	<p style="text-align: justify !important;"><a href="https://innohealthmagazine.compersona/artificial-intelligence-coming-big-way-healthcare-sector/">Artificial Intelligence (AI)</a> is a hot topic, simply put &#8211; it’s a way of making a computer think intelligently, in a way human think and over a decade now it has managed to be fairly successful. It has found application in several domains, from consumer electronics like smartphones and smart home devices like Amazon’s Alexa to very niche applications in academic research. What began as a nascent academic pursuit to enable computers to think and solve problems using human-like cognitive capabilities has now invaded most aspects of human life, <a href="https://innohealthmagazine.comtrends/first-humanitarian-medicine-delivery-drone/">medicine</a> and healthcare is no exception.</p>
<p style="text-align: justify !important;">Modern medicine has discovered around 60,000 ways things can go wrong with the human body and over thousands of years have probed these illnesses and disorders to better understand and treat them, one drug, one technique at a time. In recent years, however, there has been a dramatic shift in the pace of innovation in healthcare, especially with the advent of artificial intelligence. <a href="https://innohealthmagazine.comwell-being/artificial-intelligence-ayurveda-protocol/">Artificial Intelligence</a> is an umbrella term used to cover a wide array of algorithms which mimic human cognitive functions and are self-correcting, and can ‘learn’ from a dataset.</p>

	<p><strong>A mountain of unstructured data</strong></p>
<p style="text-align: justify !important;">One area where AI would do heaps of help to physicians and medical practitioners is to deal with the insurmountable amount of clinical unstructured data. Nearly 80% of the clinical information is “unstructured” and in a format incomprehensible to <a href="https://innohealthmagazine.comnewscope/digital-information-security-healthcare-act/">health information systems</a>. Thus, getting useful information from these so-called unstructured databases becomes a labor-intensive task. To top that, clinical data is doubling every three years; which leaves the healthcare system with a massive volume of unsorted heterogeneous patient information which may hold answers to several <a href="https://innohealthmagazine.cominnohealth-conference/challenges-redefining-healthcare-landscape/">health challenges</a>, but strictly speaking, is of little use in its current form. This <a href="https://innohealthmagazine.cominnohealth-conference/challenges-redefining-healthcare-landscape/">challenge in healthcare</a> of too much data, too little insight can be alleviated by employing Natural Language Processing (NLP), a form of AI which identifies key information from spoken or written human input, such as physical examination records, handwritten lab notes, discharge summaries etc. The promise of NLP lies in its ability to turn this big data into smart data. It can be applied to mine big blocks of clinical data and convert that into organized curated easy-for-retrieval information, which can make documentation of clinical information more manageable. In 2014, IBM’s Watson collaborated with Epic Systems and Carilion Clinic to analyze massive 21 million records in just six weeks and pulled important information about risk factors and other features from examination notes written by physicians and clinical laboratory results into organized EHR templates, and further used predictive modeling to identify patients at risk to congestive heart failure with an assuring 85% accuracy rate. Similar efforts of using NLP to tackle cancer and genomics datasets are in process. NLP algorithms thus can be employed with much effectiveness to unlock healthcare’s big data crisis to extract clinically relevant information and make it available for doctors to make smart decisions about their patients.</p>

	<p><strong>Can Artificial Intelligence replace a doctor?</strong></p>
<p style="text-align: justify !important;">Another facet of healthcare where artificial intelligence can find use is analyzing structured data namely genetic data, imaging data from X-ray scans, CT scans, MRIs, etc. and electrophysiological data obtained from electrography of the heart, brain, and other body parts. Machine learning plays a major role owing to its ability to ‘learn’ and make predictions from data without explicit programming. Of the many machine learning algorithms, two such algorithms have been used extensively in both research and healthcare, namely Support Vector Machine (SVM) and Neural Networks (NN), both use supervised learning models. SVM, in particular, has been useful in tasks involving classification and for novelty detection. For example, a 2012 study used SVM to identify imaging biomarkers of neurological and psychiatric disease. SVM has been used as prediction models for <a href="https://innohealthmagazine.comtheme/recent-breakthroughs-diabetes-research/">diabetic</a> and prediabetic patients. In 2010, a research group from Korea applied SVM to make predictions about heart failure patients and their adherence rate to their medication. Two researchers from Australia used SVM for the <a href="https://innohealthmagazine.comtrends/faster-diagnostic-tests-developed-tb/">diagnosis</a> of cerebral palsy gait with an accuracy rate of 96.8%.</p>
<p style="text-align: justify !important;">Neural Networks, on the other hand, form another major chunk of AI algorithm in healthcare. NN algorithms are vaguely based on biological neural networks, in which a collection of interconnected nodes processes the data like how neurons communicate in a human brain. The potential of NN has been multiplied manifold, thanks to the advent of Deep Learning which is an evolved form of NN, it uses multiple hidden layers that can be used to process complex multidimensional data like a human brain. A huge portion of NN algorithms is used for <a href="https://innohealthmagazine.cominnohealth-conference/advances-in-diagnostics/">diagnostic imaging</a>. Early last year, a study published in Nature used CNN, a type of deep learning NN algorithm to identify skin cancer from clinical images. The algorithm which was trained on 29,450 clinical images, was highly specific and sensitive to detection and was on par with the performance of an expert dermatologist with over 90% accuracy. A 2016 study used a variant of deep learning NN to identify interstitial lung disease using CT scan images with 85.5% accuracy. Google’s artificial intelligence team employed deep learning algorithms to study pictures of the back of the eye, for the detection of diabetic retinopathy, a blinding disorder in diabetic patients. Their results showed above 90% accuracy in both sensitivity and specificity of detection, which is at par with a skilled ophthalmologist.</p>

