Truth of AI and IoT in Healthcare

All day all round we keep hearing the buzzwords like Machine Learning (ML), IoT, connected health, AI and IoT. But do we really know what these are? Should we really be bothered and aware how these can be changing the healthcare industry? To answer and brainstorm about such questions we all gathered on a beautiful Saturday morning on June 15, 2019 in one of India’s premier hospitals, Max super speciality hospital, Saket, New Delhi.

The meeting saw some very eminent and important speakers from around the healthcare industry. The first keynote was given by Dr. Vibha Jain, AGM IT, Max Healthcare. The keynote was about how AI improves the patient doctor communication?

The rise of the Internet of Things and Artificial Intelligence is critical to the digital transformation of modern healthcare as far as healthcare monitoring and delivery are concerned. The aim of patient and doctor communication is to create a good and caring relationship, facilitate exchange of information for accurate diagnosis and treatment, and including patients in decision making and counseling. But whether the doctors are able to create a caring relationship or whether the patients are satisfied with the care, is the point of concern With the evolving technology, can Artificial Intelligence empower healthcare and improve patient doctor communication? There can be endless possibilities in medical imaging and diagnostics, patient care, RnD and management.

One of the patient doctor communications medium is voice. Companies like Google and amazon are out with devices which are voice based and are betting heavily on voice based skills. As these companies are market movers, the question arises: Are we investing in making voice one of the skills needed to help the patients?

People with chronic conditions are often advised to follow detailed treatment plans; end-of-life care often requires many complex (and emotionally draining) decisions; options for treating certain diseases are increasing drastically. How much could health outcomes be improved with more effective communication between patients and healthcare providers?

One of the panelists brought light on how the voice is substantial to the patient doctor communication. Voice as an input is a useful tool as it gets us more data to refine our systems and algorithms. Voice as an output have some issues in the current scenario, as the synthesis is not real, it is hard to deploy the voice based system. Also there is a language barrier, as the systems are mostly deployed in English, there lacks trust and empathy. There is a vast opportunity to make these systems demographically viable. The question now arises is, if we use a chat bot or a bot service, can we have an authentication that it is a bot speaking not a human being.

Countries like Sweden have a white label application which helps the patient to communicate to the hospital or doctor by the help of a virtual appointment. This makes the environment conducive. In India, big hospitals like Max healthcare can also come up with the same system to improve the patient interaction with the hospital. Government can also come up with a plan to incentify the hospitals and doctors to use this kind of application.

As we are heading towards connected health, efficient communication between doctor and patient is just a small step towards faster healthcare delivery. The pace and evolving changes in the segment of healthcare extend right across the spectrum – from the development of new treatments and medicines to the front line delivery of innovative and paramount outpatient and emergency care. Surprisingly, it is at the front line where the most amazing advancements are happening and where the advantages are registering in the biggest volumes.

With this much advancement in connected health technology we have silos in the process. Healthcare providers apparatus needs to dismantle silos to usher in jointness communication between the doctor and patient. Can we leverage digital tech to manage patient life-cycle for convenience and safety? Max healthcare is also working on many measures to help the patient such as, billing transparency and estimation of the cost, bed allocation, risk assurance etc. Implication of all these is the journey/life-cycle of the patient improves. It gives a holistic care to the patients.

The core is to maximise value for patients: that is, achieving the best outcomes at the lowest cost. In this scenario the AI is helping the healthcare providers to move away from a supply-driven health care system organized around what physicians do and toward a patient-centered system organized around what patients need.

Growing demand for out-patient and at-home care along with connectivity advancements are also driving the use of devices powered by the IoT and AI. Today, privately-based medical insurers are making use of two-way smartphone applications for smartly connecting general practitioners with customers when it comes to initial consultation and diagnosis.

This serves as a cost-efficient option for avoiding a probably unnecessary in-person consultation for a minor ailment. The next steps ahead for this is to accurately and completely share vitals with medical practitioners on a real-time basis. This will be beneficial to enable multi-faceted tele-health services by making use of affordable devices for interfacing users together. It also proves useful for data transmission through connected devices like monitors, pacemakers, and other devices.

It is evident that the core of the connected health is made of technology, but we are not talking about just tools and devices here. This topic is relevant also from a philosophical, ethical and business point of view.

Government and private payer policies are shifting toward the use of connected health; We will likely see increasing adoption of connected health services by hospitals and providers as health care systems move towards holistic care to patients. It is the innovation and execution of information-based technologies that are likely to be transformative in improving health in a world of increasingly restrained resources. And in that effort, the newly empowered consumer is likely to lead the way.

This is a growing area of health care, and despite current challenges, presents significant opportunities for health care improvement. If you are not considering connected heath and Artificial intelligence in healthcare, now is the time!

DISHA – Need of the hour

Digital Information Security in Healthcare Act (DISHA) is proposed by the Indian government to secure e-Health data.

