New Delhi, Jan 24 (India Science Wire): As the Coronavirus continues to mutate, it has become challenging to diagnose and treat different variants of concern. Some like the Omicron, though milder in symptoms and fatality, are a super spreader and are propagating like wildfire across the world.

Currently, detection of this variant depends upon tests like the S-gene dropout or by NGS (nextgen sequencing) of the whole viral genome. While the S-gene dropout method is not specific to pinpoint the type of variant, the NGS method has its limitations in terms of expense, turnaround, and the number of centres that can provide such service.

A team of scientists at CSIR-Central Drug Research Institute (CDRI), Lucknow, have developed an indigenous RT PCR Kit for the specific detection of the Omicron variant in collaboration with their industrial partner, Biotech Desk Pvt. Ltd., Hyderabad. The kit has been named INDICoV-OmTM.

Dr. Atul Goel, the team leader said that the kit provides for quick and cost-effective detection of the Omicron variant. It can also be aligned for the detection of other emerging variants of COVID infection and other respiratory infections in the future. The kit has been tested and validated by Prof. Amita Jain in several Covid positive patient samples at King George Medical University, Lucknow.

Dr. Shradha Goenka, Managing Director, Biotech Desk Pvt. Ltd., said the kit was likely to be rolled out by mid-February. “We are working on regulatory approvals and the assembly of the kit. We are keeping our fingers crossed for an early release”, said Dr Goenka.

Prof. Tapas K. Kundu, Director CSIR-CDRI, said the Institute is rapidly gaining expertise in antiviral research for therapeutics and diagnostics to combat any kind of viral infections. The team led by Dr Atul Goel is fully prepared for the detection of broad spectrum and specific pathogenic viral infection. This kit for SARS-Cov-2 omicron diagnosis has been submitted to the Indian Council of Medical Research (ICMR) for validation and should be available for the people of the country soon.

The research team consisted of Dr. Niti Kumar, Dr. Ashish Arora, Ms. Surbhi Mundra, Ms. Varsha Kumari, Mr. Kundan Singh Rawat, and Ms. Priyanka Pandey, besides Dr. Goel.

This online conference was based on the theme ‘Towards Team Science for Multi-omics Cancer Research in South Asia’.

Conference website: https://tcga.ctcr.in

TCGA is a landmark cancer genomics program that molecularly characterized over 20,000 primary cancer and matched normal samples spanning 33 cancer types. This joint effort between the US National Cancer Institute and the National Human Genome Research Institute began in 2006, bringing together researchers from diverse disciplines and multiple institutions. Over the years, TCGA generated over 2.5 petabytes of genomic, epigenomic, transcriptomic, and proteomic data. The data, which has already led to improvements in the ability to diagnose, treat, and prevent cancer, will remain publicly available for anyone in the research community to use.

On similar lines, the establishment of an ‘Indian Cancer Genomics Atlas (ICGA)’ has been initiated by a consortium of key stakeholders in India led by the Council of Scientific and Industrial Research (CSIR), Government of India in which several government agencies, cancer hospitals, academic institutions, and private sector partners. A brief outline of the proposed Indian Cancer Genomics Atlas is appended below.

The TCGA 2020 conference was inaugurated at the auspicious hands of Prof Shekhar Mande ( Director General, Council for Scientific and Industrial Research (CSIR), Government of India) and Dr. Jean Claude Zenklusen (Director, TCGA, US-NCI).

During the 3rd day online event, TCGA experts and key ICGA stakeholders deliberated on the vision, design, and plans of the ICGA project with regards to common SOPs, operations, logistics and governance models.

More importantly, conference sessions were planned around the central theme of building a culture of team science for undertaking mega-cancer research projects in India and neighboring countries. Global experts discussed technical aspects of data/ biospecimen/ multi-omics studies and applications of Precision Oncology in clinical cancer management.

In addition, eminent translational cancer scientists and onco-clinicians from India and abroad joined at the conference as speakers. Over 1000 delegates representing basic scientists, translational researchers, clinical researchers, genomics scientists, bioinformaticians, bio-statisticians, big-data experts from academia, clinical medicine, industry, and governmental agencies attend. The conference was open to everyone with prior registration.

I am excited to see the significant strides in Molecular-Oncology that are enabling more effective cancer management globally. Cancers in India are uniquely different than the rest of the world with respect to epidemiology, clinico-pathology, and treatment response. Hence, large scale, co-ordinated national cancer genomics programs such as the Indian Cancer Genome Atlas (ICGA) are timely and pertinent for our nation. This high impact multidisciplinary project which has tremendous benefits to cancer patients can be successful only if all stakeholders work as one team over a long period of time. To realize its full potential, Prashanti Cancer Care Mission is happy to be at the forefront of this effort along with like minded premier cancer hospitals across the country.

