<p>When it comes to menstruation, there is still a lot of stigma attached to it. In certain cultures, menstruation is considered filthy and disgusting. A stigmatised air around menstruation inhibits adolescent menstruators from receiving genuine, solid knowledge about what is going on with them. It is either forbidden, sacred, or invalidated. Where is the true picture?</p>



<p>Let&#8217;s go all the way back to ancient civilizations for the first known mentions of menstruation. Pliny the Elder, a Roman philosopher, was persuaded that periods were some kind of mystical trickery back in the day. He said that women on their periods could destroy large fields of crops, drive bees from their hives, and manipulate the weather. Regrettably, attitudes concerning menstruation were not much better during the Middle Ages. People went to tremendous lengths to conceal their periods due to religious embarrassment, and some even claimed cramps were a heavenly punishment for Eve&#8217;s initial sin. Those who were menstruating were not administered pain relief medication because cramps were viewed as a punishment (mostly herbal remedies at the time).</p>



<p>People in mediaeval times would either use rags to absorb their menstrual blood (ever wonder where the phrase &#8220;on the rag&#8221; comes from?) or they would free bleed. Periods entered the Bridgerton era in the late nineteenth century, and the first menstrual products were invented. People wore sanitary belts from the 1890s until the early 1980s, which were simply reusable pads tied to a belt worn around the waist &#8211; and yes, they were as terrible as they sound. Brands began to release adhesive disposable pads in the 1970s, comparable to what we see now in the menstruation goods aisle. Initially, adhesive pads were heavy and uncomfortable to wear, but they were an improvement over having to suspend your pad from a string.</p>

Once the most advanced egg is discharged, the follicle transforms into a corpus luteum that generates steadily adding situations of progesterone as well as a small quantum of estrogen. Serotonin and dopamine are two chemicals released during exercise that not only improve your mood, but can also suppress anxiety-inducing hormones. One study found that three sessions of 60-minute exercise each week for eight weeks significantly reduced PMS symptoms in women. Exercise can even help manage bloating and water retention. &#8220;Remember to do what feels comfortable, and don&#8217;t overdo it.&#8221;</p>



<p><strong>Stress Management Practices –</strong> As a result, stress management practices should be used throughout the menstrual cycle. PMS can be stressful enough on its own, especially when the symptoms disrupt your daily routine, such as cramps that make it difficult to get out of bed or mood swings that disrupt relationships. When you are stressed, your body produces increased amounts of cortisol and corticotropin-releasing hormone (CRH). They are also referred to as stress hormones. When stress has a particularly strong effect on your hormones, it can result in missed or late periods. Stress is a major cause of uncomfortable menstrual symptoms because it disrupts your hormones and can disrupt your cycle.</p>



<p><strong>Get enough sleep –</strong> when you are stressed, sleep can help you cope. Sleep improves your mental health and helps treat anxiety and sadness. During your period, try to get six to eight hours of sleep each night. If you can&#8217;t do this consistently, aim for at least six hours of sleep each night. You can also try taking naps during the day if that works better for your schedule than going to bed early at night. It also improves your mood and minimizes the harmful effects of PMS (premenstrual syndrome) hormones.</p>



<p><strong>Healthy Diet –</strong> A nutritious diet has been shown to increase mental well-being. In fact, one study found that following a Mediterranean diet was associated with lower rates of sadness and anxiety. In fact, studies have shown that eating a nutritious diet can help reduce symptoms of depression by up to 50%. For starters, consider increasing your intake of omega-3 fatty acids, found in fish like salmon and trout, as well as flaxseed oil. They help fight inflammation in the body, which can contribute to physical and mental health problems like depression. Omega-3 fatty acids also increase serotonin levels, which improves mood! Increasing your intake of vitamins B6 and B12 will also help improve your mental well-being. These vitamins are needed for the production of neurotransmitters such as norepinephrine, which affects how quickly signals are transmitted between brain cells, or neurons. In other words, they help keep everything in order.</p>



