Ms. Ishi Khosla is a practicing clinical nutritionist, columnist, author, a researcher and an entrepreneur. She is actively involved in clinical practice at the Centre For Dietary Counselling in Delhi and has spearheaded the launch of the first of its kind health food company “Whole Foods India”. She’s authored several books, including Eating at Work, The Diet Doctor, Is Wheat Killing You? and The Cholesterol Facts. She has been nominated as a member of the Programme Advisory Committee, Doordarshan Kendra; is a member of the Governing Body of Lady Irwin College, Delhi University; and is a consultant to several corporate and multinational organizations.

She has been felicitated by the President of India for work in the field of nutrition. She has also been listed among the 25 most powerful women in the country by the India Today Group.

Having spent the last 30 years focusing on the functional aspects of food in treating chronic health problems that we have faced, I have to say that it is for the first time in my professional experience that the word ‘I’, yes, the Almighty immunity has been spoken of at such a global scale. Immunity is being sold” by every brand, every food label and even toilet cleaners are being tagged with immune benefits …..

This was possibly inevitable & fast coming- the aggressive pace at which our food production & consumption have grown & changed. It is only natural that in the times of a health crisis, the entire world is pausing to, in fact, reflecting on what we are actually putting “inside our bodies”

Pick up any prescription of a patient fighting COVID-19 today and you will see what I am talking about in bold ink – Vitamins D, C + Zinc and Magnesium even probiotics – some of these elements have become the salient factors in fighting the pandemic across the world. A review & meta-analysis published in the Frontier of public health this year showed an exceptionally high mortality among patients in ICU who have Vitamin D levels less than 20 ng/ ml . Evidence indicates that vitamin D helps by preventing the cytokine storm and subsequent acute respiratory distress syndrome (ARDS ) which is commonly the cause of mortality. In addition a low Vitamin-D level was associated with an 80 percent higher risk of COVID-19 infection compared to those with normal levels.

If we zone in to the latest research we need to recognise the fact that our gut has a mind of its own with the underlying microbiome.

It was already estimated in the year 2011, that around two billion people in the world are deficient in one or more micronutrients. A review & meta analysis done in Singapore published in Nutrients in May 2021 clearly states that absence of micronutrient deficiency is strongly associated with lowered risk of infection & better outcomes among COVID-19 patients & that it offers a cost effective preventive & therapeutic approach for the pandemic. Several programmes have been launched over the years in India to improve nutrition and health status of the population; however, a large portion of the population is still affected by micronutrient deficiency. When linked with food allergies, the connection becomes even clearer – to give you all an example from my area of expertise – wheat-related disorders like Celiac Disease which have gone up by Five Fold since 1974 (that’s roughly in the last 50 years) (Annals of Medicine) such diseases are associated with multiple micronutrient deficiencies due to malabsorption, high inflammation and lower immunity; most common nutritional deficiencies include iron, calcium, folate, Vitamin B-12. Ample research has also shown that iron deficiency anaemia can affect our immune system – the body’s natural defence system. This increases our vulnerability to infection. But it has taken a global health catastrophe to shine light on this underlying issue that is affecting us in more ways than we know and there’s never been a better time in history to establish the importance of food and nutrition in bridging the gap when it comes to being future-ready as a country to fight for better health.

If we zone in to the latest research we need to recognise the fact that our gut has a mind of its own with the underlying microbiome. More than 70% of our immune system resides in our gut, so it is obvious that for maintaining a resilient immune system, we need to have the right gut composition of pro and prebiotics. The focus has been to constantly reduce systemic chronic & acute inflammation through anti-inflammatory agents like Ginger, Turmeric, Giloy, Tulsi, Honey and other Phytonutrients available in brightly coloured fruits and vegetables and healthy fats like Omega-3 and medium chain triglycerides in virgin coconut oil. By simply cutting down pro-inflammatory agents from our diets like chemical additives, trans fats and allergens plus reducing sugars and high glycemic carbohydrates ( most cereals & refined carbohydrates ) we can really make a lot of difference in fighting health issues.

