Dr. V K Singh gave a brief overview of the current technology and scenario on healthcare innovations and delivery. He presented his idea of understanding the 3 ‘P’s i.e. pain, (of) patient, (of) provider to deliver Healthcare at optimum cost and as well as scaling up the availability of care.

Without taking much time, he addressed all the stakeholders and also identified the knowledge gap that still exists and recommended increased research to fill them up through holistic approach along with further action to encourage innovation in healthcare.

Prof. Anil Kumar Gupta, Visiting faculty, Indian Institute of Management, Ahmedabad and Founder, Honey Bee Network, India gave the keynote address for the session and looked at the challenges in the areas of health. He acknowledged that, in recent times, there had been a growing awareness of health but said that more still needed to be done to allow further implementation of the necessary action. He then explained how, sharing and caring would result in good health, a significant integrated share and care approach should be adopted, thanks to the platform provided by InnoHEALTH 2018. This had enabled various stakeholders to come together. Questioned the gathering by asking them we (docs) only meet sick people, then how can we know what health is? He came up with an idea of conducting an event in which grandmothers who never visited the hospital. He was surprised by witnessing the number of grandmothers who did not visit hospitals in their lifetime. He observed that most people who live up to the age of 100 are women and unlikely to be the man. Spoke of how a healthy environment should be and considered Health as an ecosystem of both sick and healthy people.

He researched on the soil whether it had any impact on the health of a person since the cycle goes like
soil -> plant -> animal -> milk -> human

He also emphasized that there are many ways to make affordable healthcare which should be adopted in small scale at least if not on large scale. For example, a student uses iron fish in his curry which gives him 126g of iron. Surveyed and found 5 healthy states, the common thing in them was that the mothers were breastfeeding the child for 3 to 4 years and the vegetables consumed (edible wheat). We are having 10,000 to 15,000 patients every day we have enough data too. Why aren’t we using the data? Another area where development is required is in supply Chain – the challenge of innovation in healthcare, how do we connect the innovator and the user? On one side we have solutions and innovations but the scaling up foundations are less in numbers on the other side which creates the gap. Study of healthy people should be done. “Innovation comes with originality” – Prof. Gupta, He wanted the medical education system should be devised which will be helpful to all. Talked about the great scope of InnoHEALTH in this context as a platform for change. Ended the speech by throwing a question to all present there.“Innovations are coming up frequently but how to make it available to the world at large.

His excellency Klas Molin, Ambassador of Sweden to India, who was the Guest of honor, shared the healthcare experiences of Sweden. The healthcare delivery was very important in their election campaign. He expressed the concern of time taking cures for which they are looking forward to solutions. Mentioned the importance of prevention as the secret of healthcare lies in focus on prevention. The duo of government academia and the private sector should be encouraged for results in healthcare. Sweden has a separate council regulating the innovations named as National innovation council. The action plan in innovations -MOU was signed between Sweden and India. Showcased the country ranking among EU in global competitiveness and innovation.

Srijan Pal Singh CEO and Co-founder, APJ Abdul Kalam Centre – Special invitee, started with a short video about late Dr. A P J Abdul Kalam to commemorate him and his work, showing his inspiring moonshots and contributions in the healthcare sector. Srijan explained that Dr. Kalam had been a strong person with a vision and a great motivator and original driving force behind the Dr. Abdul Kalam center. Shared a memorable incident encountered by him with late Dr. APJ Abdul Kalam in Kentucky, where Dr. Kalam was asked a question by a young student – “you have been in many positions like president, missile man, nuclear power, and many more as the list bigs.

Which of these gave you happiness or bliss of the above positions?
Dr. Kalam opted a hidden option that is option E.

