<p><strong>प्रश्न:</strong> टाइप 2 डायबिटीज़ (T2D) धीरे-धीरे पूरी दुनिया में अपनी सर्वव्यापकता दिखा रही है। वर्तमान समय में, विशेषकर भारत में, डायबिटीज़ के बढ़ते प्रसार के मूल कारण क्या हैं?<br><strong>उत्तर:</strong> विश्वभर में डायबिटीज़ का बोझ लगातार बढ़ता जा रहा है। <em>IDF Atlas 2021</em> के अनुसार 20–79 वर्ष की आयु वर्ग में दुनिया में 537 मिलियन लोग डायबिटीज़ से पीड़ित हैं और 2045 तक इसमें 46% की वृद्धि होने की संभावना है। भारत पहले से ही चीन के बाद दूसरे स्थान पर है, जहाँ लगभग 74 मिलियन लोग डायबिटीज़ से प्रभावित हैं।<br>मोटापे की बढ़ती दरों के साथ डायबिटीज़ का प्रचलन भारत में और बढ़ने की संभावना है। पहले यह शहरी संपन्न वर्ग और वृद्ध लोगों की बीमारी मानी जाती थी, लेकिन आज यह मध्यम वर्ग, निम्न सामाजिक-आर्थिक वर्ग और यहाँ तक कि गाँवों और गरीब तबके में भी फैल चुकी है।<br>डायबिटीज़ अक्सर अनियंत्रित रहती है क्योंकि लोग आहार में लापरवाही बरतते हैं या पर्याप्त व्यायाम नहीं करते। भारत में भोजन का लगभग 65–75% हिस्सा कार्बोहाइड्रेट से आता है, और अधिकतर परिष्कृत कार्बोहाइड्रेट जैसे कि पॉलिश किया हुआ सफेद चावल और मैदा आधारित गेहूँ। यह जल्दी ग्लूकोज़ में बदलकर शुगर स्तर बढ़ा देते हैं। व्यायाम की कमी, नींद की कमी, तनाव और दवाओं का नियमित सेवन न करना भी मुख्य कारण हैं। ये सभी कारक नियंत्रण योग्य हैं।</p>

<p><strong>India Science Wire:</strong> Bio-printing is a method of tissue replication that temporarily or permanently supports and nurtures living cells. This technique is a potential alternative to organ transplantation, which could be useful in manufacturing functional human tissues such as skin by using specifically engineered biomaterials or bio-inks to print artificial living tissues.</p>



<p>An indigenous state-of-the-art 3D Bio-Printer ‘Mito Plus’ launched by Indian Tech Startup Avay Biosciences has been found to be helpful in printing human tissues. Mito Plus was launched at Bengaluru Tech Summit held between 16th and 18th November 2022. The prototype of Mito Plus was installed at the Indian Institute of Science (IISc), Bangalore, the top-ranked science research institute by NIRF Rankings.</p>



<p>Mito Plus is an advanced version of 3D bio-printer developed with inputs on the prototype from the research lab of Dr Bikramjit Basu at IISc, and developed by Avay which an IIT Madras alumnus co-founded. It is one of the advanced 3D bio-printers in India. Avay Biosciences provides wholly indigenous software and hardware development for end-to-end Bio 3D printing solutions in India.</p>



<p>Manish Amin, Chief Executive Officer, Avay Biosciences, said,<em> </em>“MITO plus is an advanced bio-printer at its price range which can be used to print a wide range of biomaterials. This printer comes up with an inbuilt UV curing option, HEPA filter and effective temperature control features. In this the print-head and the print-bed can be cooled up to four degrees celsius and heated to 80 degrees celsius. MITO plus can be used for pharmaceutical drug discovery and drug testing applications, it can also be used in cancer biology and cosmetology applications.”</p>

<p>Bio-printers work in almost the same way as other 3D printers do, with one major difference i.e instead of delivering materials such as plastic, metal or powders, bio-printers deposit layer of biomaterials, that may include living cells, to build complex structures like skin tissue, liver tissue, etc. 3D bio-printing is a unique gift to humanity through science and technology. However, many challenges are yet to be solved. ‘’There is still a long way to go before we can create fully functioning and viable organs for human transplant,” Amin explained.</p>



<p>“We are working on having our printers develop skin &#8211; the most common type of layered tissue that could help victims of severe burns. These tissues can also be used for toxicology screens and other testing mechanisms,” said Suhridh Sundaram, Chief Operating Officer, Avay Biosciences.</p>



<p>Typically bio-printing uses various polymers which attempt to recreate the extracellular matrix (ECM) native to the specific cell. The availability of cost-effective bio-printers is essential in developing artificial organs, since all future research depends on this infrastructure. If animal cells are used, bioprinting can also be utilized to create artificial meat, a space-age food dream.&nbsp;</p>

<p>Lifevac is a non-invasive, non-powered, single-use airway clearance device which is used to resuscitate a person with airway obstruction. The device is placed over a victim’s mouth who is choking and its one -way valve creates a vaccum to dislodge the object in the throat. This one- way valve does not allow air to enter a person on a downward motion. The patented valve prevents any air to exit through the mask and also prevents air from pushing food or objects downward, creating a one-way suction.</p>

<p>US-based, Cala Health, has recently introduced Cala Trio Therapy as the first non-invasive targeted therapy which reduces hand tremors for people living Essential Tremor (ET). According to a study by American Academy of Neurology, ET is most common in people aged 40 years or older and affects nearly 6.4 million people in the US. </p>



