<p><strong>The immune system is a shield that guards against foreign invaders like bacteria and the virus.</strong> That’s the actual functioning of the immune system. When the immune system senses the existence of any foreign particle in the body, it immediately makes the antibodies to eliminate them. In autoimmune diseases, the immune system becomes a war zone. It misinterprets the body’s healthy nerves as foreign bodies and attacks them, reducing the ability to fight against infection and viruses.</p>



<p>One such condition is Guillain Barre Syndrome (GBS), one of the rarest autoimmune disorders that affect one in one lakh people in the USA. It affects the peripheral nervous system, severity ranging from mild weakness to depending on the life support system for breathing. Fortunately, there is 80% recovery after diagnosis and 5 – 10% delayed or incomplete recovery. It can affect anyone irrespective of the age group.</p>

<h2 class="Body" style="text-align: justify; text-justify: inter-ideograph; color: #a5a5a5; font-size: 22px; line-height: 1.7;"><strong><em>The reason for GBS onset is not known, but it is evident that the immune system attacks itself. The immune system produces antibodies when foreign bodies try to attack; these antibodies are nothing but the immune response towards the foreign bodies. But in the case of autoimmune disorder, when any viral or bacterial infection attacks the body, these bacteria or the virus start to resemble the nerves, and the immune system attacks the healthy nerves by mistake.</p>



<p>The onset of symptoms can be a very mild tingling sensation in the limbs to severe breathing difficulty. It starts from the lower body and moves up to the lungs leading to breathlessness, and sometimes occasionally, it starts from the upper body and moves down the lower body. The severity of the symptoms may vary from person to person, depending on the level of nerve damage. Weakness leading to restriction in the limb movement seeks immediate medical treatment. &nbsp;</p>



<p><strong>Additional symptoms may include:</strong></p>



<ul class="wp-block-list"><li>Severe pain in the nights</li><li>Blood pressure as a secondary symptom</li><li>Indigestion and loss of control over bladder</li><li>Sometimes coordination problems due to nerve damage which fails to send signals&nbsp;</li></ul>



<p>The symptoms can get severe with the period if not treated immediately and sometimes may lead to paralysis, which can be life-threatening. When a person is affected with GBS, his nerves are damaged. Nerves are nothing but conducting wires. Nevers have axons covered with an insulating layer called myelin sheath that carries electric signals from the brain to the central and peripheral nervous system leading to immediate action or response towards the brains’ command.&nbsp;</p>



<p>In GBS patients, the myelin sheath is damaged and sometimes the entire axon results in complete failure in signal transmission, and the muscles start to lose their ability to respond to the brain’s command. Breathing can be weak, leading to a dependency on the life support system.&nbsp;Nerve damage may also cause abnormal transmission of signals that give spontaneous sensations called paresthesias, which is nothing but the tingling sensation that generates difficulty in limb movements.</p>



<p>The nerve damage occurs due to molecular mimicry theory, where the molecules of the nerve resemble the molecules of some microorganisms. Hence the microbes and the myelin sheath look similar, and when these microbes attack the immune system, they mistake the microbes for myelin sheath and attacks the healthy nerves treating them as foreign bodies. As a result, the immune system fails to recognise its nerves.</p>



<p>So the immune system basically produces antibodies for bacterial infection caused by <em>Campylobacter</em> <em>jejuni, </em>which attacks the axon in the motor nerves causing acute motor axonal neuropathy, which is one variant of GBS having acute paralysis, loss of reflexes but no sensory loss. This bacterial infection is caused by consuming contaminated food or uncooked food, especially meat.</p>



<p>This syndrome cannot be diagnosed in the initial stages due to the symptoms&#8217; variation and intensity. Generally, the physician will examine and check if the symptoms are shown on both sides. Initially, the reflexes start to slow down in the joints. Sometimes, these reflexes are absent as the velocity of signal transmission slows down, which brings us to nerve conduction velocity tests the ability to transmit signals. So in GBS, the cerebrospinal fluid starts to change, and there has been some evidence stating that in GBS patients, the cerebrospinal fluid contains higher protein levels than usual. So the sample is collected from the lumbar region to examine the fluid.</p>



<p><strong>Other diagnostic symptoms include</strong></p>



<ul class="wp-block-list"><li>Abnormal sensation like tingling in the feet along with weakness</li><li>Diminished reflexes&nbsp;</li><li>High protein levels in cerebrospinal fluid</li></ul>