<p style="text-align: justify !important;">Some areas where artificial intelligence surpasses humans is in looking for patterns in data and in making predictions about that data. Processing thousands of images and looking for a subtle discernible pattern within huge volumes of data is a tough task for humans, but that’s what Shinjini Kundu, a physician at the <a href="https://www.upmc.com/">University of Pittsburgh Medical Center</a> has been doing. Her AI algorithms examine images like MRI scans for subtle differences which may not be perceptible to the human eye, and she has employed this to study osteoarthritis and to predict its development way before it’s diagnosis with a whopping 86.2% accuracy. Similar algorithms can be used to see nuanced differences in electrocardiograms, CT scan images and even in oncology to look for invisible patterns of disease onset and progression. As artificial intelligence algorithms get better after each iteration, routine lab tests like X-rays, CT scans, MRI scans, ECG etc. would fall into the domain of artificial intelligence for more quick and reliable results.</p>

	<p><strong>Investment in AI-centered healthcare</strong></p>
<p style="text-align: justify !important;">Beyond research laboratories and hospitals, the emergence of AI has caused exponential growth in policies regarding AI and investment in AI around the world. AI-based startups have seen rampant growth. Startup Health, an incubator in the US recently reported that there were 7,600 healthcare start-ups around the world working on <a href="https://innohealthmagazine.comblog/sustainable-digital-healthcare-infrastructure/">digital health innovation</a>, a major portion of which involves AI based innovation. An Accenture report published in late 2017 states, “Growth in the AI health market is expected to reach $6.6 billion by 2021 &#8211; that’s a compound annual growth rate of 40%”. Another report by CIS India published this year states that AI could add a whopping $957 billion to the Indian economy by 2035. Even state governments are pushing for growth in AI-based sectors. The government of India aims to increase healthcare spending to 2.5% of the Gross Domestic Product (GDP) by the end of its 12th five-year plan, and to 3% by 2022. Such high rates of adoption are due to several AI start-ups and involvement of major players like Microsoft and IBM.</p>
<p style="text-align: justify !important;">Given the skewed ratio of doctors to patients in India, AI-based healthcare techniques would provide much-needed help in providing healthcare amenities to the masses. Globally, US government have made heavy investments in two of its AI-centered healthcare initiatives, with $1 billion proposed budget to its Cancer Moonshot Program and another $215 million in its Precision Medicine Initiative.</p>

	<p><strong>Ethics and issues with AI in healthcare</strong></p>
<p style="text-align: justify !important;">As rapidly as AI has been embraced by the medical and healthcare community, its benefits cannot be actualized without understanding its ethical pitfalls. But there are several concerns when applying these algorithms at a large scale to make real clinical decisions. Algorithms, albeit self-learning is products designed by human and may reflect their biases in the results they produce. These algorithms may reflect the biases of its designer or biases caused by the dataset on which the algorithm was trained. For example, algorithms developed by private sector entities can be biased to ensure outcomes of their interest or healthcare institutes may use AI systems selectively based on say, insurance plan or economic status of that patient or any other parameter.</p>
<p style="text-align: justify !important;">Even though Deep Learning algorithms can perform sophisticated predictions on imaging data, they are essentially not fed by an explicit code of information but are self-taught systems and even though the prediction score it gives, for example, whether the lesion is malignant or benign are surprisingly accurate when corroborated with the diagnostic report by a doctor, there’s no way to determine how exactly it came to that conclusion, thus rendering AI systems as a black box; with little clarity on how it works. Recently though there have been several predictions to understand how deep learning works, the information bottleneck theory being a prominent one, but the debate is far from settled.</p>

Several studies have shown that patients prefer AI chatbots and virtual nurses over humans when learning about their diagnosis as they can proceed to learn at their own pace without the embarrassment of not keeping up with the doctor’s speed. Patients are also more open to conversation with a computer than a human being, part of the reason being the diminished shame and fear associated with being vulnerable. But Allison Pugh, a Professor of Sociology at the University of Virginia and a writer for the New Yorker, thinks that virtual nurses and AI bots offer nothing more than the thinnest veil of care. She writes, “[&#8230;] automating or using AI to deliver care would be the same as relying on a “cloth monkey”—a reference to a cruel experiment, carried out in 1959, in which infant monkeys were given a choice between two surrogate mothers, one made from welded wire, the other from terry cloth. (The infants preferred the cloth mother, even when only the wire mother gave them milk.) AI-driven care was a sorry version of the real thing.”</p>
<p style="text-align: justify !important;">As demonstrated by several research groups, deep learning algorithms have achieved human-level accuracy and then some more. It can look for patterns which are invisible to the human eye. Thus, sooner or later, displacing and relegating doctors from their positions, at least in certain areas of healthcare. This can lead to massive burnouts in doctors as their roles shift drastically and may even lead to a gradual attrition of their skills. But there’s more to care than just interpreting blood reports and imaging data of a patient, it has much more to do about understanding the needs of patients, their mental state, etc. The secret of healthcare is not in reading out objective reports, but in the assurance and the warmth, a doctor’s cadence can provide. “Caring is expressed in listening, in the time-honored ritual of the skilled bedside exam &#8211; reading the body &#8211; in touching and looking at where it hurts and ultimately in localizing the disease for patients not on a screen, not on an image, not on a biopsy report, but on their bodies.”, writes Abraham Verghese, an author and a physician at Stanford.</p>
<p style="text-align: justify !important;">Employing AI to most healthcare activities might also have a negative effect on how knowledge is generated. Most medical knowledge generated in the past has been curiosity driven. AI systems can tell us whether the lesion is a benign mole or a tumor, but it can’t provide answers to why the tumor has a corrugated surface or white patches etc.</p>
<p style="text-align: justify !important;"><a href="https://innohealthmagazine.comtrends/ai-engraving-footprints-on-healthcare-transcontinental-canvas/">Artificial intelligence is going to be pervasive across the spectrum of healthcare</a>. From routine lab tests to offering a clinical decision, AI algorithms will play a major role in the future of healthcare. As deep learning algorithms get stronger and as the workings of the black box are revealed, AI technology will make further strides in healthcare. But advancements in AI-based healthcare doesn’t mean the downfall of human doctors. Healthcare is a highly emotional and human-centric field and the “human touch” will always play a pivotal role in the delivery of healthcare. Humans, even highly skilled doctors are fallible beings with inherent limitations and artificial intelligence will not sideline these practitioners but augment their abilities, in making an objectively better yet humane decision.</p>

