India is a diverse country, whether the diversity is in culture, religion, eating habits, or exercising habits. Earlier, there was this notion that if you eat healthy and exercise daily, your heart will stay healthy. The heart is an organ that needs to beat continuously so that every organ can get oxygenated blood to function. To let it function properly, one needs to take care of it. The effects of your daily lifestyle have a major impact on your heart, and along with that, heredity and environmental factors play a role in heart health. If you compare the day-to-day lives of 21st and 20th-century individuals, a massive difference in their eating and working habits will be inevitable. The difference in time showed the fast-paced life evolution where exercise habits have also changed. There were gyms earlier as well, but for boxers or sports people. Common people’s physical labor was cycling or walking to anywhere they went.

Most of them didn’t have machinery such as bikes, cars, etc. But physical exercise is not the only way to protect your heart; it involves multiple factors. Let’s see what the other factors are. One of them is eating healthy, which is another way to reduce consumption of processed food, less sugar, and less saturated fats or trans fats, as these will gradually lead to increased obesity, blockages in heart vessels, and the heart having to struggle for every beat, eventually leading to a heart attack.

The environment also has its effects on overall health as well as on heart health. Most people are aware of these facts but choose to ignore them. Some do care; they eat healthy and they workout, but what went wrong was that they got a heart attack. Sometimes, when everything is right, there is something wrong hiding behind that right. Changes in the working lifestyle and trying to be ahead of the competitive nature of today’s generation have left no space to relax and calm down. This not only affects their health but also their relationships with family and loved ones.

Thus, a lot of emotions are piling up in the form of stress, which is one of the major reasons for heart attacks in people who have a balanced diet and exercise. Not only that, but stress – smoking and alcohol consumption—and lack of sleep also contribute to it. For adults, at least – 8 hours of sound sleep is required.

“Eat right, sleep tight, and exercise light” should be the new life mantra for this generation”. Apart from this yearly checkup of your heart, invest in certain basic tests like an electrocardiogram, an echocardiogram, and a stress test (exercise tolerance test (ETT) or treadmill test) and certain advanced tests like cardiac CT, chest x- ray and Thallium scan (myocardial perfusion scintigraphy) as per your physician’s recommendations on the basis of your blood test and symptoms, if any. So, you can make it a habit to visit your physician once or twice a year to keep up with your heart health.

This discovery suggests we should change public health messages. Instead of pushing intense daily exercise for everyone, we should focus on personalized, moderate, and varied physical activities. It encourages us to tune into our bodies, to find equilibrium between activity and rest, and to recognize that health includes physical, mental, and emotional aspects. By doing this, we aim for a healthier life that reduces risks and embraces overall well-being. It also shows how important
scientific research is in updating our knowledge about what’s best for our health. “Fit to Fail” reminds us that real health is not simple.

Muscle soreness can affect every part of your body, including the chest muscles, especially when they are inflamed.

Sometimes you might feel a pressure or burning sensation in your chest, or other types of general uncomfortable situations that will indicate something is not right. Experiencing chest pain rarely is nothing to be afraid of as it might be an indication of improper digestion. However, when the feeling becomes frequent, you need to take precautionary measures. Numerous causes are associated with chest pain and it’s advised to seek immediate medical health if the sensation becomes too hard to bear.

While chest pain is an indication of heart problems, other internal organs in your body can also be the cause for the pain. Despite the causes, you need to be aware of the symptoms. The generic causes of chest pain include:

Heart-Related Causes

Heart issues are the primary cause of severe and mild chest pain. Over 15% of adults in the United States suffer from Coronary Artery disease, which is a common heart disease. When you have this heart disease, blood won’t be able to flow into your heart properly. An insufficient supply of blood in the heart can make you feel a sharp chest pain known as angina. This pain can spread from your heart to your arms, back, and other body parts.

Most people associate heart attacks when they experience even minor symptoms of chest pain. When your body suffers a heart attack, blood could not reach your heart. You will feel your chest being squeezed or crushed. If you feel any type of chest pain, call 911 immediately.

