In 1990, the World Health Organization considered obesity as a global epidemic. And guess who made it go globally that vast? We humans did. Irregular eating habits, irregular sleep patterns, stressful life and not maintaining overall health is leading to an unsatisfactory life.</p>

An individual’s upbringing can be, the way the family raises, the daily food intake, importance placed on physical activity and how a person would take their behaviour model, are the bigger influences on likelihood of obesity than genes. Health is not valued till sickness comes, true isn’t? Obesity is not because it runs in the family!! It is because no one runs in the family!!!</p>

Yes,being overweight is linked to a number of health and oral health diseases, including: heart disease, high blood pressure, diabetes, cancer, gallbladder and gallstones, breathing problems, such as sleep apnea (when a person stops breathing for a short time during sleep) and asthma, oral health problems such as caries, halitosis, poor oral hygiene, periodontal disease. Symptoms for obesity include breathlessness, Increased sweating, snoring, inability to cope with sudden physical activity, feeling tired, back and joint pain, low confidence and self-esteem, and feeling isolated.</p>

<p style="text-align: justify !important;">After being aware of the risks, proper health and oral hygiene should be maintained. And why is maintaining diet an important factor? Because a poor diet may result in lack of essential nutrients and vitamins thus potentially making it more difficult for your body to fight off an infection. A diet high in fat and calories, including large quantities of processed foods or sugar, and lack of physicalactivity, leads to obesity. Those same foods can cause oral health problems including gum disease, premature tooth loss and bad breath Our body craves for what is in our bloodstream. Wheat, sugar, alcohol, coffee and de-vitalized foods are designed to be addictive. Sugar being addictive &#8211; agreed. But how does sugar affect an individual’s health and oral health? Well, consuming sugar is bad for an individual, but consuming too much has become a norm and that can cause health issues. The only type of sugar the body needs is glucose, which it can make by breaking down carbohydrates, proteins and fats. When ingested, simple sugars are converted into plaque if not removed immediately. As plaque begins to accumulate on our teeth and gums, the risk of varying forms of gum diseases such a gingivitis and periodontitis, as well as tooth decay, may become inevitable.</p>

That’s an unpleasant operation that is completely avoidable; we have got a lot of evidence to show that the main cause is excessive sugar. There are also links with type 2 diabetes, usually chronic disease seen in adults, but we are seeing more and more under the age of 25 these days.</p>

<p style="text-align: justify !important;">So, what an oral health care professional should be aware of? Dentists must be aware of how nutrition impacts general and oral health, and how dental treatment can impact patient’s nutritional status. Oral health is strongly influenced by the daily intake of food; on the other hand, oral health can also play a significant role in nutritional intake and general health status. The dental caries is also a chronic disease and may be depend on oral hygiene, high intake of sugar added to food, lifestyle factor, and host factors (salivary flow, buffer capacity and buccal microflora). Relationship between obesity and gum disease &#8211; we actually don’t know if this is a one-way or a two-way street. For example, conditions like diabetes, the relationship is bilateral. Periodontal disease will affect diabetes and diabetes will worsen periodontal disease, so that could be a possibility here. What are the other reasons of obesity affecting gums and what would be its risk factors? Because of various dietary habits and improper maintenance of oral hygiene, chronic periodontal infection is an inflammatory state that is described by a deviation in microbial environment and composition of subgingival plaque bio films and accelerated destruction of tooth supporting structures. Thus, low glycaemic diet may be beneficial to our oral health. Periodontal diseases are also influenced by various risk factors including aging, smoking, oral hygiene, socio-economic status, psycho social stress, osteopenia, osteoporosis and various systemic diseases, which signifies that periodontitis does not merely occur as a consequence of plaque deposition but is also coupled with various other host factors. Also, on examination, obese adolescents exhibited multiple carious teeth, higher plaque index, gingival inflammation, bleeding on probing and probing depth compared to normal weight subjects of similar age.</p>