<p class="has-medium-font-size"><strong>Treatment</strong></p>



<p>Generally, short-term therapies can treat it to reduce the severity and lessen the recovery time. So the treatment includes Plasma exchange, and the other one is high dose immunoglobulin therapy (IVIg). Plasma exchange removes the blood with the help of a catheter via veins. Fresh plasma is extracted and returned to the patient. Plasma contains antibodies, so the PE removes the nerve-damaging plasma and reduces the severity of GBS.</p>

<p>Immunoglobulins are the proteins that produce antibodies against disease-causing microorganisms. In IVIg therapy, the immunoglobulins are injected through intravenous infusion into the patient&#8217;s body. These immunoglobulins have been developed from healthy donors. Researchers have stated that this therapy shortens the recovery time, has fewer complications, and lowers the chances of the antibodies attacking its nerves by diluting with non-specific antibodies.</p>



<p>As the patients recover, they are moved to rehabilitation care for physical and mental health improvement. Physiotherapy for the muscles and limbs movement and therapy for mental health is a must to regain confidence during the recovery stage. It is a slow process, and the patient may tend to lose hope.</p>

	<p><span style="font-weight: 400;">The Human Vaccines Project (HVP) is a nonprofit public-private partnership with a mission to decode the human immune system and accelerate the development of vaccines and immunotherapies across major global diseases. The Project brings together leading academic research centers, industrial partners, nonprofits and governments to answer core questions about how the human immune system fights disease and pioneer a new era in human health.</span></p>
<p><span style="font-weight: 400;">The project has three pillars: the Human Immunomics Initiative, a collaboration with the Harvard T.H. Chan School of Public Health; the Newborn Immunity Initiative, a   partnership between HVP and the Telethon Kids Institute, the Newborn Immunity Initiative Cincinnati Children&#8217;s Hospital, and Université Libre de Bruxelles; and the COVID Vaccine Initiative (CVI).</span></p>

	<p><span style="font-weight: 400;">As always, the future will tell. However, I am optimistic that SARS-CoV-2 and COVID-19 will change things for good. This pandemic has taught us that an infectious disease agent can have an enormous impact on every aspect of life. The human, social, cultural and economic toll is enormous. It has been estimated that the current pandemic will end up costing between USD 8 and 16 trillion globally, but there is also an estimate that it will cost USD 16 trillion for the US only. Surely this has opened the eyes of decision makers that a continuous investment in infectious disease R&amp;D and the underlying basic mechanisms to understand infection, disease development and severity, and immune response is necessary. We now have platforms such as mRNA and others that have allowed us to develop vaccines faster than ever. These new platforms rested on decades of research and investment and are capable, as we have seen, of bringing vaccines faster to the public faster than ever. Despite these advances we still have much work to do on the research side in understanding how to generate immunity to disease that works across all populations and including the really tough diseases like HIV or malaria where we need effective vaccines.</span></p>

A shorter vaccine development cycle is raising eyebrows, especially around safety concerns, there are population groups missing in clinical trials that we discover as the vaccine is administered. If we were to do it again, how can we do it better? Also, what can be done to improve the communication around vaccine campaigns in general?<br />
</strong></h3>