InnoHEALTH magazine team personally interacted with some of the organizing committee members (Dr Santosh Dixit, Dr Anamika Krishanpal, Disha Singh & Srikant Verma) to know their experiences and thoughts on the event organized.

1. Why do you organize the TCGA conference?

We see TCGA as an ideal team science project that can be referenced to create similar projects in India. By organizing TCGA conferences, we offer a platform where the Indian researchers can learn directly from the expert TCGA faculty members through knowledge/experience. This event is also a great place to network for collaborations, job opportunities, idea exchange, and community building.

2. What was different this year from last year?

Last year the primary objective was to learn about the TCGA project from the expert faculty members through multiple modes of learnings: plenary talks, keynote lectures, and panel discussions. But this year, the conference was aimed to deliberate on the vision, design, and plans of the Indian version of TCGA – Indian Cancer Genome Atlas (ICGA) with regards to common SOPs, operations, logistics, and governance models. Also, the number of collaborating partners increased with premier institutes from India (CSIR-IGIB) and UK (ICR) coming on board.

3. What were the challenges and how did you overcome them?

This year due to the COVID-19 pandemic, we were about to cancel the 2nd TCGA conference in India. However, we realised that there is a possibility to conduct such a mega event even through virtual means. We reached out to professional resources which could help us achieve our goal of conducting an online event.

4. What impact do you expect the 2nd TCGA conference to create?

This conference will help build a culture of team science for undertaking mega-cancer research projects in India.

5. What do you look forward to post the 2nd TCGA conference?

The ICGA project will get into action mode based on the deliberations in the conference. Several themes, topics, and teams from this conference will be the driving forces of ICGA plans.

5. What are some key takeaways from the 2nd TCGA conference? [time-stamps of YouTube links added for reference]

‘Team Science’ is the key for conducting large scale studies [Day 2: Session 1 – 20:16]

Why do we need to set up a consortium similar to TCGA in India? – because our DNA and genetics are quite different from the world [Day 2: Session 1 – 10:34]

Ethics and laboratory protocols are going to be the foundation for Indian Cancer Genome Atlas (ICGA) [Day 2: Session 7 – 13:20]

Triple-Negative Breast Cancer (TNBC) – one of the most aggressive cancer and commonly observed in young women – could be the first cancer type to be studied as part of Indian Cancer Genome Atlas (ICGA) [Day 2: Session 7 – 06:20]

The success of ICGA – a citizen science initiative, will depend on how we can engage citizens in the project. Thus, communicating the findings to the citizens on regular basis is very important [Day 2: Session 6 – 54:43]

how the developed India specific cancer database might help to diagnose, treat and prevent cancer

how the project will make cancer treatment affordable

According to research from the Global Asthma Network, some 339 million people, or one in 20 globally, are affected by the condition. Often referred to as a Chronic Respiratory Disease (CRD), it causes airways to restrict and often results in coughing, wheezing, tightening of the chest, shortness of breath and generally difficulty breathing. In the UAE, approximately 14% of the population is impacted, equating to 1.3 million residents.

Speaking ahead of the Emergency Medicine conference, Dr. Saleh Fares, Consultant of Emergency Medicine, EMS and Disaster Medicine, Founder and President, Emirates Society of Emergency Medicine (ESEM), Dubai, and moderator of the respirology session, said: “CRDs are increasing in prevalence in the Middle East and, unfortunately, are often left undiagnosed and untreated. This is having huge social and economic consequences and, in some instances, is resulting in death. With the correct diagnosis and treatment, the number of deaths can be dramatically reduced, and the impact of the condition lessoned.”

“We need to ensure there is better access to treatment and educate both frontline medical staff, as well as the patient, about the symptoms and the best course of action to take to remedy the condition,” he added.

The UAE’s above-average number of asthma cases is often attributed to the UAE’s dusty conditions due to the desert environment as well as seasonal changes and the resulting increase and decrease in temperature and humidity. A reliance on air conditioning in the home and office, if not properly cleaned, can also become the ideal breeding ground for mould and dust which can act as a trigger for some sufferers.

According to Dr Danish Sami, Consultant, Emergency Medicine, Sheikh Shakhbout Medical City (SSMC), Abu Dhabi, who will outline the factors contributing to the region’s asthma problems as part of his session titled Asthma Exacerbation in 2020: “The issue we face is that only 30% of asthma sufferers are treated in a controlled environment. Similarly, only 30% are either partially controlled, and 40% have no diagnosis and therefore no treatment. There is still a need for education, particularly with children, who we need to encourage to exercise more to help negate the impact of asthma, however steps taken within the education system and health authorities are helping to address these issues.”

