	<p style="text-align: justify !important;"><em><strong>Lt General (Dr.) Rajesh Pant</strong></em> is an internationally recognized Cyber Security expert, presently tenanting the prestigious appointment of National Cyber Security Coordinator at the Prime Minister’s Office, India. General Pant brings to the table an interesting mix of military operations, academic excellence, corporate governance, and cybersecurity wisdom. Prior to this, he was the Head of the Army’s Cyber Training establishment for three years. He served in the Army Signals Corps for 41 years wherein he was awarded three times by the President of India for distinguished service of the highest order. He also served as the Chairman of Precision Electronics Ltd as a Governing Council Member of IETE (India). <a href="https://www.linkedin.com/in/sachgaur/"><em><strong>Sachin Gaur</strong></em></a> interviewed him on his viewpoint on India’s vision for cybersecurity.</p>

	<p><strong>Q. On behalf of InnoHEALTH Magazine, we congratulate you on your new assignment. For our readers, we would like you to share your short-term and long-term vision for Cybersecurity from national</strong><strong> security perspective</strong>.<br />
Short-term vision is to issue National Cyber Security Strategy 2020-25 early next year. Task force is working overtime on this by consulting all stakeholders. Long-term vision is to create an all-encompassing cyber vertical at the national level, to handle incident response, cybercrimes, legal issues and capacity building.</p>

<strong>Q. We know that there are some fundamental technological shifts waiting to happen like 5G, and along-with it massive (Internet of Things) IoT deployments and especially use cases of connected healthcare. Can you share your views on the cybersecurity implications of the connected devices?</strong><br />
<a href="https://innohealthmagazine.comtheme/iot-can-truly-transform-rural-healthcare-india/">IoT security</a> is a priority topic world over and this is because the limited security capabilities of these devices are also an afterthought. We need to work on a framework, to bring baselinesecurity through the manufacturers and developers of these devices. These devices are omnipresent in our lives, we find them in our home environment to industrial environments including hospitals. We have seen attacks in the past, where such devices are compromised to launch massive denial of service attacks to manipulate the workings of critical infrastructure.<br />
Also, the issue of IoT security is multidimensional, from <a href="https://innohealthmagazine.comissues/patients-sensitive-health-data/">data security</a>, privacy to device security. As we discuss this, there are multiple acts and bills pending in the Parliament on these topics. While the bills and acts will provide a framework, we need to also create awareness on both sides, supplier and consumer on the possible risks and mitigation strategies.</p>

<strong>Q. What steps can be taken to improve the security in such <a href="https://innohealthmagazine.comtrends/medical-iot-future-of-connected-health/">connected devices</a>?</strong><br />
When I say baseline security framework, it can be achieved in multiple ways.<br />
As of today, most devices that we use including mobile phones, do not have a security testing certification. So, we can agree with the industry and look at important test cases and if they can do self-certification on such test cases.<br />
<em>For </em>example: the device should not have weak default login credentials, it is sending data to a remote server and can be operated remotely. So, we can come up like a 5-star rating framework like that of the energy consumption but for the security of IoT devices basis what kind of tests they clear.<br />
Industry bodies can agree on various levels of security and what it takes to achieve that level. Such a framework, when implemented, can provide confidence to consumers and users on the kind of device they are using vis-a-visthe use case they have at hand. So, they might use a higher security rating device in a use case where the stakes are high.<br />
The other approach is to get the security testing done with notified agencies. Department of Telecom for example has announced mandatory security testing of network elements for telecom given telecom is a part of the critical infrastructure and security issuescannot be taken lightly.<br />
Also, some of the emerging concepts in connected devices are missing in the various governing acts of the industrial connected devices. So, we also need to update our legal frameworks to cover software-based tempering of such devices and make the manufacturers and service providers accountable and proactive towards the security of the systems they provide.</p>

<strong>Q. What are the threats that you foresee for the health sector? </strong><br />
There are three areas we see where health sector can be impacted:<br />
First is the data breaches and <a href="https://innohealthmagazine.comissues/ransomware-epidemic/">ransomware attacks</a> on healthcare data. As we know, among all the data, healthcare is the most sensitive and sought after by malicious actors. Outside of India, we have seen umpteen cases where ransomware has crippled the health system and it is only after paying the ransom the hospitals can start operation again. Timely backups and encryption of healthcare data during storage is a preventive measure that clinical establishments can take to mitigate the breach and ransomware attacks.<br />
Second is the manipulation of connected devices. The topic of IoT and connected devices security, as discussed in the above sections, directly apply to the medical devices. Healthcare is a domain where attacks on such devices can be life threatening, especially when there are implantable devices. As we have the new Medical Device Regulation Act in India since 2018, we should also consider cyber security aspect in the devices which have a communication interface. For example, a pacemaker which has a communication interface can be manipulated remotely and the patient’s life is at risk.<br />
Third is the manipulation of health system including the building management. We are probably not very far from the days when sophisticated attacks, as we see in the movies, on high security establishments by manipulating the building controls. The building management systems are very weak when it comes to security. Every hospital is a building and imagine what a false fire alarm would mean to patients in Intensive Care Unit. Or even loss of air conditioning or sudden spikes in electrical power.<br />
There is a proposed act <a href="https://innohealthmagazine.cominnovatiocuris/disha-act/">DISHA</a>, Digital Information Security Healthcare Act, which might address some of the legal aspects of security in the healthcare setting. A lot needs to be done in this area, and we are on our way.</p>