Since the medical space is growing rapidly from 20 million markets to 70 million by 2025, describing a business opportunity for the younger ones and existing players of the <a href="https://innohealthmagazine.comtrends/internet-of-medical-things/">Internet of Medical Things (IoMT)</a>, insisted on mastering the technology instead of technology mastering us!</p>
<p style="text-align: justify !important;">Our inaugural speaker Dr. Partha Dey of IBM emphasized the importance of rapidly growing medical space in the next 5 to 7 years (million to billion to trillion) and that the numbers are promising. The question “Is there anything we can do to get our pie of the share from that increasing number?” Connecting internally (institution/hospitals) and with the outer world, there are two aspects. He went on to highlight a concern that connected health should be targeting illness rather than wellness. Illness market would have a better opportunity and that&#8217;s a fact of life.</p>
<p style="text-align: justify !important;">Integration of many devices also causes a problem in the implementation of IoT in healthcare. The reasons can be many.One of them is that the medical stakeholders do not have any consensus regarding the consent, generation, storage and transmission of data. But IoT changes the way!</p>

A paradigm shift in the way of offering services through connected health services and emphasizing that privacy and security concerns are of utmost importance when it comes to connected health. Responsibility and accountability after acquiring data are very important. The question is how to analyze the data and get actionable insight. Analytics and IoT should be considered together for actionable insight. The challenge of interoperability has turned up with an increase in the number of devices being used.</p>

	<p><strong>Trends and Opportunities of Medical IoT</strong></p>
<p style="text-align: justify !important;">IoT or Internet of Things is not new, but it has been gaining traction at a very high pace. Since the inception of the word IoT, there has been an ongoing conversation about how it might change healthcare, improve patient safety, affordability and accessibility for patients.</p>
<p style="text-align: justify !important;">Healthcare in India has a huge potential in terms of IoMT or medical IoT solutions. There will always be challenges and opportunities in this sector. Connected health is contributing towards the growth in space of IoT, but is lagging behind many other connected sectors.</p>
<p style="text-align: justify !important;">One of the key challenges in developing the algorithm for medical devices such as CT scans and X-rays. Application of artificial intelligence and machine learning in healthcare should be cautiously done because in case of a CT scan if we miss a module it has life risk and hence should be used carefully and should be a well-validated clinical algorithm.</p>
<p style="text-align: justify !important;">Since the infant mortality rate in India is twice the rest of the world, and that is certainly alarming as the bottom of the pyramid is affected; there is a company which focuses on the mortality rate of low weight birth babies. They have come up with developing a bracelet which diagnoses hypothermia in low birth weight babies. This was a very simple and innovative device, but then they analyzed the intermissions they can work on and came up with baby cradle. The main challenges to develop and design such a product is to cope with the cost factor and the startups definitely need funds to develop such a technology.</p>
<p style="text-align: justify !important;">The goal of connected health is not just early identification of health issues, but also effective patient care and safety. Today IoT platforms support integration of <a href="https://innohealthmagazine.comtheme/medical-devices-churning/">medical devices</a> and wearables. The need for security is as important as when it comes to IoT and accessing healthcare data and strengthening advocacy for connected health.</p>
<p style="text-align: justify !important;">Things change very rapidly and in very small time, and IoT is taking shape with it. But who will fund the cost of development/ Can government help the industry in making such policies?</p>

	<p><strong>Business Models of Successful IOMT in India and Challenges</strong></p>
<p style="text-align: justify !important;">The IoT healthcare business strategy is not yet robust because it involves a set of elements with new requirements such as new operational processes and policies, new infrastructure systems, distributed target customers and transformed organizational structures. Therefore, there is a need for a new business model. The IoT technology opens a new dimension of business opportunities for healthcare companies and the IoT platform becomes a key artifact in this transformation. Early movers who are taking a proactive approach to establishing their IoT healthcare ecosystem will tap into new business models. In this part of the business model, the panel discussed the basis of pricing and the way of charging for a product or service. Besides this, the panel also discussed the ways of revenue generation.</p>
<p style="text-align: justify !important;">While there are many benefits of the Internet of things in healthcare, it isn&#8217;t without its challenges. As with any new technology in healthcare, hospital and IT executives are concerned about data security and IoT device management. What are the potential barriers to IoT adoption and how healthcare IT can overcome those obstacles?</p>
<p style="text-align: justify !important;">AI and IoT change the way healthcare is delivered and we obviously are on the cusp of a revolution. This definitely poses a fair amount of concerns but also huge possibilities. The panel also discussed how Medicare is 100th the cost per person when compared to Obama care. So clearly, there must be more innovations coming from IoT and AI; they are going to help us reduce the cost.</p>
<p style="text-align: justify !important;">In the past, how people tried to use the data for the benefit of both patients and providers, starting from the patient side faced huge failures, as nobody was willing to pay. And that&#8217;s where the thinking came from that; how do we really make this work? How do we really turn this data into a business model? And that&#8217;s where they pivoted the model, working with providers and using their data sets. But then the question comes up, “Why the providers would share their data?”. Thus, the first pitching that started was to use the provider’s data for their patient care. And it didn&#8217;t work well. The whole narrative was, “What&#8217;s in it for providers?”.Therefore, the business model turned out to be, as we kept on talking to people, we realized that the data is going to be useful for provider’s revenue success, and then the same datasets will help the patients. A solution that they projected is to help retain the provider with patients’ data, grow their revenues and help in patient care.</p>
<p style="text-align: justify !important;">The MedTech certainly seems more complex. Where they are trying to come out with a dermatology solution and oral health solution for serious diseases where a cell phone image taken from an off the shelf mobile phone camera is sent to an AI backend and it gives a diagnosis and possible treatment options for a variety of skin diseases, particularly oral cancer. Such cases are very prevalent. What the panelist found is a landscape extremely complex in terms of the business model because when someone comes out with something like this, then figuring out who would it be paying is difficult. And suddenly the AI and IoT thing comes up and say that it can perform this diagnosis at par the level and in certain cases better than certain trained specialists. This does not support their case.</p>
<p style="text-align: justify !important;">One of the panelists shared her thoughts on important factors to concentrate and realize why health care has not moved digitally as compared to other industries like banking, insurance and how a lot of digital interventions can happen in healthcare. There are a whole lot of opportunities.</p>
<p>The main thing to focus on is What is the problem? What is the solution? And who&#8217;s going to pay you? Because that&#8217;s where the panelists believe that lots of startups have a scope.</p>
<p style="text-align: justify !important;">The customer is one, the doctor is another and hospital, the other spectrum. So how to manage this change and address the change management. If you give a thought on what to overcome, it gives you a kind of value that you try and achieve success and how do we overcome the quality assurance issues, change issues, challenges that one sees in terms of adoption. Let&#8217;s say for the doctors, it took a hundred years to the doctor to adopt stethoscope. Today, it&#8217;s widely accepted. IoMT is something that is obviously very innovative, very useful. But then not every doctor will accept it right away. How does one plan to overcome that barrier? So, they might just want one to focus on change management issues.</p>
<p style="text-align: justify !important;">There are three components of the IT embedment in a hospital. One is the business side of it, which gets adopted immediately. There&#8217;s just no resistance to it. The second side is the paramedical side of it; it has pathology, radiology, and other such things. They take a little time but they come on board. The learning curve is much shorter over there. The third one is the clinical side, which is the most difficult, as in a hospital system there are doctors who are most resistant to changes and the reason being their upbringing and learning of clinical practices in a certain way.</p>
<p style="text-align: justify !important;">The question arises “How does one change their practices?” Changed management is already happening at the college level. The medical students who are coming out now are more hands-on and are accepting the new technological changes. We will have to wait a little while, but they will accept it. When we talk about a change in the management, the problem that arises is not specific to India only. In the USA, George Bush had said that he wanted EMR to be adopted by everyone within the next ten years. Surprisingly, he didn&#8217;t even get 15% conversion in those ten years.</p>
<p style="text-align: justify !important;">It is important to secure the data and look over the cost implications of data security issues in connected health work moving forward. There is a need for appropriate interventions at the transition level of doctors and patients, as well as the importance of advocacy and awareness‐raising at community level about connected health. There is also a need for financial support for encouraging the tech enthusiasts of the country to address health issues in India with connected health solutions. One needs to understand customer value as well. How do we speak the right language in terms of what would appeal to that particular user or customer or doctor or hospital for that matter only then we will see more IoMT adoption happening. Yes, there is a need to understand the regulatory framework. Startups needs to adopt a lean structure in collaboration with the medical partners and come up with innovative solutions. India has a fantastic opportunity, but clearly it does need elements to come together and work together.</p>

