He stressed the importance of holistic approaches to healthcare that prioritize patient safety and satisfaction.</p>

<p>As digital transformation accelerates, cybersecurity emerged as a critical concern. Dr. Kousar Shah highlighted the increasing vulnerability of healthcare systems to cyberattacks. &#8220;Cybersecurity must be integrated into the clinical quality framework of hospitals,&#8221; he urged, noting that data breaches can have severe financial and reputational consequences.</p>

<p>In the latest episode of the InnoHealth Magazine podcast, host Dr. Soumya Singh sat down with Mr. Sanjay Jain, Founder and CEO of Akhil Systems Pvt. Ltd., to explore the evolving landscape of healthcare IT in India. With over three decades of experience in healthcare digitization, Mr. Jain offered rich insights into the journey of Akhil Systems, the digital transformation of hospitals, and the challenges and opportunities that lie ahead.</p>



<h3 class="wp-block-heading">A Three-Decade-Long Journey</h3>



<p>Founded in 1994, Akhil Systems emerged out of a vision to fill the glaring gaps in healthcare automation. Mr. Jain recalled joining the company as a software developer, eventually becoming its co-leader with Rekha Murthy, his business and life partner. Their commitment to improving healthcare delivery through technology has fueled the company’s success across India and in more than 10 countries globally.</p>



<p>The company’s first major milestone came in 1995 with Mulchand Hospital in North India. This was followed by key partnerships with hospitals like Yashoda and Teerath Ram. An international breakthrough came in 2001 with an implementation project in Saudi Arabia—an experience that exposed Akhil Systems to sophisticated insurance and electronic medical record (EMR) requirements, far ahead of India’s at the time.</p>



<h3 class="wp-block-heading">Turning Points in Health IT</h3>



<p>A critical turning point arrived in 2004 with an ambitious project for STS Apollo Hospital in Dhaka, Bangladesh (now Evercare). The goal was to build a paperless hospital on par with European standards. Mr. Jain proudly shared that for the last 20 years, patients at that facility haven’t carried a single paper prescription—proof of the system’s enduring success.</p>



<h3 class="wp-block-heading">Challenges of EMR Adoption</h3>



<p>Despite technological advancements, EMR adoption remains inconsistent in India. Mr. Jain emphasized that the barrier is rarely technological—it’s often behavioral. Even when hospitals are equipped with capable software, implementation falters due to inadequate planning, insufficient training, and resistance to change among medical staff. He stressed the importance of top-down commitment and continuous support to ensure successful adoption.</p>



<p>“One of the biggest lessons,” Mr. Jain said, “is that mindset matters more than the system. The same EMR works wonders in one hospital and is barely used in another.”</p>



<h3 class="wp-block-heading">Key Features for Successful Implementation</h3>



<p>Akhil Systems developed unique solutions like single-window billing and multilingual pharmacy packaging to streamline patient experiences. These innovations, supported by user-friendly apps and doctor-focused tools, highlight the importance of customizing IT systems to suit local workflows and user needs.</p>



<p>Mr. Jain offered practical advice to hospitals considering a Health Information System (HIS) purchase:</p>

<h3 class="wp-block-heading">Data Security: A Growing Concern</h3>



<p>With increasing digitization, cybersecurity has become a critical issue. Mr. Jain admitted that this area has often been neglected but is now gaining attention following recent ransomware attacks. He stressed the importance of data encryption, secure communication protocols, regular backups, and robust firewall protections—whether data is stored on-premise or in the cloud.</p>



<p>“Security should be baked into the infrastructure and application from the start,” he warned, highlighting the role of standards like HIPAA, GDPR, and India’s upcoming DPDP Act.</p>



<h3 class="wp-block-heading">The Future: AI, IoT, and Digital Ecosystems</h3>



<p>Looking ahead, Mr. Jain is optimistic. He believes the next five years will be transformative for Indian healthcare, driven by:</p>

<li>Remote patient monitoring using IoT and wearable devices<br></li>

<p>His company is preparing for this future with “Agastam,” a new cloud-native, microservices-based platform aimed at overcoming traditional EMR adoption challenges.</p>