<strong>Q. Our readership consists of health experts all over the world. Any message for them?</strong><br />
We are at the cusp of a new age where we look to take advantage of <a href="https://innohealthmagazine.comexpert-opinion/ai-iot-healthcare-need-future/">Artificial Intelligence</a> to Internet of Things. For such a knowledge economy to take off, health sector is at the center of it and health experts need to pay attention on what they are buying and how such systems are managed and operated. Through intervention of Ministry of Health &amp; Family Welfare and responsible bodies such as National Accreditation Board of Hospitals &amp; Healthcare Providers (NABH) of Quality Council of India, we plan to recommend a cyber audit and increased awareness of information security.<br />
We would not want our hospitals and clinical establishments to be a prey for malicious actors. Rather we would want our experts to leverage technology to take the country to the next level in providing care to a wider population at a lower cost and of the highest quality.</p>

	<p><strong>Q. What is the Swedish interest in India and your priority topics for the Indian health sector?</strong></p>
<p style="text-align: justify !important;">We have a very fruitful bilateral collaboration with India in the health sector, beginning with our development cooperation in areas such as mother- and child health, midwifery, and work against communicable diseases. Cooperation has now increased to mutually beneficial partnerships. For the past ten years, we have had a Memorandum of Understanding on Health between our countries and next year we are going to celebrate this with an India-Swedish Health Year.</p>
<p style="text-align: justify !important;">Many collaborations of mutual interests are ongoing and being planned that focus in a wide range of areas like antimicrobial resistance, elderly care, pharmaceuticals, non-communicable diseases and digital innovations including artificial intelligence, where the emphasis will be on knowledge transfer, capacity building and skills training.</p>

<p style="text-align: justify !important;">We are in the process of putting out calls and setting up incubators which will encourage close collaboration between the innovators in Sweden and India. Solutions are only sustainable if both sides contribute and share a strong sense of ownership. Progress has already been made in laying the groundwork for such partnerships within the sectors of clean energy and smart cities, and we hope to launch initiatives targeted to healthcare during the coming year. <p style="text-align: justify !important;">India and Sweden plan to commemorate the ten years completion of the MoU in 2019. We hope to enhance our collaborations in new areas with new stakeholders like the new AIIMS that have come up in some states. We also hope to have high-level visits from both sides to enhance policy dialogues and bilateral relations. Some of the collaborative partners are also developing new plans to strengthen their cooperation.</p>

	<p><strong>Q. What can Indian healthcare businesses gain from Swedish healthcare businesses?</strong></p>
<p style="text-align: justify !important;">Any suggestions on possible partnerships? It is not so much what the Indian healthcare system can gain, but rather, how can we join forces to overcome common challenges. Swedish businesses have been in India for many years. Within healthcare, we have AstraZeneca, who recently celebrated 40 years in India, and Getinge who has been investing in the Indian ecosystem for many years. Their focus, like many others, has been in building the capacity of their employees and the users of their products to increase efficiency. The businesses also bring with them lessons from Sweden so that we can help India leapfrog outdated technology. Sweden also ranks high in most innovation indexes – it can be argued that our relatively small size makes collaborations outside traditional silos necessary in order to succeed. Public actors, private companies, and institutions often work together in finding innovative solutions to serious healthcare challenges. We would like to work with India to build spaces for similar ecosystems across India.</p>

	<p><strong>Q. What are the common areas for healthcare research and ongoing partnerships for both sides to collaborate?</strong></p>
<p style="text-align: justify !important;">Under the MoU, there are extensive collaborations between public authorities and universities from both countries. For instance, the Swedish Public Health Agency, Medical Product Agency and Forte (the Swedish Research Council on Health, Working Life and Welfare) have an ongoing collaboration with their Indian counterparts. There are ongoing discussions on extending collaborations with new players like our National Board of Health and Welfare and E-Health Authority as well as between our medical universities such as Karolinska Institute, Uppsala University and the different AIIMS.</p>
<p style="text-align: justify !important;">Through these collaborations, we hope to demonstrate best practices and technologies. We have research calls in different areas for example with EU in big data and artificial intelligence for monitoring health status and quality of life after cancer treatment, under JPIAMR call on diagnostics and surveillance 2019.</p>
<p style="text-align: justify !important;">Innovation-the UNNATI way is an India-Sweden Innovators for strategic business development to address global challenges. It is an industry-supported initiative coordinated by Business Sweden in collaboration with AGNIi-Invest India and ATAL Innovation Mission, NITI Aayog.</p>