<p><strong>Do yoga –</strong> According to healthcare experts, rehearsing yoga during your period can help ameliorate your emotional state and overall health. Yoga can help you fight internal problems and sleep better. Yoga is a great approach to reducing stress, which can be particularly dangerous for those suffering from hormonal problems similar to PMS or menopause. Yoga can also help you feel more and help acne flare- ups during your period. 	<h2 class="Body" style="text-align: justify; text-justify: inter-ideograph; font-size: 22px; line-height: 1.7;"><strong>Q. How did you turn yourself from clinical cardiology to preventive cardiology and adopt meditation to prevent/reverse heart disease.</strong></h2>

	<p><span style="font-weight: 400;">While working in one of premier Heart Institute in Delhi in 1994-95, I noticed the number of heart patients in the 30-40 yrs age group, coming to the hospital with acute coronary events have increased. After managing the patient the next day we used to perform coronary angiography and we used to put a stent in the culprit lesion. I remember one patient, who was about 36 years old, got admitted to the emergency room with a heart attack. Next day in coronary angiography we found 80% blockage in </span><span style="font-weight: 400;">Left</span><span style="font-weight: 400;"> artery and 60% blockage in </span><span style="font-weight: 400;">right coronary artery.</span></p>
<p><span style="font-weight: 400;">I informed the patient’s wife about the urgent</span> <span style="font-weight: 400;">coronary intervention</span><span style="font-weight: 400;"> by putting stent. Patient was discharged from the hospital three days after the procedure. About 3 months later, same patient was again admitted to our hospital with chest discomfort, he was managed conservatively in CCU and repeat coronary angiography revealed 100% in stent restenosis in one artery and  80% in other artery    his wife had sold their house and for the last 3 months, he is not able to join his work and now what should they sell to get angioplasty. I had no answer to his query and after the round I went to the library and found few research articles, which related psychosocial stressors e.g Type A behaviour, anxiety, depression, anger, hostility, lack of social and emotional support etc. with rapid progression of CAD (Coronary Artery disease) and acute coronary events e.g. unstable angina or heart attack. </span></p>
<p><span style="font-weight: 400;">I met Brahmakumaris Rajyoga Meditation in 1986 when I was at AIIMS, New Delhi. I have been doing Rajyoga Meditation since then and experienced a lot of changes in my thought patterns. Just a few days of practice of Rajyoga Meditation made me experience internal peace, true love, and happiness.  </span><span style="font-weight: 400;">Then, one day after early morning meditation I had clairvoyance, in which I realised how psychosocial stressors are responsible for rapid progression of coronary blockages and acute coronary events. Practice of Rajyoga Meditation along with a healthy diet, healthy exercise, and proper sleep as per our biological clock/circadian rhythm can regress CAD and prevent fresh angina or heart attack.</span></p>
<p><span style="font-weight: 400;">After few days of this realization, when I was delivering a talk on “Stress &amp; Heart Diseases” in Doctors meeting, I shared my thoughts on how stress can lead to rapid progression of coronary blockages &amp; acute coronary events and how meditation and healthy lifestyle changes can reverse coronary blockages and prevent further heart attacks. </span></p>
<p><span style="font-weight: 400;">After this, we planned a research project in collaboration with Defence Institute of Physiology &amp; Allied Sciences and Defence Institute of Psychological Research, DRDO, New Delhi. To conduct this research project I shifted to J Watumull Global Hospital &amp; Research Centre, Brahmakumaris, Mount Abu in April 1995.</span></p>

	<p><span style="font-weight: 400;">In the CAD Research Project, for every angiographically proven CAD patient psychological analysis along with physiological (EEG, HR variability), endocrinological (adrenaline, nor-adrenaline, cortisol; beta-endorphins, serotonin) was carried out. With 24 long years of research, we have been able to co-relate every coronary artery blockage with various thought patterns (type A behavior, anxiety, depression, anger, hostility, lack of social and emotional support, etc). </span></p>
<p><span style="font-weight: 400;">We educate our CAD patients how to change various thought patterns with Rajyoga Meditation. This change in thought patterns along with better adherence to health-promoting behavior e.g. healthy diet, exercise, sleep and de-addiction from smoking and alcohol leads to normalization of endothelial function thereby reverse cholesterol transport of LDL from the plaque into the lumen, hence regression in coronary blockages.</span></p>

