	<p><strong>World Health Day</strong></p>
<p style="text-align: justify !important;">April 7th marks the birth anniversary of the World Health Organisation (WHO) and along with drives worldwide attention by spreading the message of the importance of global health each year.</p>
<p style="text-align: justify !important;">Ranging from imperative issues such as food safety, diabetes, depressions, etc., this year the World Health Day embarks the mission of people-centered care irrespective of their financial hardships. The 2019 theme envisions the Universal Health Coverage (UHC), i.e., health for all. There are millions of people who have no access at all to healthcare. Majority of the populations are forced to choose between healthcare and other daily needs such as food, clothing, and shelter. In India, a lot of efforts need to be put in to ensure adequate medicines, affordable health policies, and financing strategies to enable everyone to get access to equal healthcare. “Universal” in UHC means “for all”, without discrimination, leaving no one behind, i.e. providing healthcare facilities which are</p>

	<p><strong>Where India Stands &#8211; The New &#8220;Health-Y&#8221; Politics</strong></p>
<p style="text-align: justify !important;">The much-awaited general election will be held in India in April 2019 when the Indian voters will go to the polls. It is anticipated that whichever government comes into play will surely set a course for newer and better <a href="https://innohealthmagazine.comnewscope/medical-errors-india/">health policy</a>.</p>
<p style="text-align: justify !important;">Recent researches reveal an alarmingly rapid epidemiological transition. The occurrence of diseases such as cardiovascular diseases, diabetes, and cancer are rapidly on a rise. For instance, since 1990, the number of Indians with diabetes has increased from 26 million to approximately 65 million to date. The incidence of all cancers has increased by 28% between 1990 and 2016, with new cases of cancer emerging each year. India not only seems terribly engulfed in the swirling epidemic of non-communicable diseases, but it is also in the grip of a mental health emergency. The rate of global suicide deaths is speculated to be 1.1 times higher among women and 1.4 times higher among men than global averages in 2016.</p>
<p style="text-align: justify !important;">It would be unfair if we highlight only the negative side of the picture. After many years of neglect, the Government of India has finally recognized and considered the perils of public discontent relating to health. Prime Minister Narendra Modi has initiated a new programme called Ayushman Bharat, which along with focusing on preventive health, emphasizes affordable health.</p>
<p style="text-align: justify !important;"><a href="https://innohealthmagazine.comnewscope/ayushman-bharat/">Ayushman Bharat</a> majorly comprises two new flagship programmes. First, the creation of 1.5 lakh health and wellness centers across the whole country. These centers will primarily be responsible for providing primary healthcare facilities to provide health coverage for all. The second includes the National Health Protection Mission (NHPM)—a system of health insurance that intends to cover 500 million people. It covers secondary and tertiary care for the poorest and most vulnerable strata of society. These twin programmes together must help India improve access to quality health services and reduce out-of-pocket health expenditures.</p>
<p style="text-align: justify !important;">Modi is the first Indian Prime Minister who has grasped the importance of health for Indian citizens and has prioritized universal health coverage as part of his political platform. “Modicare” as it is sometimes addressed to, can become a decisive issue in the upcoming general elections.</p>

	<p><strong>Way Forward for a Healthy Nation</strong><br />
Does Universal health Coverage still seem a distant goal in India!</p>
<p style="text-align: justify !important;">The scant funding from the Central and state governments seems the root cause and the major hindrance in achieving UHC. The Central Government has recently announced a massive expansion of healthcare facilities.</p>
<p style="text-align: justify !important;">Through its ambitious National Health Protection Scheme (NHPS), the Government of India provides a framework for universal health coverage in which it prioritizes healthcare delivery and its easy access. By reducing out-of-pocket payments, expanding service coverage, improving quality of services, and ultimately advancing UHC, countries and in particular, India with schemes like NHPS in place can transform the health of their people and bring drastic changes in society. As stated by WHO Director-General: “No one should have to choose between death and financial hardship. No one should have to choose between buying medicine and buying food.” UHC indeed is a way to go ahead.</p>