<p style="text-align: justify !important;">The Swedish Public Health Agency and the Indian National Centre for Disease Control have an ongoing collaboration with a focus on antimicrobial resistance. Additionally, the Indian Ministry of Health has proposed that Sweden collaborate closely with a new-AIIMS and its associated Health and Wellness Centres. AIIMS-Jodhpur has been suggested to us and we are currently considering the scope of such collaboration.</p>

<p style="text-align: justify !important;">Do you see any collaboration in this field with Indian providers? This is a hot topic all over the world right now. I know India is well ahead in AI technology while Sweden has more long-term experience in extracting relevant information from vast amounts of data. Our patient registries have enabled us to have one of the highest cancer survival rates in the world and that is only one example. I believe we will see some collaborations in the coming year, especially as you are arranging a delegation to Sweden addressing this exact topic in May next year. I will follow this progress closely.</p>

	<p style="text-align: justify !important;">Our best wishes for the Year 2019 to all our readers, advisory group and wellwishers. We have completed three years and this year has been very eventful; we have launched <a href="http://icfhe.in/">IC Foundation of Healthcare and Excellence (ICFHE)</a>, a not-for-profit organization for social interventions and are now connected to the community of 40,000 like-minded people. We have put this issue on the theme of &#8220;<a href="http://innohealth.in/">Innovation in Diabetes-Unmet needs</a>&#8220;given India has 73 million population suffering from Diabetes and 50% of people are not even diagnosed properly which means 7% of the Indian population is diabetic. World’s 18% diabetics are in India, second largest in the world. At present, the world has 425 million diabetics which would become 629 million in the year 2045.</p>
<p style="text-align: justify !important;">We intend to handle this menace by conducting awareness programs, find innovative methods to bring diagnostic tools and treatments at a lower cost with quality &amp; behavior modification programs for preventive and curative measures.</p>
<p style="text-align: justify !important;">The lifestyle changes are the key to the treatment of lifestyle diseases like hypertension, diabetes, obesity and select cancers. They cause complications for other parts of the body such as eyes, heart, kidney, and foot. People must be told of the importance of diet, exercise, and meditation in order to have a healthy lifestyle. We have taken this initiative and created a website to raise awareness of the latest scientific interventions <a href="http://www.experimentswithsugar.in">www.experimentswithsugar.in</a></p>
<p style="text-align: justify !important;">We intend to publish articles regularly of common interest and would also encourage the professionals in this field to share their experiences through our magazine. Our team participated in ESICON 2018, to learn latest on the diabetes treatment from the top experts. We have also surveyed the clinicians for their unmet needs and will soon be releasing the findings.</p>
<p style="text-align: justify !important;">We plan to have the next issue on the theme Cyber4Health: security and privacy issues linked with the health sector and their solutions. Healthcare providers are welcome to share their articles and help us increase awareness in this new arena. Our cybersecurity training programs for senior officers of the Armed Forces and various healthcare organizations were very well received.</p>

It may look as though Copenhageners crave the outdoors, get a kick out of exercise, want to be fit and healthy. It may or may not be the reason. Maybe, it’s just the easiest way to get anywhere.</p>

My perception regarding health sector is to use innovation for reducing healthcare delivery cost keeping quality and benefit at the last mile and bottom of Pyramid. It is to provide the solution to the pain of patients and providers to streamline many processes. The use of technology to achieve this aim and to foster innovation is needed. AI, the blockchain, <a href="https://innohealthmagazine.comresearch/cyber-biosecurity/">cyber security</a> &#8211; technology terms &#8211; are in vogue to be spoken by many without understanding them.</p>