	<p style="text-align: justify !important;">An obese patient, whose lungs are often not large enough to lift heavy chest walls, can have trouble during breathing, especially when active. Also, during sleep the person might breathe through his/her mouth instead of nose, thus jaw is forced into an unnatural position for a longer period of time. This can lead to teeth grinding and development of an overbite, over jet or spacing between the anterior teeth.How can we, as dentists, help the obese patients? Dentists should include the evaluation of BMI in routine patient examination and eventually refer obese periodontal adolescents for weight reduction interventions like diet therapy, behavioural therapy, pharmacotherapy, so that they can have better control over periodontal inflammation.What are the practical considerations in dentistry? It can be difficult carrying out dental care for an obese patient: the maximum lifting weight for modern dental chair is approximately 140kg (23 stones). The position of anatomical landmarks may be less obvious if surrounded by fatty tissue, the landmarks for placement of an inferior alveolar nerve block can be impossible to palpate through excess soft tissue. It may be impossible to palpate cervical lymph nodes in a large neck. Also due to constant secretion of saliva, the mouth and teeth are rinsed and kept free off food debris and foreign particles, thus excess of buccal pad of fat hinders with self-cleansing action in the mouth. Because of lack of saliva present in the mouth, it can lead to dry mouth. Sip water frequently throughout the day and use alcohol-free antiseptic mouthwash daily for cure. A normal sized mirror is unsafe for retraction of soft tissues: a ‘lax’ tongue retractor can be useful.What is the alternative to conventional dental chair? Refer the patient to the hospital environment where the patient may be treated on an operating table or trolley. Another possible solution for obese patients is a custom-made chair such as DIACO dental chair. What is the difficulty faced during conscious sedation? When thinking about conscious sedation for obese patients, the potential difficulties in airway management and intravenous cannulation should be considered. The provision of inhalation sedation with nitrous oxide during which oxygen levels are maintained at or above 30% may be more appropriate. If intravenous sedation with midazolam is proposed, the overall benefit to the patient must be carefully weighed up against the increased likelihood of significant respiratory depression and the difficulties in managing a respiratory complication. New research suggests that the health of your mouth mirrors the condition of your body as a whole. A healthy smile can surely transform our visual appearance and the positivity of our mind-set.</p>

	<p><strong>Increasing protein and dairy intake may help reduce burden of diabetes: A study</strong></p>
<p style="text-align: justify !important;">Diabetes &#8211; increase in blood glucose levels &#8211; is an emerging health problem, especially in developing countries. According to the World Health Organisation, India had 69 million diabetic individuals in 2015 and the number is projected to go up to 98 million by 2030. The problem is more serious for Asians as their genetic make-up puts them at a greater risk of diabetes at a younger age than their European counterparts.</p>
<p style="text-align: justify !important;">If left untreated, diabetes can intensify to serious health conditions like blindness, kidney failure, heart problems, etc. Apart from genetic factors, the occurrence of diabetes is related to food preferences and lifestyles. Therefore, understanding the link between the consumption of various foods and the prevalence of diabetes in different states can help in devising effective strategies to address the problem.</p>

<p style="text-align: justify !important;">A new study has linked the food preferences of individuals to the prevalence of diabetes in various states of India. The study has found that eating calorie-rich food like sugar and honey increases the risk of diabetes while the addition of protein-rich food and dairy products in a meal can help reduce the risk of diabetes.</p>
<p style="text-align: justify !important;">The study is based on data from the National Family Health Survey-4 (2015– 2016), 2011 census, data on per capita crop production, and consumption figures of different food groups from the 68th round of the National Sample Survey. It evaluated trends of food availability and preferences in various states with diabetes numbers.</p>

<p style="text-align: justify !important;">It was found that diabetes was more common in states like West Bengal and Kerala where people consume more non-vegetarian food than the one where the diet is predominantly vegetarian. This is because a vegetarian diet helps in diabetes management by making the body sensitive to the insulin hormone.</p>
<p style="text-align: justify !important;">Researchers also noted that there were more diabetics in the states where people consumed more sugar and honey though this trend was not valid in the case of Maharashtra, Rajasthan, and Haryana. The study also found that eating protein-rich food such as pulses and nuts decreased the chances of diabetes. Similarly, states, where people ate more dairy products, had a low prevalence of diabetes.</p>