	<p><span style="font-weight: 400;">It is important to realize here that the risks taken were financial, not on safety.  We were able to bring vaccines to market so fast because we had invested billions in the development of new platforms like mRNA in the decades before this outbreak. Second, governments around the world started building highly expensive manufacturing plants </span><i><span style="font-weight: 400;">before</span></i><span style="font-weight: 400;"> we knew the vaccines would work at a great cost and risk to capital. Usually the vaccine industry waits to see if the vaccines look like they will work before making such an investment stretching the development time to years. So the acceleration of the cycle was really due to previous investments and willingness to take financial risk.</span></p>
<p><span style="font-weight: 400;">I believe it has been shown that a quicker vaccine development cycle and market authorization do not have to go against safety. Safety is the key issue for industry, regulatory agencies and policy makers, and the vaccines went through the standard safety testing and clinical process. That said we have to acknowledge that risks from vaccines are few, but real.  The harms coming from vaccines are almost always seen in a short time—allergic reactions being the most common by far.  So large trials with shorter observation periods are adequate to find these even if they are unusual.  We also have systems for seeking very rare events after vaccines are in use (one event per million doses), and those are now being used.  But traditional vaccine and drug development has a lot of delays that don’t really add to safety of the product. That being said, policy makers and politicians should not put pressure on industry and regulatory agencies to move faster if that would jeopardize safety. </span></p>
<p><span style="font-weight: 400;"><strong>Secondly:</strong> Yes, population groups were missing in the clinical trials, which is common practice in vaccine R&amp;D: children, pregnant and lactating women, people with (severe) underlying health conditions were not included in the first trials. Elderly people were included, but not in very large numbers.  This means that additional studies are needed, as we are witnessing now: studies are carried out amongst immune-depressed patients, cancer patients, people with auto-immune diseases, children etc. This is not an easy issue to be fixed in clinical trials, because the golden rule is that you enrol ‘healthy’ volunteers, usually people between 18 and 55. This needs a lot of thinking and a careful balancing of risks and benefits if you are going to enrol vulnerable populations. Again, this is where post-marketing surveillance systems or additional clinical trials after the vaccine is being distributed, can help without slowing down access to a life-saving intervention. </span></p>
<p><span style="font-weight: 400;"><strong>Thirdly,</strong> improving communication around vaccine campaigns in general. Vaccine hesitancy is a big issue worldwide. WHO identified vaccine hesitancy as one of the top 10 threats to global health. The challenge for public health authorities stretches beyond COVID-19. I believe that we are entering a completely new paradigm around vaccination. Social media and social networks have become important shapers of the frame, as we have seen in so many countries with the introduction of the HPV vaccine; opinion makers and leaders &#8211; whether in media, science, politics, religion – can make or break vaccination programs; lack of consistency and trust are critical issues. </span></p>
<p><span style="font-weight: 400;">On top of that, given the enormous amount of time spent on reporting on COVID-19, people are becoming more knowledgeable about vaccines and vaccine development: herd immunity, level and duration of efficacy; side effects; halting trials because of (serious or severe) adverse events.  The public also is gaining an understanding of gaps in our knowledge after a vaccine has been licensed such as the level of protection against new virus variants, the efficacy of a vaccine in the aging population and in people with pre-existing morbidity. Last but not least, the top 3 categories for vaccine hesitancy (scientific evidence; knowledge and awareness; religion/culture/gender/socio-economic) remain relatively constant, but even the most cited category (scientific evidence – risk benefit) only accounted for less than a quarter of all categories cited, emphasizing the complexity and variability of vaccine hesitancy globally. So this is a daunting challenge for public health authorities. </span></p>
<p><span style="font-weight: 400;">I believe it is key to (re)establish trust. And trust is created by being honest / transparent, fair, reasonable and realistic. A recent study showed that the most important determinant for being vaccinated is that people believe and are confident that vaccines are important for their own safety and health. That in my opinion is going to be critical. Communicate clearly and consistently about the impact of vaccination on life expectancy; invest in understanding why some are hesitant and some confident; invest in making immunisation accessible, affordable and acceptable. Understand community dynamics and social and peer group pressure. I believe it is critically important to involve social and behavioral scientists in order to understand the social and behavioural dynamics.</span></p>

However, we would not survive if our body did not host its share of viruses. Wishing to be ‘virus free’, actually is a bad idea. The great majority of viruses are actually not ‘interested’ in humans at all, but in bacteria. These ‘bacteriophages’, or bacterial viruses destroy bacteria and keep bacteria populations under control. </span></p>
<p><span style="font-weight: 400;">Everybody carries around a huge number of microbes, known as the microbiome. The microbiome is made up of around 100,000 billion microbes, which is more than all the human cells of your body. Most of these microbes are bacteria, and they are vital to your health. Nevertheless, these bacteria need to be kept under control. That is why the bacteriophages in your body outnumber the bacteria 10 to 1. Nature uses bacteriophages as a sort of ‘biological pest control’ for bacteria, which prevents humans from being overwhelmed with bacteria. We would die without viruses. </span></p>

