How crucial are business models for the healthcare organizations?

Have you ever thought, long and hard about what can be key to your success in a business startup? You might be having all ideas to accomplish your dream. But you don’t have any clue how to jot down the proposal to clinch the deal.

Sitting thousands of miles away from comforts of his home in Finland. A Finnish Professor Paul Lillrank, with one hand in his trousers, adjusts his specks on a crisp winter morning. Recently in the posh Lutyen zone of India’s national capital to address a select group of professionals. Aspiring entrepreneurs and experts from prestigious business and management institutes.

A calm composure of the professor, writer of many books on healthcare. Prof was infectious as he along with another expert Sachin Gaur started presenting a threadbare account of points to be highlighted in the business proposal before it is thrown to the practice and before financers and other stakeholders.

In the third IC Innovators Club meeting, Prof Paul Lillrank of Aalto University, Finland and Sachin Gaur, Director, InnovatioCuris deliberated with club members on the business models. The name of the workshop was ‘the business model canvas’. The participants from successful health ventures as far as Nagpur participated.

Many of the businesses do not experiment with their business model. Hence they are not able to maximize the revenues, profits or impact! Alexander Osterwalder has examined the business models. And designed a canvas on which a business model workshop can be run as a simulation. And the betterment of businesses can make with new adjustments. Ever since its creation business model canvas has seen fifty million plus downloads. This IC Club meeting used the business model canvas as a tool.

Prof Paul Lillrank has been studying the topic already for more than a decade. He brings the academic rigor in understanding the dynamics of healthcare businesses. Prof not only shared the underlying constructs for efficient and robust healthcare models but also the understanding of what makes the most successful models like that of Arvind Eye Care work. He also shared how a desire to copy Arvind Eye Care in Finland resulted in the creation of a mega dental clinic. Which clearly illustrates that the fundamentals of good business models are universal cutting geographies and specializations.

Sachin Gaur further added to the workshop on the practical side by running the business model and value proposition canvases by asking two volunteers among the IC Club participants to share their business cases. The participation of the audience in sourcing the business model for the two business cases was an enriching exercise not just for the volunteers but for the entire room.

As the transaction cost for soliciting relevant feedback from an accomplished crowd of forty plus diverse set of people would have taken the two example businesses more than a month, which the club as a platform could solicit in a matter of few hours for them! This remains as the core motto of the IC Club which is to act as a force multiplier for new. And innovative businesses by connecting them with a diverse set of health experts. Who are constructive and see themselves as a change agent for good!

New India, Digital India, Make India, Innovate India in making a ‘Healthy’ India

India is a country of diversity with 1.3 billion population of which 70 per cent resides in villages and have access to 30 per cent of medical assets of the country. Poverty is a significant issue of the country, despite having one of the fastest-growing economies in the world, clocked at an economic growth of 7.6 per cent in 2015. It is estimated that 23.6 per cent of Indian population, or about 276 million people, live below $1.25 per day.

As Health is a state subject-there is lot of mismatch in states like Kerala and Punjab have best health indicators, while Uttar Pradesh is the poorest as per the NITI Aayog report. Indian government spends approx. 1.5 per cent of its GDP on health sector and mplans to make it 2.5 per cent which is much less than many developing countries while the USA has dedicated 16 per cent of its GDP. India is riddled with very basic public health issues leading to disease burden. Keeping this in mind, Prime Minister Narendra Modi has launched six initiatives: Open Defecation free country by 2019, Swachh Bharat Mission, National Health Policy 2017, Digital India with e-health, medical device manufacturing and door-to-door screening of chronic diseases.

The healthcare needs holistic approach which depends on multiple factors. The present government has taken many positive steps including launching of National Health Policy 2017 after the gap of 12 years. It has announced many initiatives like health insurance of people who cannot afford basic healthcare and upgrading of health infrastructure.

The regulation of medical devices have been brought out and is applicable w.e.f January 1, 2018 ending uncertainty of medical device manufacturers having global market of 220 billion US dollar. Challenges bring opportunities such as Indian healthcare market is around US$ 100 billion while it is expected to grow US$ 280 billion by 2020. The healthcare IT market is US$ one billion and is expected to grow 1.5 times by 2020. There is requirement of 7 lakh hospital beds which need investment opportunities of 25-30 billion US dollar. We need to bring innovations in hospital planning,devices, diagnostics, drugs and use of technology to reduce healthcare delivery cost and yet quality.

We failed targets of Health for All by 2000, National Rural Health Mission, and Millennium Development Goals and now launched Universal Health Coverage; its success would depend on providing healthcare facilities and strict accountability. We need to focus on primary health care and customise our healthcare delivery system by learning from experiences of other countries.

The concept of Diagnosis Related Group (DRG) making financial package for group of diseases which is known to patients, providers and third party payors should be considered by improving deficiency found in its execution by the USA.

The various schemes of present government are appreciable but success would depend on strict monitoring, corporate hospitals have high cost and many unethical practices are reported every day in media. Public hospitals to National Rural Health Mission have not delivered as required because of corruption, lack of resources and application of management practices. The need is not old wine in new bottle but strict control in implementation of various schemes launched.