In 1980, Mission AGNI was a tough task to achieve as the nose cone of the missile was a big challenge to design as there was a lot of heat as well as the pressure acting on it. They couldn’t use titanium also since the missile had to be very light in weight, then found a carbon-carbon composite which solved the purpose. One day his friend called from Hyderabad pleading his help for the children who were physically challenged and asked for a solution for their prosthetics. Kalam suggested why not make them a carbon-carbon composite prosthetics, which made the children very happy as the prosthetics were very light and they could now easily walk, run and didn’t feel the weight. The happiness in their faces made kalam sir happy. That gave him happiness by seeing the kids happy.

The message he wanted to is human compassion is strength and no joy can replace that.

He then suggested, Art + music + soft skills + medicine + healthcare should be taken into the mainframe, a holistic approach should be adopted in healthcare. He Started a non-profit organization named after the missile man to try achieving the vision which Dr. Kalam dreamt of.

Came up with a unique idea of collecting engineering solutions for the upcoming challenges. Illustrated how poor and remote area people aren’t getting proper drinking water by which around 3000 people die every year. Mission to get all the citizens free and clean drinking water have been started and the PIL will be filed on Oct 15th, 2018.

Book ‘‘Innovations in Healthcare Management: Cost-Effective and Sustainable Solutions”, written by VK Singh & Paul Lillrank, bags coveted Shingo Publication Award

A Book “Innovations in Healthcare Management: Cost-Effective and Sustainable Solutions”, jointly written by an Indian Doctor and a Finnish Professor, Dr. VK Singh and Paul Lillrank respectively, has been picked for this year’s coveted Shingo publication award.

The award recognizes and promotes writing that has had a significant impact and advances the body of knowledge regarding operational excellence. The Shingo Institute is most interested in recognizing thought leadership.

The book was published in the USA in 2015. Special Indian edition published in 2017.

Elated over the achievement, a beaming Dr Singh has told newspersons recently that a team of Shingo examiners judged the book on the various yardsticks, “and then observed Singh and Paul have written a thoughtful and engaging book describing various aspects of healthcare management innovations and the journey organizations have followed to streamline the delivery of healthcare while facing unique challenges. The book was written by the two authors using 34 contributors and compiled into 22 chapters of interesting content covering a wide range of topics. Most of the chapters provide a snapshot of approaches taken to improve healthcare management but do not delve into the roadmap for implementation.

The examiners said the book would be a good companion guide to stoke innovative thinking for those involved with a healthcare system.

Innovations in Healthcare Management covers a wide range of topics that can be used to create a Lean transformation roadmap. Not only does the book cover the systemic burning platforms shared by various healthcare organizations (i.e. improve quality, reduce cost, improve patient satisfaction), but the book puts a high value on healthcare improvement from the end-user perspective. Additionally, the book identifies opportunities that impacted communities, supply chain, and stakeholders.

According to them, the book adds to the existing body of knowledge by providing not only tools and techniques for transforming an organization, but also includes real-world examples of Lean transformation within healthcare organizations such as Tan Tock Seng Hospital in Singapore, Stanford Health Care, and the Mayo Clinic. Also, there are examples of Lean tools such as A3, Just in Time, and Quantitative versus Qualitative impact analysis.

The book provides a comprehensive approach to Lean transformation within diverse healthcare environments.

It is full of insights and supported with real examples that benefit the reader regardless of Lean knowledge or level of experience.

On the impact front, they said the demonstrated impact for Innovations in Healthcare Management on the body of knowledge is difficult to ascertain. The number of books sold is not reported. However, it can be inferred there is demand and there has been sufficient impact to justify a second printing with a special Indian edition.

The book has been prescribed in Indian Institutes of Management (IIMs) prestigious institutes established by Government of India. The authors indicate in the application that the book has reached all continents, there has been “very big demand” in the USA and Europe, and demand in India is “huge”.

There are 12 endorsements from a very impressive list of healthcare experts from the United Kingdom, United Arab Emirates, India, United States, Tanzania, Brunei, South Africa, Sweden, and the Cayman Islands. This information all indicates there is some level of impact being made. However, sales figures and the number of books sold is not available from the publisher or any other resource. A general online search does not generate any appreciable insight on demonstrated impact.