										<content:encoded><![CDATA[<p style="text-align: justify !important;">Brig Arvind Lal, CMD of Dr Lal Path Labs, is a pioneer in bringing <a href="https://innohealthmagazine.compersona/testing-times-for-india/">laboratory services</a> in India at par with the western world. In 1977, he took charge of the <a href="https://innohealthmagazine.cominnohealth-conference/advances-in-diagnostics/">medical diagnostics laboratory</a> founded in 1949 by his late father. Under his expert guidance and leadership, the initiative has become one of the most reputed laboratories in Asia, having to its credit quality accreditations from various national and international bodies.</p>
<p style="text-align: justify !important;">The critical care <a href="https://innohealthmagazine.comnewscope/disruptive-technologies-save-environment/">environment</a> has undergone significant alterations in the past several years. This has happened because our <a href="https://innohealthmagazine.comwell-being/contracting-lifestyle-disease-adulthood/">lifestyles</a> in the fast-paced lives of modern India are ensuring that most people, in the age group of 30-50 years are falling prey to life-threatening <a href="https://innohealthmagazine.comtrends/indias-first-smartphone-compatible-insertable-cardiac-monitor/">cardiac diseases and strokes</a>, in addition to diabetes, hypertension, cancers, liver, kidney and lung diseases &#8211; these diseases being called <a href="https://innohealthmagazine.cominnohealth-conference/solution-non-communicable-diseases/">Non-Communicable Diseases or NCDs</a>. They are now responsible for killing more than 65% of our population says Dr Arvind Lal, known for his diagnostic labs across the country.</p>

<p style="text-align: justify !important;">Flagging concerns on such trends, these patients need high-cost intensive care, be it for complications of heart attacks, strokes, diabetes, hypertension, cancer or lung diseases. This is where the importance of Point-of-care testing (POCT) comes in. It helps in almost diagnosing the patient instantly and improves the physician’s ability to take immediate corrective action and decreases hospital stay. One such test is Troponin – that has revolutionized cardiac care by diagnosing heart attacks or myocardial infarction.</p>
<p style="text-align: justify !important;">This article is based on the keynote address on the occasion of <a href="https://innohealth.in/archive/2017/">2nd Annual International InnoHEALTH Conference 2017</a> – ‘<a href="https://innohealthmagazine.cominnovatiocuris/transforming-healthcare-through-innovation/">Transforming Healthcare Through Innovation</a>’ in New Delhi, said there are numerous promising diagnostic technologies. The key message is that in a country where 70% of the population lives in rural surroundings, ‘it is our duty to rapidly adopt disruptive innovative affordable technologies including telemedicine. Thus, our underserved population would be able to avail of the best treatment possible and bring in massive visible change’. He said the importance of bringing quality healthcare needs no reminder and the time has come for India to change the direction of healthcare for the masses.</p>
<p style="text-align: justify !important;">Healthcare is a right &#8211; and access to good healthcare should not depend on where one lives and how much he or she earns. But sadly, that is exactly what plagues India’s healthcare today, he lamented. India faces a severe shortage of both hard infrastructure and talent. With about one doctor and one functional bed per 1000 population, healthcare is truly underserved in India. Add to this the regional imbalances and variations in healthcare delivery. The healthcare infrastructure is skewed towards urban over rural India.</p>
<p style="text-align: justify !important;">Although rural India accounts for about 70% of the population, it has less than one-third of the nation’s hospitals, doctors and beds, resulting in large disparities in health outcomes across <a href="https://innohealthmagazine.comnewscope/ayushman-bharat/">urban and rural India</a>. British Medical Journal (BMJ) has observed that there is a remarkable saving of lives in India if good healthcare facilities consisting of operation theatres, surgeons, anaesthetists, blood banks are available within 50 kilometres of the patient providing quality medical services within the ‘golden hour&#8217;.</p>
<p style="text-align: justify !important;">Though there has been a sea change in the last five decades, India now needs to reinvent the field of diagnostics as laboratory tests are responsible for 70% of all clinical or medical decisions.</p>
<p style="text-align: justify !important;">In today’s life where internet rules the roost, the patients have become very knowledgeable, thanks to the globalisation of healthcare, and are demanding very high-quality healthcare for themselves. They are insisting on a very wholesome and satisfying experience rather than being told that the ‘treatment is over’.</p>
<p style="text-align: justify !important;">Soon, a time will come when the tests shall be ordered by the patients based on clinical history and clinical findings that shall be answered by an Artificial Intelligence (AI) application. Artificial intelligence has already found several areas in healthcare from the design of treatment plans to assist in repetitive jobs to medication management and drug designing. The most obvious application of artificial intelligence in healthcare is data management. Collecting it, storing it, normalizing it, tracing its lineage – it may well be the first step in revolutionizing the existing healthcare systems.</p>
<p style="text-align: justify !important;">Recently, the AI research branch of the search giant, Google, launched its <a href="https://deepmind.com/applied/deepmind-health/">Google Deepmind Health project</a>, which is used to mine the data of medical records in order to provide better and faster health services. The project is in its initial phase, and at present, they are working with <a href="https://www.moorfields.nhs.uk/">Moorfields Eye Hospital</a> of NHS Foundation Trust, UK to improve eye treatment.</p>