Transforming healthcare through innovation

Executive opinion

At the outset, I wish all our innovator community best wishes and exciting New Year 2018. At our end, we continue to add value through our knowledge activities of InnovatioCuris (IC). An international conference will be organised on October 5 & 6 2018. We also continue to arrange webinars, training, IC club meetings, magazine publication and launch of new initiatives like security audit of healthcare facilities. We have already published two books in the U.S.A. First being “Innovations in Healthcare Management: Cost-Effective and Sustainable Solutions” in 2015 which has been adopted by Indian Institutes of Management (IIMs) and University in UK as a reference book. It has now special Indian edition as well. The second is “Planning and Designing Healthcare Facilities: A Lean, Innovative, and Evidence-Based Approach” published in October 2017. We bring global leaders to our platform and share experiences to benefit all. We thank our members for continuous support to take all initiatives to the next level. We have already partnered with many credible organisations to work together.

As developed economies enter a period of slower growth, emerging economies such as India have become prime examples of how more can be achieved with less. We need to share best global practices to optimise healthcare delivery cost and can reduce concerns of patients and yet keep high degree of quality. We impart knowledge through various medium and presents new concepts, methods and tools to achieve low cost, high volume solutions that improve access to patient care by better understanding of transforming healthcare through innovations. Our focus is grass root innovation to benefit have nots by organising resources and flow to achieve with available resources. It also covers non-traditional ecosystems of innovation outside of pure technology, such as social persuasion, rural health delivery, exploitation of economies of scale, prevention oriented primary care, medical education, and hospital design.

The ecosystem is very conducive for innovations right now, with growing number of start-ups and open mindset. We should take advantage of the new initiatives launched by the government like Startup India, Make in India, Standup India, Skill India and many more to take innovations to logical conclusion. The government has created enough funds to support. Innovators should take advantage of these schemes to fulfil their mission. To make members aware of funding opportunities, a workshop was conducted in the last club meeting by experts evaluating such proposals. DISHA – Need of the hour

Digital Information Security in Healthcare Act (DISHA) is proposed by the Indian government to secure e-Health data.

The act:

The Act mandates the clinical establishments to secure the digital health data and defines functions of the new regulatory bodies at the center and state level except the State of Jammu and Kashmir. Clinical establishments here means, any hospital, clinic, dispensary, etc., be it public or private.

Setup:

National Electronic Health Authority (NEHA) and State Electronic Health Authority (SEHA) will be setup as the governing bodies to formulate standards, operational guidelines and protocols for the generation, collection, storage and transmission of the digital health data for this Act. It will also ensure data protection and prevent breach or theft of digital health data. It will establish data security measures for all stages of generation, collection, storage and transmission of digital health data, which will at least include access controls, encrypting and audit trails.

Digital Health Data:

Digital health data comprises of one’s physical or mental health condition, sexual orientation, use of narcotic or psychotropic substances, consumption of alcohol, sexual practices, Human Immunodeficiency Virus (HIV) status, Sexually Transmitted Infections treatment, and abortion etc;

The required health data can be obtained by consent from the owner, thus informing them the purpose of collection, identity of the recipients to whom the health data may be transmitted or disclosed, identity of the recipients who may have access to the data on a “need to know” basis.

Major points:

As per the draft, the owners have the right to privacy, confidentiality, and security of their digital health data and the right to give or refuse consent for generation and collection of such data.

The Act also lists down factors affecting data transmission as to who can transmit, how they can transmit and monitoring data transmission. The Act further lists down the guidelines on accessing this data, with regards to who can access, how they can access and purpose of data access by various entities.

Penalties in contravention of serious breach of healthcare data shall be punishable with imprisonment, which shall extend from three to five years; or fine, which shall not be less than five lakh of rupees.

There are many debatable points that arise from this Act such as the technical measures a clinical establishment should take, standardization of data security, measures to be taken in times of breach, training and capacity building of the clinical establishment, best practices of data collection and storages.

The clinical establishments might be worried on implementing the Act, as they might lack the technical resources to bring the robust solutions. On the other hand the security industry in the country might be looking to engage clinical establishments and respond to this situation by providing cost effective solutions and safeguarding the privacy of the patient.

To implement the Act, clinical establishment might need hand holding. However, we appreciate the efforts of the government to propose this Act and safeguard the interest of patients and citizens.

On the third day of the event, a special seminar was hosted by IC Innovator Club (IC)2 with its first meeting held on 20 Sept 2017 at India International Center (IIC, Annexe), New Delhi.

The club brought all innovators, investors, start up, technology persons, policy makers, health experts and legal advisers under one large umbrella to be members and hold hands by sharing success stories and challenges and to incubate their ideas to take the dream and vision of India forward. It has a very strong band of advisers to support the activity of club and mentor projects. The aim is to bring down healthcare delivery cost yet maintain quality through innovations. IC has an outreach of 30,000 people globally to share its activities. The first of the club meetings was supported by Amity University, represented by Dr. Ajit Kumar Nagpal and three of the leading investors in the field of health and medical facilities.

Hence, we are having Prof Paul Lillrank from Aalto University, Finland to share the methodology on how to evaluate and improve upon the healthcare organisations when it comes to their business models.

About the workshop expert

Paul Lillrank is a professor of quality and service management at Aalto University since 1994. He received his Ph.D. from Helsinki University in 1988 after post-graduate studies in Japan, where he studied Quality management in Japanese industry. He worked at Science University of Tokyo, the Boston Consulting Group, and Stockholm School of Economics. He has pioneered industrial management applications in healthcare, co-founding The Institute of Healthcare Engineering, Management and Architecture (HEMA) at Aalto, Finland and The Nordic Healthcare Group (NHG), a consultancy. He has published in the areas of healthcare operations management, technology transfer, service quality, and process analysis. He teaches regularly at the Indian Institute of Technology, Kharagpur.