The act:

The Act mandates the clinical establishments to secure the digital health data and defines functions of the new regulatory bodies at the center and state level except the State of Jammu and Kashmir. Clinical establishments here means, any hospital, clinic, dispensary, etc., be it public or private.

Setup:

National Electronic Health Authority (NEHA) and State Electronic Health Authority (SEHA) will be setup as the governing bodies to formulate standards, operational guidelines and protocols for the generation, collection, storage and transmission of the digital health data for this Act. It will also ensure data protection and prevent breach or theft of digital health data. It will establish data security measures for all stages of generation, collection, storage and transmission of digital health data, which will at least include access controls, encrypting and audit trails.

Digital Health Data:

Digital health data comprises of one’s physical or mental health condition, sexual orientation, use of narcotic or psychotropic substances, consumption of alcohol, sexual practices, Human Immunodeficiency Virus (HIV) status, Sexually Transmitted Infections treatment, and abortion etc;

The required health data can be obtained by consent from the owner, thus informing them the purpose of collection, identity of the recipients to whom the health data may be transmitted or disclosed, identity of the recipients who may have access to the data on a “need to know” basis.

Major points:

As per the draft, the owners have the right to privacy, confidentiality, and security of their digital health data and the right to give or refuse consent for generation and collection of such data.

The Act also lists down factors affecting data transmission as to who can transmit, how they can transmit and monitoring data transmission. The Act further lists down the guidelines on accessing this data, with regards to who can access, how they can access and purpose of data access by various entities.

Penalties in contravention of serious breach of healthcare data shall be punishable with imprisonment, which shall extend from three to five years; or fine, which shall not be less than five lakh of rupees.

There are many debatable points that arise from this Act such as the technical measures a clinical establishment should take, standardization of data security, measures to be taken in times of breach, training and capacity building of the clinical establishment, best practices of data collection and storages.

The clinical establishments might be worried on implementing the Act, as they might lack the technical resources to bring the robust solutions. On the other hand the security industry in the country might be looking to engage clinical establishments and respond to this situation by providing cost effective solutions and safeguarding the privacy of the patient.

To implement the Act, clinical establishment might need hand holding. However, we appreciate the efforts of the government to propose this Act and safeguard the interest of patients and citizens.

TELEMEDICINE: THE JOURNEY SO FAR…

Telemedicine: The journey so far…

It has been a lifetime experience to venture out into the world of Telemedicine post retirement from Army after 2013. The government of Rajasthan restarted the Telemedicine program by designating the Mahatma Gandhi hospital telemedicine centre as the nodal centre for all district hospitals.

The telemedicine team in Mahatma Gandhi hospital started with the Rajasthan government project of connecting to all district hospitals in the state. Our team visited a number of district hospitals and contacted all of them on phone. With lot of Persuasion we have managed to connect to 14 district hospitals.

Telemedicine consultations were sporadic as the workload in the district hospitals is overwhelming. We realised that it would be prudent to reach out to the rural population through NGOs and private clinics. The private clinics and NGOs were more than willing to participate in spreading Telemedicine.

Telemedicine awareness workshops were planned and audience we sought were Gram Panchayat members, lay population and paramedics. Our first workshop was in Chaksu, a small town about 40 kms from Jaipur organised by a NGO. The Gram Panchayat Pradhanalongwith a couple of Sarpanchs from nearby villages attended the workshop. Their queries started when we had a live demonstration of a telemedicine consultation. The enthusiasm shown by the audience motivated the team further. Next stop, after a couple of weeks,was in Roopkabas village in Alwar district.

The crowd that gathered was eager to understand what Telemedicine was. Waiting for connectivity a computer presentation was made. A lot of questions came up and a few villagers sere keen to consult physicians in Mahatama Gandhi Hospital.

As the Internet connectivity could not be established we moved on with a promise that the day the village has proper internet connectivity we will set a Telemedicine node there. Ratangarh, Churu district workshop was organised by Goswami Medicare clinic. The audience had a first experience of telemedicine consultation. Our last workshop was in Rawatsar, Hanumangarh district organised by SafeCare hospital, which has been using telemedicine services for past year or so. Great response from the audience travelling to these rural areas was full of thrilling and anxious moments. At Roopkawas the rains damaged the road and at places our vehicle got stuck. Ratangarh had flash floods with water rising to 3 to 4 feet and coming into the clinic where teleconsultation were taking place. Rawatsar journey was though the desert and spotting chinkaras (deer) in herds was a great sight.

To sum up there was tremendous and wholehearted support from the NGOs and private clinics to set up telemedicine nodes. The rural population was more than willing to use this facility as their out of pocket expenses would be reduced considerably and they would get services of Specialists. The need is for a robust infrastructure in terms of trained manpower, Internet connectivity and willingness of urban hospitals to provide teleconsulations.

Our next venture will be participating in school health using the telemedicine kit as an educational tool.