<p style="text-align: justify !important;">Diabetes, often dubbed as a lifestyle disorder, was also found to be high in urban districts than in rural areas. This is so because people in cities tend to be less physically active. This makes urbanization an important factor for the growing number of diabetics in the country. Also, diabetes was found to be more common in males than females as has been indicated in the earlier studies.</p>
<p style="text-align: justify !important;">‘Our study has identified hotspot districts having a high prevalence of diabetes and recommended them to be targeted in public health programs,’ said Dr. Preeti Dhillon, author of the study and Assistant Professor at the International Institute for Population Sciences (IIPS), Mumbai. The analysis showed that 6.9% of adults in India between the age of 15-49 years have diabetes. Among the states and union territories, diabetes is highly prevalent in Andaman and Nicobar Islands, while the least in Rajasthan. The prevalence of disease was found to be more in districts that are close to the coastal areas. ‘The high prevalence of diabetes in Andamans and Lakshadweep is of interest as this has not been reported earlier and it would be worthwhile exploring whether any genetic or environmental factors contribute to this,’ commented Dr. V Mohan, Director, Madras Diabetes Research Foundation. Though he was not associated with the study.</p>

<p style="text-align: justify !important;">Dr. Anoop Misra, Vice President, Diabetes Foundation (India), commented that while this study provided new information about dietary context and prevalence of diabetes, it must be understood that diabetes is a multifactorial disease. Genetics, physical activity, alcohol consumption, and other lifestyle factors should be taken into consideration before establishing conclusive links. Food intake is a continuously changing and dynamic process and cannot be completely captured in a cross-sectional snapshot. He agreed, however, that high protein intake is needed for Indians as it boosts glucose metabolism in muscles reducing incidences of diabetes. The research team at IIPS included Koustav Ghosh and Gopal Agrawal apart from Dr. Dhillon. The results of this study have been published in the Journal of Public Health.</p>

<p style="text-align: justify !important;"><strong>Where there&#8217;s a will there’s a way.</strong> For a young man &#8211; Sachin Gaur, a medical urgency that too on an alien land sent things topsy-turvy for a while, but he envisioned latent prospect in the disgusting scenario. Soon after stepping out from the <a href="https://innohealthmagazine.cominnohealth-conference/innovations-for-hospitals-2/">hospital</a>, he summarily stitched an innovative group to combat challenges being faced by millions of diabetic patients globe over.</p>
<p style="text-align: justify !important;">Astoundingly, strange are the ways of nature to lend you a helping hand in a well-intentioned initiative. On way back home from Hyderabad, Sachin shared the flight coincidentally with one of the key chemists working with MSD for the last 22 years. He is a Harvard alumnus and was part of the team which designed the molecule called Januvia, which is one of the key drugs for diabetes. The whole gambit of <a href="https://innohealthmagazine.comtrends/needle-free-diabetes-care/">diabetes</a> and afflicted persons were discussed threadbare on board.</p>