Additionally, a chemical in ginger called Gingerol helps decrease inflammation and blocks nerve pathways that process pain. Consuming ginger may help limit nausea and motion sickness and may help treat arthritis by decreasing inflammation. It also contains small amounts of fiber, essential vitamins and minerals, but due to the small amount usually consumed in any given meal, it is unlikely to greatly affect your nutrient intake. Macro nutrients &#8211; one-fourth cup of ginger contains 19 calories, 0.2 grams of fat, 0.4 grams of protein and 4.3 grams of carbohydrate, including 0.5 grams of fiber. Higher consumption may cause heartburn,stomach upset and diarrhea and may interfere with certain medications, including blood thinners, diabetes medication and blood pressure medications. Vitamins &#8211; eating 1/4-cup of raw ginger would provide you with 2% of the daily value for both vitamin B-6 and vitamin C, as well as smaller amounts of niacin, riboflavin, folate, thiamine and vitamin E. Vitamin B-6 is necessary for metabolism, immune function and red blood cell function, and vitamin C is important for healing wounds and for limiting damage to your cells from free radicals. 	<p><b>Nutrition And Cognitive Health</b></p>
<p style="text-align: justify !important;"><span style="font-weight: 400;">A child belonging to the lower socioeconomic status, not only has to struggle with a lack of educational opportunities but also a lack of healthy nutrition. This child will most likely go through their formative years having a poor diet which lacks most essential nutrients. This may lead to a lower academic performance throughout the school years, which in turn may lead to fewer job opportunities resulting in lower socioeconomic status. When the same child grows up and starts own family, the cycle continues to the next generation. On the other side of the socio-economic spectrum, a child belonging to an affluent urban family also deals with poor nutrition but in a very different way. More than one-third of such children and adolescents, aged 6yrs to 19yrs, are considered <a href="https://innohealthmagazine.comwell-being/contracting-lifestyle-disease-adulthood/">overweight or obese</a>. This is because these children are overfed but remain undernourished. Studies show that more than 90% of children in urban areas, belonging to affluent families, consume more than the recommended amounts of saturated fats and added sugars. The consumption of fruits and vegetables in these children is also observed to be less than the recommended intake. Thus, even though they may appear to be well nourished, they have many underlying nutritional deficiencies. Some of these deficiencies impact the performance of these children in academics and extracurricular activities.</span></p>

	<p><b>Relationship Between Breakfast And Academic Performance </b></p>
<p style="text-align: justify !important;"><span style="font-weight: 400;">There have been many studies that have looked at the relationship between breakfast consumption and academic performance in children, and most have concluded that </span><span style="font-weight: 400;">eating breakfast improves cognitive functions like memory recall. A study was done in Norway which looked at 475 high school children, found that those who ate breakfast regularly had fewer difficulties in learning, especially mathematics, besides improved reading and writing. Another study carried out in China on kindergarten students found that kids who ate breakfast had higher IQs (intelligent quotients). This was independent of the education level of the parents, or the family’s <a href="https://innohealthmagazine.comresearch/socioeconomic-inequalities/">socioeconomic</a> status. Many similar studies carried out over the years have reported that skipping breakfast was associated with decreased cognitive performance (e.g., alertness, attention, memory, processing of complex visual display, problem-solving, etc.) among the students. Children who do not eat an adequate breakfast can also suffer from hunger pangs while in school, headaches, and stomach pains due to excessive acid, etc. This, besides being a source of distraction for children, can also lead to absence from classes, which in turn can affect academic performance in school.</span></p>

<p style="text-align: justify !important;"><span style="font-weight: 400;">Interestingly, studies have not just found a positive association with breakfast consumption but also between the quality of meals and academic performance. A study carried out in Scotland investigated the consumption of sugary drinks, sweets, chocolates, pizza, savory snacks and hot dogs with learning abilities. The study found that children who had a higher consumption of these foods had increased mathematical difficulties as compared to children who had a more nutritious dietary intake. Another study found that children who ate <a href="https://innohealthmagazine.comwell-being/consumption-of-fruits-cuts-death-risk/">healthier foods</a> with a low glycemic index had better attention span, word recall and reaction time than children who consumed food with a higher glycemic index that leads to <a href="https://innohealthmagazine.comresearch/new-reading-high-blood-pressure/">high blood glucose levels</a> soon after consumption Sub-optimal intake of foods, such as <a href="https://innohealthmagazine.comwell-being/consumption-of-fruits-cuts-death-risk/">fruits</a>, vegetables, and dairy products, has also been associated with lower grades among students.</span></p>