Lung-Related Causes

Lung pain is closely associated with chest pain. Pleurisy is a common case when a thin membrane will cover your entire lungs and make them inflamed. When you develop pleurisy inflammation, you will feel tenderness and sharp pain when taking deep breaths or coughing. This sheer pain in your chest might get worsened over time, especially if you don’t consider treatments.

Some pains associated with your lungs will require immediate medical attention. Pulmonary embolism is one of the most common causes. This situation takes place when one lung’s arteries form a blood clot. Problems in breathing along with sheer burning sensations in your chest are the symptoms.

Muscle-Related Causes

If you have been exercising at the gym before, you will know the feeling of muscle soreness. Muscle soreness can affect every part of your body, including the chest muscles, especially when they are inflamed. Trauma can also affect the chest muscles, causing chest pain or a burning sensation. Lifting heavy weights incorrectly and coughing vigorously can also cause chest pain. As per Everyday Health, muscle soreness is normal after light weights.

Stress-Related Causes

When you’re stressed about something, feeling extremely worried, vulnerable, and restless is normal. However, depending on your stress level, your mental health conditions will go through massive changes. These negative emotions will create a feeling of tightness in your chest, which will prevent you from breathing properly.

To summarise we can understand that chest pain can take place in the upper section of your body for various reasons, including heart and lung problems. It can also spread to other parts of the body quickly. You should always take symptoms of chest pain seriously and seek medical attention.

Brig Arvind Lal, CMD of Dr Lal Path Labs, is a pioneer in bringing laboratory services in India at par with the western world. In 1977, he took charge of the medical diagnostics laboratory founded in 1949 by his late father. Under his expert guidance and leadership, the initiative has become one of the most reputed laboratories in Asia, having to its credit quality accreditations from various national and international bodies.

The critical care environment has undergone significant alterations in the past several years. This has happened because our lifestyles in the fast-paced lives of modern India are ensuring that most people, in the age group of 30-50 years are falling prey to life-threatening cardiac diseases and strokes, in addition to diabetes, hypertension, cancers, liver, kidney and lung diseases – these diseases being called Non-Communicable Diseases or NCDs. They are now responsible for killing more than 65% of our population says Dr Arvind Lal, known for his diagnostic labs across the country.

Flagging concerns on such trends, these patients need high-cost intensive care, be it for complications of heart attacks, strokes, diabetes, hypertension, cancer or lung diseases. This is where the importance of Point-of-care testing (POCT) comes in. It helps in almost diagnosing the patient instantly and improves the physician’s ability to take immediate corrective action and decreases hospital stay. One such test is Troponin – that has revolutionized cardiac care by diagnosing heart attacks or myocardial infarction.

This article is based on the keynote address on the occasion of 2nd Annual International InnoHEALTH Conference 2017 – ‘Transforming Healthcare Through Innovation’ in New Delhi, said there are numerous promising diagnostic technologies. The key message is that in a country where 70% of the population lives in rural surroundings, ‘it is our duty to rapidly adopt disruptive innovative affordable technologies including telemedicine. Thus, our underserved population would be able to avail of the best treatment possible and bring in massive visible change’. He said the importance of bringing quality healthcare needs no reminder and the time has come for India to change the direction of healthcare for the masses.

Healthcare is a right – and access to good healthcare should not depend on where one lives and how much he or she earns. But sadly, that is exactly what plagues India’s healthcare today, he lamented. India faces a severe shortage of both hard infrastructure and talent. With about one doctor and one functional bed per 1000 population, healthcare is truly underserved in India. Add to this the regional imbalances and variations in healthcare delivery. The healthcare infrastructure is skewed towards urban over rural India.

Although rural India accounts for about 70% of the population, it has less than one-third of the nation’s hospitals, doctors and beds, resulting in large disparities in health outcomes across urban and rural India. British Medical Journal (BMJ) has observed that there is a remarkable saving of lives in India if good healthcare facilities consisting of operation theatres, surgeons, anaesthetists, blood banks are available within 50 kilometres of the patient providing quality medical services within the ‘golden hour’.

Though there has been a sea change in the last five decades, India now needs to reinvent the field of diagnostics as laboratory tests are responsible for 70% of all clinical or medical decisions.