<p>AI is revolutionizing the healthcare industry and driving it towards digital transformation. AI algorithms and robots are altering the nurse&#8217;s role and challenging the profession and the very true essence of nursing: The human connection.&nbsp;</p>



<p>AI is the ability of a machine to emulate intelligent human behaviours and represents an umbrella term for machine learning (ML), computer vision, and natural language processing (NLP) technologies.AI is already changing the patient experience, how clinicians practice medicine, and how the pharmaceutical industry operates.&nbsp;</p>



<p>In a single day or shift, nurses have to juggle a number of tasks woven with hospital accreditation standards from general patient care and monitoring to administering medications and treatments to communicating with doctors. They also have to stay on top of administrative tasks like charting patient records, interacting with insurance providers, and handling other types of paperwork. Robots and AI in healthcare facilities will augment and automate some tasks, allowing nurses more time to care for their patients.&nbsp;</p>



<p>In future, artificial intelligence-enabled robotics may act as assistants to nurses, freeing staff to dedicate in,evidence-based care and focus on human element of nursing. Nurses will likely become more patient-centric and focused as AI absorbs more of the routine work.&nbsp;</p>



<p>With the future practice of nursing in a technologically advanced AI health care settings, It is question how human nurses can preserve the very true essence of nursing? Nurses should be involved in deciding which aspects of their practice can be delegated to technology and oversee the introduction of automated technology and artificial intelligence ensuring they practice on holistic care while embracing the artificial intelligence to achieve predictable patient outcomes.</p>



<p>Since the AI in healthcare has started coming up, variety of ethical implications come along too. Healthcare decisions exclusively made by the humans in the past, and the use of smart AIs to work would raise the issues of accountability, transparency and privacy. <strong><em>Nurses know the difference between being &#8220;cared for&#8221; and &#8220;caring”, Cared for him not just his body but his soul too.</em> </strong>Over the course of a nurse’s shift, conflicts may come from differing opinions, personality clashes among health care team members and general stress with patients and family. </p>



<p>Nurses with their level of emotional intelligence can resolve the conflict while keeping up the therapeutic relationships. So, it is fully clear that AI systems would not replace nurses and clinicians on large scales, but rather would strengthen their efforts in patient care. Over the time, health care workers effectively utilize their time, towards tasks and job designs that draw on unique human skills which include emotional elements like empathy, touch, persuasions and communication. </p>



<p>Bringing the nurse leaders, nurse clinical specialists, nurse academicians and faculty, altogether is an intelligent way to keep pace with AI advancement while preserving the essence of nursing- the human element of this noble profession.</p>



<p>Given the potential of this technology for patient care and its impact on clinical providers, it is essential for nurses to have a basic understanding of AI concepts. Nurses at all levels and fields must update their knowledge adequately on AI technologies and involve to contribute in the developing stages of AI creation which directly involved in nursing functions. Providing opportunities for continuing education and research is mandatory to update knowledge, attitude and skill sets to incorporate AI in nursing teams.&nbsp;</p>



<p>Moving forward Nursing education with advanced new curriculum and nursing research will change to encompass a differentiated demand for professional nursing practice with robots in healthcare. Nursing educators in clinical as well as academic setups would be the leading faculty in teaching the new complex roles and responsibilities with the modern AI.&nbsp;</p>



<p>&nbsp;It is the time for the nurse leaders &nbsp; to be among the leaders and drivers of conversations around AI in health systems to preserve the true essence of Nursing profession.&nbsp;</p>



<p>Nurse’s eyes have seen pain, hands have touched hearts, heart has felt brokenness, feet have walked a thousand miles all for patients. The trained nurse has become a blessing to the humanity, taking a place besides the physician and the priest. Nurses can use AI to enrich the nursing practice by making sound clinical judgements. I would end up saying that artificial intelligence would need to be complementary to each other and can never take away the true essence of nursing profession.</p>