	<p><span style="font-weight: 400;">Angiographically documented CAD patients along with spouse are invited for “3D Healthcare Rajyoga Meditative Lifestyle or Healthy Heart, Happy Mind  &amp; Healthy Body”, 7 days in-house program in RMM Global Hospital Trauma Centre &amp; Brahmakumaris shantivan campus. </span></p>
<p><span style="font-weight: 400;">In the first day morning weight, height, BMI, skin for thickness, Heart Rate, blood pressure, ECG, and various tests e.g. Haemoglobin, Serum creatinine, serum lipid profile, blood sugar: fasting, two hours after the breakfast, Glycosylated Haemoglobin (only on day 1), Serum uric-acid are assessed on day one and day 6 (after 6 days of workshop). Along with these baseline assessments detailed Clinical and Psychological Analysis is also carried out. After these baseline assessment patients are given intense information, education and counselling about body, heart, mind, mind-body connection, about Heart Disease, conventional and psychological risk factors of heart disease. Brahmakumaris Rajyoga meditation uses Rajyoga meditation to open the blockages, role of healthy diet, exercise, sleep and de-addiction from smoking and alcohol. On day 6 repetition of clinical, psychological, pathological assessment is carried out. On day 7 every patient along with the spouse is given individual clinical, psychological healthy lifestyle counselling, overall risk factors modification and optimization of medication is advised.</span></p>

	<p><span style="font-weight: 400;">It has been observed in many studies that stress e.g. type A behaviour, anxiety, depression, anger, hostility etc leads to prolonged endothelial dysfunction. Rajyoga Meditation normalizes endothelial function thereby reversing the heart disease. When we are in stress our thought speed increases from 10-15 thoughts per minute to 30-45 thoughts per minute. </span></p>
<p><span style="font-weight: 400;">Rajyoga Meditation is conscious training of the mind for enjoying inner self consciousness for enduring the immortal spirit/soul and it leads to stability and full proof security. This further leads to the understanding of truth, purity, peace, love, happiness, bliss and powers from outer self-consciousness of the mortal body, or ever-changing role &amp; material things which leads to instability and insecurity therefore they in-turn leads to type A behaviour, anger, anxiety, depression, isolation and chronic life stresses. 	<p><span style="font-weight: 400;">We have observed that more than 10,000 heart patients also had other lifestyle-related diseases e.g. anxiety, depression, hypertension (about 35%), diabetes (about 30%), high cholesterol (65-70%), obesity (35-40%), GERD/Acidity (45-50%), migraine (10-15%), cervical &amp; lumbar spondylosis (20%), osteo-arthritis of knee joints (25%). We observed that most of these heart patients had better control of all lifestyle-related diseases of mind and body with significant reduction in requirement of medication.</span></p>

<p><i><span style="font-weight: 400;">Self-responsibility through self-empowerment</span></i><b>:</b><span style="font-weight: 400;">  The word ‘healthy’ is derived from two words; ‘heal’ + ’thy’ i.e., to heal yourself. To heal oneself, one needs to be empowered by appropriate information about the mind-body connection, psychological and conventional risk factors, stress management, diet, exercise, sleep, substance abuse, and usual medical care. </span></p>

<p><span style="font-weight: 400;">The Hindi word for healthy is ‘swasth’, which consists of swa-, meaning &#8216;inner self&#8217;, and -asth, meaning &#8216;conscious&#8217;. The word ‘healthy’ could also mean &#8216;inner self-conscious&#8217;. Outer self-consciousness of the mortal body, or ever-changing role &amp; material things, leads to instability and insecurity, which in turn leads to anger, anxiety, depression, type A behavior, isolation, and chronic life stresses.  Conversely, inner self-consciousness of an enduring and immortal spirit leads to stability and security, which in turn leads to peace, love, and happiness.  A healthy lifestyle is an &#8216;inner self-conscious lifestyle&#8217;.</span></p>

<p><span style="font-weight: 400;">More than 93% of patients adhere to this program sincerely. It has been observed that “Better the program adherence, more the reversal of heart blockages and other lifestyle-related diseases-in a dose-response manner.”</span></p>