Today, many mothers with children as young as 10 years old, feel that enrolling their children in extracurricular activities will take time away from their studies and result in poorer academic results. On the contrary, studies have shown that students who are physically active, perform better in school, compared to children who are physically inactive. These kids not only have better grades but also better attendance and fewer absenteeism, better memory recall, concentration during lessons and lesser disciplinary problems. Physically inactive children who gradually increased their activity showed improved memory and cognitive function than when they were physically inactive.</p>

	<p><b>Relationship Between Specific Nutrients And Academics Performance</b></p>
<p style="text-align: justify !important;"><span style="font-weight: 400;">There are specific micro-nutrients that have been associated with better academic performance.</span></p>
<p style="text-align: justify !important;"><span style="font-weight: 400;">Children with deficiencies of micronutrients like vitamin A, B6, B12, folic acid, iron, zinc, and calcium, have shown to score lower grades and result in higher absenteeism. Two nutrients, known to have the strongest relation to school performance, are iron and zinc. Iron is an integral part of hemoglobin which is the oxygen-carrying molecule in the blood. Deficiency of iron is called anemia. Symptoms of iron deficiency include fatigue or tiredness, lethargy, slowed growth and development, poor appetite, abnormally rapid breathing, behavioral problems or disciplinary issues and susceptibility to frequent infections. All these individually and collectively can lead to increased absenteeism and thereby decreased academic performance. Iron deficiency also directly affects the ability to undertake physical activity, which in turn can further affect performance in school. Zinc is mainly needed for the body’s immune system to work properly. In addition, it is involved in cell growth, wound healing and for the sense of taste and smell. Symptoms of zinc deficiency are <a href="https://innohealthmagazine.comtrends/exercise-in-big-parks-can-reduce-depression/">depression</a> and psychological disorders, </span><span style="font-weight: 400;">impaired immunity, impaired growth, diarrhea, delayed sexual maturation, alopecia or abnormal hair growth, impaired appetite, altered cognitive functions and altered sense of smell and taste. Zinc deficiency also leads to poorer performance in school. This can be due to the fact that children with zinc deficiency are more prone to catching an infection and falling sick, thereby missing classes, their ability to memorize is affected, there are psychological disorders that can affect learning, etc. So, how can parents ensure that they give their children the best advantage for improving academic performance? 	<p style="text-align: justify !important;">Have you ever heard someone say, “I’m exhausted”, or “I’m dead tired”? This is often how people unknowingly express the sensation of burnout. It is widely known that burnout is closely linked with <a href="https://innohealthmagazine.comwomen-corner/stressful-can-events-increase-womens-obesity/">stress</a>, but it is much more than simply being tired, and it is often the case that the seriousness of burnout is overlooked.</p>
<p style="text-align: justify !important;">Let us then ask the questions that need to be asked when talking about the subject of burnout. Is it possible to prevent burnout? How do we know when we have been exposed to either too much &#8211; positive or negative stress? How can we change our <a href="https://innohealthmagazine.comwell-being/contracting-lifestyle-disease-adulthood/">lifestyles</a> accordingly to make the meaningful and sustained change to make ourselves more immune to burnout?</p>
<p><strong>Stress is perception based</strong></p>
<p style="text-align: justify !important;">It is primarily our perception or evaluation of something that determines whether or not we become stressed. If we perceive an object to be too challenging or potentially dangerous in some way, the “flight” type of emotional and mental reactions surface, eventually influencing the body – this is the route of negative stress. If something is challenging enough and we deem the odds of success leading to a potential opportunity in our favour, the “fight” type of emotional and mental reactions occur, and we experience positive stress.</p>
<p style="text-align: justify !important;">Regardless of the polarity of stress (positive or negative), biochemically your body reacts in the same way. Whenever our subconscious autopilot system detects a stressor, adrenaline and cortisol are released from our adrenal glands located on top of the kidneys. Our blood pressure rises, our muscles tense, heart rate elevates, and digestion is inhibited, all preparing us to either fight or take flight. We all have experienced this feeling, like a sudden rush. However, being constantly exposed to prolonged episodes of stress impacts the health of our body and reinforces unhealthy patterns mentally as well as emotionally. It can weaken the immune system, cause more regular mood swings, and can manifest as if being ‘on the ashes’, ultimately resulting in burnout.</p>

