	<p>The detrimental effect of the outbreak of COVID-19[the disease caused by Severe Acute Respiratory Syndrome coronavirus 2 (SARS-COV-2)] was evident not only on global healthcare systems but also on other aspects of human life.</p>
<p>The World Health Organisation (WHO) declared the COVID-19 outbreak as a global emergency on 30th January 2020. According to various scientific literatures, and a response to ‘flatten the curve’ governments have enforced border shutdowns, travel restrictions and quarantine in countries which constitute the world’s largest economies.All these factors have anticipated the fears of an impending economic crisis and recession.</p>
<p>The COVID-19 pandemic has caused an unprecedented challenge for healthcare systems worldwide with healthcare workers as the frontline warriors .<br />
In upcoming times there may be a higher surge in cases reporting critical situations or may be even fatalities due to previously existing illnesses like Chronic Heart Failure, Chronic Renal Failure, COPD, etc. getting complicated because of improper or not adequately managed.</p>
<p>Several reports have shown the predicamemt of patients having chronic diseases and those require proper medical maneuvering other than COVID-19 infected patients.The overall revenue generation for hospitals and healthcare sectors were greatly affected e.g an official data from a 350 bedded hospital in NCR reports more than 50 % reduction in bed occupancy rate and more than 60 % reduction in routine elective procedures thus making the system very fragile.</p>
<p>Maharashtra Govt. has undertaken 80 percent of private hospital beds with capping of treatment costs Another such step has been taken by the Delhi Govt which has directed the private hospitals with capacity of 50 beds or more to provide 20 % of its total capacity for COVID-19 patients.</p>
<p>With the prevalence of such regulations, adequate measures are taken to safeguard the workforce of the organization. With such regulations, it becomes foremost important to take adequate measures to protect our workforce amid this health crisis as they are the only weapons all through this fight. For enabling life towards getting near to normal , first thing that has to be made sustainable is the healthcare system.</p>

	<p>Non-COVID ZONED HOSPITAL gets divided in the following:</p>
<ul>
<li><strong>RED zone</strong>: Consists of Emergency Room , RED ICU and RED Ward
<ul>
<li>Every HCW working in Red Zone works with all precautions including PPEs. (LEVEL 4)</li>
<li>Patient treated as per his condition with all necessary monitoring and investigations thereby making things more clear</li>
<li>First COVID-19 testing done at Red zone</li>
<li>If patient is tested positive for COVID-19 then the patient is shifted to COVID center and if report comes out negative then patient is transferred to Yellow zone</li>
</ul>
</li>
<li><strong>ORANGE or YELLOW zone</strong>: Consists of another ORANGE ICU and an ORANGE WARD
<ul>
<li>Only patient once tested negative for COVID Negative gets entry to Yellow / Orange zone</li>
<li>Health care workers use at least level 2 PPE adjunct with an antiviral kit<br />
Patient are treated as per the symptoms with all necessary monitoring.</li>
<li>Clinical follow up of patient done with investigations like CBC to look for lymphopenia, coagulation profile to look for any derangement, LDH levels and serial Chest X-rays to look for radiological evidences .</li>
<li>Also any suspected patient to be kept in isolation or cabin with Negative pressure if available</li>
<li>Second COVID-19 testing done at Orange/ Yellow zone in case of any suspect either clinically or derangement in any of the significant investigations</li>
<li>If patient is unfortunately tested Positive , patient is referred to COVID Unit</li>
<li>If Second report also comes out Negative , then patient is transferred to Green zone</li>
</ul>
</li>
<li><strong>GREEN zone</strong>: consists of GREEN ICU and GREEN Ward
<ul>
<li>Patient that has been tested as COVID-19 negative twice is admitted in Green Zone</li>
<li>HCWs use N95 and Gloves only</li>
<li>Patients are treated as per his condition including all necessary monitoring and investigations</li>
<li>Patients gets discharged from Green zone</li>
</ul>