	<p style="text-align: justify !important;">It was Sachin Gaur’s tryst with destiny &#8211; narration of the sugary tale. “My quest for <a href="https://innohealthmagazine.comresearch/night-shifts-may-trigger-type-2-diabetes/">diabetes cure and management</a>: a health accident that has given me a purpose in life.</p>
<p style="text-align: justify !important;">Surrounded by healthcare experts, my quest of promoting healthcare innovations could not have been better. Until I was diagnosed with high blood sugar and the week immediately after with a <a href="https://innohealthmagazine.comnewscope/national-medical-commission-bill/">medical emergency</a>, which has no direct link with my diabetic condition, but the treatment is such that I need to have a strict regime of a brisk walk, frequent meals, and schedule of oral drugs and insulin. As I am writing this I have spent 6 days in a hospital bed (which was the first time since I was born) and 14 days out of it. The condition I had, essentially, I felt reborn but this time it seemed with a greater purpose. So, I am writing this article &#8211; not as 35 years old but a 20-day infant.</p>
<p style="text-align: justify !important;">It seems the drugs that I am taking have made me as curious as a child &#8211; nothing seems impossible. I was able to synthesize a few lines on the hospital bed, which summarize my state of the mind. For English readers, this poem essentially means that I want to win my diet, in my mind and the exercise that this body can do. I need a resolve, determination, and courage from the almighty combined with the blessings of my elders and love of dear ones to defeat the voices of the world.</p>
<p style="text-align: justify !important;">With this thought process in my mind, the project “Experiments with Sugar” (<a href="http://www.experimentswithsugar.in/">www.ExperimentsWithSugar.in</a>) was born. With a team of 11 &#8211; from different walks of life: medical experts, researchers, a filmmaker, and more joining every day; we aim to enable 1 million people living with diabetes either cure or better manage their diabetes by the end of September 2021. I will be reporting the progress to you for the next three years via the InnoHEALTH magazine. This is the first social cause project we have undertaken at “<a href="http://www.icfhe.in">InnovatioCuris Foundation for Healthcare and Excellence” (</a>ICFHE), a not-for-profit organization as a mission mode project.</p>
<p style="text-align: justify !important;">With the support of our experts, I managed to meet with some of the top experts and we are getting a great response. To name a few: Dr. Rakesh Sahay, Head of Department at Osmania Medical College; Joseph Duffy, Executive Director, Head of A Neo Diabetic’s Tryst with Destiny &amp; Birth of Innovative Enterprise to Combat Challenge External Discovery Chemistry – Kenilworth, Merck &amp; Co.</p>
<p style="text-align: justify !important;">We plan to release a few episodes every month as a web series to provide scientifically correct information but in an accessible way to the common man. So, as a reader do guide us and send good suggestions our way. By the year-end, we also aim to produce a very high-quality report on the unmet needs of the stakeholders involved in the life of a person living with diabetes. This will then feed into the Smart India Hackathon planned in February 2019 as a diabetes track to support innovation in the country. We need suggestions for a planned roundtable in December 2018 in Hyderabad to validate our findings and further record the proceedings on the best scientific knowledge on diabetes.</p>

<p style="text-align: justify !important;">For the person who is extremely thin (i.e., your bones are protruding on some parts of your body), it’s more a problem of eating the basics for survival. These types need to carefully plan their daily meals in order to ingest the perfect ratio of fat:protein:carbs (30:40:30). 30% of the calories they ingest needs to be fat and carbohydrates, while 40% needs to be protein.</p>
<p style="text-align: justify !important;">This, coupled with moderate exercise, will help pack on some pounds gradually. People in this type of category need to change the way they view food. Food is not the enemy; it can be your friend. Actually, it can be your savior. Without basic sustenance from vitamins, minerals, fats, carbs, and proteins, the body will self-destruct.</p>
<h5><span style="color: #0071b2;">The ideal weight person</span></h5>
<p style="text-align: justify !important;">For the person who is already at his ideal weight, it’s more a problem of mental image and fear. They are usually so afraid of becoming fat that they deny themselves the smallest pleasures like a nice cold ice-cream cone on a hot summer’s day.</p>
<p style="text-align: justify !important;">Fitness buffs who are in top shape burn themselves out by exercising too much and not letting their bodies recuperate. They harbor this fear (whether consciously or unconsciously) that they’ll go over their ideal 10% body fat limit, and become the next Guinness World Record holder for the heaviest guy alive. Since these types are already hitting the gym 6 times a week and eating sensibly, the only thing they should change is their mental reasoning. Pushing the body too much too fast is never a good thing.</p>
<h5><span style="color: #0071b2;">The overweight</span></h5>
<p style="text-align: justify !important;">Finally, for the person who is obese (i.e., you can’t see your own feet while standing straight and looking down at the floor), it’s a matter of eating too much and exercising very little &#8212; if at all. This is when the calories start piling up and everything you eat gets stored as fat (yes, even protein and carbs).</p>
<p style="text-align: justify !important;">Following the aforementioned fat:protein:carbs ratio helps, but you also need to exercise at least 4 times a week for 45 minutes, doing a lot of cardio and aerobics in order to melt away those love handles and that large bottom.</p>

