	<p style="text-align: justify !important;">Artificial Intelligence can help decode epileptic brains. Epilepsy is the fourth most common neurological disorder affecting nearly 65 million people worldwide. The seizures or ‘fits’ as is commonly known, arise due to unusual electrical activity in the brain and is the chief symptom of epilepsy. Neither dependent on age or gender, the onset of the seizure is unpredictable without a set pattern of frequency of occurrence or severity, often posing a challenge to the caregiver.</p>
<p><em><strong><a href="https://innohealthmagazine.comissues/one-student-commits-suicide-every-hour-india/">Did you know that one student commits suicide every hour in India?</a></strong></em></p>
<p style="text-align: justify !important;">Although epilepsy can be related to previous brain injuries or genetic factors, neurologists have found unprovoked, recurrent seizures in healthy individuals too. How and why these seizures occur remains a mystery. However, research has found that the source of seizures is within the brain. In other words, the brain itself is the generator of epilepsy.</p>
<p style="text-align: justify !important;">Spatial maps of top 10 networks: If the origin is within the brain, then are there any fingerprints that can be detected? Does the brain often tell-tale signs which can be mapped to predict the tendency of epilepsy?</p>
<p><a href="https://innohealthmagazine.comresearch/aggression-after-drink/"><em><strong>Reason for Aggression After Drink</strong></em></a></p>
<p style="text-align: justify !important;">Seeking answers to these questions, a team of interdisciplinary researchers conducted a study to peep inside epileptic brains. The results indicate that there exist independent neural networks that can carry disease sensitive information about the anomaly. With the help of machine learning models and artificial intelligence, researchers were able to detect and reveal the hidden patterns.</p>
<p style="text-align: justify !important;">“Epilepsy is not a disorder but the manifesting of something from within the brain’s electrical activity. Interestingly, each one of us has the neural map of epilepsy within our brain. It is only when the network gets fired and manifests externally, in a recurrent manner, it becomes disorder or epilepsy,” explained Dr. Tapan Kumar Gandhi, lead researcher of the study from Indian Institute of TechnologyDelhi, while speaking to India Science Wire.</p>
<p style="text-align: justify !important;">The usual diagnosing tool for epilepsy is by EEG (Electroencephalography) readings of epileptic patterns and visible symptoms like convulsions, loss of consciousness or sensory disturbances.</p>
<p style="text-align: justify !important;">Existing studies reveal specific patterns that represent synchronous activities of sensory, auditory, cognitive and other functions. These activities are indicated by the change in blood flow to the brain and seen as BOLD signals or changes in the Blood-Oxygen-Level-Dependent output.</p>
<p style="text-align: justify !important;">Recent developments in Magnetic Resonance Imaging or MRI help picture these activities in the brain and detect the cause of seizures such as a lesion or scar. However, MRI is not very useful when a seizure flares up. Whereas, functional MRI — another scanning method — can record regional interactions in the brain when a particular task is being performed.</p>
<p style="text-align: justify !important;">In 1995, Indian researchers had found that the brain shows prominent neural network connections even in its resting state. Termed as resting-state functional MRI or rsfMRI, the images from this scanning indicate neural patterns in an individual’s brain even when no action is performed.</p>
<p style="text-align: justify !important;">In the present study, the team utilized rsfMRI technique and performed brain scans on individuals with Temporal Lobe Epilepsy (TLE), which is the most common form of epilepsy.</p>
<p style="text-align: justify !important;">Dr. Gandhi said, “We hypothesized that there could be ‘disease-specific networks’ in epilepsy prone brain that can be identified with the help of the machine learning model.” Machine learning involves artificial intelligence to read live data instead of pre-programmed information. Such a building block of a machine is analogous to a neuron cell in the brain.</p>
<p style="text-align: justify !important;">Researchers used a tool called Support Vector Machine (SVM) to deal with the complex and non-linear data obtained from the scans. By using another algorithm called Elastic-net based ranking, the relevant features of the neuroimaging data were extracted. The signals were integrated to reveal the patterns.</p>