<li>Protection of healthcare workers &#8211; with double testing and proper zoning of hospitals depending upon the risk associated, can achieve a system where minimize of the infection among HCWs by using proper PPEs depending upon the zones</li>
<li>We will be able to cater to life-saving emergencies efficiently, also the routine services can be escalated with such kind of strategies</li>
<li>The hospitals can achieve a system where they can provide their services near to normal (pre-covid times) thereby reaching a sustainable system both in terms of services and revenue generation</li>
</ol>
<p>The current article mentions the zoning of a hospital for handling the emergency admissions, likewise for routine health services as well such zoning can be formulated and adopted. E.g. an elective surgery can be performed by performing a pre-admission COVID-19 test and then admitting the patient in ORANGE zone ward isolation rooms and then performing another test if required.</p>
<p>Recently there has been guidelines by Delhi govt suggesting a division of NonCovid hospitals into Risk Zones ( i.e mild , moderate, and severe ) addressing the issue of infection among HCWs and thereby suggesting the use of a different level of PPEs in different zones depending upon the risk associated . Also the advisory states formation of Holding area (corresponds to Red zone) for patients with influenza like illness (ILI ) in Non-Covid hospitals.</p>
<p>We appreciate the authorities for addressing the problem and taking such suggestions into consideration while forming the advisories and formulating the guidelines . Though there can be much more that can be done to achieve a more sustainable system which helps us fight the current pandemic in much more efficient manner as in our consideration achieving a SUSTAINABLE HEALTH SYSTEM ( HEALTH 2.0 ) is the only weapon amidst this crisis that can help us reach a safe bank.</p>

Part A includes A to G sections that need to be filled by the claimant and Part B is filled by the hospital. Part A of form informs the insurer about the incident, medical expenses, and details of the policyholder and dependent who needs coverage, details of hospitalization, and a checklist of documents that one needs to submit with the claim form to the insurer. <strong>2. Intimation &amp; claim within the time limits</strong><br />
The insurance company expects the insured to intimate the insurance company about the hospitalization 3 days before for planned hospitalization and within 24 hours in emergency hospitalization. Once the intimation is done, you can file a claim within 30 days from the date of hospitalization. On exceeding this time limit, you will have to submit the reason for the delay with your claim, failing which your claim can get delayed.<br />

<strong>3. Whether treatment is covered or not</strong><br />
Having a health insurance policy doesn’t mean that you can claim any medical expenses arising out of any medical conditions. Every health insurance plan comes with certain limitations mentioned in the terms and conditions of the policy. Before filing a claim, you must read the policy wordings which include the exclusion list and all the terms and conditions of your policy. Claim filed for anything that falls under the limitation or exclusion list will be rejected by the insurance company.<br />

<strong>4. Documents</strong><br />
After filing a claim form, you need to submit the documents as evidence and proof of the incident. The documents should be in the order that it should include the first consultation letter to all the medical bills related to medicine, hospitalization, lab reports and for which you are covered under the policy. The documentation process plays a very crucial role in your claim process as the entire decision of approval depends upon the documents you have submitted to your insurer. When people fail to justify the medical expenses by not submitting the supporting documents, the insurance company raises query which makes the process complex and time taking. It is important to note that all the supporting documents are to be submitted in original to the insurance company.<br />

<strong>5. Submit it to the right address</strong><br />
Now when you are ready with your duly filled claim form and all the supporting documents, it is time to verify the correct address of your insurer to send your claim. This point might sound obvious but let me tell you insurance companies have a specific address to receive the claim and process them. Make sure you find the correct address and send your claim via mail or submit by hand to the right address.<br />

Insurance companies take somewhere around 5 to 7 weeks to process your claim. Wait, your job is still not complete, you will have to do a regular follow-up of your claim with your insurer. First, you will have to verify if the insurance company has received your claim. Second, you will have to follow up for your claim id, which is usually generated by the insurance company within 3 to 4 days after receiving your claim documents. Third, take regular follow-up with your insurer for your claim status via call or online.</p>