<p style="text-align: justify !important;">The bringing out of Innovation does not need qualification, experience but idea and passion to do it. There are many success stories of such innovations where young school students have brought out miraculous innovations. These innovators need only hand-holding, mentorship and resources. The innovator need not be an entrepreneur as they exit many times for want of capital to scale-up.</p>
<p style="text-align: justify !important;">The ecosystem created is best at present for innovators due to many initiatives placed by the Government. There is a need to put all the information in one booklet because the number of schemes from different ministries is available and many of these are not known to people in the arena of innovation. We have evolved in the last four years to greater heights because of constant endeavor from the government as well as our partners.</p>
<p style="text-align: justify !important;">The idea of creating this <a href="https://innohealthmagazine.com">magazine</a> is sharing of experience of each other and providing a platform to publicize good work of innovators to the global community of 38,000 people with whom this electronic and print magazine is shared. We have published stories of grassroots innovators, InnoHEALTH executive editor <a href="https://www.linkedin.com/in/sachgaur/?originalSubdomain=in">Mr. Sachin Gaur</a> traveled to Nagaland for two-weeks to find innovators in the interiors. This is a very interesting time for innovators who are ready to take the risk can only succeed.</p>
<p style="text-align: justify !important;">I would like to urge all innovators to use knowledge platform created by us to disseminate and share information through international InnoHEALTH conference which also has young innovators’ award, B2B meetings, meet-ups, master classes, <a href="https://innohealthmagazine.com">InnoHEALTH magazine</a>, innovators’ club, <a href="http://icfhe.in/">webinars</a>, <a href="http://innovatiocuris.com/">training, and workshops</a>, and planned study tours in 2019. These activities are under Pvt. Ltd. company InnovatioCuris (IC) means innovation in curing a Latin word which also has an extended arm that is not-for-profit called <a href="http://icfhe.in/">IC Foundation of Healthcare and Excellence (</a>ICFHE).</p>
<p style="text-align: justify !important;">Our journey is just three years old but has grown because of support of our <a href="http://innovatiocuris.com/ecosystem-partners/">ecosystem</a>, partners and <a href="http://innovatiocuris.com/team/">advisors</a>. These words sound more relevant in the contemporary phase in the wake of fast changing global scenario and emerging inevitability for updating virtually by spilled of seconds.</strong></p>
<p style="text-align: justify !important;">Amalgamation of biotechnological and information technology advancement in the 21st century has altered our world at the fundamental level which is evident in the area of health, manufacturing, and food security. Humankind has also witnessed malicious bugs like Y2K and Wanna cry in biological and cyber field respectively. We have reached new landmarks on our understanding of how biological systems work and also discovered ways to meaningfully manipulate these systems as per our advantage/ requirement. Biotech tools, such as gene drives, can deliberately engineer inheritable genetic traits into wild populations, offering a powerful new way to escape from certain vector-borne diseases.</p>
<p style="text-align: justify !important;">Gene editing tools such as CRISPR-CAS9 (Clustered Regularly Interspaced Short Palindromic Repeats associated protein-9 nuclease) are being used globally for quick and precise gene editing. Researchers like to use computers to analyze DNA, operate lab machines and store genetic information. In the health sector, the digitization of biology &amp; metabolic engineering accelerated the development of new vaccines, drugs, and painkillers. Agriculture is becoming smarter/digitized, with farmers relying on data-driven decision acquired through sensors planted in the ground, satellites guiding tractor movements and other new practices. But these emerging capabilities come with a whole new category of vulnerabilities and risks.</p>

<p style="text-align: justify !important;">Over the last five years “technological barriers to acquire and use biological weapon has been significantly eroded.” The security impact of biotech advances goes beyond bioweapon. For example, developments in metabolic pathway engineering also offer opportunities to produce illegal drugs such as heroin. Scientists have already identified how to make the active compounds in other narcotics, such as for cannabis and precursors of LSD. What if a terrorist group or a despotic regime tries to spread modified organisms aimed at striking troops, frightening civilians, or putting food production in disarray? The failed attempt of Japanese cult to obtain Ebola strains from South Africa is one such indicator.</p>
<p style="text-align: justify !important;">Recently Jean and coworkers (2018) highlighted the risks of using gene sequencing technologies to corrupt the databases by altering sequences or annotations. In this article, computer scientists designed a DNA sample that when sequenced, resulted in a data file which enabled the hacker to control the sequencing computer remotely and gave access to the hacker to make changes in DNA sequences. These alterations could delay a research program causing capital, labor loss or can be used in act of terrorism for uncontrolled production of toxins or infectious agents. To mitigate these risks, the culture of the life sciences community needs to shift from trusting blindly to a highly aware and trained community. This also requires intricate relationships between the computational and experimental dimensions of product development workflows.</p>
<p style="text-align: justify !important;">The diverse nature of pathogens and toxins with their potential to be used as a biowarfare agent (BW) could be attributed to multiple factors. These include infectivity (the number of organisms required to cause disease), virulence (the severity of the disease caused), transmissibility (ease of spreading from person to person), and incubation period (the time from exposure of a biological agent to the onset of illness). All these attributes are manageable by modern biotechnology and information related to such experimentation trials is key to any covert attack using these for BW.</p>
<p style="text-align: justify !important;">Similarly, in the cyber world, there is a diversity of malicious codes. These include viruses (programs that replicate in target machinery); worms (self-sustaining programs) and carriers such as a trojan horse to perform a legitimate function with malicious activity. Additionally, Botnets, or networks of computers infected with malicious code, can be coordinated to perform distributed denial of service attacks. For biological weapons, delivery vehicles range from advanced aerial spray technology to contamination of food products or water, while malicious code in cyberspace can be delivered by usage portals, email, web browsers, chat clients, web-enabled applications, and updates. The cyber threat has expanded dramatically in recent years with a series of damage. Terrorists are using cyber capabilities over traditional methods to target 104 countries including India.</p>