<p style="text-align: justify !important;">The team conducted a pilot study on 132 subjects &#8211; 42 with epilepsy, and the rest with healthy individuals. Parameters like age, gender, history of epilepsy, genetic predisposition, incidents of injuries, medications and more, were taken into account. The epilepsy patients underwent three rsfMRI while those in the healthy group were scanned once.</p>
<p style="text-align: justify !important;">In all, 88 independent components or networks were obtained from the whole brain imaging data and fed as input to the SVM. From the patterns, top 10 strong networks were correlated with clinical features using another standard method called Pearson’s Correlation to generate the rsfMRI epileptic neural networks.</p>
<p style="text-align: justify !important;">From the pattern inputs, the SVM could identify epileptic individuals to an accuracy of 97.5% and specific lobes in the brain responsible for the condition. The model also revealed correlations such as the age of onset, frequency of seizures, or duration of illness.</p>
<p style="text-align: justify !important;">By this, researchers concluded that the independently derived rsfMRI contains epilepsy-related networks. ‘Our research establishes that with the help of machine learning methods, we can identify these networks, as we had hypothesized. Increased strength in these networks indicates the possibility of a progressing Temporal Lobe Epilepsy’, explained Dr.Gandhi.</p>
<p style="text-align: justify !important;">The team included Rose Dawn Bharath, Sujas Bharadwaj, Sanjib Sinha, Kenchaiah Raghavendra, Ravindranadh C. Mundlamuri, Arivazhagan Arimappamagan, Malla Bhaskara Rao, Jamuna Rajeshwaran, Kandavel Thennarasu and Parthasarathy Satishchandra (National Institute of Mental Health and Neurosciences, Bengaluru); Tapan K. Gandhi and Jeetu Raj (IIT, Delhi); Rajanikant Panda (Universitè de Liège, Belgium); Ganne Chaitanya (Thomas Jefferson University, USA) and Kaushik K. 	<p><strong>Where India Stands &#8211; The New &#8220;Health-Y&#8221; Politics</strong></p>
<p style="text-align: justify !important;">The much-awaited general election will be held in India in April 2019 when the Indian voters will go to the polls. It is anticipated that whichever government comes into play will surely set a course for newer and better <a href="https://innohealthmagazine.comnewscope/medical-errors-india/">health policy</a>.</p>
<p style="text-align: justify !important;">Recent researches reveal an alarmingly rapid epidemiological transition. The occurrence of diseases such as cardiovascular diseases, diabetes, and cancer are rapidly on a rise. For instance, since 1990, the number of Indians with diabetes has increased from 26 million to approximately 65 million to date. The incidence of all cancers has increased by 28% between 1990 and 2016, with new cases of cancer emerging each year. India not only seems terribly engulfed in the swirling epidemic of non-communicable diseases, but it is also in the grip of a mental health emergency. The rate of global suicide deaths is speculated to be 1.1 times higher among women and 1.4 times higher among men than global averages in 2016.</p>
<p style="text-align: justify !important;">It would be unfair if we highlight only the negative side of the picture. After many years of neglect, the Government of India has finally recognized and considered the perils of public discontent relating to health. Prime Minister Narendra Modi has initiated a new programme called Ayushman Bharat, which along with focusing on preventive health, emphasizes affordable health.</p>
<p style="text-align: justify !important;"><a href="https://innohealthmagazine.comnewscope/ayushman-bharat/">Ayushman Bharat</a> majorly comprises two new flagship programmes. First, the creation of 1.5 lakh health and wellness centers across the whole country. These centers will primarily be responsible for providing primary healthcare facilities to provide health coverage for all. The second includes the National Health Protection Mission (NHPM)—a system of health insurance that intends to cover 500 million people. It covers secondary and tertiary care for the poorest and most vulnerable strata of society. These twin programmes together must help India improve access to quality health services and reduce out-of-pocket health expenditures.</p>
<p style="text-align: justify !important;">Modi is the first Indian Prime Minister who has grasped the importance of health for Indian citizens and has prioritized universal health coverage as part of his political platform. “Modicare” as it is sometimes addressed to, can become a decisive issue in the upcoming general elections.</p>