I thank my team Dr. Dhanajay K Mangal, Dr. Vikas Gaur and Surendra Singh for having participating in these Telemedicine Awareness workshops. It has, indeed, been a journey with lot learnings and to meet the large hearted villagers who look forward to this mode of health care delivery media.

When companies, governments, universities and healthcare institutions work in tandem to push the frontiers of knowledge, they become a powerful engine to find innovative solutions to major social problems including health.The article talks about the healthcare scenario in Sweden and how the country is reforming its healthcare system to benefit its people.

Across Europe the rising cost of healthcare,caused among other things by the ageing of the population and the parallel rise in chronic illness; has made policymakers support the development of more effective preventive measures and early diagnosis products. At the same time, patients have started to take more responsibility for their own health, treatment and care, thus becoming a major cost driver of the healthcare system.

Innovation within material sciences, genetics, biotechnology and computational information has escalated in recent years, bringing significantly improved chances of

surviving disease. In Europe and especially in Sweden, patient-centered care, evidence-based care, and early detection are dictating the path to follow for designing the healthcare of the future.

Sweden spends about 11% of its GDP on health care and nearly 80% of it is publicly financed. Healthcare expenditure as percentage of GDP is higher in Sweden compared to many countries of similar or larger size.

The following are some of the major innovations and reforms introduced during the last years for containing costs.

• A shift from hospital inpatient care towards outpatient care at hospitals and primary care facilities

• More choice of provider, competition and privatization to support the development of primary care

• Regionalization of health care services including mergers between county councils

• Privatization of the pharmacy sector

• Continued specialization and concentration of services within the hospital sector

• National reforms for shortening waiting times for services

• Cost-effectiveness and social perspective are becoming key indicators for reimbursement.

Health economics is a priority for the 21 county councils and 290 municipalities in charge of health provision in Sweden. Some of the latest initiatives include:

• New mechanisms to support evidence-based and cost–effective vertical priorities

• Health outcomes and benefits from the patient perspective

• Process orientation

• New valid performance indicators

• Increase abilities to monitor performance on a regular basis by investments in health quality registers and new information technology

Cost-effective methods for early diagnosis and treatment can anticipate future societal costs

The Swedish Cardiopulmonary Bioimage Study (Scapis)

The Swedish CardioPulmonary BioImage Study, one of the largest studies of its kind in Sweden, is a major joint national effort to reduce mortality and morbidityfrom cardiovascular disease (CVD), chronic obstructive pulmonary disease (COPD) and relatedmetabolic disorders, all of which are important issues for public health. A Swedish cohort of 30,000 men and women aged 50-64 years are characterized with help of advance imaging techniques (Ultrasounds, MRI, Computer tomography, 3D ECG, etc.) together with information obtained by proteomics, metabolomics and genomics.

A comprehensive pilot study with over 1000 patients were completed at the

Sahlgrenska University Hospital in West Swedenin 2012 and recruitment to the nationwide multicenter study is ongoing.

Centre for imaging and intervention at Sahlgrenska University Hospital

In 2015, a new centre for imaging and intervention opened at the Sahlgrenska University Hospital, one of the largest hospitals in Northern Europe.

The center is unique among the Nordic imaging centres in that it was initiated by a hospital, rather than by academia. The regional health authorities have recognized that advanced imaging and intervention are critical not only to the provision of the best healthcare possible but also to making the care cost-effective. The newly built 21 000sqm-center houses Hi-tech operating rooms with advanced imaging equipment such as gamma and PET cameras, X-rays, computed tomography, ultrasound and magnetic resonance.

Academy, healthcare, county council and Industry are already collaborating in a model in which technology development and transfer can evolve from the traditional ‘pull’ and ‘push’ model to an integrated process in which problems and solutions are addressed on a day-to-day basis.

The use of microwave techniques for the treatment of hyperthermia of deep seated cancer tumours; breast cancer detection with microwave tomographyorstroke diagnostics are some of the cross-disciplinary collaborations that have resulted of this long-term strategic triple helix collaboration.

Smart textiles with medical applications

Companies, universities and research centers in western Sweden have been collaborating for almost a decade now around the design, development and production of the next generation of textile products.With Smart textiles, a large part of the health monitoring can be done from home in the future. One example is a regular shirt with integrated sensors that can be used to measure breathing and heart activity and thereby reducing the number of visits to the hospital. Textile based sensors made of conductive fibres incorporated in clothing to record electrical activity from the heart (ECG), brain (EEG), or muscles (EMG), might become part of the future health care of patients suffering from sleep apnea, breathing difficulties or stress-related conditions.

When companies, governments, universities and healthcare institutions work in tandem to push the frontiers of knowledge, they become a powerful engine to find innovative solutions to major social problems including health.However, product innovation, value networks and ecosystems alone will not be enough. How we innovate and connect with customers and patients is crucial. We need to involve patients and patient organizations in the innovation process and decision-making. We need to take the time to see the patient in all aspects of his life. It might be the best alternative for both providing more quality care and at the same reducing the spiralling healthcare costs.
