Wound care today is one of the most expensive and overlooked threats to patients and our overall healthcare system. It is very difficult for doctors to identify a wound which is becoming infected. Clinical signs and symptoms are imprecise and methods of identifying bacteria can be inaccessible or time-consuming. Keeping this in mind, an International team of Researchers have deduces a solution in the form of ‘SWIFT RAY 1’ which is a device run from a smart phone or tablet application that allows advanced imaging of a wound to identify infection.

This device can be attached to a smart phone and connected to the Swift Skin and Wound software to take medical-grade photographs, bacterial fluorescence images (which reveal bacteria using violet light) and infrared thermography images (which measure body heat). By capturing the heat produced by a wound and the fluorescence of bacteria, it helps clinicians tell the difference between inflammation and infection. This could help in faster intervention by catching infections before they pose serious threat to the patient.

Testing of the device was done by recruiting 66 wounded patients. Their wounds showed no sign of infection spreading further, did not contain foreign bodies and had not previously been treated with antibiotic or growth factors. The wounds were cleaned, uncovered and dried before imaging and later cared for as usual.

A researcher who was not present for the wound care process, reviewed the images which identified four patterns. ‘Non-inflamed’ wounds were the ones where the wound was not warmer than healthy skin and no bacterial flouroscence was present. ‘Inflamed’ wounds were the ones which were slightly warmer than healthy skin and no or slight bacterial fluoroscence. Last two patterns of wounds which were substantially warmer with or without bacterial fluorescence were both designated as ‘Infected’ because all the clinicians who had examined these wounds has considered them infected. So out of the 66 wounds, 20 were non-inflamed, 26 were inflamed and 20 were infected.

The researchers performed principal component analysis and used an algorithm called k-neighbor clustering to see if a machine learning model could accurately identify these different categories of wound. It was found out that the model could identify all three very well with an accuracy of 74%. When differentiating between infected and non-infected wounds, the model correctly identified 100% of infected wounds and 91% of non-infected wounds.

This was a pilot study and follow-up studies are planned. This research has demonstrated bacterial imaging helps guide clinicians work to remove nonviable tissue, yet it cannot identify infection by itself and also that the images should always be considered in their medical context. The research paper is published in Frontiers in Medicine.

In the United States, lung cancer is the leading cause of cancer-related deaths. Some tumors are extremely small and hide deep within the lung tissues making it nearly impossible or extremely difficult for surgeons to reach them. To handle this challenge effectively, researchers at the Department of Medicine, University of North Carolina have been working on an extremely bendy but sturdy robot which is capable of moving through the lung tissue without damaging it. Their research has reached a new milestone where the team has proven that their robot can autonomously go from Point A to Point B while avoiding important structures like blood vessels and tiny airways in a living laboratory model. This steerable needle is highly compact but the system is packed with a suite of technologies that allow the needle to navigate in real-time.

The needle can also account for respiratory motion. The researchers tested their robot while the laboratory model performed intermittent breath holding. Every time the subject’s breath is held, the robot is programmed to move forward. This technology gives you that extra few millimeters or centimeters which would help with pursuing small targets in the lungs. Thus, allowing us to reach targets we cannot otherwise reach with a standard or even robotic bronchoscope.

The robot is made of many separate components. A mechanical control provides controlled thrust of the needle to go forward and backward and the needle design allows for steering along curved paths. The needle is made from nickel-titanium alloy and is etched with laser to increase its flexibility and effortless movement through the tissue. Other attachments like catheters could be used together with the needle to perform procedures like lung biopsies.

CT scans of the patients’ thoracic cavity were used and Artificial Intelligence (AI) engaged to create three-dimensional models of the lung including the blood vessels, airways and the chosen target. Using this 3-Dmodel and after positioning the needle for launch, their AI-driven software instructs it to automatically travel from point A to point B while avoiding vital structures.

This robot is a clear amalgamation of medicine, computer science and engineering which promises to improve medical outcomes for patients facing a variety of health challenges while providing guarantees on patient safety. This research study is published in Science Robotics.

Early stage detection of breast cancer can have a survival rate of nearly 100% but if tumours are detected in later stages, this rate drops to 25%. With a vision to improve the overall survival rate of cancer patients, researchers at the Massachusetts Institute of Technology (MIT) have designed a wearable ultrasound device which would allow women to detect tumours when they are still in early stages. This new device could be more valuable for patients at high risk of developing breast cancer in between mammograms by allowing more frequent monitoring.

This device is a flexible patch which can be attached to a bra, allowing the wearer to move an ultrasound tracker along the patch and image the breast tissue from different angles. The device is shown to obtain ultrasound images with resolution comparable to that of the ultrasound probes used in medical imaging centres. The USP of this wearable device is that it is easy to use, portable and provides real-time user-friendly monitoring of breast tissue. This research study is published in Science Advances.

In a prospective, six month, single-arm study with use of a novel clinical-decision support tool and shared decision-making in 94 patients above 65 years old with type 2 diabetes managed in a primary care practice and at high risk for hypoglycemic episodes led to a 46% decrease in the number of at-risk patients and discontinuation of hypoglycemic agents in 20% patients.