<p style="text-align: justify !important;">Governments and security experts have singled out the life sciences sector as being significantly vulnerable to cybercrime. In cybersecurity terms, innovation is fast 50 <strong><a href="https://innohealthmagazine.comvolume-3-issue-3/">Volume 3 | Issue 3 | July-September 2018</a></strong> becoming a double-edged sword for life sciences clients. Recently <a href="https://www.fireeye.com/">FireEye</a> disclosed the threat posed by two Advance Persistent Threat (APT) groups which gained access to the environment of a leading pharmaceutical company for up to three years prior to detection. They stole IP and business data from the victim, information on bio cultures, products, cost reports, and other details pertaining to the company’s operations abroad. There is nothing more important to a pharmaceutical organization than the formula for one of its new drugs.</p>
<p style="text-align: justify !important;">For cyber biosecurity, employee training should be given priority. It can greatly increase an organization’s general awareness of these new risks. Similar to biosafety training, cybersecurity training modules and policies should be introduced. Secondly, organizations should perform a thorough analysis of its exposure to cyber biosecurity risks not covered by existing biosafety and biosecurity policies.</p>
<p style="text-align: justify !important;">Training exercises based on this type of analysis will encourage participants to review their workflows and identify their vulnerabilities. It is high time now to evolve a policy framework to detect and prevent security threats that may compromise life sciences assets. It includes guidelines on synthetic DNA targeted companies that provide DNA synthesis services to monitor research focus and relates features.</p>
<p style="text-align: justify !important;">Bioinformatics software is still not hardened against attack. Encouragement of widespread adoption of standard software best security practices like input sanitization, the use of memory safe languages or bounds checking at buffers, and regular security audits is necessary. Patching still remains challenging as the analysis software are often located in individually managed repositories and not regularly updated. One solution is to use a centralized repository to manage updates and deliver patches, similar to the APT package manager.</p>
<p style="text-align: justify !important;">These could also be signed to ensure their authenticity. In the case of file sharing, the sequencing files themselves could be signed by verified research groups before uploading them to centralized databases. This is just the glimpse of the long list of strategies that need immediate deployment, continuous review, and improvement with time.</p>

	<p style="text-align: justify !important;">India’s Environment, <strong>Forest and Climate Change Minister Dr. Harsh Vardhan</strong>, in an exclusive interview to <strong>InnoHEALTH Magazine</strong>, appealed to the people to give up single use of plastic and expressed confidence that a perceptible and measurable difference in the country will be brought about on plastic pollution and other environmental issues on the occasion of the <strong>World Environment Day 2018</strong>.</p>
<p style="text-align: justify !important;">He is also the Science and Technology Minister. In March this year, Prime Minister Narendra Modi called for a &#8220;<strong>Fit India</strong>&#8221; movement, focusing on the benefits of yoga, in ‘<strong>Mann Ki Baat</strong>’. Mr. Rajyavardhan Rathore, the newly appointed Minister for Information and Broadcasting, seems to have taken a cue from the PM to take the initiative forward. He recently tweeted a video of himself doing push-ups with the hashtag <em><strong>#HumFitTohIndiaFit</strong></em>. Since then, the fitness campaign zoomed into a debate and many people accepted the challenge and started posting their videos.</p>
<p style="text-align: justify !important;">International awards winner filmmaker Janet Tobia who has won laurels for her documentary “Unseen Enemy” flags major concerns over lurking threats on the environment. She was interviewed by our magazine to highlight her views on the Environment Day. The Emmy award winner said that her movie Unseen Enemy examines why in the 21st century. We are experiencing a rash of diseases that were once only outbreaks but have now become full-blown epidemics. She is precisely right on her perceptions and the fresh outbreak of Ebola in Congo vindicates her concerns.</p>
<p style="text-align: justify !important;">Amid growing number of violent incidents against young children, we interacted with UNICEF’s India In-charge. Fresh from her experience in managing emergency situations, Dr. Yasmin Ali Haque handles the tsunami of problems during her current assignment. She has disclosed that the Indian government in collaboration with UNICEF is developing a comprehensive plan on digital safety for children. Digital literacy is key to ensuring that all boys and all girls stay informed, engaged and safe online. There is a need to galvanize ‘zero tolerance’ towards violence against children.</p>

Poverty is a significant issue of the country, despite having one of the fastest-growing economies in the world, clocked at an economic growth of 7.6 per cent in 2015. It is estimated that 23.6 per cent of Indian population, or about 276 million people, live below $1.25 per day.</p>

	<p style="text-align: justify !important;">As Health is a state subject-there is lot of mismatch in states like Kerala and Punjab have best health indicators, while Uttar Pradesh is the poorest as per the NITI Aayog report. Indian government spends approx. 1.5 per cent of its GDP on health sector and mplans to make it 2.5 per cent which is much less than many developing countries while the USA has dedicated 16 per cent of its GDP. India is riddled with very basic public health issues leading to disease burden. Keeping this in mind, Prime Minister Narendra Modi has launched six initiatives: Open Defecation free country by 2019, Swachh Bharat Mission, National Health Policy 2017, Digital India with e-health, medical device manufacturing and door-to-door screening of chronic diseases.</p>