	<p><strong>Way Forward for a Healthy Nation</strong><br />
Does Universal health Coverage still seem a distant goal in India!</p>
<p style="text-align: justify !important;">The scant funding from the Central and state governments seems the root cause and the major hindrance in achieving UHC. The Central Government has recently announced a massive expansion of healthcare facilities.</p>
<p style="text-align: justify !important;">Through its ambitious National Health Protection Scheme (NHPS), the Government of India provides a framework for universal health coverage in which it prioritizes healthcare delivery and its easy access. By reducing out-of-pocket payments, expanding service coverage, improving quality of services, and ultimately advancing UHC, countries and in particular, India with schemes like NHPS in place can transform the health of their people and bring drastic changes in society. As stated by WHO Director-General: “No one should have to choose between death and financial hardship. No one should have to choose between buying medicine and buying food.” UHC indeed is a way to go ahead.</p>

He presented his idea of understanding the 3 ‘P’s i.e. pain, (of) patient, (of) provider to deliver Healthcare at optimum cost and as well as scaling up the availability of care.</p>
<p style="text-align: justify !important;">Without taking much time, he addressed all the stakeholders and also identified the knowledge gap that still exists and recommended increased research to fill them up through holistic approach along with further action to encourage innovation in healthcare.</p>

<p style="text-align: justify !important;"><a href="http://innohealth2018.innohealth.in/speakers/dr-anil-kumar-gupta/">Prof. Anil Kumar Gupta</a>, Visiting faculty, Indian Institute of Management, Ahmedabad and Founder, Honey Bee Network, India gave the keynote address for the session and looked at the challenges in the areas of health. He acknowledged that, in recent times, there had been a growing awareness of health but said that more still needed to be done to allow further implementation of the necessary action. He then explained how, sharing and caring would result in good health, a significant integrated share and care approach should be adopted, thanks to the platform provided by <a href="http://innohealth2018.innohealth.in/">InnoHEALTH 2018</a>. This had enabled various stakeholders to come together. Questioned the gathering by asking them we (docs) only meet sick people, then how can we know what health is? He came up with an idea of conducting an event in which grandmothers who never visited the hospital. He was surprised by witnessing the number of grandmothers who did not visit hospitals in their lifetime. He observed that most people who live up to the age of 100 are women and unlikely to be the man. Spoke of how a healthy environment should be and considered Health as an ecosystem of both sick and healthy people.</p>
<p>He researched on the soil whether it had any impact on the health of a person since the cycle goes like<br />
soil -&gt; plant -&gt; animal -&gt; milk -&gt; human</p>
<p style="text-align: justify !important;">He also emphasized that there are many ways to make affordable healthcare which should be adopted in small scale at least if not on large scale. For example, a student uses iron fish in his curry which gives him 126g of iron. Surveyed and found 5 healthy states, the common thing in them was that the mothers were breastfeeding the child for 3 to 4 years and the vegetables consumed (edible wheat). Insisted on adopting alternatives for supplementation of health.</p>

We are having 10,000 to 15,000 patients every day we have enough data too. Why aren&#8217;t we using the data? There was a need to raise awareness of the benefits of more data analytics in the healthcare sector. emphasized to Improve data analytics. Another area where development is required is in supply Chain – the challenge of innovation in healthcare, how do we connect the innovator and the user? On one side we have solutions and innovations but the scaling up foundations are less in numbers on the other side which creates the gap. Study of healthy people should be done. “Innovation comes with originality” – Prof. Gupta, He wanted the medical education system should be devised which will be helpful to all. Talked about the great scope of InnoHEALTH in this context as a platform for change. Ended the speech by throwing a question to all present there.“Innovations are coming up frequently but how to make it available to the world at large.</p>
<p style="text-align: justify !important;">His excellency <a href="http://innohealth2018.innohealth.in/speakers/klas-molin/">Klas Molin, Ambassador of Sweden to India</a>, who was the Guest of honor, shared the healthcare experiences of Sweden. The healthcare delivery was very important in their election campaign. He expressed the concern of time taking cures for which they are looking forward to solutions. Mentioned the importance of prevention as the secret of healthcare lies in focus on prevention. The duo of government academia and the private sector should be encouraged for results in healthcare. Sweden has a separate council regulating the innovations named as National innovation council. The action plan in innovations -MOU was signed between Sweden and India. Showcased the country ranking among EU in global competitiveness and innovation.</p>
<p style="text-align: justify !important;">Srijan Pal Singh CEO and Co-founder, <a href="http://www.kalamcentre.org/">APJ Abdul Kalam Centre</a> &#8211; Special invitee, started with a short video about late Dr. A P J Abdul Kalam to commemorate him and his work, showing his inspiring moonshots and contributions in the healthcare sector. Srijan explained that Dr. Kalam had been a strong person with a vision and a great motivator and original driving force behind the Dr. Abdul Kalam center. Shared a memorable incident encountered by him with late Dr. APJ Abdul Kalam in Kentucky, where Dr. Kalam was asked a question by a young student &#8211; “you have been in many positions like president, missile man, nuclear power, and many more as the list bigs.</p>
<p>Which of these gave you happiness or bliss of the above positions?<br />
Dr. Kalam opted a hidden option that is option E.</p>
<p style="text-align: justify !important;">In 1980, Mission AGNI was a tough task to achieve as the nose cone of the missile was a big challenge to design as there was a lot of heat as well as the pressure acting on it. They couldn’t use titanium also since the missile had to be very light in weight, then found a carbon-carbon composite which solved the purpose. One day his friend called from Hyderabad pleading his help for the children who were physically challenged and asked for a solution for their prosthetics. Kalam suggested why not make them a carbon-carbon composite prosthetics, which made the children very happy as the prosthetics were very light and they could now easily walk, run and didn’t feel the weight. The happiness in their faces made kalam sir happy. That gave him happiness by seeing the kids happy.</p>
<p>The message he wanted to is human compassion is strength and no joy can replace that.</p>
<p style="text-align: justify !important;">He then suggested, Art + music + soft skills + medicine + healthcare should be taken into the mainframe, a holistic approach should be adopted in healthcare. He Started a non-profit organization named after the missile man to try achieving the vision which Dr. Kalam dreamt of.</p>
<p style="text-align: justify !important;">Came up with a unique idea of collecting engineering solutions for the upcoming challenges. Illustrated how poor and remote area people aren&#8217;t getting proper drinking water by which around 3000 people die every year. Mission to get all the citizens free and clean drinking water have been started and the PIL will be filed on Oct 15th, 2018.</p>