Truth of the matter remains that prescriptive advice only makes sense in times of adversity when it’s often too late, when the fight is already underway and a lot has to be done.

Truth of the matter remains that prescriptive advice only makes sense in times of adversity when it’s often too late, when the fight is already underway and a lot has to be done. Preventative health continues to be a buzz word in a country like ours where lifestyle related disorders are growing at a super high pace and are expected to cross 1 billion cases by 2022.

It has to start much earlier, this entire process of awareness and the responsibility lies equally in all our worlds – right from agricultural practices, soil quality to fortification, biodiversity, introduction of alternate grains & millets , manufacturing practices , waste management, positive labelling to disseminating the right information. There are so many glorious examples of best practices we can look upto to address some of the issues we are facing today. Addressing Vitamin D deficiency through food fortification in the West (and in India), supplying probiotic shots of fermented milk to citizens of Japan to fight gut infections at scale under Dr Minoru Shirota since 1935 , the innovation of FOSHU and FFC ( foods & functional claims ) system of labelling health claims in Japan , elimination of pro-inflammatory trans fatty acids started in Denmark, Canada and New York.

I, in my own small way, have taken the opportunity in the past to not only preach but showcase a model to manufacture and retail safe foods, high on nutrients and functional foods free of undesirable constituents like chemical additives, trans fats using healthy ingredients, apart from this the allergen free foods including gluten and dairy free, and more. Among the first of its kind food company Wholefoods started as a home kitchen outfit, expanded to have presence in some of the leading hospitals across the NCR region. It clearly established to me and my colleagues that there is an appetite for health if delivered in a sound holistic manner.

It is time for collective action, integrated thinking and innovative solutions to find ways that can actually work for the people and the planet. Coming together of all stakeholders with a shared vision will ensure that the ultimate goal of ending all forms of malnutrition and nutritional security is achieved and we need not have to wait for a pandemic to build a robust immune system. It is the only sound investment to build a healthy population that will eventually pave the way for a healthy nation.

World Health Day

April 7th marks the birth anniversary of the World Health Organisation (WHO) and along with drives worldwide attention by spreading the message of the importance of global health each year.

Ranging from imperative issues such as food safety, diabetes, depressions, etc., this year the World Health Day embarks the mission of people-centered care irrespective of their financial hardships. The 2019 theme envisions the Universal Health Coverage (UHC), i.e., health for all. There are millions of people who have no access at all to healthcare. Majority of the populations are forced to choose between healthcare and other daily needs such as food, clothing, and shelter. In India, a lot of efforts need to be put in to ensure adequate medicines, affordable health policies, and financing strategies to enable everyone to get access to equal healthcare. “Universal” in UHC means “for all”, without discrimination, leaving no one behind, i.e. providing healthcare facilities which are

• Adequate

Addressing the Financial Crisis

“Universal health coverage is also about ensuring access to essential quality care and financial protection,” says Dr. Mahjour. It aids in enhancing people’s health at large and increasing their life expectancy. Under this, many countries are protected from epidemics and the risk of hunger. This paves a way for new jobs, enhanced economic growth, and gender equality.

To make the phenomenon of Universal health coverage achievable globally, there should be concrete roadmaps which highlight the gaps and obstacles patients face in accessing proper healthcare. This can be done only when there are identified challenges, key actions and sustainable goals to be taken care of.

• There must be a holistic range of health and related services
• Quality care is a must

Where India Stands – The New “Health-Y” Politics

The much-awaited general election will be held in India in April 2019 when the Indian voters will go to the polls. It is anticipated that whichever government comes into play will surely set a course for newer and better health policy.

Recent researches reveal an alarmingly rapid epidemiological transition. The occurrence of diseases such as cardiovascular diseases, diabetes, and cancer are rapidly on a rise. For instance, since 1990, the number of Indians with diabetes has increased from 26 million to approximately 65 million to date. The incidence of all cancers has increased by 28% between 1990 and 2016, with new cases of cancer emerging each year. India not only seems terribly engulfed in the swirling epidemic of non-communicable diseases, but it is also in the grip of a mental health emergency. The rate of global suicide deaths is speculated to be 1.1 times higher among women and 1.4 times higher among men than global averages in 2016.