The examiners said the premise of the book is quite simple: “A lot can be done with a little. It is reducing healthcare delivery cost yet maintaining quality through technology, innovation, and various quality models like Lean and others.”

The authors’ statement that “trying to do more with more ends up accomplishing less with more” is the reason for any organization to embrace becoming Lean. The examples in this book describe the successful actions taken inpatient treatment goals of better healthcare in less patient time. The authors have done a great job of compiling a wealth of knowledge from across the globe to demonstrate how innovation, technology, and Lean principles can transform the delivery of quality healthcare and reduce costs.

Healthcare is a troubled industry all over the world. There are new approaches in this book we can all learn from. At the same time, what is needed in America may not be the same as what is needed or will work in India with over 500 million people and incredible levels of poverty. In essence, the book focuses on a country with lesser developed infrastructure, and therefore the problems encountered are not unique to India, but also not universal.

The institute which conferred the award said Innovations in Healthcare Management is a compilation of 22 articles from 34 contributors and two authors. The styles of writing used in the articles flow together quite well and contribute effectively to the authors’ objectives. The stories and examples provided throughout the book offer compelling ideas for understanding how to positively impact the delivery of healthcare from Britain, United States, Singapore, and India.

The book contains many simple illustrations and is grounded with facts and visual examples. All of the articles are well referenced. The summary at the end of each chapter provides a linkage back to the overall premise. The book is an easy read and effective at getting the reader to understand the concepts.

Examiners agreed that there are many takeaways in the publication. The authors successfully support the premise that more can be done with less. They prove that people can be very creative and innovative when faced with a meaningful challenge that can be embraced. They also effectively show the importance of relationship management between the patient, medical staff, and ancillary stakeholders and dispel the concept that patient care only occurs between the hospital and the patient.

Additionally, not all the important healthcare innovations have come from the USA, as India is becoming a leading innovator in healthcare, and others can learn from them.

It was a crisp winter morning when a motely crowd of innovators assembled quietly in a conference hall of the national capital’s prestigious India International Center to learn craft of presenting their case before government bodies, philanthropists and other agencies for funding.

With a promise to give an encouraging platform to innovators and aspiring Startups, InnovatioCuris had organised the second IC Innovator Club meeting on November 26, 2017.

A medium height simpleton emerged on the scene and occupied center stage of the Sunday morning show, but patient listeners within split of seconds shed off doubts whether spending a precious Sunday morning in such a meet was rewarding or a mere waste.

The club meeting brought some big names of healthcare including PHFI, INMAS (DRDO), Amity University, IIHMR to name a few, under a single roof including experts from investing agencies, Startups, policy makers, academia and many more.

The members quickly envisioned pots of money for their dream projects mainly strategized to fulfill unmet needs of common people and the industry.

One should avoid the following for a positive outcome:

Putting two different applications at the same time to same funder.
Doubling the funder (two funders for the same proposal).
Diverse and varied application should be avoided, instead thematic calls are better.

Glued to a widescreen where slides were scrolled for thumb rules while applying to such bodies for funding, innovators lent ears to Mission Director who at an ease rolled out drill for a successful applicant.

Batting with a straight blade, he minced no words to articulate the harsh reality that there was no scope to dodge funders to siphon money from them on bogus facts and figures. No cutting of corners; no double play with different funders; no concealment of facts; no new sets of facts and figures for same projects with different bodies; and no lobbying were among many during his opening remarks.

Dr. Shirshendu Mukherjee, Mission Director of Program Management Unit, jointly supported by the Department of Bio Technology, Bio Technology Industry Research Assistance Council (BIRAC) –BMGF(BILL & MELINDA GATES) Foundation and Wellcome Trust unveiled points which play crucial role in attracting funds from various bodies.

In life of a researcher or innovator one important pillar for the success of his idea is getting the right funding. Innovation is not only about ideas.It’s about making the ideas happen and sometimes difficult to convert a good idea on to a paper.