In today’s life where internet rules the roost, the patients have become very knowledgeable, thanks to the globalisation of healthcare, and are demanding very high-quality healthcare for themselves. They are insisting on a very wholesome and satisfying experience rather than being told that the ‘treatment is over’.

Soon, a time will come when the tests shall be ordered by the patients based on clinical history and clinical findings that shall be answered by an Artificial Intelligence (AI) application. Artificial intelligence has already found several areas in healthcare from the design of treatment plans to assist in repetitive jobs to medication management and drug designing. The most obvious application of artificial intelligence in healthcare is data management. Collecting it, storing it, normalizing it, tracing its lineage – it may well be the first step in revolutionizing the existing healthcare systems.

Recently, the AI research branch of the search giant, Google, launched its Google Deepmind Health project, which is used to mine the data of medical records in order to provide better and faster health services. The project is in its initial phase, and at present, they are working with Moorfields Eye Hospital of NHS Foundation Trust, UK to improve eye treatment.

Just a few years ago the patient after giving the sample used to come back in the evening to the lab to collect a physical copy of the test report. This was replaced by making the report available on the internet that could be downloaded by the patient in the comfort of his home. These days this has been further replaced by making available an App on his mobile phone wherein he can book an appointment for the sample to be collected at home and the report being later available on the same mobile App.

‘IBM Watson, whose headquarters I had the privilege of visiting a few months back in the Silicon Valley, is an AI-based engine that has launched its special program for oncologists to provide clinicians evidence-based treatment options. The program has an advanced ability to analyze the meaning and context of structured and unstructured data in clinical notes and reports in its encyclopedic memory that may be critical to selecting a treatment pathway’. IBM launched another algorithm called Medical Sieve. It is an ambitious long-term exploratory project to build a next-generation ‘cognitive health assistant’ that is able to analyze radiology images to spot and detect abnormalities faster and more reliably. This shall help radiologists in the future to look at the most complicated cases where human supervision is essential.

‘Wearable Tech is another area which I am personally very excited about. It has the potential to change the world as it helps people understand their own bodies by using mass data collected on a daily basis. From fitness bands to smartwatches to eye based wearables, they are being adopted widely. Take the case of Zephyr’s Anywhere Bio Patch which is an FDA-approved, small device that is attached to a patient’s chest and monitors their vitals minute-by-minute and collects medical-grade data for doctors’ use. These devices will connect our organs digitally, enabling disease detection at very early stages. It has the potential to bring down cardiac and other deaths drastically. This offers immense potential to do remote testing, monitoring and thus assisting the doctor in timely treatment’.

Point of Care Testing: Technological advancements in laboratory automation, including POCT, and initiatives to increase patient satisfaction are transforming the clinical laboratory market. POCT has come a long way from a handful of simple tests to a multibillion-dollar global market that holds great promise for the future. Not so long ago, laboratory data would often arrive at the bedside too late to be of significant use in the active, continuing care of critically ill patients. Now, most clinicians acknowledge that POCT is a prerequisite for early recognition of life-threatening conditions as they require that laboratory results are made available in real-time and, if possible, at the critically ill patient’s point of care. The College of American Pathologists defines POCT as tests designed to be used at or near the site where the patient is located, that do not require permanent dedicated space, and that are performed outside the physical facilities of the clinical laboratories.

Examples include kits and instruments that are hand-carried or transported to the vicinity of the patient for immediate testing at that site (e.g. capillary blood glucose) or analytical instruments that are temporarily brought to a patient care location (like operating room, intensive care unit). In many cases, the simplicity was not achievable until technologies developed that was simple and affordable. For example, various kinds of urine test strips have been available for decades, but portable ultrasonography did not reach the stage of being advanced, affordable and widespread until recently. Similarly, pulse oximetry can test arterial oxygen saturation in a quick, simple, non-invasive, affordable way today, but in earlier eras, this required an intra-arterial needle puncture and a laboratory test. Thus, over decades, testing continues to move toward the point of care.