	<p style="text-align: justify !important;">While World Health Organization (WHO) recommends 800 grams or 5-9 servings of fruit or vegetables per day, the new study has shown that it takes just half – that is, 375 grams to get the desired health benefits. This is important for people living in countries who cannot afford a lot of fruits and vegetables.</p>
<p style="text-align: justify !important;">Potatoes and other tubers were not included and fruit juices were not considered as fruits while calculating the intake. Legumes included beans, black beans, lentils, peas, chickpeas, and black-eyed peas.</p>
<p style="text-align: justify !important;">The researchers attribute beneficial effects of consuming fruits and vegetables to presence of antioxidants like vitamin C, vitamin E and carotenoids, and fiber in them, which reduces bad cholesterol, improves insulin response, lowers the blood pressure, prevents fat deposition in blood vessels, and improves cellular function in the body.</p>

	<p style="text-align: justify !important;">The decade-long research was done in 18 countries with 135,335 participants aged 35 to 70 years. Healthy individuals with no reported diseases and complications were enrolled for the study. They were given questionnaires to record daily diet, lifestyle habits like smoking, physical activity and alcohol intake, and their socioeconomic status like education, income, and employment.</p>
<p style="text-align: justify !important;">At the end of the study, researchers recorded the number of deaths, cases of cardiovascular diseases, heart attack, and stroke. Then the data was analyzed to see if consuming higher amounts of fruits, vegetables, and dal is related to the number of deaths and adverse outcomes on health. “We found that regular consumption of vegetables, fruits and legumes protectedpeople from cardiovascular disease and death”, states Dr. Mohan.</p>

	<p style="text-align: justify !important;">“This study does not distinguish between cooked vegetables and raw ones, although it is common knowledge that cooking destroys some of the vitamins and minerals, hence as far as possible we should use raw vegetables like tomato, cucumber, carrot and green leafy vegetables which can be consumed raw”, pointed out Dr. Mohan.</p>
<p style="text-align: justify !important;">According to Estefania Toledo, professor at the Department of Nutrition of the Harvard School of Public Health, Boston, USA, who is not connected to the study,“Increased consumption of fruits and vegetables should beat the expense of reducing other foods and drinks, such as sugar sweetened beverages, red and processed meat, saturated and trans fat, refined cereals, and sugar rich desserts”. She added that consuming more plant-based foods helps replace detrimental foods, which benefits the overall dietary pattern.</p>

<p style="text-align: justify;">Jayashree is one the CHW who works relentlessly in her village Hubli(Karnataka). One day, she visited a woman named Pushpa who was in her first trimester and looked quite unwell.On enquiring, Pushpa was found to suffer from Epilepsy. Her family also revealed that she has been prescribed with medicines by a local doctor, but she didn’t take them regularly.</p>
<p style="text-align: justify;">Epilepsy is a disorder of the nervous system and such patients during pregnancy have major concerns. Jayashree felt the need and promptly captured her vitals on her smartphone loaded with Wireless Health Incident Monitoring Systems, (WHIMS), and pushed the patient’s vitals on to the cloud for a quick doctor reference.</p>

<p style="text-align: justify;">In the context of rural settings, it is never possible to create doctors or health clinics as required. To bridge the gap, the grassroots community members are empowered to provide clinical careenabled through WHIMS.The technology application is designed with an intuitive graphics user interface that can be efficiently used by a health worker even with a basic education.</p>
<p style="text-align: justify;">For instance, WHIMS screen has a complete photograph of a human body.When a health worker clicks on the head, WHIMS gives options of potential symptoms of the head, such as ache, dizziness, seeing dark spots, and so on.</p>