<p style="text-align: justify !important;"><strong>I can handle everything &#8211; Always!</strong> Burnout usually works in either of the two ways, more frequently as a manifestation of chronic stress, or the result of one sudden life-changing event. Because burnout is closely linked with increasing stress, you may already assume that this isn’t a result of one single event but is rather a question of <a href="https://innohealthmagazine.comtrends/indoor-lifestyle/">lifestyle choices</a> and skills to handle the inner turmoil that gets triggered by certain events.</p>
<p style="text-align: justify !important;">Burnout can happen to anyone and everyone, however, there are certain types of people more prone than others. Those who are success or achievement oriented, so-called “career” people, managers and entrepreneurs tend to be more susceptible. They are the people who seem capable of anything. These sorts of people live for their work, strive for success and never switch off.</p>
<p style="text-align: justify !important;">They are burdened with numerous to-do lists and they check their work emails on a Sunday. They are always on the go and find it difficult taking time out. They validate themselves based on their achievements and getting things done. Even during their vacation, they feel the need to be on the go, filling their days with planned activities and engagements. The bigger picture is very often completely lost; they just don’t seem to be able to stop the lifestyle of ‘busyness’.</p>

However, their lives are filled with dangerous amounts of positive stress or eustress. Of course, this positive stress feels good in the beginning and is indeed useful because it helps to get things done and achieve goals. However, in positive stress ‘mobilizing us for action’ means the constant release of adrenaline and cortisol from the adrenal glands – the stress hormones.</p>

Most of the cases of burnout today are caused by people “riding the wave” of positive stress because people do not expect it to be dangerous – on the contrary, it is almost actively encouraged and often glorified.</p>

	<p><strong>Lifestyle Changes &#8211; No Easy Way Out </strong></p>
<p style="text-align: justify !important;">It is very much the common and accepted prevailing attitude of our modern working world that it&#8217;s perfectly normal to be “stressed”. It is no wonder then that more and more of us are facing burnout.</p>
<p style="text-align: justify !important;">In order to prevent or recover from burnout, we must adjust our whole lifestyle. Already the term “lifestyle” suggests that this is not just a minor phenomenon that can be simply fixed with a few good night’s sleep or a vacation. This is not a bacterial infection that can be cured by taking a few pills and then rushing back to our “normal” life where stress reaction is considered inevitable. It is this attitude that is what really needs changing leading to skills that help in handling reactions.</p>
<p style="text-align: justify !important;">One must consider both the physical and psychological sides, understanding that psychological arousal leads to a stress reaction in the physical body. It’s not just about reassessing values, setting priorities or time management. The most crucial elements to address are the chronic emotional and mental reactions.</p>
<p style="text-align: justify !important;">What can be done to prevent burnout from taking hold? By educating ourselves on how we function internally will help us further understand the problems associated with burnout. This will help you make better and more informed choices for yourself.</p>
<p style="text-align: justify !important;">Learning relaxation exercises, in particular stress relieving breathing exercises will also help, but in essence it’s already alleviating a consequence caused by a reaction. It is important to take time out during our busy days to just sit and simply be. It seems obvious, but many of us feel guilty for investing in sufficient leisure time. Make it a point to put aside a certain time each week to do that thing you have been meaning to do, make good on that promise to meet a friend, do something with your loved ones, or do something entirely for yourself. Just make sure you do it. Making sure we are getting 7-9 hours per night of continuous quality sleep can make all the difference in how we think and feel.</p>

	<p style="text-align: justify !important;">Scientists at Pune-based National Institute of Virology (NIV) have found a new genotype of dengue virus in patients who suffered due to one of the worst epidemics in recent years in Tamil Nadu. The study shows that the strain originated in Singapore and emerged in Tamil Nadu in 2012 and Kerala in 2013.</p>
<p style="text-align: justify !important;">Even a single cell bacterium has not only DNA but also cell organelles. But viruses are just bits of DNA material in a wrap, and they use cellular mechanism of the host to multiply themselves. In doing so they damage the infected cells. The immune system of the body responds with production of specific antibodies, which destroy specific viruses.</p>

	<p style="text-align: justify !important;">Dengue virus comes in different types and each with different flavours. There are four serotypes of dengue virus &#8211; DENV-1 to 4. Each of them has multiple genotypes. The genotype variation can be subtle either in DNA material or the envelope. For example, DENV-1 comes in as many as five genotypes &#8211; Asia, South Pacific, Thailand, Malaysia and AM/AF.</p>
<p style="text-align: justify !important;">When infected first time, the patient developed a life time immunity for that serotype due to presence of antibodies produced earlier. However, if the secondary infection is by another serotype, the immune system is confused, and the infection can become life threatening.</p>