The agenda of IC Innovator Club’s second meeting was to share the views and knowledge on how to write a research or a grant proposal so that the innovator is successful in bringing his innovation to real world by implementing ideas.

Dr. Mukherjee insisted that art of communication is trick of the trade as in the world of tearing hurry ambience and cut-throat competition no one has time to flip through bulky dossiers.

According to Dr Mukherjee, “Winning a grant is how compelling story (of your idea) you tell”. He said writing a grant application is like narrating a story with once upon a time to a happily lived ever after. By this what he meant was, it is important to narrate the beginning, ending and all the important milestones to the funding agency very clearly and in chronological order with a proper flow of events to take place for the innovation to happen.

Dr. Mukherjee beautifully divided the prospective challenges of innovation broadly under three stages: Discovery, Development and Delivery. Major challenges under the discovery stage being to understand the problem, limited market pull and push in non-commercial indications and lack of local research infrastructure. Under the development stage, he told the major challenge faced by an innovator could be the translation of the idea to the product. Adding to it, lack of local development infrastructure, complex IP and regulatory pathway also contribute to the challenges faced during the development stage of an innovation.

Dr Mukherjee also told “One should keep in mind the unmet need of the market and the funding space available for area of interest”. About the dedicated experts and advisors being appointed in the funding agencies, he advised participants to seek advice on the funding space relevance of their proposal through these experts.

According to the expert, few of the challenges that one can expect during his project/work could be regulatory, IP or IT related, business plan development challenges, challenges in developing business exit plan etc.

Dr. Mukherjee encouraged the participants by saying “A successful proposal is 60% science and rest 40% is the foresightedness” that is how accurately down the line one can see and plan.

Sharing about Program Related investments, Dr. Mukherjee made a bold statement which could make an enormous impact in the applicant’s life. He said, “The applicant should also do due diligence on the funder, it is not only the right of a funder to do due diligence on the applicant, but also the applicant should do due diligence on the funder”. He also discussed few points on how one can perform the above said task, like visiting their respective websites, noticing what they have funded in last two years and others.

Best approach for applying a grant is ‘Two funder approach’. Approach is also a crucial factor in determining the progress of the application says Dr. Mukherjee. Benefit of doubt always rests in court of applicant, meaning if one of the reviewers is in favor the application and rest are not then also the applicant would get a chance to defend his application at first.

Further discussing about the ideas and points to be taken care of while drafting the proposal application, views were discussed on how one can write a comprehensive application with a suitable title and an impactful abstract.

Budgeting is very important in every aspect of life and so is the case while writing a grant application. The budget should be exhaustive and self-explanatory says Dr. Mukherjee. One should be realistic in quoting the budget and should avoid any kind of hidden costs or inflation in costing.

At the outset, I wish all our innovator community best wishes and exciting New Year 2018. At our end, we continue to add value through our knowledge activities of InnovatioCuris (IC). An international conference will be organised on October 5 & 6 2018. We also continue to arrange webinars, training, IC club meetings, magazine publication and launch of new initiatives like security audit of healthcare facilities. We have already published two books in the U.S.A. First being “Innovations in Healthcare Management: Cost-Effective and Sustainable Solutions” in 2015 which has been adopted by Indian Institutes of Management (IIMs) and University in UK as a reference book. It has now special Indian edition as well. The second is “Planning and Designing Healthcare Facilities: A Lean, Innovative, and Evidence-Based Approach” published in October 2017. We bring global leaders to our platform and share experiences to benefit all. We thank our members for continuous support to take all initiatives to the next level. We have already partnered with many credible organisations to work together.

As developed economies enter a period of slower growth, emerging economies such as India have become prime examples of how more can be achieved with less. We need to share best global practices to optimise healthcare delivery cost and can reduce concerns of patients and yet keep high degree of quality. We impart knowledge through various medium and presents new concepts, methods and tools to achieve low cost, high volume solutions that improve access to patient care by better understanding of transforming healthcare through innovations. Our focus is grass root innovation to benefit have nots by organising resources and flow to achieve with available resources. It also covers non-traditional ecosystems of innovation outside of pure technology, such as social persuasion, rural health delivery, exploitation of economies of scale, prevention oriented primary care, medical education, and hospital design.