<p style="text-align: justify !important;">Just a few years ago the patient after giving the sample used to come back in the evening to the lab to collect a physical copy of the test report. This was replaced by making the report available on the internet that could be downloaded by the patient in the comfort of his home. These days this has been further replaced by making available an App on his mobile phone wherein he can book an appointment for the sample to be collected at home and the report being later available on the same mobile App.</p>
<p style="text-align: justify !important;">‘IBM Watson, whose headquarters I had the privilege of visiting a few months back in the Silicon Valley, is an AI-based engine that has launched its special program for oncologists to provide clinicians evidence-based treatment options. The program has an advanced ability to analyze the meaning and context of structured and unstructured data in clinical notes and reports in its encyclopedic memory that may be critical to selecting a treatment pathway’. IBM launched another algorithm called Medical Sieve. It is an ambitious long-term exploratory project to build a next-generation ‘cognitive health assistant’ that is able to analyze radiology images to spot and detect abnormalities faster and more reliably. This shall help radiologists in the future to look at the most complicated cases where human supervision is essential.</p>

	<p style="text-align: justify !important;">‘Wearable Tech is another area which I am personally very excited about. It has the potential to change the world as it helps people understand their own bodies by using mass data collected on a daily basis. From fitness bands to smartwatches to eye based wearables, they are being adopted widely. Take the case of Zephyr’s Anywhere Bio Patch which is an FDA-approved, small device that is attached to a patient’s chest and monitors their vitals minute-by-minute and collects medical-grade data for doctors’ use. These devices will connect our organs digitally, enabling disease detection at very early stages. It has the potential to bring down cardiac and other deaths drastically. This offers immense potential to do remote testing, monitoring and thus assisting the doctor in timely treatment’.</p>

	<p style="text-align: justify !important;">Point of Care Testing: Technological advancements in laboratory automation, including POCT, and initiatives to increase patient satisfaction are transforming the clinical laboratory market. POCT has come a long way from a handful of simple tests to a multibillion-dollar global market that holds great promise for the future. Not so long ago, laboratory data would often arrive at the bedside too late to be of significant use in the active, continuing care of critically ill patients. Now, most clinicians acknowledge that POCT is a prerequisite for early recognition of life-threatening conditions as they require that laboratory results are made available in real-time and, if possible, at the critically ill patient&#8217;s point of care. The College of American Pathologists defines POCT as tests designed to be used at or near the site where the patient is located, that do not require permanent dedicated space, and that are performed outside the physical facilities of the clinical laboratories.</p>

	<p style="text-align: justify !important;">Examples include kits and instruments that are hand-carried or transported to the vicinity of the patient for immediate testing at that site (e.g. capillary blood glucose) or analytical instruments that are temporarily brought to a patient care location (like operating room, intensive care unit). In many cases, the simplicity was not achievable until technologies developed that was simple and affordable. For example, various kinds of urine test strips have been available for decades, but portable ultrasonography did not reach the stage of being advanced, affordable and widespread until recently. Similarly, pulse oximetry can test arterial oxygen saturation in a quick, simple, non-invasive, affordable way today, but in earlier eras, this required an intra-arterial needle puncture and a laboratory test. Thus, over decades, testing continues to move toward the point of care.</p>

<p style="text-align: justify !important;">The lab-on-a-chip (LOC) is another device that integrates one or several laboratory functions on a single integrated circuit (commonly called a &#8220;chip&#8221;) of only a few square centimetres to achieve automation and high throughput screening. Imagine that a patient comes to one of our 2,100 collection centres in the remote tier three or tier four towns in India with the high fever. We take a drop of blood from his finger and inform the clinician almost immediately that the patient is suffering from Chikungunya and not from <a href="https://innohealthmagazine.comtrends/early-detect-dengue/">Malaria or Typhoid, or Dengue fever</a> or Japanese Encephalitis – all in a matter of minutes! The driving notion behind POCT is to bring the test conveniently and immediately to the patient. Needless to add, the patient’s data by POCT shall be made available to update the patient’s electronic health records (EHR).</p>

<p style="text-align: justify !important;">Talking about POCT Instruments: Currently, two broad type of POCT instruments are available: Small benchtop analyzers (for example, blood gas and electrolyte systems) and handheld, single-use devices (such as urine albumin, blood glucose, and coagulation tests). Now let us talk about if POCT is Boon or Bane? The strong point of POCT is speed and the rapidity with which it shall save lives in emergencies. As India marches towards quality healthcare delivery, in course of time regulatory compliances shall have to be adhered to in the interest of the patient’s health.</p>