Challenges bring opportunities such as Indian healthcare market is around US$ 100 billion while it is expected to grow US$ 280 billion by 2020. The healthcare IT market is US$ one billion and is expected to grow 1.5 times by 2020. There is requirement of 7 lakh hospital beds which need investment opportunities of 25-30 billion US dollar. We need to bring innovations in hospital planning,devices, diagnostics, drugs and use of technology to reduce healthcare delivery cost and yet quality.</p>

Public hospitals to National Rural Health Mission have not delivered as required because of corruption, lack of resources and application of management practices. The need is not old wine in new bottle but strict control in implementation of various schemes launched.</p>

	<p style="text-align: justify !important;">Hospitals in India play a vital role in healthcare with most of the high-end medical facilities available only at the hospitals. But these large institutes are very far away from the masses as more than 70 percent of the Indian population lives in rural areas. That leads to a reality that medical facilities must go beyond hospitals, with an innovative approach, to serve the masses and make India a healthy place.</p>
<p style="text-align: justify !important;">Addressing the issue as keynote speaker, Professor Krishnan from IIM, shed light on the management aspect of innovations and spelled out the needs of the industry for the sake of ensuring that all innovations become a reality and benefit the masses.</p>

	<p style="text-align: justify !important;">In his words, he said, “Innovations in healthcare must consider three aspects: Begin with identifying core functionalities of equipment without having to sacrifice on the safety aspect. Secondly, start from scratch. Build upwards and incorporate all the technical features that you feel are essential to the product; downscaling features will not necessarily downscale your cost; lastly, collaborate and team up with others who will complement your product, effort and intentions. There’s no one entity that has all the knowledge and resources to develop any product in its entirety. So build up on sound partnerships using complementary technology and process.”</p>
<p style="text-align: justify !important;">In support of the view, Dr. S. Venkataramanaiah highlighted data usage as one of the most effective tools to implement an innovative approach for spreading good health. He said, “Data clearly indicates that there’s a huge demand for private players in the health sector. Everyone is and needs to be medically insured and ensured of good health by private or a government body. On the other hand, healthcare has some major deficiencies like availability of medical help at anytime and anywhere. Like you have roadside assistance available all over the country, such a facility for health and medical urgencies is still not available.</p>
<p style="text-align: justify !important;">“So innovative approach in production, processing, delivering service or business management is the answer to finding solutions to new challenges faced in the health sector,” he said</p>

	<p style="text-align: justify !important;">In a more myopic approach to the topic of distance solutions Dr. Mukesh Taneja shared some startling data from the WHO.</p>
<p style="text-align: justify !important;">“According to World Health Organization’s 2010 data 285 million people are visually impaired; 90 percent of these are in the developing world; and 80 percent of these are preventable,” he said.</p>
<p style="text-align: justify !important;">And speaking of India, 70 percent of the population lives in rural areas, however 90 percent of the medical facilities are located in urban areas. So, there’s a huge gap between the care providers and the population who they should be reaching. And we believe that telemedicine – a very innovative approach &#8212; is the answer to bridge this gap. Eye problems can more effectively be pictured and online imaging of affected eye can easily be examined and diagnosed by a distant located ophthalmologist.<br />
“We, at LV Prasad Eye Institute, extensively use this telemedicine platform and are reaching out to the rural population with the help of health workers and specially trained technicians who operate our remotely located Vision Centers.”</p>
<p style="text-align: justify !important;">The next panelist, Dr. Santanu Biswas, from the UAE shared his international perspective and said, “If you are to work on remote distance technology, you cannot work in isolation. You choose partners in technology, data procurement and delivery system. This means your dependence on your partners is even more critical as you depend on their credibility, technical compatibility and similar intentions. This is the reason that even though we have been talking about digital health and e-health; but other than the fact that it’s been a buzzword for the last 15 years, it has not become a well-charted module. We are still talking about the concept and exploring innovations that will truly support the telemedicine or e-health platform. This is because we are trying to marry and merge a conventional health system with an imposed telemedicine process.”</p>
<p style="text-align: justify !important;">Concluding his views on the subject he summed it up saying, “I think we need to rediscover a totally new innovative approach to telemedicine, which will use innovative technology, innovative approach and an all-new processing and delivery system. And this will evolve, as this is the future.”</p>
<p style="text-align: justify !important;">Dr. Tarun Ramole, speaking in the role of an investor and entrepreneur spoke candidly about the business aspect of telemedicine. In his opinion, Telemedicine, as a business is a very lucrative platform. The gap between the resources and the users remains very high and in a country like India is not going to be easy to fill that.</p>
<p style="text-align: justify !important;">“Telemedicine will be a very reliable option to bridge the gap and that makes it a very viable business option. But, like most businesses, telemedicine also has challenges that are very unique. Starting with, most start-ups end up replicating what is already there in the market “with improvisations”. What we need is new solutions and not improved solutions. You must think innovatively to innovate things,” he concluded his opinion with a very upfront approach on the subject.</p>