The lab-on-a-chip (LOC) is another device that integrates one or several laboratory functions on a single integrated circuit (commonly called a “chip”) of only a few square centimetres to achieve automation and high throughput screening. Imagine that a patient comes to one of our 2,100 collection centres in the remote tier three or tier four towns in India with the high fever. We take a drop of blood from his finger and inform the clinician almost immediately that the patient is suffering from Chikungunya and not from Malaria or Typhoid, or Dengue fever or Japanese Encephalitis – all in a matter of minutes! The driving notion behind POCT is to bring the test conveniently and immediately to the patient. Needless to add, the patient’s data by POCT shall be made available to update the patient’s electronic health records (EHR).

Talking about POCT Instruments: Currently, two broad type of POCT instruments are available: Small benchtop analyzers (for example, blood gas and electrolyte systems) and handheld, single-use devices (such as urine albumin, blood glucose, and coagulation tests). Now let us talk about if POCT is Boon or Bane? The strong point of POCT is speed and the rapidity with which it shall save lives in emergencies. As India marches towards quality healthcare delivery, in course of time regulatory compliances shall have to be adhered to in the interest of the patient’s health.

The death of his grandfather due to heart attack led Manoj Akash, a class 10th student of Ashok Leyland School in Hosur, Karnataka, to invent skin patch which has to be attached to the wrist or the back of the ear and it will release a small ‘positive’ electrical impulse, which will attract the negatively charged protein released by the heart to signal a heart attack. If the quantity of this protein – FABP3 — is high, the person must seek immediate medical attention.

Since class eight he started visiting the library of the Indian Institute of Science in Bengaluru – an hour away from his hometown. He could not afford expensive books and journals. Only option left to visit library for this enterprising adolescent who had an extra knack to comprehend complexities of cardiology just because of his penchant for the heart diseases studies. He was chosen for the President’s Innovation Scholar’s In-Residence Programme at Rashtrapati Bhavan.

The 16-year-old says that he investigated a novel concept that could potentially allow patients to detect silent heart attacks by non-invasively sensing the FABP3 biomarker in the bloodstream. ABP3 is a lightweight protein, releases quickly from heart muscle into the bloodstream during a heart attack, and therefore, it is an optimal cardiac diagnostic marker.

According to him, doctors may test a patient’s blood for FABP3. If he or she experiences characteristic symptoms like chest pain. However, not all heart attacks make themselves known through easily noticeable signs. Silent heart attacks, which are becoming increasingly common, are asymptomatic, making them more dangerous than conventional heart attacks.

Patients often “drop dead” while feeling completely normal. This collapse happened to my grandfather on July 3rd, 2015 which served as an impetus for me to find a solution to this problem. In these “silent” cases, doctors are unlikely to administer the crucial FABP3 blood test because there is no visible presentation of symptoms to warrant a diagnostic test. As a result, silent heart attacks go unnoticed.

“I realized that if at-risk patients could test themselves daily for the presence of FABP3 in their blood. They would have higher chances of detecting silent heart attacks as they occur. A method that allows daily self-testing would have to be non-invasive, safe, and easy to use. Ultimately, it would have to involve a transcutaneous blood analysis, which examines the contents of one’s blood without penetrating the skin.

In searching for ways to tackle this challenging prospect. I examined the various distinguishing characteristics of blood proteins that would allow them to be identified transcutaneously. I found that proteins have distinctive masses and electric charges in blood.

So, I used a model to test whether different magnitudes of charged electricity. When applied to a thin area of skin, would isolate FABP3 from the other blood proteins and attract FABP3 to the capillary walls. My results showed that this is true.

This means that the technique that I investigated can potentially be coupled with transcutaneous UV-protein quantification to non-invasively measure the amount of FABP3 in a patient’s blood and alert him or her of a silent heart attack.”

Help Million Hearts Stay Healthy! Is his web page that narrates his vision.

He was frequenting scientific conventions to further his knowledge in the science field. Internet gave him a lot of insight, he admits Clinical trials for the medical device are on. And it could be approved for a human trial. The product would be fit to be launched in the market after two months of human trial, assuming nothing goes wrong.

“I have already filed for a patent and I would tie up with the department of biotechnology for the trial. I would want the Government of India to take the project instead of selling it to a private company because it is for the public good,” he says on his website.