This flagship project was launched on September 25. The project Ayushman Bharat-National Health Protection Mission (AB-NHPM) aims to supply coverage of Rs. 5 Lakh per family annually, benefiting more than 10 crore poor families. Poors and deprived rural families will be targeted under this scheme.</p>

	<p><strong>Eligibility and benefits of the scheme:</strong><br />
<strong>1.</strong> The benefit of cover Rs. 5 lakh per family per year. More than 10 crore families belonging to poor and vulnerable population based on SECC database will be the target beneficiaries of the scheme.<br />
<strong>2.</strong> The cover will take care of almost all secondary care and most tertiary care procedures.<br />
<strong>3.</strong> Under this project, the entitlement will be decided on the basis of deprivation criteria in the SECC database. The different categories in rural area include families having only one room with Kucha walls and kucharoof; families having no adult member between age 16 to 59; female-headed households with no adult male member between age 16 to 59; disabled member and no able-bodied adult member in the family; SC/ST households; and landless households deriving major part of their income from manual casual labour, as mentioned on the official website of Ayushman Bharat scheme-abnhpm.gov.in.<br />
4. Automatically included families in rural areas having any one of the following: households without shelter, destitute, living on alms, manual scavenger families, primitive tribal groups, legally released bonded labor, will also come under this scheme. For urban areas, 11 defined occupational categories will be entitled under the scheme.<br />
5. There will be no restriction on family size, age, and gender under &#8216;Ayushman Bharat&#8217; scheme to ensure that nobody is left out (especially women, children and elderly).<br />
6. Under this scheme, the treatment in case of hospitalization will be free of cost for the family.<br />
7. All pre-existing conditions will be covered from day one of the policy. The benefits cover will include pre and post hospitalization.<br />
8. The beneficiary will be able to go to public or empaneled private hospitals across the country and get free treatment.<br />
9. The beneficiary will only need to carry any prescribed ID to receive treatment at the hospital.<br />
10. According to Dr. Dharminder Nagar, MD, Paras Healthcare, &#8220;Government is acting strongly and actively in making &#8216;Ayushman Bharat&#8217; a reality.  PM&#8217;s announcement of rolling the policy under &#8216;Pradhan Mantri Jan Arogya&#8217; through a technologically driven transparent platform speaks of government&#8217;s vision. We hope that Ayushman Bharat paves way for a concerted effort on improving country&#8217;s healthcare facilities.&#8221;</p>

Amid growing awareness for the insurance coverage due to the high cost involved in medical treatment at super speciality hospitals, InnoHEALTH magazine interviewed seasoned player of the insurance discipline Mr Sethi, Vice President <strong>Insurance Foundation of India</strong> and sought answers for simple queries by people.</p>
<p style="text-align: justify !important;"><strong>Mr. Sethi</strong>, who is closely following recently announced National Health Protection scheme by the Union government for ten crore poor people and enthusiastically await roll out of the full scheme, said it would herald a new era in the health sector as the country has pan India facilities to implement the plan, but “ only little bit intelligent planning” is warranted.</p>