The ecosystem is very conducive for innovations right now, with growing number of start-ups and open mindset. We should take advantage of the new initiatives launched by the government like Startup India, Make in India, Standup India, Skill India and many more to take innovations to logical conclusion. The government has created enough funds to support. Innovators should take advantage of these schemes to fulfil their mission. Keeping hospitals in the best of health

Hospitals will remain at the core of healthcare and with good quality medical facilities limited to corporate hospitals or a few of the government run large hospitals, altering the health map of the country has to start from the hospital premises.

Hospitals in India play a vital role in healthcare with most of the high-end medical facilities available only at the hospitals. But these large institutes are very far away from the masses as more than 70 percent of the Indian population lives in rural areas. That leads to a reality that medical facilities must go beyond hospitals, with an innovative approach, to serve the masses and make India a healthy place.

Addressing the issue as keynote speaker, Professor Krishnan from IIM, shed light on the management aspect of innovations and spelled out the needs of the industry for the sake of ensuring that all innovations become a reality and benefit the masses.

In his words, he said, “Innovations in healthcare must consider three aspects: Begin with identifying core functionalities of equipment without having to sacrifice on the safety aspect. Secondly, start from scratch. Build upwards and incorporate all the technical features that you feel are essential to the product; downscaling features will not necessarily downscale your cost; lastly, collaborate and team up with others who will complement your product, effort and intentions. There’s no one entity that has all the knowledge and resources to develop any product in its entirety. So build up on sound partnerships using complementary technology and process.”

In support of the view, Dr. S. Venkataramanaiah highlighted data usage as one of the most effective tools to implement an innovative approach for spreading good health. He said, “Data clearly indicates that there’s a huge demand for private players in the health sector. Everyone is and needs to be medically insured and ensured of good health by private or a government body. On the other hand, healthcare has some major deficiencies like availability of medical help at anytime and anywhere. Like you have roadside assistance available all over the country, such a facility for health and medical urgencies is still not available.

“So innovative approach in production, processing, delivering service or business management is the answer to finding solutions to new challenges faced in the health sector,” he said

In a more myopic approach to the topic of distance solutions Dr. Mukesh Taneja shared some startling data from the WHO.

“According to World Health Organization’s 2010 data 285 million people are visually impaired; 90 percent of these are in the developing world; and 80 percent of these are preventable,” he said.

And speaking of India, 70 percent of the population lives in rural areas, however 90 percent of the medical facilities are located in urban areas. So, there’s a huge gap between the care providers and the population who they should be reaching. And we believe that telemedicine – a very innovative approach — is the answer to bridge this gap. Eye problems can more effectively be pictured and online imaging of affected eye can easily be examined and diagnosed by a distant located ophthalmologist.
“We, at LV Prasad Eye Institute, extensively use this telemedicine platform and are reaching out to the rural population with the help of health workers and specially trained technicians who operate our remotely located Vision Centers.”

The next panelist, Dr. Santanu Biswas, from the UAE shared his international perspective and said, “If you are to work on remote distance technology, you cannot work in isolation. You choose partners in technology, data procurement and delivery system. This means your dependence on your partners is even more critical as you depend on their credibility, technical compatibility and similar intentions. This is the reason that even though we have been talking about digital health and e-health; but other than the fact that it’s been a buzzword for the last 15 years, it has not become a well-charted module. We are still talking about the concept and exploring innovations that will truly support the telemedicine or e-health platform. This is because we are trying to marry and merge a conventional health system with an imposed telemedicine process.”

Concluding his views on the subject he summed it up saying, “I think we need to rediscover a totally new innovative approach to telemedicine, which will use innovative technology, innovative approach and an all-new processing and delivery system. And this will evolve, as this is the future.”