	<p><strong>Q. What is the Swedish interest in India and your priority topics for the Indian health sector?</strong></p>
<p style="text-align: justify !important;">We have a very fruitful bilateral collaboration with India in the health sector, beginning with our development cooperation in areas such as mother- and child health, midwifery, and work against communicable diseases. Cooperation has now increased to mutually beneficial partnerships. For the past ten years, we have had a Memorandum of Understanding on Health between our countries and next year we are going to celebrate this with an India-Swedish Health Year.</p>
<p style="text-align: justify !important;">Many collaborations of mutual interests are ongoing and being planned that focus in a wide range of areas like antimicrobial resistance, elderly care, pharmaceuticals, non-communicable diseases and digital innovations including artificial intelligence, where the emphasis will be on knowledge transfer, capacity building and skills training.</p>

<p style="text-align: justify !important;">We are in the process of putting out calls and setting up incubators which will encourage close collaboration between the innovators in Sweden and India. Solutions are only sustainable if both sides contribute and share a strong sense of ownership. Progress has already been made in laying the groundwork for such partnerships within the sectors of clean energy and smart cities, and we hope to launch initiatives targeted to healthcare during the coming year. We hope to enhance our collaborations in new areas with new stakeholders like the new AIIMS that have come up in some states. We also hope to have high-level visits from both sides to enhance policy dialogues and bilateral relations. Some of the collaborative partners are also developing new plans to strengthen their cooperation.</p>

	<p><strong>Q. What can Indian healthcare businesses gain from Swedish healthcare businesses?</strong></p>
<p style="text-align: justify !important;">Any suggestions on possible partnerships? It is not so much what the Indian healthcare system can gain, but rather, how can we join forces to overcome common challenges. Swedish businesses have been in India for many years. Within healthcare, we have AstraZeneca, who recently celebrated 40 years in India, and Getinge who has been investing in the Indian ecosystem for many years. Their focus, like many others, has been in building the capacity of their employees and the users of their products to increase efficiency. The businesses also bring with them lessons from Sweden so that we can help India leapfrog outdated technology. Sweden also ranks high in most innovation indexes – it can be argued that our relatively small size makes collaborations outside traditional silos necessary in order to succeed. Public actors, private companies, and institutions often work together in finding innovative solutions to serious healthcare challenges. We would like to work with India to build spaces for similar ecosystems across India.</p>

	<p><strong>Q. What are the common areas for healthcare research and ongoing partnerships for both sides to collaborate?</strong></p>
<p style="text-align: justify !important;">Under the MoU, there are extensive collaborations between public authorities and universities from both countries. For instance, the Swedish Public Health Agency, Medical Product Agency and Forte (the Swedish Research Council on Health, Working Life and Welfare) have an ongoing collaboration with their Indian counterparts. There are ongoing discussions on extending collaborations with new players like our National Board of Health and Welfare and E-Health Authority as well as between our medical universities such as Karolinska Institute, Uppsala University and the different AIIMS.</p>
<p style="text-align: justify !important;">Through these collaborations, we hope to demonstrate best practices and technologies. We have research calls in different areas for example with EU in big data and artificial intelligence for monitoring health status and quality of life after cancer treatment, under JPIAMR call on diagnostics and surveillance 2019.</p>
<p style="text-align: justify !important;">Innovation-the UNNATI way is an India-Sweden Innovators for strategic business development to address global challenges. It is an industry-supported initiative coordinated by Business Sweden in collaboration with AGNIi-Invest India and ATAL Innovation Mission, NITI Aayog.</p>

	<p><strong>Q. Anti-Microbial Resistance is an important issue right now. How do you propose the Swedish Govt. support in this area?</strong></p>
<p style="text-align: justify !important;">The Swedish Public Health Agency and the Indian National Centre for Disease Control have an ongoing collaboration with a focus on antimicrobial resistance. Additionally, the Indian Ministry of Health has proposed that Sweden collaborate closely with a new-AIIMS and its associated Health and Wellness Centres. AIIMS-Jodhpur has been suggested to us and we are currently considering the scope of such collaboration.</p>