<p>This ground-breaking therapy, cala trio, is caliberated to treat tremor which is a common motor symptom of parkinson’s, that was earlier irreversibly debilitating. It is a wearable worn on the wrist which measures specific tremor and sends a gentle stimulation to the radial and medial nerves that travels to the brain. Thus interrupting the tremor in the ventral intermediate nucleus of the thalamus. </p>

<p>The USP of the device is that it is a personalized therapy for essential tremor and is the only non-invasive prescription therapy for ET. The device targets the same site in the brain where tremor is well-controlled by deep brain stimulation (DBS)- thalamus. Cala health recently received FDA breakthrough device designation for expansion into hand tremors associated with parkinson’s disease. Cala health was also a nominee for Prix Galien’s best medical technology award in 2021.</p>

<p>Boston Scientific, a biotechnology/biomedical engineering firm and multinational manufacturer of medical devices used in interventional medical specialities, also a global leader in cardiovascular medicine is one of the world’s largest medical device companies. In a path breaking discovery, recently Boston Scientific introduced Eluvia drug-eluting vascular stent system as a drug delivery device for the treatment of peripheral artery disease (PAD).</p>



<p>Eluvia has a hydrophobic promus polymer which protects the drug from dissolving in the blood and allows for highly controlled drug delivery to the target lesion. Eluvia uses a drug polymer combination to deliver the chemotherapy drug paclitaxel over a period of one year. Observation reveals that Eluvia reduces downstream particulates and has low systemic drug exposure with no measurable levels of paclitaxel in the bloodstream within thirty minutes. </p>

<p>The one year long clinical trial demonstrated superiority of the Eluvia stent compared to self- expanding bare metal stents for the treatment of peripheral artery disease (PAD) and superficial femoral artery (SFA) or popliteal artery (PPA) lesions up to 210mm in length. There were 775 patients involved in the trial, making it the largest randomized trial of a drug-eluting stent for the treatment of PAD till date. Eluvia stent showed superiority with a primary patency rate of 85.4% as compared to 76.3% with BMS. Eluvia stent which does sustained release of the lowest dose of paclitaxel of any peripheral drug-eluting device, re-opens blocked arteries and restores blood flow along with using a drug-polymer combination to prevent tissue regrowth.</p>

<p>As we enter a new stage of the pandemic in 2022 wherein the omicron variant begins to split into new ‘subvariants’, reminding the world of the ever-evolving nature of the virus and its capability to change its response to the tools developed to fight it alongwith its effect on the body. So in order to better equip ourselves to respond to it, Roche, pharmaceutical and diagnostics company based in Switzerland and TIB Molbiol, its subsidiary have developed a COVID-19 test for researchers which detects and also differentiates the most recent BA.2.75 subvariant of interest by targeting the two of the unique mutations in BA.2.75 which are different from other notable subvariants.</p>

<p>BA.2.75 variant related to the omicron variant has cases which have been reported in more than a dozen countries since it was first discovered in India in May 2022. Though it has not yet reached the level of variant of concern but differentiating it from BA.4 and BA.5 permits its better monitoring and tracking of how it is spreading. Both the World Health Organisation (WHO) and the Centers for Disease Control and Prevention (CDC) are keeping a vigil on this subvariant.</p>



<p>The COVID-19 test kits developed by Roche and TIB Molbiol includes kits for detecting the recent BA.4 and BA.5 subvariants alongwith other mutations found in the omicron SARS-COV-2 variant.</p>

<p>German pharmaceutical firm,Bayer has recently launched a potential blockbuster drug in India under the brandname ‘Kerendia’ to treat chronic kidney disease (CKD) in type2 diabetic patients. The drug is called Finerenone which is a first-in-class-non-steroidal, selective mineralocorticoid receptor antagonist.</p>



<p>The differentiating point of finerenone from the existing drugs for CKD in diabetic patients is that it works by blocking overactivation of the mineralcorticoid receptor (MR), which is thought to contribute to the progression of cardiovascular damage and chronic kidney disease.</p>

<p>This drug has been approved in India with an aim to lower the risk of an end-stage kidney disease, sustained estimated eGFR decline, non-fatal myocardial infarction, cardiovascular death and hospitalization for heart failure in adult patients with chronic kidney disease (CKD).</p>



<p>More than 13000 patients were enrolled in the phase III clinical trial program to explore the efficacy and safety outcomes, particularly cardiovascular and renal in patients with chronic kidney disease and type 2 diabetes. It was observed that Finerenone reduced kidney composite outcome by 23% on top of optimized Renin-angiotensin system (RAS) blockade and chance of a 57% Glomerular Filtration Rate (GFR). It also significantly lowered the risk of a composite CV outcome by 14%.</p>

<p>Nearly 21 million people are suffering from major depressive disorder (MDD) in Europe. Keeping this in mind Flow Neuroscience, a medical technology startup from Sweden has developed a brain stimulation headset which is used in the treatment for depression.</p>



<p>This stimulation headset developed by neuroscientist Erik Rehn and clinical psychologist Daniel Mansson is known as ‘FLOW’ which is a medically approved home treatment for depression. This technology uses a device which looks like a pair of headphones to deliver transcranial direct-current stimulation.&nbsp;</p>

<p>Here a very little amount of electricity is applied to an area at the front of the brain also known as the dorsolateral prefrontal cortex which is shown to be less active in people suffering from mental depression.</p>



<p>It has been observed that when this headset, Flow was used by patients in conjunction with a behavioral therapy application that features a virtual therapist to empower and motivate its users’ to manage their treatment, more than 81% which is three-quarters of cases displayed a reduction in their symptoms after three weeks of its usage. This combination offers a unique way to treat depression by removing access barriers. It has been proven that Flow is very effective at treating mild symptoms. Flow has been designed with a vision to change how treatment for depression is accessed.</p>