Eat a lot of vegetables for regulating your digestion.<br />

Eat sensible amounts of food at mealtimes. Do not deny yourself normal portions.<br />

	<p style="text-align: justify !important;">Brig (Hony) Arvind Lal is a pioneer in bringing laboratory services in India at par with the international world. He has modernized Indian medical diagnostics and initiated the first Public Private Partnership (PPP) in the field of laboratory testing in India. Under his guidance, Dr Lal PathLabs (LPL) has become one of the most reputed laboratories in Asia having to its credit quality accreditations from various national and international bodies. Currently they operate over 172 labs, including Asia’s biggest lab at Rohini, New Delhi with 1,500 collection centers and pick-up samples from another 7,000 medical establishments all over India. The lab tests over 50,000 patients every day or nearly 10 million patients in a year.</p>
<p style="text-align: justify !important;">He holds the honorary rank of a Brigadier in the Indian Army and the Government of India conferred him Padma Shri (Indian national award) in 2009, for his contributions to Medicine.</p>

	<p style="text-align: justify !important;">My late father Dr. Major S.K Lal started this lab in April 1949. He was a displaced person from the partition and came to India from Rawalpindi in 1947. After coming to Delhi he worked in a government lab for one-and-a-half years and then started the first pathology lab in North India. At that point of time there were hardly any pathology tests as we know neither there were any routine tests like, HP, TLC, ESR, Urine Test, Blood sugar Test, Cholesterol test. So there was not any competition then.</p>

	<p style="text-align: justify !important;">We are running 1700 laboratories in India and about 1600 collection centers. In addition we take another 5000 collections of blood samples from our pick up points. For example Medanta, Fortis, Moolchand and Apollo are some of the pick-up points we have.</p>
<p style="text-align: justify !important;">Last year we tested about 1.35crore patients, so the average of the samples we can test is about 55000 in a day. We are the biggest Histopathology Biopsy Centre in the world.</p>
<p style="text-align: justify !important;">Histopathology Biopsy is a test basically to detect cancer. Suppose if a lady has a lump in her breast, so she feels something has to be done about the lump, a part of the lump is surgically taken out which is known as the biopsy, it is the biopsy we test and find out the results. There is a possibility of two things either the tumor is benign (Non-Cancerous) or malignant (Cancerous). If the lady has malignancy she under goes with further tests, chemotherapy and radioactive therapies as per clinical requirement. We are the largest Histopathology in the world. We test about 1000 biopsies in one day and we are the second largest kidney biopsy center in the world. We are the only laboratories in India which have their own electro-microscope for very high end testing. We have our centers in all the major cities of India.</p>
<p style="text-align: justify !important;">Our next step would be to set up another comprehensive laboratory in Lucknow. We have such research center in Kolkata and next year we will make one in Lucknow. With this we will provide high end services for the people in the region for whom the services are not available.</p>

	<p style="text-align: justify !important;">We have many reasons, first of all people know that we are not fly by night operators and also they have faith in us since last 70 years plus people come to us for their pathological tests because 70% of all the medical decisions are nbased upon the pathology test of a patient, hence if the pathological tests result aren’t in place 70% of the medical decision can’t be taken. Gone were the days when people used to visit the “vaids” and doctors and they felt the nerves and pulse gave medicines, now it is evidence based medication. If someone has fever the doctor would suggest the person to get a blood test because it could be malaria, chikungunya, dengue or it could be anything. And if the person is diagnosed with anything she or he will get the treatment for the same. Now the medicines are completely evidence based, there is no hit and trial. These are the reasons why people come to us.</p>