	<p><span style="color: #0071b2;"><strong>Introduction</strong></span></p>
<p style="text-align: justify !important;">The famous Silicon Valley investor Marc Andreessen says, “software is eating the world”. By, which he means that increasingly we are bringing software into systems to increase efficiency, lower down the cost or time involved in the process. Interestingly, humans also do software writing and humans are prone to make mistakes. It is estimated by various experts that 1000 lines of code (KLoC) has approximately 15-50 bugs present. Bugs here mean mistakes made by the software programmer while writing the software code. Bugs often result in some kind of malfunction or wrong output. Some of these bugs can lead to exploit by a third party making the larger system vulnerable or as we call hackable. As long as humans will write software, bugs will be there.</p>
<p style="text-align: justify !important;">In a typical software company as bugs are discovered, new code is written to fix these bugs. The new code might further result into new bugs hence the cycle continues. At the consumer end, we keep receiving software updates over the air, as we use our phone / laptops or other devices, which are many times an attempt of the software company to overcome the past mistakes.</p>
<p style="text-align: justify !important;">A lone computer hacker or an organized crime group looks at these software updates (sometimes called patches) very curiously as for them this could be a chance of hitting the jackpot! They reverse engineer it and try to understand the bug, that the patch is trying to cover. Very often systems are not updated with latest updates. Leading to most system having a known vulnerability, which the hacker can take advantage after understanding it well. Hackers further can create a simple script (programming code snippet) to some sophisticated software, which can then take advantage of the vulnerable system. We often call such a program as malware, as it is built with bad intention.</p>
<p style="text-align: justify !important;">Today, as we talk it has become from a hobby crime to organized crime! Software companies regularly receive communication from bounty hunters about exposing their critical software bugs and in exchange not to do so, hackers want to charge them bounty money. Some software companies have gone further and engaged these bounty hunters to reduce security risks in their software.</p>
<p style="text-align: justify !important;">In some cases the hacker is not interested in the bounty money (hence they do not inform the software maker) but rather interested in exploiting the bug. Sometimes, the bug is not known to the software maker or anyone else in the world and can be converted into a lethal attack. Such attacks are known as a zero day attack! As prior knowledge of such a vulnerability does not exist. Hence, most software security solutions, like anti virus software do not work on them. Further selling the knowledge of exploit as lethal software is now called as a cyber weapon. Nation states are now engaged in buying or building such software to infect systems of enemy states. Hence, we have come very far in the business of software bugs, where the enemy could be a lone developer, an organized crime group or a Nation state.</p>