Dr. Tarun Ramole, speaking in the role of an investor and entrepreneur spoke candidly about the business aspect of telemedicine. In his opinion, Telemedicine, as a business is a very lucrative platform. The gap between the resources and the users remains very high and in a country like India is not going to be easy to fill that.

“Telemedicine will be a very reliable option to bridge the gap and that makes it a very viable business option. But, like most businesses, telemedicine also has challenges that are very unique. Re-Shaping of Healthcare Through Innovations

The World economy is changing drastically from agriculture to manufacturing to services. Among all services, healthcare services are one of the significant contributors to the economic growth. Further, it has become easy to implement due to advances in information and communication technologies (ICT ), Internet of Things (IoT), efficient network of organisations. IoT is a network of organisations based on their complementarity strengths / resources available. IoT is connecting homes, cars cities as never before to make life simple and more comfortable [1].

Innovations has become critical capability of many business organisations including healthcare organisations. New technologies such as Internet of Things (IoT), nano technology, semiconductor products, stem cells, genetic engineering are driving the healthcare across the world. Among all, medical sciences have advanced significantly in terms of adopting latest technologies throughout the process of care delivery. Innovations in healthcare industry primarily aimed at improving quality of life, life expectancy, treatment options and cost efficiency.

Healthcare Innovation can be defined as the introduction of a new concept, idea, service, process, or product aimed at improving treatment, diagnosis, education, outreach, prevention and research and with the long term goals of improving quality of life, safety, outcomes, efficiency and costs [2].

There are several innovations happened in healthcare delivery. Some of these are given below (figure 1):

• Product innovation
• Process Innovation
• Service design Innovation
• Service delivery Innovation
• Business model Innovation
Innovations in healthcare improves adaptation and affordability.

Product Innovation is concerned with introduction of new types of goods and services for the external markets. This includes significant improvements in technical specifications, components and materials, software, user friendliness or other functional characteristics. Examples include wearable devices, portable ECG of GE, Intra Ocular lens of Aravind Eye Hospital, Jaipur foot etc. These innovations help in generating incremental revenues [2]. Some of the main products include patient monitoring devices, home care and health and fitness. Wearable devices market is growing at the rate of 4.8% on year-on-year growth [3].

Process Innovation is mainly implementation of a new or significantly improved production or delivery method. This includes significant changes in techniques, equipment and/or software. These innovations help in improving internal capabilities and quality improvement [2]. Some of the examples include use of computerisation of medical records, improvement in the use of medical equipment like in Aravind Eye Hospital where surgical equipment is being used for two tables.

Service Design Innovation mainly focus on design of man-machine system compatibility including customisation of devices and its functioning. For example, customising the pulse rate of pacemaker implanted to a patient. Examples include telemedicine, mobile healthcare units, small clinics in the vicinity of patients, collaboration with other stakeholders etc. Figure 3 shows

Service Delivery Innovation include the use of new methods in delivering the service and helps in cost reduction as well as turnaround time of care delivery. This include innovation in medication, surgical interventions, procedure etc.

For example, use of iphone for taking picture of eye and checking for preliminary diagnosis in place of a computer or other equipment. MRI, CT Scan, Cataract surgery etc. Figure 4 shows arrangement of eye check-up equipment on a two wheeler which can reach to the location where four wheeler mobile unit cannot reach.

Apart from these four innovations, one of the most important one is business model innovation. This is mainly focuses on number of alternatives and pricing of products and services. Examples include offering two or three options for surgical services along with other support services like room with air conditioner or without air conditioner etc. Providing enough number of winning alternatives refers to business model innovation.

Diffusion of innovations in healthcare requires acceptance of innovations by the different stakeholders mainly the care receivers (patients), care givers (doctors and others). This requires alignment of knowledge, technology and institutionalisation of practices. Innovations in healthcare helps government to provide conducive environment for innovation and collaboration opportunity between public and private organisations. Facilitate in implementing healthcare initiatives in large scale by optimising resources and their deployment.
