable candidates for it.</p>



<p>Keeping this in mind, a nonhormonal intervention called NK3R antagonists has come up as a viable and effective alternative to hormone therapy. This group of drugs disrupts a signaling pathway in the brain that is implicated in the development of hot flashes. In clinical trials these nonhormonal drugs have shown positivity for relieving moderate to sever hot flashes as effectively as hormones. Still more studies need to be done to completely understand the safety and effectiveness of these non- hormonal treatments for patient safety. But introduction of NK3R clearly shows that the next generation of nonhormonal treatments is on the horizon.</p>

<p><strong>In 2020,</strong> an estimated 375,304 people worldwide died from prostate cancer, making it the second leading cause of cancer death in men in the United States. Every year nearly 200,000 Americans are diagnosed with prostate cancer, making it the most commonly diagnosed cancer among the men in America. Early detection and successful imaging are very crucial for localization of the tumour, staging of the disease and detection of any recurrences. A potential biomarker of the disease is the prostate-specific membrane antigen (PSMA) which is found in high levels on the surface of prostate cancer cells.</p>

<p>PSMA PET uses a radioactive tracer to locate and attach to PSMA proteins which makes them visible by PET imaging. So when this approach is used in conjunction with MRI or CT scans, it can visualize where prostate cancer cells reside. This technology got approval from FDA after it showed in its phase 3 trials, substantial increase in accuracy for detecting prostate cancer metastases compared to conventional imaging with bone and CT scans.</p>

<p>Open AI’s ChatGPT was launched for the public in November 2022 and since then it has been a hot topic amongst healthcare professionals WORLDWIDE whether ChatGPT is ready for healthcare? These discussions inspired a team of doctors led by Dr. John Myers, vice chief of Innovation in the UC-San Diego School of Medicine Division of Infectious Disease and Global Public Health to see if chatbot could accurately and empathetically respond to patient queries related to their real health issues. In this recent study which is published in the April 2023 issue of JAMA Internal Medicine, scientists have explored how well ChatGPT can answer health-related questions from real patients and their result shows that ChatGPT outperformed human doctors, especially on ground of empathy when assessed by a panel of medical experts.</p>

<p>To carry out the study, the team randomly selected 195 question and answer exchanges posted last October on Reditt’s AskDocs which has a community of 481,000 members where experienced doctors respond to publicly asked medical questions. Then the same queries were posed to ChatGPT version 3.5 in December 2022. The answers were collected from the chatbot and presented to a panel of healthcare professionals to evaluate both the doctors’ and AI’s answers to the questions. To ensure that raters did not know which answers were from AI and which ones were from doctors, responses were randomly ordered and labelled as Response1 and Response 2 to blind evaluators to the identity to the author. On a scale of 5, the answers were rated for empathy and quality.</p>



<p>On tabulation of the ratings, the results were highly in favour of ChatGPT over doctors. The evaluators preferred the AI response 78.6% of the time, giving ChatGPT answers an average rating of 4.1/5 for quality, corresponding to ‘Good’. Doctors’ answers were given an average rating of 3.26 corresponding to ‘Acceptable’. For the empathy component, evaluators rated ChatGPT answers as ‘empathetic’ or ‘very empathetic’ 9.8 times more often than they did doctors’ responses. Bedside manners were found to be better with ChatGPT. It is to be noted here that ChatGPT gave lengthy responses with a use of average 211 words whereas doctors’ responses had a usage of average 52 words. But this can be attributed to the fact that doctors’ responses were drawn from a casual, anonymous online forum and not a professional setting so their answers to questions may not have been very thoughtfully worded as they could have been had they seen the patients’ in person. This above-mentioned point can be considered as a significant limitation of this study.</p>



<p>This clearly shows that ChatGPT cannot claim a conclusive victory over trained and experienced doctors. However, it clearly suggests that ChatGPT could satisfactorily assist medical professionals in responding to patient queries by drafting a message based on the query for doctors or support staff to edit. Conclusively one can say that while AI won’t replace your doctor but doctored working together with technologies like ChatGPT may revolutionise medicine and definitely AI-augmented care is the future of medicine.</p>

<p>Glioblastoma is a fast and aggressively growing tumour in the brain and spinal cord. The average life expectancy post diagnosis is 14-16 months, just about 1% of patients survive at least 10 years. This form of tumour does not have any cure but treatments might slow cancer growth and reduce symptoms. Despite the ongoing research being done to understand this tumour, therapeutic options remain very limited and expected survival rate of less than 2 years post initial diagnosis. Keeping in mind these facts about this deadly tumour, researchers at IIT Madras, India have developed a machine learning-based computational tool named ‘GBMDriver’ (GlioBlastoma Mutiforme Drivers)for better detection of cancer-causing tumours (both driver mutations and passenger mutations) in the spinal cord and brain.</p>

<p>The research team has successfully identified the important amino acid features for identifying cancer-causing mutations and achieved the highest accuracy for differentiating between driver and neutral mutations. The team hopes that this tool will aid in prioritizing driver mutations in Glioblastoma and assist in identifying potential therapeutic targets to thus help in developing drug design strategies. Presently this tool is publicly available online.</p>

<p>Researchers at Johns Hopkins University, USA has recently developed an electronic skin which is made of fabric and rubber laced with sensors to mimic nerve endings and it recreates a sense of touch and pain by sensing stimuli and relaying the impulses back to the peripheral nerves. This e-dermis can be integrated into robotics and be used to create prosthetic limbs allowing amputees to regain sensation.</p>



<p>The inspiration for making such a product comes from what is happening in human biology, with receptors for both touch and pain. The newly made sensor goes over the fingertips of a prosthetic hand and acts like one’s own skin would. Prosthetic hand which is already available in the market can be fitted with an e-dermis which then can tell the wearer whether he or she is picking up something that is either round in shape or has sharp edges. The e-dermis also enables its user to sense a continuous spectrum of tactile perceptions ranging from light touch to painful stimulus.</p>

<p>The ’neuromorphic model’ prepared by the researchers mimics the pain and touch receptors of the human nervous system, enabling the e-skin to electronically encode sensations just as the receptors in the skin would. The team determined that the test subject was able to perceive these sensations in his phantom hand by tracking his brain activity via electroencephalography. The e-dermis output was then connected to the volunteer by using a non-invasive method known as trans-cutaneous electrical nerve stimulation (TENS). It was also determined that the user and the prosthesis were able to experience a natural and reflexive reaction to both pain while touching a pointed object and non-pain when he touched a round object.</p>



<p>The USP of this e-skin is that it not only aids its user to sense touch but also pain and it can be used with an already existing prostheses available in the market currently.&nbsp; The design team kept it in mind that protective sense of touch is lacking in the prostheses which are currently available to the amputees. The ability to detect pain could be useful not only in prosthetic hands but also lower limb prostheses, alerting the user of potential damage to the device. The e-dermis literally brings the phantom limb come to life by stimulating the peripheral nerves in the arm in a non-invasive way, through the skin.</p>



<p>The testing also reveals that e-dermis is not sensitive to temperature. The team thinks that this e-dermis technology could be used to make robotic systems more human which could also be extended to space suits and astronaut gloves.</p>