	<p><span style="color: #0071b2;"><strong>How is software eating the health sector and the threats linked to it?</strong></span></p>
<p style="text-align: justify !important;">In the above section we discussed in general, how the exploitation of software is increasingly becoming a serious business. While, we have seen many examples in last 30 years from a hobby software programmer to Nation states taking advantage of the software driven vulnerabilities. We would like to share some examples closer to the health sector.</p>
<p><strong>1. Malware affecting data systems</strong></p>
<p style="text-align: justify !important;">Hospital information systems and similar information systems as part of the healthcare delivery have become very commonplace and one of the core component of the system. As pointed out in the introductory section, organized crime groups are now looking to exploit software bugs for commercial purposes. One of the ingenious way that they have developed is a malware known as ransomware . Ransomware is a malicious computer program which when executed on a system encrypts the data with very strong encryption making it unusable for hospitals or any other care provider to access patient records or other vital information. It then demands a ransom inform of bitcoins (a crypto currency) in order for the victim to have the key to decrypt the vital information. In recent incidents of ransomware infection, some hospitals in USA have even demanded millions of dollars as ransom and some have even paid.</p>
<p style="text-align: justify !important;">The mitigation strategy for countering ransomware for any organization would be a strong backup of data. Also, creating awareness among the employees on sources of malware and reducing the chances of accidental infection of the workplace systems.</p>
<p style="text-align: justify !important;">The long-term solution of tackling such organized crime is a better international legal framework, which allows international prosecution and cooperation among law enforcement agencies.</p>
<p><strong>2. Denial of service attacks on ehealth services</strong></p>
<p style="text-align: justify !important;">In 2007 there was a distributed denial of service attack that took place in Estonia. A statue of the Russian soldier was removed from the Tallinn Square, capital of the country. Which sparked a response from sympathizers from Russia and it brought down the Estonian economy for three days. Estonia being one of the most advance countries when it comes to take up of e governance services, ehealth being one of them. The entire attack costed less than 50,000 US dollars. That was the first Denial of service attack the world saw at the level of a nation state.</p>
<p style="text-align: justify !important;">The basic premise behind such an attack is that you have a service (e-service) to be provided to citizens over Internet like their own health records for example. The provider would have some finite amount of bandwidth and computing power at the backend of the service correlating to the average load on the service. In a distributed denial of service attack, the attacker uses compromised computing devices (commonly known as a bot) to access the Internet service. The botnet, which is a collection of such bots could be having thousands or millions of such devices that simultaneously access the service. The service provider is not able to distinguish the normal traffic from the bot traffic and often the server crashes under the heavy load. For a normal user trying to access the service, the service is unavailable because of the finite resources of the server being exhausted by the bot traffic. Hence, it is called a denial of service attack.</p>
<p style="text-align: justify !important;">Hence, when a city, state or a nation is considering providing an eservice to citizens it could witness such attacks. One strategy to mitigate such attacks is to have tracking of the server traffic for any anomalies and having redundancy available in the system. This is achieved many times by putting the service on a cloud, which can tolerate such traffic fluctuations.</p>
<p><strong>3. Data leak and breaches</strong></p>
<p style="text-align: justify !important;">Many health systems or systems require some kind of authentication mechanism to log in to the system in order to access the service. Many a times these are text password based systems behind which, important patient profile or health records information is stored. The largest of the companies like that of Google, Microsoft etc have seen attacks where the attacker is able to leak the passwords of millions of their customers. Such scenarios result in massive breach of data privacy and compromise for customers.</p>
<p style="text-align: justify !important;">Good security practices, proper encryption of data and regular updates of the system are some of the key considerations for avoiding such instances. Nowadays, two-factor authentication has become a standard practice for making the authentication systems more robust. However, still some user awareness is needed to opt for better security practices whenever possible.</p>
<p><strong>4. Hacking medical devices and health system</strong></p>
<p style="text-align: justify !important;">If we look at the building blocks of the health systems, where information technology is deeply integrated. We have already covered the health information systems, eservices and patient interface of authentication into the services. However, increasingly we hear about Internet of Things (IoT) devices in the health sector domain. Which means the integration of Internet services into traditional medical devices or new age devices, which have also connectivity. For example, a thermometer which can send the temperature data to your phone or a stethoscope which can record the patient breathing sound and upload in a server for finding patterns of lung diseases. These are powerful use cases and provide great opportunity to clinicians and care providers, where they have greater computation power available to them and they are able to do more with less. However, these IoT devices are prone to the same kind of attacks as any other communication device or a software program. They can be compromised to show wrong values and totally messing up the diagnosis. There are already such instances. One such instance not related to health sector but important is of the Stuxnet. Stuxnet was designed for the SCADA systems of Iranian nuclear program by USA and Israel in order to delay their nuclear program.</p>
<p><strong>5. Stealing identity information</strong></p>
<p style="text-align: justify !important;">As mentioned in the point 3, about data leaks and breaches at the system level. One problem, which can arise from such an attack, is a further more damaging attack that is stealing of identity information. In India, mobile phones to receive an sms message containing one time password is increasingly becoming a standard practise because of being cost effective, simple and secure. Any such application, which you may install on your phone, can also get access to the sms and other features of your phone. Meaning the incoming sms or calls can also be stolen by this application to complete the transaction on your behalf. As increasingly we have to prove ourselves using biometrics or passwords to online systems. It is possible for the attackers to steal these credentials and access our records without our knowledge. Hence, any third party applications that we install on our devices (especially phone) , we need to be very careful about the type of access control they have on our devices.</p>
<p><strong>6. Implantable medical devices with communication interfaces:</strong></p>
<p style="text-align: justify !important;">In 2007, the former US Vice President, Dick Cheney’s implanted pacemaker’s wireless communication was disabled fearing a terrorist attack. This sounds like science fiction to many but incident has already happened ten years back! Many of the medical devices are built with a communication interface and it is quite normal for a typical pacemaker or other such devices to have a Bluetooth or a similar communication technology based interface for remote diagnosis and other purposes. While, the communication ability of such a device was planned for looking at the state of the pacemaker it was not designed with keeping security in mind. Hence, it is possible that someone can connect to a critical device like pacemaker and shuts it down remotely.</p>
<p style="text-align: justify !important;">One more reason that such exploits are possible increasingly as computing is becoming cheaper. What seems strong security today might not be strong tomorrow. However, an implantable device might stay in the patient’s body for tens of years. Hence, we need to have a long-term view on the communication interfaces and their capabilities on such devices. We need to make considerations on control and information capabilities of these interfaces. Misuse of control capabilities can lead to even death and misuse of information capabilities can lead to breach of patient privacy.</p>

	<p><span style="color: #0071b2;"><strong>Way forward: why Internet is the new breeding ground for crime?</strong></span></p>
<p style="text-align: justify !important;">The law of the land governs the Internet in every country and hence the legal regime globally is very fragmented. However, a user of Internet does not see any borders or walls and so is the criminal. They build their criminal businesses where they do not fear strict government action and often for paltry sums the user or the national law enforcement agencies do not pursue the criminal cases cross border.</p>
<p style="text-align: justify !important;">On top of it newer crypto currencies like Bitcoins, makes it easy to make such transaction in an anonymous manner. Dark net marketplaces provide a breeding ground for criminals to conduct illegal transactions of billions of dollars without getting caught. So, the three important components, weak legal enforcement, anonymous currency and secret marketplaces are enabling the cyber crime to flourish. If we want to slow it down, we will need greater international collaboration among lawmakers and user awareness at all levels.</p>