To monitor, regular use small silicon patch stuck to your wrist or back of your ear. Whether there has been a heart attack instead of waiting for a doctor to prescribe a test.

The patch uses a positively charged electrical impulse to draw negatively charged —protein to the surface; If the amount of FABP3 is high, then the person would need immediate medical attention; Experts recommend to use the device twice a day. The product can soon be in the market. It would cost around R 900, cheaper than a glucometer symptoms at all.

Diabetes, high blood pressure, high cholesterol levels all put you at risk of a silent heart attack, experts say. Having a silent heart attack puts people at a greater risk of having another heart attack, which could be fatal. Having another heart attack also increases the risk of complications, such as heart failure.

Chest pain, pain in the left arm or shortness is when of breath characterizes a silent heart attack. A silent heart attack may not show symptoms of chest pain and shortness of breath that doesn’t feel heart attack.
While World Health Organization (WHO) recommends 800 grams or 5-9 servings of fruit or vegetables per day, the new study has shown that it takes just half – that is, 375 grams to get the desired health benefits. This is important for people living in countries who cannot afford a lot of fruits and vegetables.

Potatoes and other tubers were not included and fruit juices were not considered as fruits while calculating the intake. Legumes included beans, black beans, lentils, peas, chickpeas, and black-eyed peas.

The researchers attribute beneficial effects of consuming fruits and vegetables to presence of antioxidants like vitamin C, vitamin E and carotenoids, and fiber in them, which reduces bad cholesterol, improves insulin response, lowers the blood pressure, prevents fat deposition in blood vessels, and improves cellular function in the body.

The decade-long research was done in 18 countries with 135,335 participants aged 35 to 70 years. Healthy individuals with no reported diseases and complications were enrolled for the study. They were given questionnaires to record daily diet, lifestyle habits like smoking, physical activity and alcohol intake, and their socioeconomic status like education, income, and employment.

At the end of the study, researchers recorded the number of deaths, cases of cardiovascular diseases, heart attack, and stroke. Then the data was analyzed to see if consuming higher amounts of fruits, vegetables, and dal is related to the number of deaths and adverse outcomes on health. “We found that regular consumption of vegetables, fruits and legumes protectedpeople from cardiovascular disease and death”, states Dr. Mohan.

According to Estefania Toledo, professor at the Department of Nutrition of the Harvard School of Public Health, Boston, USA, who is not connected to the study,“Increased consumption of fruits and vegetables should beat the expense of reducing other foods and drinks, such as sugar sweetened beverages, red and processed meat, saturated and trans fat, refined cereals, and sugar rich desserts”. She added that consuming more plant-based foods helps replace detrimental foods, which benefits the overall dietary pattern.

We often hear of people dying in sleep. But, do we have an understanding of what can cause this sudden death? In most cases, the cause is sudden cardiac arrest due to an undetected heart rhythm disorder, or an underlying coronary artery disease.

In India, however, there is limited awareness both among people and healthcare providers about mechanisms needed to be adopted to reduce risk of sudden cardiac death and save lives.

There is a need to talk about the importance of timely identifying and treating patients at high risk of sudden cardiac death.

Sudden cardiac death (SCD) is defined as death which occurs within one hour of onset of symptoms due to cardiac causes. In USA about 350,000 deaths occur every year which are sudden. While we do not have consolidated statistics for India, clinical evidence suggests the numbers of people dying a sudden death due to heart ailments is high. Most important cause of sudden death in India is coronary heart disease or artery blockage but very few people understand the concept of sudden cardiac death or do anything to mitigate their risks.

In many cases, especially in smaller towns patients do not report fainting episodes, and carry on with their lives after regaining consciousness. There could be several reasons for a person passing out including a heart rhythm disorder coming from lower chambers of heart which can turn fatal if ignored. If the predictors are detected on time, the sudden cardiac arrest can be prevented by implanting a pacemaker-like device called ICD (Implantable Cardioverter Defibrillator) under the skin (usually below the collarbone) to send life-saving electrical signals that bring the heart back to a normal rhythm whenever abnormal heart rhythm is detected.