Mr. Karnal Singh, Ex-Enforcement Director, India raised during his speech Narayana Murthy’s concern that there have been no recent innovations from India. “Are we so bad?” He said we would have to think differently for finding solutions. The education which we receive in our country is more competitive than collaborative. There are many unmet needs in health sector, for example, for majority of injections, cold chain is not maintained. It’s been a long time that we could have insulin tablets rather having the existing insulin injections. India, prominently being an agricultural land faces a major unmet need of water requirement, which is only 50% against the 80% requirement. He acknowledged that 2010-2020 has been the decade of innovations for India. To everybody’s pride, the ranking of India in the global innovation index has improved by five places to 52nd in 2019 from 57th position last year.

Mr. Mohan Krishnan introduced the discussion with a brief presentation on the global trends in healthcare and their affordability. He particularly emphasized on access to healthcare resources and quantity shortage in India. He highlighted how lifestyle associated diseases have taken over the communicable diseases. He discussed the key trends in supply dynamics, their relationship to healthcare sector, and the challenges and opportunities for their future implementation. The functional and central aspect of this session was majorly about healthcare which is accessible and affordable to the masses. Other key points that emerged during the session are outlined as:

switching of communicable to non-communicable lifestyle associated diseases

lack of medical care in rural areas

Ayushman Bharat Yojana or Pradhan Mantri Jan Arogya Yojana or National Health Protection Scheme: one of the largest and promising healthcare initiatives which aims to address healthcare holistically

patient feedback must be a pre-requisite while thinking of the innovations which can fulfill the unmet needs

biggest challenge in healthcare is information symmetry

one of the most remarkable point that came up was that the healthcare provider should be “paid to prevent”, and not paid to cure

5 P’s should be prioritized: People, Plant (equipment in hospitals), Processes, Premises, Patient care

This enlightening session focused on four most important factors, viz., better health outcome, better patient experience, better staff experience and better costs.

The diagnostics could be done by the help of paramedics or people other than doctors and then they could be replaced by automated systems or AI. There are schemes such as Ayushman Bharat, which give accessibility to each one. The innovations could be patient centric, healthcare centric or in terms of sustainability. The focus was on trends impacting medical devices and diagnostics, for example, lab-on-chip, wearable health monitoring devices, AI, 3D printing, etc.

The brainstorming session provided meaningful insights into the remedies which innovations can bring to the unmet needs in healthcare. Prime Minister’s health and wellness program were much talked about. Challenges in the not-much-explored area of management of blood banks were discussed. The lack of a centralized system of blood banking is a major concern.

The role of medical robotics, which is a type of disruptive technology, is very fascinating. The thought of switching from open surgery to key-hole-surgery to no-hole surgery is in fact very intruding. Another major concern across the globe is diabetes and how the diabetologists across the nation are ignorant in not examining the feet of their diabetic patients as 30% of the diabetics suffer from severe foot diseases. There are ample cases of diabetic amputation, which must raise an alarm within the diabetologists.e lab-on-chip, wearable health monitoring devices, AI, 3D printing, etc.

The fourth session was a very inspirational one wherein the dais had seven women innovators, each one unique in their own fields. As Prime Minister Modi once said, “The progress of humanity is incomplete without the empowerment of women.
”InnovateHER gave a common platform to few wonderful women where they shared their stories of inspiration, falls, and experience. This session was indeed a beautiful stretch of one hour where these women from varied backgrounds and aspirations showcased their expertise and their words gave hope, path and motivation to everybody present in the auditorium. They were an epitome of women empowerment and proved it greatly “Komal Hai Kamzor Nahi Tu, Shakti Ka Naam Hi Naari Hai, Jag Ko Jeevan Dene Wali, Maut Bhi Tujhse Haari Hai”.

Ms. Priya Prakash, Founder & CEO, HealthSetGo, was one of the youngest innovators, who herself realized at a very tender age, the importance of good health. This drove her to initiate her existing foundation, which focuses on the wellness of children. “If the kids are healthy in the first place, the coming young generation would be disease-free”, was her take on the evolution of her organization.

Another eminent panelist Ms. Preeti Rao, Founder & CEO, Weljii, inspired everybody around to focus on being healthy. Instead of talking about the illness, she emphasized more on how people must take care of their wellness which includes both physical and mental health.

Ms. Veena Sehgal, a pharmacist and entrepreneur mentioned the gap in the availability of medicines which the doctor prescribes. They are most of the times available on area-specific basis which makes it difficult and time consuming for the patients. There should be an emphasis on generic medicines, rather than brands.