	<p style="text-align: justify;">Stockholm, September 12, 2017. <strong>Beactica, BluSense Diagnostics and Optomed </strong>are the recipients of the Nordic Stars 2018 Award. The highly coveted award was presented to the winners from three Nordic countries at the conclusion of the Nordic Life Science Days (NLSDays) in Stockholm. Growing numbers of one-on-one meetings and licensing opportunities year by year to a new all-time high underpin that the conference has become an established arena for high quality meetings between, companies, service providers, investors, research communities, and government and regulatory bodies. Approximately 1,280 delegates representing almost 800 companies from 40 countries joined the conference, thus confirming NLSDays’ role as the premier Nordic life sciences event.</p>

But growing back to the future, India is also taking lead to promote its age-old Ayurveda that is as effective as any other modern science.Opening the floor to this high voltage discussion, Dr. Shiban Ganju Atrimed Pharmaceuticals in the US said, “Until now human body has been an amalgamation of cells, tissues and organs. Now, the human body has emerged as a conglomeration of biochemical process that are scattered around and work in close coordination with each other with the sole purpose of keeping the human body alive.</p>

	<p style="text-align: justify !important;">“Another baffling reality for me is that Ayurveda introduces us to the benefits of plants and their properties. After all, trees and plants appeared much before the pharma industry. Plants have been in existence for more than two billion years and our pharma industry is less than 200 years old. Not only that, plants have a self-preserving system and can fight any bacterial attack or any foreign infection; that makes them the most natural source for our pharma industry – even in these modern times. More so in the modern times!” he concluded his thoughts with his faith in age-old Ayurveda.</p>
<p style="text-align: justify !important;">Closer to the ground, Dr. V.K. Sharma shared some startling facts about the Indian pharma sector: “We are the most economical medicine manufacturer in the world. India is the third largest producer in volume and 10th largest in business globally. Thirty percent of US requirements are catered by Indian pharma manufacturers. We export to almost 200 countries in the world. One out of every three pills consumed in the world is from India. And all this has been achieved in the last 30 years when the Indian government came out with its Pharma policy in 1986.”</p>
<p style="text-align: justify !important;">“Despite that India pharma industry has been facing many challenges and to address those issues now there’s a new Drug &amp; Pharma policy that will encourage more research and development in the country making the pharma industry more robust and bigger than ever.”</p>
<p style="text-align: justify !important;">Dr. Vinod Nikhra highlighted the contribution of Indian pharma industry beyond the “pill factor” as he emphasized the broad developments in the sector and what makes it click internationally.</p>
<p style="text-align: justify !important;">“India has been on the forefront of providing some of the leading medical facilities that are uniquely innovative and ingenious. Indian pharma industry has gone beyond molecular contributions and is in fact manufacturing some ground breaking gadgets and machines that contribute to curing people from extreme ailments like heart and lung diseases, transplants and coronary diseases,” he said.</p>
<p style="text-align: justify !important;">On the flip side, he added, “Of course pricing is an issue that is faced by the industry and the irony is that despite the price being much cheaper than the international market, it still remains out of reach of most Indians. For this we are counting on government to extend some encouraging and bold steps to make medical facilities and good health within the reach of all.”</p>

	<p style="text-align: justify !important;">Highlighting the problems of the Indian Pharma industry, Ashish Kumar from Medicea Technology Solutions said that, “The problem that we are trying to solve is two-fold. First one is facing the encroachment of fake and counterfeit drugs which is growing at 25% CAGR per year and is valued at $10 billion annually. We are working closely with IBM to address this problem and are on the brink of perfecting a fool-proof system that will block every fake or counterfeit drug in the country.”</p>

	<p style="text-align: justify !important;">“Second project that we are working on is to track and design demographics of drug consumption. Use of a particular drug in a zone or a larger region can give a very useful insight to health authorities as to what is the cause of those ailments that are leading to high consumption of particular drugs in that particular area.”</p>
<p style="text-align: justify !important;">Dr. Anjana Batni, an Ayurveda researcher from Atrimed Biotech brought glory to Indian past highlighting the relevance and importance of age-old Ayurveda that is getting increasingly accepted universally and is completing the modern medicine and not competing with it.</p>
<p style="text-align: justify !important;">Highlighting the contributions of her organization, she said, “With the intention to modernize Ayurveda, we are collating data about the plants and their use. Ayurveda texts identify how to diagnose, manage and how to cure diseases; and they also have vast information on plant molecules and so far, less than 10 percent of these plant molecules identified in Ayurveda texts are in practice. Our efforts are to classify all the plant molecules and store them digitally for use by Ayurveda practitioners and other medical platforms. 	<p style="text-align: justify !important;">Access to good health is a right and should not depend on where you live and how much you earn. But sadly, that is the reality in our health care system today. With less than 7.5 doctors to a thousand patients, we have a severely underserved health care industry. Even though we have more than 70 per cent of the country’s population in rural areas, but less than a third of the medical resources including doctors, laboratories, hospitals and hospital beds.</p>
<p style="text-align: justify !important;">Emphasizing his and the establishment’s 50 years contribution to providing good health, he highlighted that he may have to rediscover the business of diagnostics, yet again.</p>
<p style="text-align: justify !important;">Laboratory tests are responsible for 70% of clinical or medical decisions. This makes diagnostics the spine of the entire medical and healthcare system anywhere in the world.</p>

	<p style="text-align: justify !important;">Session moderator Dr. Ravi Gaur of Oncquest Laboratories opened the floor with his opinion on the subject. “Healthcare industry has probably seen the maximum innovations in India. Diagnostics has been moving from incremental to being disruptive with innovations; and that’s what the need of the hour is today. We need to cut short the timings; we need to cut short the process of diagnosis and arrive at a conclusion at the earliest so as to be usefully effective to the clinical process.”</p>
<p style="text-align: justify !important;">Dr. Pieter Spee from Estonia, the Partner Country, gave international perspective to the process of diagnostics. He highlighted that innovations are even more important in diagnostics as it’s the early detection that enhances the medical treatment.</p>
<p style="text-align: justify !important;">Estonia is emphatically developing new products, facilities, techniques and tests that support the process of diagnosis. Beyond that, access to lab results and sharing data in real time is the key to improved medical facilities and clinical treatment.</p>
<p style="text-align: justify !important;">Diagnostics is complemented with very high intervention by innovations and inventions, and IT companies like IBM are also pitching in with their state of the art products like the Watson and Block Chain.</p>
<p style="text-align: justify !important;">Taking the diagnosis beyond a laboratory process, Siddharth Sangwan of Hindustan Wellness highlighted the newly defined telemedicine process with new innovations and a new approach.</p>