<p><strong><span style="color: #0071b2;">How is the Health Insurance Premium determines?</span></strong></p>
<p style="text-align: justify !important;">It depends on the age, sum assured, whether a person to be insured has any pre-existing disease or adverse health conditions. The rates are fixed (approved by <a href="https://www.irdai.gov.in"><strong>IRDA</strong></a>) and are available in the brochure published by the insurance company.</p>
<p><strong><span style="color: #0071b2;">My employer provides me with health insurance coverage. Is it advisable to take another policy on my own?</span></strong></p>
<p style="text-align: justify !important;">If the sum assured is good and the company is doing fine then you should think positive and do not go in for the additional sum. When you plan to leave the organization then you must verify from the new employer whether they have suitable sum assured available for you as part of compensation package. If no health insurance is available, then you must buy Health Insurance immediately so that 30 days cooling off period of the new policy (purchased by you) is matching with last 30 days with your employer. Try to have this health insurance from the same Insurance Company.</p>
<p><span style="color: #0071b2;"><strong>Will the agent be helpful in settling of claims in future?</strong></span></p>
<p style="text-align: justify !important;">Yes, your agent will be helpful. But he is getting 100% of his income from the insurance company and he may be getting .001 % of his income from you. If he has to make a choice, it is natural that he will favour Insurance Company. You are the best person to decide whether to buy directly from the company or through an agent. Theoretically, the agent is supposed to help you in claim settlement, but time will tell when the claim is to be lodged. Be positive and buy it through an agent but better buy through an Insurance Brokerage Firm as it offers a wider choice and is large enough to protect your interests.</p>
<p><span style="color: #0071b2;"><strong>How much should you invest in health insurance?</strong></span></p>
<p style="text-align: justify !important;">You have to invest according to your health the requirements and the resources you have. You can invest by deciding about your budget for health insurance. If your budget is not a constraint, you can invest by deciding about the sum assured and services required by you.</p>
<p><span style="color: #0071b2;"><strong>I might be laid off soon. Should I see my doctor before I lose my insurance?</strong></span></p>
<p style="text-align: justify !important;">No purpose will be served by seeing your doctor. You should consult an Insurance Brokerage Firm and buy a health insurance, the policy which should be issued at least 30 days before your last day in your present company. This protects your interest.</p>
<p><span style="color: #0071b2;"><strong>Will my health insurance coverage Clinical Trials?</strong></span><br />
We feel that if treatment is;<br />
1. Part of hospitalization in a reputed hospital like AIIMS, capable of conducting clinical trials under controlled conditions.<br />
2. Under the supervision of a reputed doctor<br />
3. Medical Council (Ministry of Health) approves clinical trials</p>
<p style="text-align: justify !important;">Then it will be payable. If it does not fulfill these conditions, then you may have difficulty in getting it paid as the issue is around treatment as well as the price of the medicine.</p>
<p><strong><span style="color: #0071b2;">I own a pet and my vet bills are sky-high. Is there such a thing as health insurance for animals</span></strong>?</p>
<p style="text-align: justify !important;">It depends on a pet. But definitely, it is not part of Health Insurance policy which is for human beings only. Cattle Insurance covers cow and buffalo.</p>
<p><strong><span style="color: #0071b2;">Should I pay my premiums through the a</span><span style="color: #0071b2;">gent</span><span style="color: #0071b2;">?</span></strong></p>
<p style="text-align: justify !important;">If he is ready to provide you with the service – yes. It is your duty that cheques should reach the insurance company well in time. Not picking up on the cheques by the agent is no excuse. If your cheque has not reached the insurance company on time, your policy may have a break and you may lose the benefits. You can also pay the premium online.</p>
<p><span style="color: #0071b2;"><strong>I already have a health insurance policy. Should I still buy a life insurance policy with the rid<span style="color: #0071b2;">ers</span>?</strong></span></p>
<p style="text-align: justify !important;">Health Insurance Policy covers your hospitalization needs. Life insurance covers your retirement needs. Adding of critical illness rider will take care of incidence of you getting a critical illness like cancer. Every product is different and fulfills different needs. Study it carefully and make a decision.</p>
<p style="text-align: justify !important;"><span style="color: #0071b2;"><strong>Are the new clinical and pathological tests, not available in India and done abroad covered under the</strong> <strong>mediclaim<span style="color: #0071b2;"> rule</span>s of the country? If no, are efforts being made to do?</strong></span></p>
<p style="text-align: justify !important;">Some insurance companies permit you to take the opinion. Your suggestion is very good, and we feel Insurance companies will start offering the same. If the Textile industry is permitted to get flammability tests done on garments in Hong Kong then why Insurance companies should not permit clinical and pathological tests (not available in India), which are to be conducted in the foreign country.</p>