	<p style="text-align: justify !important;">“Dengue virus has four antigenically defined serotypes and each serotype has multiple genotypes with several clades based on phylogenetic analysis of envelope or whole genome sequences” says D Cecilia of the Dengue Group at ICMR-NIV, Pune, who is the lead author of the new study. This makes developing an effective vaccine a major challenge.</p>
<p style="text-align: justify !important;">During the last five years, the National Vector Borne Disease Control Programme (NVBDCP) reported 80,725 cases of dengue per year with a fatality rate of about 0.24%. The ten-year data for Tamil Nadu, 2007–2016 show an average of 2539 cases per year with fatality of 0.21%. This is less than the national average. However, during 2012 the cases spiked to 12,826 and deaths rose to 66, which implied fatality of 0.51%. This was a clear signature of a dengue epidemic. Epidemiological studies indicated that Tirunelveli in Tamil Nadu was the epicentre of the epidemic.</p>

	<p style="text-align: justify !important;">Past research had shown that the emergence of new genotype was the cause of the major dengue outbreak in late 1980s in India. Taking a cue, scientists led by Dr Cecilia collected blood samples from patients in South India during the 2012 outbreak with the help of Vellore-based Christian Medical College. The genome sequence of the virus collected from the samples were compared against the GenBank library which hosts all known dengue virus samples from 1943 to 2015.</p>
<p style="text-align: justify !important;">The study revealed that DENV-1 Asian genotype had replaced the AF-AM type that was hitherto dominant in Tamil Nadu. “All four serotypes were circulating but DENV-1 was dominant, present in 52% of the serotyped samples” says Cecilia. Further, the study showed that the DENV-1 Asian genotype had also developed a new phenotype in the E gene. “This is the first time after 20 years we are observing change in genotype in India,” the scientist added.</p>

	<p style="text-align: justify !important;">DENV-1 originated in US and Japan during 1932. However, the Asian genotype of DENV-1 that emerged in Thailand. The Indian strains that caused havoc during the 2012-15 emerged in Singapore sometime in 2005. The same strain caused an epidemic in Singapore in 2005 and later in 2009 in Sri Lanka. “Phylogenetic analysis revealed that the Asian genotype was introduced from Singapore and shared 99% similarity with viruses, associated with large outbreaks in Singapore and Sri Lanka. The movement of DENV can affect dengue outbreaks and underscores the need for close molecular monitoring of DENV,” says Dr Cecilia.</p>
<p style="text-align: justify !important;">The findings are published in journal Virology. The research team included D. Cecilia, J.A. Patil, M.B. Kakade, A. Walimbe, K. Alagarasu, B. Anukumar, from  National Institute of Virology and A. Abraham from Christian Medical College, Vellore.</p>

	<p style="text-align: justify !important;">Due to adoption of urban lifestyles, vitamin D deficiency has reached epidemic proportions; 90% of patients we see are severely vitamin D deficient. The issue with Vitamin D deficiency is that patient complaints are extremely vague and possible to be missed by most doctors and sometimes even patients may not report them putting them down to the general wear and tear of life. Some can, however, get severe symptoms including stress fractures as well as neurological symptoms. Studies have shown that vitamin D decreases the risk by half in certain forms of Cancer like Colon and Breast Cancer. People with deficiency have been shown to have significantly higher incidences of various neurological disorders like Dementia, Depression, Multiple sclerosis, Psychosis and even Schizophrenia. The biggest plus point is that people who have good vitamin D levels have a much lower risk of developing Type 2 Diabetes, which is another epidemic we are facing in urban India.</p>
<p style="text-align: justify !important;">Vitamin D plays another important role; boosting the immune system. Multiple studies have shown that people with high vitamin D levels are less likely to get Influenza and Flu like infections. Other less well established but actively under research benefits of Vitamin D includes protection against heart disease, high blood pressure and Autoimmune Disorders. Vitamin D supplementation is a no-brainer. Supplementation is extremely easy and after an initial weekly dose can be consumed once a month. However, you can obtain it naturally; the only problem lies in the consistency of sun exposure. To obtain a day of sufficient vitamin D supply, you have to spend time in direct sunlight, approximately between 60 and 120 minutes with 30% body surface exposed.</p>