	<p style="text-align: justify !important;">The first reason is, India is not having the kind of quality labs that it should have. There is a demand and supply problem. That is because there are no large laboratories. There are lots of facilities that claim to do testing, but they are not testing laboratories but testing shops. This is what is to be changed and people give the example of our establishment and say that “they should be like Dr. Lal’s laboratory.” All these countries you have mentioned, we are getting their samples for testing but only one lab is set up in Nepal and next would be in Bangladesh and then we will go to other countries. The reason we are slow on this is because our own country is huge. I usually say to people that if we will take out Uttar Pradesh and put it in the Indian Ocean, India would be the 5th largest country in the world. So this is the reason that we want to look at our country first and then look at the outside market.</p>

	<p style="text-align: justify !important;">These are known as point of care testing and people can test a few things but in India people can only test blood sugar. For which we suggest the patient to check his or her blood sugar on a regular basis of 15 or 20 days, so these are called Point of Care Testing (POCT).</p>
<p style="text-align: justify !important;">The POCT in India is not established because firstly, it is very expensive and secondly the Indian people aren’t savvy. There are seven types of care testing which is emanated by a lady. A lady can go to the bathroom and test her pregnancy by a UPT (urinal pregnancy test) in which she gets to know whether she is pregnant or not then in this case there is no need of a pathological test. When I was young we never had cats and dogs as pets but we had rabbits , so I never couldn’t understand why my father killed those rabbits and I got my answer when I was studying medicine. The reason why rabbits were killed during that time i.e, in early 1950s people used to check the pregnancy through it. A pregnant woman’s urine was injected into a female rabbit and after killing them they use to dissect the abdomen area to find ovulating ovaries. This is how pregnancy test were conducted.</p>

We have done 10 inorganic growths till now and yes we will buy some more from south of India and west of India.</p>

	<p style="text-align: justify !important;">There is no start-up in my line, if one is a qualified pathologist either he can work in a pathology lab or can start his own lab. The start-up in Delhi is difficult because Delhi has become a saturated city. there are many people who are running many pathology labs and they shouldn’t run these pathology labs and the reason they are running the labs is because it is very lucrative, which is very unethical and should not be done.</p>
<p style="text-align: justify !important;">So the government of India has also started the voice to our ways and they said that we are going to have a clinical establishment act which has been passed by the parliament in 2010. Since the healthcare is state subject so all the 29 states have not adopted it or implemented it. There are a chain of pathology labs which are not run by the pathologists they are run by the non-pathologists they are not even doctors they are quacks. So this brings out the quality and efficiency of the whole industry.</p>
<p style="text-align: justify !important;">They are many diseases which affect the mankind these days and these diseases are called the non-communicable diseases. The non-communicable diseases can’t communicate with others, whereas malaria, chikungunya and dengue are communicable diseases because it happens through a mosquito bite. TB is a communicable disease too and if a person’s lungs are infected by TB people around them will get infected. In India we lose two patients of TB in every three minutes, India has a burden of communicable diseases which includes Hepatitis A.</p>
<p style="text-align: justify !important;">But a bigger set of diseases have come out which are called NCDs (Non Communicable Diseases) and the NCDs are killing 65% of our people and if we will not control the NCDs it will also affect the two per cent of our GDP. The NCDs are headed by the high blood pressure, obesity, smoking, cardiac diseases, lung diseases, cancer, kidney diseases and stroke. Why it is becoming so important to us because it is also called the life style diseases, it changes the lifestyle of the person which has suddenly come up. India has large number of cancer patients and also large number of blood pressure patients. There is an element of genetic transfer of physical appearance of a person but some diseases are also transferred which only run in families.</p>
<p style="text-align: justify !important;">The problem with Indian people is they only come in illness, but they should know that they should get their self-check on a regular basis to keep a check on their health, my main point is to make people aware of the annual healthcare which is very important for one’s health which is in between the doctor and a patient and also make them aware of the testing in wellness not in illness.</p>