<p style="text-align: justify !important;">Do you see any collaboration in this field with Indian providers? This is a hot topic all over the world right now. I know India is well ahead in AI technology while Sweden has more long-term experience in extracting relevant information from vast amounts of data. Our patient registries have enabled us to have one of the highest cancer survival rates in the world and that is only one example. I believe we will see some collaborations in the coming year, especially as you are arranging a delegation to Sweden addressing this exact topic in May next year. I will follow this progress closely.</p>

	<p style="text-align: justify !important;">Women’s health in India has emerged as a major nutritional challenge with the country wrestling largest number of anemic women in the world and the other having to tackle diseases related with obesity –that is on the rise, warns the latest Global Nutrition Report, 2017. It says there is malnutrition among adults globally.A total of 614 million women aged between 15–49 years were affected by anemia. India had the largest number of women impacted, followed by China, Pakistan, Nigeria and Indonesia.In India and Pakistan, more than half of all women of reproductive age have anemia.</p>

	<p style="text-align: justify !important;">Overweight and obesity are increasing in almost every country and are a real concern for many low and middle income countries, not just high-income ones.The problem affects more women than men in all the world’s regions, reflecting a wider global gender disparity.</p>
<p style="text-align: justify !important;">Hypertension is most common (28%) among African women and European men, and lowest (11%) among North American women. A quarter of Asian and Latin American men suffered from raised blood pressure in 2015. While more women worldwide are affected by obesity, the case for diabetes and hypertension is mixed. There is more diabetes among men than women in Asia, Europe, Northern America and Oceania, and more hypertension among men than women in all regions except Africa.</p>
<p style="text-align: justify !important;">The world now faces a serious nutrition- related challenge, whether stemming from under nutrition or obesity, states Global Nutrition Report 2017.</p>
<p style="text-align: justify !important;">The report found the vast majority (88%) of countries studied face a serious burden of two or three of these forms of malnutrition. It highlights the damaging impact this burden is having on broader global development efforts.</p>
<p style="text-align: justify !important;">“The world can’t afford not to act on nutrition or we risk putting the brakes on human development as a whole,” said Corinna Hawkes, Co-Chair of the Global Nutrition Report’s Independent Expert Group and Director of the Centre for Food Policy at City, University London. “We will not achieve any of the Global Goals for Sustainable Development (SDGs) by the 2030 deadline unless there is a critical step change in our response to malnutrition in all its forms. Equally, we need action throughout the goals to tackle the many causes of malnutrition.”</p>
<p style="text-align: justify !important;">The Report calls for nutrition to be placed at the heart of efforts to end poverty, fight disease, raise educational standards and tackle climate change.</p>
<p style="text-align: justify !important;">&#8220;We know that a well-nourished child is one-third more likely to escape poverty,” said Jessica Fanzo, Bloomberg Distinguished Professor of Global Food and Agriculture Policy Ethics at Johns Hopkins University and Global Nutrition Report CoChair.</p>

	<p>In all 140 countries studied, the report found ‘significant burdens’ of three important forms of malnutrition used as an indicator of broader trends:<br />