WHIMS is accompanied with low-cost portable instruments to measure basic statistics and loaded on a tablet is carried by the CHWs in the remotest villages to diagnose patients. The patients pay an affordable rate for basic diagnostics and consultations, through which they earn additional income.</p>
<p style="text-align: justify;">Unlike the Accredited Social Health Activists (ASHA workers) who work as community mobilizer and not authorized to deliver clinical services, these CHWs meet both the ends.</p>
<p style="text-align: justify;">Equipped with a medical kit that consists of devices including BP machine, stethoscope, pulse oximeter, thermometer, first aid kit, weight machine, Height measuring scale and gloves,they carry medicines for basic common ailments, non-invasive hemoglobin meter, haemoglobin kit, mobile phone based sphygmomanometer, urine analyzer, calorimeter, ECG, flipcharts/ Hoardings &amp; registers.</p>

DTH gets updated with latest clinical indicators. The health data obtained from the health cards is synced to a cloud for effective utilization in the upward heath chain at both secondary and tertiary levels.</p>
<p style="text-align: justify;">CHW’s contact and familiarity within the community drive effective community mobilization measures. As they become an integral part of the healthcare delivery model, CHWs act as a catalyst to drive behavioral change. While they interacts and communicates the necessity on the ground and identifies appropriate health needs, it is through street plays, hoardings and message dissemination on mobile phones, theymobilize patients at different levels. They are also attuned to provide nutritional counselling, and spectacle services based on the community needs.</p>
<p style="text-align: justify;">Further, WHIMS is upgraded with robust supply chain management framework, innovative algorithm, and Data Warehousing solutions. Driven by its CHWs, iKure aims to bring efficiency in public system &amp; programe and improve lives.</p>
<p style="text-align: justify;">The CHW model of empowering the women community and gender mainstreaming represents transformational change in rural healthcare delivery. This has been a critical step in rural healthcare delivery with a positive outcome on prevention, curative care and treatment procedures”.</p>

	<p style="text-align: justify !important;">Do you know anybody who does not wish to get rid of the fat on one particular area of their body? I have not met anybody like that!</p>
<p style="text-align: justify !important;">“If I could only get rid of my big belly”! “Oh, I wish my thighs were slimmer”! Does it sound familiar?</p>
<p style="text-align: justify !important;">Even skinny people are often unhappy with the excess fat in some areas of their body. We’ve all seen those skinny men with huge bellies!</p>
<p style="text-align: justify !important;">This common problem is accompanied by a widespread belief that you can lose the fat on a “problem area” by exercising that particular area. Some people do hundreds of crunches in attempts to flatten their stomachs, others buy the “miracle” ab gadgets and bulky “hip slimming” machines from TV.</p>
<p style="text-align: justify !important;">Were you about to call the 800 numbers to buy the “latest and greatest” ab roller?</p>
<p style="text-align: justify !important;">Hold on to your hard-earned money!</p>
<p style="text-align: justify !important;">The belief that you can lose fat on a specific part of your body by exercising that area is a myth! Targeting the “spots” of your body for fat loss is called “spot reducing”, and it simply does not work.</p>
<p style="text-align: justify !important;">Exercising certain areas of your body will strengthen the muscles there, but it will not remove the fat from them. Your newly developed muscles will be hidden under the layer of fat.</p>
<p style="text-align: justify !important;">What about those heavily pitched creams, wraps and rubber belts that supposedly “melt the fat away”? Those are just plainly scams.</p>
<p style="text-align: justify !important;">The creams and body wraps can temporarily give your skin smoother appearance, but they will do absolutely nothing for fat loss. All kinds of weight loss belts, pants and other clothing will make you sweat, thereby causing some loss of water weight which is mistaken by most people for fat loss. But they will not help you lose even one gram of the actual fat!</p>
<p style="text-align: justify !important;">There is no way to tell your body where exactly it should lose its fat.</p>
<p style="text-align: justify !important;">Each person’s body has its own unique shape and fat distribution pattern. The places where the fat goes first are, unfortunately, where it comes off last. So the only way to lose that fat is by losing weight in general.</p>
<p style="text-align: justify !important;">The only healthy permanent way to lose weight is by making lifestyle changes to incorporate healthy eating and increased activity. When you do that, the fat will come off everywhere on your body, including those “trouble spots”. And yes, you will then have that body that you always dreamed of!</p>