We have now integrated the telemedicine system where the doctor explains the report to the patient and in the process takes the customer/patient in a walk through his report and diagnosis. Next step of telemedicine will be to integrate Artificial Intelligence so that patients can contribute to the clinical process and recover faster, with the additional help of doctors and medicines.</p>

	<p style="text-align: justify !important;">Hospitals are the core of health and medical care and adaptation of new technology and innovations will make the biggest difference to our medical needs while improving the health conditions of all.</p>
<p style="text-align: justify !important;">Highlighting the concern of data security and reluctance to depend on technology because of that concern, the keynote speaker for the first session Dr. Vijay Agarwal shared his views on the subject emphasizing on the fact that technology is rolling out at a very fast pace. Stating an example of landline telephones that took more than 100 years to reach the one billion mark of users, was reduced to less than 20 years for mobile technology to do the same. This way he highlighted the fact that newer technology will be very rapid in implementation requiring hospitals and clinics to adapt and be prepared to utilize it rather than be apprehensive about it.</p>

	<p style="text-align: justify !important;">Professor AK Gupta of the esteemed PGI Hospital in Chandigarh, also, supported this view and shared his personal experiences and experimentation with management and administration of the hospital. He emphasized that it’s not just in the medical and clinical field that we have to embrace innovations and technology, but in fact it will fetch very positively encouraging results if adopted in the management and administration of hospitals. He shared his views on reducing the time stay of patients in hospitals by means of using an innovative approach provided by tailored software and mobile apps. These efforts were recognized by the hospital; national level authorities and even internationally and was rightly awarded for this approach. He concluded his speech by asserting that you don’t have to discover new technology, but innovative utilization of existing technology can also provide very good results.</p>
<p style="text-align: justify !important;">Touching the humane element in the hospitals, Prof Arti Maria of RML Hospital shared her experiences of involving mothers and non-hospital staff in caring for the young ones in ICU units. “A soft touch by the mothers, who are otherwise distanced outside the ICU units, has resulted on faster recovery of infants in the ICU.”</p>
<p style="text-align: justify !important;">“In fact, this experimentation has also resulted in enhanced confidence of mothers and improved their social status with respect to their husbands, families and the social fabric as such.” Prof Maria, who specializes in Neonatology as Head of that Department is keenly working with other authorities beyond the hospital to promote this concept and is getting very encouraging results from hospitals, and relevant bodies.</p>

The three products and services of IBM, Watson, Block Chain and Cloud data are the way forward, according to Mr. Dey, Healthcare Leader and SME and IBM, India.</p>
<p style="text-align: justify !important;">Watson, according to Mr Dey, is the most innovative friend for health care workers and doctors alike, as it understands many languages, including regional and Oncology. This helps medical staff to interpret and diagnose ailments with a high grade of accuracy and ease. Watson can, not only, analyze and conclude at a faster pace, but can also eliminate human error contributed in the form of emotional stress and personal bias.</p>
<p style="text-align: justify !important;">Block Chain, on the other hand is the best mode of data utilization and can contribute to tracking of product and their status. Block Chain can also help eliminate counterfeit medicines by clever tracking of products and their batches.</p>
<p style="text-align: justify !important;">IBM is constantly pushing the boundaries of innovations with introduction of new products and services. Watson and Block Chain are latest in the field of health and medical welfare.</p>

	<p style="text-align: justify !important;">The healthcare industry has been a victim of various cyber attacks in the last few years. According to recent studies, healthcare has outnumbered financial services and become the most cyber attacked industry. The latest in cyber-attack is ransomware wherein the hacker encrypts the data and threatens to publish it until the ransom is paid in form of bitcoins. In US alone, healthcare industry was the victim of 88 per cent of all ransomware attacks across industries last year.</p>
<p style="text-align: justify !important;">The recent case of WannaCry ransomware crippled the IT systems of NHS, UK. And after hitting NHS, it spread globally targeting more than 99 nations. The hackers demanded payment of £300 &#8211; £600 to unlock systems and have earned about £55,000 in ransom.</p>
<p style="text-align: justify !important;">Ransomware has indeed become a lucrative revenue source for hackers due to which the number of attacks is predicted to quadruple by 2020. Medical records have 10-20 times more value than the credit card data in the internet black market. Ransomware epidemic is spreading in healthcare like wildfire due to its increasing digitalization which is and will attract more attention of hackers. Also, the vulnerability of the health data tends the organizations to pay the ransom to get the data back to maintain privacy and confidentiality of patient data.</p>
<p style="text-align: justify !important;">Even after so many cases of cyber attacks compromising millions of electronic health records each year, the healthcare industry is inadequately prepared to prevent and resolve these attacks. Whether it is India or US, cyber security is always discussed in forums and budget is allocated for the same but is not put to proper use. Cyber attacks happen due to outdated security infrastructure or employee negligence.</p>
<p style="text-align: justify !important;">Hospitals and insurance companies have been the main targets of hackers. But, a new vulnerability is catching everyone’s attention i.e. medical devices. The next nightmare in ransomware attacks could be hacking of medical devices such as insulin pumps, pacemakers, defibrillators, implants etc.</p>
<p style="text-align: justify !important;">Disfunctioning of medical devices can be catastrophic. Just imagine, hackers take control of one’s pacemaker and ask for ransom or else they would manipulate the device which could be fatal. This kind of attack has been showcased in the very famous TV show Homeland wherein the Vice President dies due to hackers remotely disable his pacemaker.</p>