	<p style="text-align: justify !important;">“They will learn better in school, be healthier and grow into productive contributors to their economies. Good nutrition provides the brainpower, the ‘grey matter infrastructure’ to build the economies of the future.”</p>
<p style="text-align: justify !important;">Rates of undernutrition in children are decreasing, the report said, with recent gains in some countries. But global progress is not fast enough to meet internationally agreed nutrition goals, including the Sustainable Development Goals (SDG) target 2.2 to end all forms of malnutrition by 2030.</p>
<p style="text-align: justify !important;">EmornUdomkesmalee, Co-Chair of the Global Nutrition Report’s Independent Expert Group and Senior Advisor, Institute of Nutrition, Mahidol University, Thailand, said, “It’s not just about more money – although that is important &#8211; it’s also about breaking down silos and addressing malnutrition in a more joined-up way alongside all the other drivers of development. There’s a powerful multiplier effect here that we have to harness.”</p>
<p style="text-align: justify !important;">The report found that overweight and obesity are on the rise in almost every country.With 2 billion of the world’s 7 billion people are now overweight or obese and a less than 1 per cent chance of meeting the global target of halting the rise in obesity and diabetes by 2025.</p>
<p style="text-align: justify !important;">Rising rates of anemiaamong women of reproductive age are also cited as a concern with almost one in three women affected worldwide and no country on track to meet global targets. “Historically, maternal anemia and child undernutrition have been separate problems to obesity and noncommunicable diseases,” said MsFanzo. “The reality is they are intimately connected and driven by inequalities everywhere in the world. That’s why governments and their partners need to tackle them holistically, not as distinct problems.” Donor funding for nutrition rose by just two per cent in 2015, to US$867 million, representing a slight fall in the overall percentage of global aid. The report says funding needs to be ‘turbo charged’ and calls for a tripling of global investments in nutrition, to $70bn for over next 10 years to tackle childhood stunting, wasting and anemia and to increase breastfeeding rates. Crucially, donors are only spending 0.01 per cent of official development assistance on diet related Non-Communicable Diseases, a ‘disturbingly low’ level.</p>
<p style="text-align: justify !important;">Pledges to invest in nutrition must be ‘concrete’ and ‘acted upon’, not ‘empty rhetoric’, the report said. Of the 203 commitments made at the Nutrition for Growth Summit in 2013 those most likely to be classified as ‘on course’ are the UN agencies’ at 86 per cent, followed by ‘other organisations’ at 75 per cent and NGO policy commitments at 73 per cent.</p>
<p style="text-align: justify !important;">The report found there is a critical need for better data on nutrition &#8211; many countries don’t have enough data to track the nutrition targets they signed up to and to identify who is being left behind.</p>

	<p style="text-align: justify !important;">Non-communicable diseases (NCDs) represent a major peril to the economic growth and development and human health of any nation. The economic loss to India is pegged at $4.58 trillion before 2030 due to NCDs and mental health. The vicious cycle of high out-of-pocket expenditure on health, poor economic growth, workforce productivity loss and rising disease burden are intricately linked to the rising menace of NCDs.</p>
<p style="text-align: justify !important;">The South Asian region─with a population of more than 1.7 billion─is experiencing a marked demographic transition─characterised by declining birth and death rates and an increasingly aging population. This increase in longevity has also been accompanied by a rise in the prevalence of NCDslike diabetes (78 million cases in 2015). India has the second highest number of diabetic patients in the world, at about 69 million, after China, and this figure is likely to reach 140 million by 2040 and almost half of them remain un-diagnosed. A recent study showed that seven out of ten diabetics in India &#8211; especially those in the 20-24 age group &#8211; do not take enough steps to control their blood sugar level &#8211; even after diagnosis.</p>