It would be unfair if we highlight only the negative side of the picture. After many years of neglect, the Government of India has finally recognized and considered the perils of public discontent relating to health. Prime Minister Narendra Modi has initiated a new programme called Ayushman Bharat, which along with focusing on preventive health, emphasizes affordable health.

Ayushman Bharat majorly comprises two new flagship programmes. First, the creation of 1.5 lakh health and wellness centers across the whole country. These centers will primarily be responsible for providing primary healthcare facilities to provide health coverage for all. The second includes the National Health Protection Mission (NHPM)—a system of health insurance that intends to cover 500 million people. It covers secondary and tertiary care for the poorest and most vulnerable strata of society. These twin programmes together must help India improve access to quality health services and reduce out-of-pocket health expenditures.

Modi is the first Indian Prime Minister who has grasped the importance of health for Indian citizens and has prioritized universal health coverage as part of his political platform. “Modicare” as it is sometimes addressed to, can become a decisive issue in the upcoming general elections.

Way Forward for a Healthy Nation
Does Universal health Coverage still seem a distant goal in India!

The scant funding from the Central and state governments seems the root cause and the major hindrance in achieving UHC. The Central Government has recently announced a massive expansion of healthcare facilities.

Through its ambitious National Health Protection Scheme (NHPS), the Government of India provides a framework for universal health coverage in which it prioritizes healthcare delivery and its easy access. By reducing out-of-pocket payments, expanding service coverage, improving quality of services, and ultimately advancing UHC, countries and in particular, India with schemes like NHPS in place can transform the health of their people and bring drastic changes in society. As stated by WHO Director-General: “No one should have to choose between death and financial hardship. No one should have to choose between buying medicine and buying food.” UHC indeed is a way to go ahead.

Innovation has become a buzzword for every sector and similarly, the next buzz would be Hackathon, many without knowing the meaning of these words tend to use it to show their updated knowledge. My perception regarding health sector is to use innovation for reducing healthcare delivery cost keeping quality and benefit at the last mile and bottom of Pyramid. It is to provide the solution to the pain of patients and providers to streamline many processes. The use of technology to achieve this aim and to foster innovation is needed. AI, the blockchain, cyber security – technology terms – are in vogue to be spoken by many without understanding them.

The bringing out of Innovation does not need qualification, experience but idea and passion to do it. There are many success stories of such innovations where young school students have brought out miraculous innovations. These innovators need only hand-holding, mentorship and resources. The innovator need not be an entrepreneur as they exit many times for want of capital to scale-up.

The ecosystem created is best at present for innovators due to many initiatives placed by the Government. There is a need to put all the information in one booklet because the number of schemes from different ministries is available and many of these are not known to people in the arena of innovation. We have evolved in the last four years to greater heights because of constant endeavor from the government as well as our partners.

The idea of creating this magazine is sharing of experience of each other and providing a platform to publicize good work of innovators to the global community of 38,000 people with whom this electronic and print magazine is shared. We have published stories of grassroots innovators, InnoHEALTH executive editor Mr. Sachin Gaur traveled to Nagaland for two-weeks to find innovators in the interiors. This is a very interesting time for innovators who are ready to take the risk can only succeed.

I would like to urge all innovators to use knowledge platform created by us to disseminate and share information through international InnoHEALTH conference which also has young innovators’ award, B2B meetings, meet-ups, master classes, InnoHEALTH magazine, innovators’ club, webinars, training, and workshops, and planned study tours in 2019. These activities are under Pvt. Ltd. company InnovatioCuris (IC) means innovation in curing a Latin word which also has an extended arm that is not-for-profit called IC Foundation of Healthcare and Excellence (ICFHE).

Our journey is just three years old but has grown because of support of our ecosystem, partners and advisors. We would like to have regular feedback to make it better and achieve our vision of “Healthcare delivery to all at optimum cost with quality”. Let us join hands to make India a healthy nation-leading way for many countries, even developed ones looking for frugal and reverse innovations from India.