	<p style="text-align: justify !important;">The rise of the Internet of Things and Artificial Intelligence is critical to the digital transformation of modern healthcare as far as healthcare monitoring and delivery are concerned. The aim of patient and doctor communication is to create a good and caring relationship, facilitate exchange of information for accurate diagnosis and treatment, and including patients in decision making and counseling. But whether  the doctors are able to create a caring relationship or whether the patients are satisfied with the care, is the point of concern With the evolving technology, can Artificial Intelligence empower healthcare and improve patient doctor communication? There can be endless possibilities in medical imaging and diagnostics, patient care, RnD and management.</p>
<p style="text-align: justify !important;">One of the patient doctor communications medium is voice. Companies like Google and amazon are out with devices which are voice based and are betting heavily on voice based skills. As these companies are market movers, the question arises: <strong>Are we investing in making voice  one of the skills needed to help the patients?</strong></p>

	<p style="text-align: justify !important;">People with chronic conditions are often advised to follow detailed treatment plans; end-of-life care often requires many complex (and emotionally draining) decisions; options for treating certain diseases are increasing drastically. <strong>How much could health outcomes be improved with more effective communication between patients and healthcare providers?</strong></p>
<p style="text-align: justify !important;">One of the panelists brought light on how the voice is substantial to the patient doctor communication. Voice as an input is a useful tool as it gets us more data to refine our systems and algorithms. Voice as an output have some issues in the current scenario, as the synthesis is not real, it is hard to deploy the voice based system. Also there is a language barrier, as the systems are mostly deployed in English, there lacks trust and empathy. There is a vast opportunity to make these systems demographically viable. The question now arises is, if we use a chat bot or a bot service, can we have an authentication that it is a bot speaking not a human being.</p>

The pace and evolving changes in the segment of healthcare extend right across the spectrum &#8211; from the development of new treatments and medicines to the front line delivery of innovative and paramount outpatient and emergency care. Surprisingly, it is at the front line where the most amazing advancements are happening and where the advantages are registering in the biggest volumes.</p>

	<p style="text-align: justify !important;">In a recent healthcare conference, the people were unaware when I spoke about how on October 21, 2013, the former US Vice President Dick Cheney’s doctors disabled his pacemaker’s wireless capabilities to thwart possible assassination attempts. To this, everyone had an issue on accountability. It remains unanswered that who would be liable, the doctor, hospital, company which provided pacemaker or the insurance company. Amidst all this, nobody was concerned about the patient as none present there questioned it. Maybe we are awaiting a major catastrophe to happen to put our minds to work. It is our endeavor to equip the healthcare community to this aspect of digital health.</p>

A recent audit of a corporate hospital in Delhi by our team revealed that 80% of equipment is vulnerable to cyber attack, thus, risking the patients. Healthcare providers are not sensitized to issue as required hence this magazine is dedicated to Cybersecurity. 	<p style="text-align: justify !important;">Health leaders and executives globally are focused on addressing a key strategic question: “How do we activate, cultivate, manage and accelerate digital innovation within our organizations?”</p>
<p style="text-align: justify !important;">There’s a right and a wrong way to address this question and successfully ignite digital transformation. Let’s start by taking a look at the wrong way.</p>
<p style="text-align: justify !important;"><span style="color: #0073b4;"><strong>Why the traditional technology-centric approach towards digital transformation is flawed ?</strong></span><br />
Customarily, health organizations have taken a very technology-centric approach to fostering digital transformation. Here is a simplified overview of how this strategy is usually applied:<br />
<strong>• Step I:</strong> Learn about a new technology trend or tool<br />
<strong>• Step II:</strong> Decide to implement pilot projects to test it<br />
<strong>• Step III:</strong> Exemplify it for how the organization is digitizing its operations and approach<br />
While simply testing various technologies is clearly an important first step for organizations seeking to further their digital transformation efforts, this approach has the following flaws.<br />
<strong>• It promotes silo building:</strong> In many cases, the digital health innovation work is disconnected from the larger organizations, promoting the creation of innovation silos that are distant from other parts of the organization, and thereby receiving less support in due course of time.<br />
<strong>• It is difficult to scale:</strong> Pilots are by their very definition, short-term experiments. Without careful consideration of how they fit into larger strategic and tactical objectives, they can be difficult for the organization to scale over time.<br />
<strong>• Lessons and best/worst practices are often not shared broadly:</strong> Because these pilots are often being led by small teams or innovation leaders, sometimes — because they are disconnected from the larger organization — lessons from these pilots are not being shared widely or used to tackle related businesses and organizational issues.<br />
<strong>• Leadership and budgetary support is limited:</strong> Leadership may be aware of these technology-focused projects, but view them as short-term “side projects”, disconnected from the larger strategic and tactical imperatives. This lack of leadership support translates into more limited budgets for the initial and ongoing digital innovation.<br />
<strong>• It is hard to prove return on investment (ROI):</strong> Without carefully thinking about how these innovations will either reduce expenses or drive revenue (via improvements in workflows, better health outcomes, etc.) it is often difficult to determine a positive ROI for these efforts.<br />
It is clear that the technology-centric approach may deliver short-term benefits in terms of improving understanding and the ability to implement certain digital innovations. However, it has significant limitations when viewed from the context of helping health organizations develop a more viable and sustainable digital transformation strategy.<br />
Fortunately, there is a better way!</p>
<p style="text-align: justify !important;"><span style="color: #0073b4;"><strong>A superior approach: Understand and manage the four fundamental forces shaping digital innovation at health organizations</strong></span><br />
For more than 10 years, I have worked with leading health organizations (including government, pharmaceutical firms and payers) on various digital transformation-related initiatives. And, late last year, I developed a unique research initiative called the State of Digital Health Innovation. This study is designed to identify the forces that are accelerating or slowing digital initiatives at health organizations. This experience and research clearly demonstrates that while technology plays an important role in fostering digital transformation, it is far from the only factor. In fact, as illustrated in the image below, there are four.</p>