If you have a history of coronary artery disease, a record of high blood pressure or cholesterol, or a tendency towards diabetes or obesity, you are at a higher risk of sudden cardiac arrest. People who smoke, have family history of irregular heart rhythm or sudden cardiac death are other groups who have higher chance of sudden cardiac death.

Also, chances of sudden cardiac arrest are higher in people whose ejection fraction is less than 35% or who have history of fainting intermittently. Ejection fraction is a measure of the heart’s pumping power. A healthy person’s ejection fraction ranges between 60-70%. This is diagnosed on performing ECHO.

A sudden cardiac arrest occurs when the electrical impulse system of the heart malfunctions, disrupting regular heartbeat. It is more dangerous that heart attack because the rate of survival in case of sudden cardiac arrest is very low. Implanting an ICD in persons at high risk of sudden cardiac arrest can save lives. The key lies in improving identification of persons at high risk and recommending them for ICD implantation in time. Another important aspect of prevention is preventing coronary artery disease.

The single most important and cost-effective way to prevent sudden cardiac death is to prevent damage to heart which occurs due to heart attacks caused by blockages in the arteries. Cardiovascular disease is a huge health burden in India, magnified by the changing lifestyles, increase in sedentary jobs and lack of physical activity.

How to Reduce the Risk of Sudden Cardiac Arrest:
Control your weight: People who are over overweight especially at the abdomen have high risk of cardiovascular disease. Control diabetes, hypertension and LDL. Diabetes, hypertension and bad cholesterol magnify risk of both coronary artery disease and sudden cardiac arrest.
Exercise: 45 minutes of moderate daily exercise, at least 5 days a week, performing yoga or meditation, will go a long way to prevent heart attacks & sudden cardiac death.
Avoid Smoking: Smoking increases risk of coronary heart disease by narrowing arteries over a sustained period of time. Quitting immediately cuts the risk.
Managing heart health: People who already have suffered damage to their left ventricle (Ejection Fraction 35 % or less) can prevent sudden death by implantation of small pacemaker like device called ICD. ICD is highly effective device which can revive more than 95% of victims who suffer from cardiac arrest.

It is believed that by 2030, India alone will account for 60 percent of the world’s cardiac patients, nearly four times its share of the world’s population. The major reason for these alarming statistics is lack of awareness towards- ‘healthy’ lifestyle and the lack of knowledge of few simple steps that can be taken during emergency or before reaching the hospital.

Out of the people who die from heart attack, about 50 percent die within an hour of their initial symptoms before they reach the hospital. The chances of survival of the victim can increase significantly if he is provided Cardiopulmonary resuscitation (CPR) immediately. Hence, people present at the scene can help avert the risk of death by proving CPR promptly to the victim.

Providing BLS and CPR during the golden hour can help in preventing pre hospital deaths from injuries as well as heart attack if there is a delay in receiving hospital treatment or medical care. Mostly, the steps we take in haste while attending to an emergency, makes a deep-rooted impact in saving the life of the patient. Thus, it is vital for all to learn the BLS and CPR techniques, so that we can take the right measures when required.

These are simple steps like early recognition and initiating chest compression in cardiac arrest or protecting cervical spine with c-collar in road traffic accident victims and for this you don’t need to be an expert in medicine to take these lifesaving steps. Therefore, CPR and BLS should be made mandatory in secondary level of education and to all public service job holders as the chances of survival of the victim increase double fold if appropriate emergency medical care is provided immediately.

Time is the essence while treating a heart attack and cardiac arrest victims. “When the heart stops beating, blood flow to the body organs ceases. Of all the organs, brain is affected first. People become unconscious within 15 -20 seconds after the heart stops and chances of brain recovery decreases with time so much so that irreversible brain damage is common if circulation is not restored within 5 minutes of cardiac arrest. The need of the hour is to train common people to do CPR and BLS before the victim reaches the hospital. This training will help them to learn special skills required to deal with heart related emergencies. Skills to perform CPR through chest compressions, maintaining victims breathing by providing appropriate rescue breaths, can greatly improve the chances of survival of these unfortunate people.

You too can save a life. And for this you don’t need a medical degree.