	<p style="text-align: justify !important;">Just like the serial, the former US Vice President Dick Cheney’s doctors disabled his pacemaker’s wireless functionality due to fear of possible assassination attempts as revealed by him during an interview in 2013. This clearly showcases that medical devices can be the next target for hackers.</p>
<p style="text-align: justify !important;">Regulators such as FDA are increasingly getting concerned about medical device security and have issued warning. In 2015, for the first time FDA issued safety notice to hospitals which strongly discouraged hospitals to use an infusion pump which was found to be vulnerable to cyber attacks. But it has been observed that FDA did not force the company to fix the devices being used in the hospitals and didn’t investigate other insulin pump models. This shows that FDA needs to be more stringent towards medical device security. The vulnerability of infusion pump was pointed out by a white hat hacker Billy Rios during his hospital stay.</p>

	<p style="text-align: justify !important;">Few of the medical device companies/providers have been proactive in strengthening their device security such as Johnson &amp; Johnson in Oct 2016 warned 114,000 diabetic patients about a security lax that a hacker could exploit in one of its insulin pumps (J&amp;J Animas OneTouch Ping). The hackers can disable or alter the dosage which could be fatal. J&amp;J suggested ways to the patients for mitigating risk.</p>
<p style="text-align: justify !important;">There have been no documented cases of medical device hacking till date but demonstrations have been conducted in research environment. One such example is of Barnaby Jack who succeeded in hacking an insulin pump and demonstrated giving off lethal dose of insulin without the pump alerting the user. Another example is that of St Jude Medical’s implantable devices such as pacemakers, defibrillators, and resynchronization devices. The radio frequency (RF) enabled St. Jude medical implantable cardiac device and corresponding Merlin@home Transmitter enables transmitting and receiving patient data stored on the device to the physician to monitor his health. But FDA reviewed the device and confirmed about cybersecurity vulnerabilities, if exploited, could be fatal.</p>
<p style="text-align: justify !important;">Also, researchers at TrapX Security analysed three hospitals for medical device hacking. The deception technology was installed which utilized emulated medical devices in the hospitals. These emulated devices attract and trap hackers so that TrapX could trace the hackers activity. These fake medical devices such as Radiation Oncology system, LINAC , Fluoroscopy, PACS and Xray system appeared real to the hackers and TrapX could monitor hacker’s activity.</p>
<p style="text-align: justify !important;">According to TrapX, these hospitals utilized older version of Windows that made it vulnerable and most medical devices did not have additional endpoint security software which made the attack undetectable. It was also noticed that the main goal of hackers was to steal medical records not to manipulate the device.</p>
<p style="text-align: justify !important;">Another research at University of South Alabama showcased how they hacked pacemaker and killed a medical simulator called iStan. The $100,000 medical dummy comes equipped with robotics that mimic the human cardiovascular, respiratory, and neurological systems. The researchers could speed the heart rate up or could slow it down. Not only pacemaker, researchers could manipulate an insulin pump or a number of things that would cause life-threatening injuries or death. This clearly illustrates why medical device security is important.</p>
<p style="text-align: justify !important;">With the advent of IoT, where devices are connected via internet should focus on cyber security. Industrial experts are realizing that cyber security is prime priority for all the devices connected to the internet.</p>
<p style="text-align: justify !important;">Devices such as wearables, smart bed, smart emergency system, etc. are all lagging behind in cyber security. Apart from medical devices, surgical robots are not being scrutinized for cyber security. Just imagine, surgical robots been hacked which could lead to life threatening situation of the patient. One such demonstration has been showcased by researchers at University of Washington in 2015. They hacked a tele-operated surgical robot, Raven II. The experiment demonstrated three types of attacks that made telesurgery vulnerable with this robot. The researchers demonstrated how they took complete control over the robot and disrupted the operation.</p>

	<p style="text-align: justify !important;">All of this sounds scary but it can be prevented if we are well prepared. It is important to understand that not only regulators like FDA need not address the challenge of cyber security but also the medical device vendors and providers should take shared responsibility. It has been observed that providers point the device manufacturers to be accountable for cyber security for responding to vulnerabilities and providing fixes for the same.</p>
<p style="text-align: justify !important;">On the other hand, the device vendors hold providers responsible for their negligence and having outdated network  protection. To be safe from such attacks, organisations should review their cyber defence strategies and budget. Also, employee training and awareness needs to be tackled to avoid falling for opening phishing mails and change passwords regularly.</p>

	<p style="text-align: justify !important;">It has also been observed that providers such as middle scale hospitals, clinics or laboratories have often overlooked cyber security as priority as they believe not much data is present with them and only the big organisations are in trouble. Which is not true, as hackers are aware of the precious financial and patient data these clinics hold and are aiming at clinics or small hospitals also to get the data. So, they should also focus on medical device security.</p>
<p style="text-align: justify !important;">The next big thing in helping fight against cybersecurity is artificial intelligence (AI). According to some analysts, the advantage of using AI is it can help predict cyber attack before it happens with the use of behaviour analysis. It alerts security team on any behaviour deviation or authentication failures while accessing records. AI not only helps in detecting threats quickly but it is also cost efficient compared to the money paid by companies in ransom. It does not replace security tools but acts as an additional layer of security. AI can also help in analysing employee behaviour for avoiding any internal security breach. AI can help in bridging the shortage of skilled cyber security professionals also. According to Centre for Cyber Safety and Education, there is a shortfall of 1.8 million cyber security professionals by 2022 worldwide. Companies such as IBM are already investing in AI system Watson for cyber security.</p>
<p style="text-align: justify !important;">Also start-ups such as Cognetyx are providing cognitive cyber surveillance solution to healthcare organizations. Use of AI for cyber security in other areas has been showcased, for example, the Las Vegas city officials and UK government to monitor their Public Services Network and protect their records from security threats. Whereas, the successful implementation of AI in healthcare cyber security is yet to happen.</p>
<p style="text-align: justify !important;">The next wave of medical device cyber attacks can be prevented by collaborative approach and commitment from all the stakeholders. Not only the healthcare organizations should make sure their security practices and strategies are updated but the government should also help in skill development of cyber security professionals and encourage more research on medical device security by providing medical device at low cost. Since medical devices are expensive and require license, it makes it difficult for researchers to explore this area. At the end, we should not forget that we have to stay a step ahead of hackers to be a hard target for them.</p>