	<p>Health organizations that fail to consider, account for and manage the four fundamental forces shaping digital innovation will ultimately be less successful in their transformation efforts. These forces are:</p>
<ul>
<li><strong>Human:</strong> This involves:</li>
<li style="text-align: justify !important;">Ensuring that innovation initiatives — whether large or small — are supported by leadership. This does not just mean garnering support from the C-suite, but also ensuring that the team and functional leaders understand how these innovations can benefit them in due course of time.</li>
<li style="text-align: justify !important;">Creating opportunities for people throughout the organization to be educated about innovation — not only the ones of immediate relevance, but also other, potentially high-impact trends and technologies that are being developed almost daily. We found that a major drag on innovation is personnel who are incurious and uneducated about the evolving digital technology landscape. Going to events, and holding innovation days are not enough. Organizations must find ways to inject innovation into executives’ daily routines and workflows in easy and unobtrusive ways.</li>
<li style="text-align: justify !important;"><strong>Economic:</strong> This is about answering a fundamental question on innovations — even at the earliest stage. Can technology increase revenues or reduce health spending (via improved workflows, better health outcomes, etc.)? Failing to address this question early and often is another major barrier to successful digital transformation. If internal innovators can’t demonstrate the potential positive impact of these digital activities on the organization, they will be less likely to be funded or receive support from team, functional and organizational leadership.</li>
<li style="text-align: justify !important;"><strong>Technological:</strong> This is not just about ensuring that people within the organization have the ability to execute around one or more innovations. It is also about asking tough questions regarding whether the key stakeholders who will utilize these technologies, have the capacity to use them. There are many examples of organizations that have deployed digital solutions to patients, providers and other stakeholders; solutions that have failed because innovations were too difficult to use, people were unfamiliar with the technology and other factors. Digital transformation will be stalled if internal and external technology-related issues are not considered and addressed.</li>
<li style="text-align: justify !important;"><strong>Policy/regulatory:</strong> This area is one of the vital, but frequently overlooked, forces impacting digital transformation efforts. There are two sides to this issue:</li>