Dr. Arjun Dang, CEO, Dr. Dangs Lab, who was chairing the session referred India as the “diabetic capital of the world”, referring to the alarming scenario of the affected Indians wherein around 16 million Indians are affected with Type 2 diabetes, and this number is expected to rise to more than 100 million in the next ten years. He also insisted not to commoditize healthcare, rather consider it as a service which requires a lot of compassion and empathy.

One very relevant question that came up was that “Why Indians are most prone to diabetes?”. Probably it is because of the harmful oils and highly processed foods available in the Indian markets. Stress is a huge component in diabetes management. Exercise and breath-management (pranayama, etc.) have a major impact on diabetes management. One of the major contributors of diabetes is non-nutritional food. Sleep which is often understated has a direct impact on blood sugar level which increases due to lack of proper sleep. Diabetic patients can improve only to fall back again into the same pit and destroy their health, hence it is of utmost importance to rescue them.

Way forward

With such inspirational, informative and interactive sessions, the two-day conference came to an end, raising too many new questions and challenges that may impact healthcare, especially in India. It also gave answers to many existing issues through the numerous innovations that were presented. The generic thought which probably each one had was on the affordability and accessibility of good healthcare. If the innovations are not scalable, easily accessible and cost-effective to the masses, they cannot persist for long.

Of utmost importance were the views on how AI can save lives, and the importance of end-to-end affordable healthcare. IoT can connect to anyone, anytime, anywhere in rural and urban areas where reach is low, and how AI helps in diagnosis and innovation. Early intervention can resolve health problems and there is a huge scope in the use of the apps which can enable timely and real time intervention. With highly potential schemes like Ayushman Bharat, we can create value chain in affordable and accessible healthcare for everyone.

The healthcare sector sees immense opportunities with the innovations happening across the globe. Innovation pitches from global and Indian companies that were conducted wherein organizations from various sectors came forward to present their portfolio of services and products.

Innovators and researchers are working on various aspects wherein they can target various chronic as well as lifestyle associated diseases. For example, the Nano 4 TB kit helps detect TB easily. There are various state-of-art laboratories across the nation which are deployed and use high-end technology, for example next generation sequencing is used to perform operations to detect aneuploidy or any kind of anomaly which is responsible for spontaneous miscarriages and mental retardation in live born babies. In the light of technology, innovation and healthcare, we must look at technology with a business view; how technologies work in society, sustainability of the model with traditional delivery, and about new models in healthcare.

Introducing The Patient Experience Excellence Awards 2019
The Patient Experience movement is growing within India and around the world. Both hospitals andhealthcare providers are realising the significance of patient feedback which is being used in theirown teams to improve patient care.
The CRM ACADEMY OF ASIA – TERRAGNI – ICC Patient Experience Excellence Awards 2019, recognise invaluable service in celebrating what is best about what we do, encouraging others andraising the bar a little higher each year. Fortunately with the benchmarks these awards willrecognise, everyone will leave with at least one new idea to put into practice back home. The awardswill select those ambassadors with a patient-centred approach – with a critical role to play in qualityimprovement. As the quality of Patient Experience also affects patient safety and self-reported outcomes, this work has much wider effects. The move to new care models will show the newopportunities to improve care across boundaries where it has traditionally fallen short.
The PEXA2019 Awards are the only recognition for contribution to Patient Experience in the countryand are based on innovation and impact.

India’s business of healthcare continues to grow steadily – with its wider coverage, better services, and increasing investment. As per the India Brand Equity Foundation – the Healthcare Market in India is expected to be the US $ 372 billion in size by 2022, driven by rising incomes, greater awareness levels, higher incidence of lifestyle diseases and access to healthcare insurance. While the industry continues to be focused on operations and revenue and notwithstanding the launch of Ayushman Bharat – the world’s single largest Government-funded Healthcare program, the need for understanding and defining what can bring about change from within – is critical.

An increasingly competitive market driven by aware and technology-enabled patients and caregivers is seeing greater levels of involvement and their rapidly increasing need for driving their own health-care experiences. The Harvard Business Review believes that “the future of the global health care market lies in delivering high-value care that concentrates on Patient Experience.”

It is important to understand that Patient Experience is not just about an individual patient’s interactions – but also his or her perceptions of that caregiving experience. A Healthcare organization that consistently delivers a better quality of patient experience will be able to drive better quality of patient outcomes and in turn – better business performance.

It is in this scenario – that Team CRM Academy of Asia has partnered with Terragni Consulting and Indian Chamber of Commerce to bring you the Patient Experience Conclave & Awards 2019, to establish the Patient Experience paradigm and benchmark its performance standards – and drive the change for the entire Indian Healthcare Industry.