The HypoPrevent study was done in a Pennsylvania primary care centre where 94 enrolled patients had type 2 diabetes and were at risk for hypoglycemia because of treatment with sulfonylureas or insulin with HbA1c of less than 7%. Patients averaged 74 years old, 57% were women, 955 were white, 61% had diabetes for more than 10 years, 51% were on insulin, 48%had chronic kidney disease and 47% were on sulfonylurea. Out of 94 patients, 80 completed all three study visits.

So, the patients and clinicians used a new hypoglycemia reduction clinical-decision support tool developed by the Endocrine Society and a healthcare consulting company. Resultantly, 20% patients reduced their dose of or discontinued insulin or sulfonylurea. In patients with both baseline and follow-up A1 measures, A1c rose from 6.29% at baseline to 6.82%. 50 patients set an A1c goal and had a timely follow-up A1c measurement and in this subgroup, the number of patients at risk for hypoglycemia decreased significantly by 46%. This proves that this decision support tool can be effectively used in primary care practice with positive results.

The drawback of this study clearly lies in the fact that there was lack of control group which makes it impossible to conclusively determine whether the intervention led to the observed increases in A1c levels nor can the study exclude regression to the mean as the cause for lowered A1c levels. The study received funding from Lilly, Merck, Abbott, Sanofi and Novo Nordisk. This study is published in the Journal of the American Geriatrics Society.

Source: www.Technologynetworks.Com

Researchers at University of Surrey warn that the world needs to be prepared for the next pandemic in the post COVID era and in this light they have developed a new molecular test for viruses and bacteria, including Sars-CoV-2 called ‘Electro-chemical LAMP’ (Elamp) that is rapid, sensitive, affordable and can be performed at home, in a GP surgery or in a hospital laboratory. This rapid electronic diagnostic test for infectious diseases has the sensitivity of lab-based polymerase chain reaction (PCR) tests but when connected to a smartphone could be done at home.

Researchers tested human blood, saliva and nasal and throat swabs and found that their test had a 93.33% detection accuracy rate. The test also performs well at room temperature and gives the results within 45 minutes. This diagnostic tool seems to be effective, rapid and affordable which can be used at home to monitor and contain infectious diseases. The USP of this test is that it can detect lower amounts of the Sars-CoV-2 virus compared to other home-based tests.

This electrochemical test device is also compact and the team is further exploring its potential to be miniaturized into a simple memory stick-sized device that could be operated by anyone with an application. This would make the test to be performed at home where then the result would be instantly sent to health services to monitor the spread of infection along with giving timely advice to the patient.

A study on the test has been published in iScience. This study is very relevant today because undoubtedly it is time to keep working on better ways to test for the coronavirus and other new difficult viruses which are likely to emerge and cause imbalance in the health scenario around the globe.

India is a country with a billion people wherein there is one allopathic doctor available for every 10,189 people whereas as per World Health Organisation (WHO) the ratio should be one doctor for 600 people. This itself reflects that India is facing a dire need for more doctors. Keeping the current scenario in mind, low-cost healthcare devices can often replace the need for trained medical personnel. Taking a note of this statistic, thirteen centres of Biomedical Engineering Technology incubation Centre (BETiC), starting with the one at IIT Bombay, have till date created 40 different novel and affordable medical devices. BETiC has become a centre of innovation where more than 100 engineers and doctors are closely working together to solve the healthcare problems of a billion people.

BETiC centre at IIT Bombay has made many low-cost medical devices like smart stethoscopes to easy diabetic checkups which can do many tasks allowing for easy and affordable healthcare with the aim of saving valuable time of highly trained doctors. An example here is of the CollabDDS system developed by National Informatics Centre (NIC) which allows x-ray images taken in a remote hospital to be shared and examined in real-time by an expert radiologist in a tertiary care hospital.

The SMART STETHOSCOPE developed by Ayu Devices is used by primary healthcare doctors to record and share chest or heart sounds to experts in tertiary care hospitals for a second opinion and diagnosis. The HYBRID PLASTER SPLINT is a simple formulation to hold injured body parts together after an accident to prevent further damage during transport of the patient to the hospital and is being used in the remotest of villages. This splint is made from a combination of paper pulp and Plaster of Paris which is extremely light weight and can be carried in a first aid kit for emergencies. College of Engineering, Pune (one of the centres of BETiC) has developed a patented hybrid orthopedic splint which can be used as an instant plaster on a fractured limb to ensure that the broken bone stays in normal position and avoid propagation of crack.

DIABETIC FOOT SCANNER has been developed by a team at IIT Bombay to help healthcare workers in far-reach areas to prevent long-term ulceration and amputation among patients. Statistics show that India is home to more than 60 million diabetics and every year over one lakh people undergo amputation. A diabetic foot can be managed better if rural healthforce is trained to use this scanner device, saving diabetics from amputation.

The team has developed a semi-automatic non-invasive device for screening a diabetic foot condition wherein it detects the stiffness of tissues on the sole of feet and categorizes the condition of the foot in green, yellow and red risk zones. The clinicians can uprise the patients susceptible to diabetic foot ulceration in advance, potentially saving them from an amputation.