<li style="text-align: justify !important;"><strong>Internal policies and procedures:</strong> For many organizations, well-crafted policies and procedures can be the lubricant of innovation progress. If effective policies exist, executives and leaders can move forward with a clear understanding of the guardrails that shape their decisions and processes. All-to-often policies are too restrictive, but if crafted well, they can also spur innovation because people can move forward with confidence and certainty.</li>
</ul>
<p style="text-align: justify !important;">If policies don’t exist, it will be much harder to scale innovations because people will be uncertain about what will be and will not be acceptable. For example, in the United States, the lack of clear policies around how social media technologies could be used by internal stakeholders, prevented many pharmaceutical companies from utilizing social tools more effectively and widely. A major reason for the lack of policy development was limited guidance from government agencies about what was acceptable from a social media implementation perspective.</p>
<ul>
<li style="text-align: justify !important;"><strong>Legal/regulatory clarity:</strong> Another major issue impacting digital transformation across organizations is the legal and regulatory landscape, specifically, whether government is developing innovation-friendly legislation and regulations. This means that even if organizations get everything right internally, external legal and regulatory forces can have a significant impact on their success.</li>
</ul>
<p style="text-align: justify !important;">One relevant example has been the slow uptake of telemedicine technologies in South Korea. Although innovators have developed unique solutions, government has been slow to test these tools and produce legislation enabling physicians to examine patients and prescribe medications via remote technologies such as mobile and video. Failing to become educated about the shape, scope and future of the legal and regulatory landscape can doom digital acceleration efforts.</p>
<p style="text-align: justify !important;"><span style="color: #0073b4;"><strong>What is the status of health organizations in their efforts towards digital transformation and what are the common barriers to success?</strong></span></p>
<p style="text-align: justify !important;">It can be helpful for health organizations seeking acceleration of digital transformation to have a sense of how their national and international peers are doing. In addition, it can be beneficial for them to have a clear communication during the overall digital innovation process and for quickly identifying (and addressing) areas of strengths and weaknesses.</p>
<p style="text-align: justify !important;">To help, I developed and released an industry-first digital maturity framework called the Digital Health Innovation Acceleration Curve (displayed below) in mid-2015.</p>

	<p style="text-align: justify !important;">This framework organizes organizational innovation activity into three stages:<br />
<strong>• Stage I &#8211; Awareness and education:</strong> Becoming aware of and informed about new digital innovations<br />
<strong>• Stage II – Engagement and capacity boosting:</strong> Engaging in pilot projects and other implementations that help build knowledge and understanding<br />
<strong>• Stage III – Proficiency and scaling:</strong> Having the knowledge, ability and focus to scale innovations across the enterprise, use case studies, patients’ groups, therapeutic categories and more</p>
<p style="text-align: justify !important;">This innovation maturity model is at the center of our State of Digital Health Innovation research initiative. Study participants from health organizations (and their partners, e.g., startups, technology firms, consultants) rated organizational readiness and ability in each of the four areas (economics, human, policy, technological) discussed previously. Scores in each of these areas were calculated and aggregated to determine organizations’ overall position on the Digital Health Innovation Integration Curve and provide important information about their strengths and weaknesses.</p>

	<p style="text-align: justify !important;">Overall, (as illustrated above) Wave I of this study reveals that only 5% of organizations globally are at Stage III, i.e., operating at the highest level of proficiency and benefitting from a digital innovation perspective. <p style="text-align: justify !important;"><span style="color: #0073b4;"><strong>What is holding back digital innovation and transformation efforts?</strong></span><br />
We found that:<br />
<strong>•</strong> Among Stage I organizations: Limited budgets, lack of technological ability and low levels of innovation education and knowledge among executives and others are major reasons behind organizations not being at Stage II</p>
<p style="text-align: justify !important;"><strong>•</strong> Among Stage II organizations: Digital transformation efforts are hampered by limited budgets, lack of internal policies guiding innovation deployment and, in some technology areas (particularly wearable), limited technical ability and knowledge</p>
<p style="text-align: justify !important;"><span style="color: #0073b4;"><strong>The road ahead: Towards a more holistic digital transformation approach</strong></span></p>
<p style="text-align: justify !important;">Leaders and executives at health organizations struggling to ignite innovation and guide digital transformation have long understood the problem, but have had limited tools they could use to treat it successfully.<br />
What the State of Digital Health Innovation research initiative provides is a means they can use to quickly assess (by participating in the research), identify and remove roadblocks to digital transformation progress.<br />
It may be difficult to change organizational culture to move towards strategies that favor holistic rather than technology-centric digital transformation approach. However doing so is vital if we are to explore new opportunities to realize the promise of digital revolution for physicians, patients, caregivers, institutions and others in the health sector.</p>