	<p style="text-align: justify !important;">The Economic Impact of Diabetes, As per a study conducted by the Indian Institute of Public Health, the number of diabetes patients in the country is likely to go up to 120 million in next 20 years as against the current 70 million. On an average a diabetic patient spends about INR 25,000 annually to manage the disease and its associated complications. Beyond placing a financial strain on the patient due to treatment expenditures, poor management of diabetes significantly impacts the patients’ quality of life: it may lead to loss of productivity due to multiple hospital visits, elevating rates of absenteeism, diminishing the energy and focus of a productive workforce, and depleting critical workplace skills ultimately leading to reduced mobility and span of life. Diabetes related complications like kidney failure, nerve damage along with obesity, dyslipidemia and hypertension make diabetes management quite complex for clinicians.</p>
<p style="text-align: justify !important;">In this context, it becomes important that healthcare professionals have access to more educational resources to remain updated about how to tackle the diabetes epidemic. But due to scarcity of time and high patient load in South Asian healthcare facilities, medical practitioners find it very difficult to update their knowledge. Remaining updated can help clinicians offer better diabetes management options to their patients.</p>
<p style="text-align: justify !important;"><strong>A Possible Solution:</strong> E-Learning Courses e-Learning programs can help clinicians in acquiring skills required to address the growing complex management of diseases. E-learning courses can be tailored to fit the clinicians schedule, delivering content right at the moment when it is demanded by the doctor. Interactive e-learning programs also offer peer to peer learning, thus enabling clinicians to discuss learning from key cases. Those e-learning programs which also offer the opportunity of contact classes, offer a richer and immersive learning experience.</p>
<p style="text-align: justify !important;">An effective e-learning program will incorporate guidelines for determining the correct and appropriate blend of instructional strategies, including on-line learning, face-to-face instruction, and skill demonstrations, thus facilitating the learning of a clinician far beyond than traditional learning materials. Unlike traditional CMEs, e-learning can be designed to be self-paced, and research has indicated that e-learning reduces the learning time by at least 25-60% when compared to traditional learning, which can be an added advantage for time-constrained clinicians.</p>
<p style="text-align: justify !important;">Intelligent e-learning management systems also adapt and learn from the learners’ aptitude and change and deliver a personalized learning experience. 	<p style="text-align: justify !important;">Highlighting his concern, Biten Kathrani of Boston Scientific voiced his opinion saying, “Affordability today is very important not just for emerging markets but for the global health care environment. But in the guise of affordability and much talked about affordability, one of the critical things that we have forgotten to do in health care is to figure out challenges associated with awareness, access and adoption. So today we must look beyond affordability and not ignore awareness, access and adoption.”</p>
<p style="text-align: justify !important;">Addressing the flip side of the subject of beating around about affordability, Dr. Karthik Anantharaman of BPL Medical Technologies highlighted, “Traditionally developing medical devices is very capital intensive with years of R&amp;D that adds to the cost and makes it very unviable for the product to be of any commercial viability. The best way forward is to produce products that are of need by the doctors and not indulge in ambitious research programs that can take as long as 4-5 years of development time and just the research alone can cost a couple of millions of dollars.”</p>

	<p style="text-align: justify !important;">Highlighting the concern of the market towards the newly-launched products and services, another panelist Stephen Victor said, “Life cycle of a diagnostic device is not predictable as some competition may arrive in the market within six months of its launch, with additional facilities, extra benefits and probably at a cheaper cost. And there are other factors that can contribute to failure of the product despite it being very innovative and technically superior. For this, you need Curators for startups or need big corporates to invest in service and maintenance for new innovative products.”</p>
<p style="text-align: justify !important;">With an emphatic conclusion about the worries of many of the startup aspirants, M.V. Amaresh Kumar, who works for Abdul Kalam Institute in Andhra Pradesh, a quasi-government establishment, emphasized, “Cost of innovation is always high as it involves immense research and investment in resources with no guarantee of market share. Government establishments like the Andhra Metric Zone and Abdul Kalam Institute have come to the rescue of such innovators who need hand-holding by someone for making new products commercially viable and relieving the inventor of such mental burden. Only such a marriage between innovators and non-profit establishments can ensure innovations to be introduced at reasonable price – not necessarily at a cheap rate, but affordable.”</p>

	<p style="text-align: justify !important;">The Hyderabad-based National Institute of Nutrition (NIN) has developed the country’s first Nutrition Atlas to provide all the relevant data and information about nutrition.</p>
<p style="text-align: justify !important;">The Nutrition Atlas provides information and data on nutritional status of population groups at national and state levels, along with an overview of nutrition-related deficiencies, disorders and prevalence levels in various parts of the country along-with information on nutrients, nutrient rich foods, nutritional deficiency disorders and a host of other topics.</p>
<p style="text-align: justify !important;">The portal also includes information on nutrition rich foods and nutri-guide for various nutrients, minerals, essential amino-acids, fatty acids, dietary fibers and proteins, along with their biochemical cutoffs, recommended dietary allowances, signs and symptoms and dietary sources.</p>
<p style="text-align: justify !important;">The Atlas is interactive and displays information in the form a user may select and has been developed through a long drawn, cohesive and technical exercise by a multidisciplinary team in the bioinformatics division of NIN. It makes use of publicly available data sources like reports of National Nutrition Monitoring Bureau, National Family Health Survey, World Health Organisation and other public databases. The Dashboard acts like an information management tool, helping the user track data on under nutrition, over nutrition, overweight, obesity and communicable and noncommunicable diseases. It can also provide different time trends on each of these parameters. The development conducted the research under ICMR.</p>