These all NCDs including cardiovascular disorders (such as hypertension, heart attacks, and stroke), cancers, chronic respiratory diseases (such as chronic obstructive pulmonary disease and asthma), depression, diabetes, etc. are psychosomatic/lifestyle-related diseases and only permanent solution for all these NCDs is integrating Meditation with modern state of art medication to enjoy healthy and happy life. Yes,being overweight is linked to a number of health and oral health diseases, including: heart disease, high blood pressure, diabetes, cancer, gallbladder and gallstones, breathing problems, such as sleep apnea (when a person stops breathing for a short time during sleep) and asthma, oral health problems such as caries, halitosis, poor oral hygiene, periodontal disease. Symptoms for obesity include breathlessness, Increased sweating, snoring, inability to cope with sudden physical activity, feeling tired, back and joint pain, low confidence and self-esteem, and feeling isolated.</p>

<p style="text-align: justify !important;">After being aware of the risks, proper health and oral hygiene should be maintained. And why is maintaining diet an important factor? Because a poor diet may result in lack of essential nutrients and vitamins thus potentially making it more difficult for your body to fight off an infection. A diet high in fat and calories, including large quantities of processed foods or sugar, and lack of physicalactivity, leads to obesity. Those same foods can cause oral health problems including gum disease, premature tooth loss and bad breath Our body craves for what is in our bloodstream. Wheat, sugar, alcohol, coffee and de-vitalized foods are designed to be addictive. Sugar being addictive &#8211; agreed. But how does sugar affect an individual’s health and oral health? Well, consuming sugar is bad for an individual, but consuming too much has become a norm and that can cause health issues. The only type of sugar the body needs is glucose, which it can make by breaking down carbohydrates, proteins and fats. When ingested, simple sugars are converted into plaque if not removed immediately. As plaque begins to accumulate on our teeth and gums, the risk of varying forms of gum diseases such a gingivitis and periodontitis, as well as tooth decay, may become inevitable.</p>

Dentists must be aware of how nutrition impacts general and oral health, and how dental treatment can impact patient’s nutritional status. Oral health is strongly influenced by the daily intake of food; on the other hand, oral health can also play a significant role in nutritional intake and general health status. The dental caries is also a chronic disease and may be depend on oral hygiene, high intake of sugar added to food, lifestyle factor, and host factors (salivary flow, buffer capacity and buccal microflora). Relationship between obesity and gum disease &#8211; we actually don’t know if this is a one-way or a two-way street. For example, conditions like diabetes, the relationship is bilateral. Periodontal disease will affect diabetes and diabetes will worsen periodontal disease, so that could be a possibility here. What are the other reasons of obesity affecting gums and what would be its risk factors? Because of various dietary habits and improper maintenance of oral hygiene, chronic periodontal infection is an inflammatory state that is described by a deviation in microbial environment and composition of subgingival plaque bio films and accelerated destruction of tooth supporting structures. Thus, low glycaemic diet may be beneficial to our oral health. Periodontal diseases are also influenced by various risk factors including aging, smoking, oral hygiene, socio-economic status, psycho social stress, osteopenia, osteoporosis and various systemic diseases, which signifies that periodontitis does not merely occur as a consequence of plaque deposition but is also coupled with various other host factors. Also, on examination, obese adolescents exhibited multiple carious teeth, higher plaque index, gingival inflammation, bleeding on probing and probing depth compared to normal weight subjects of similar age.</p>

	<p style="text-align: justify !important;">An obese patient, whose lungs are often not large enough to lift heavy chest walls, can have trouble during breathing, especially when active. Also, during sleep the person might breathe through his/her mouth instead of nose, thus jaw is forced into an unnatural position for a longer period of time. This can lead to teeth grinding and development of an overbite, over jet or spacing between the anterior teeth.How can we, as dentists, help the obese patients? Dentists should include the evaluation of BMI in routine patient examination and eventually refer obese periodontal adolescents for weight reduction interventions like diet therapy, behavioural therapy, pharmacotherapy, so that they can have better control over periodontal inflammation.What are the practical considerations in dentistry? It can be difficult carrying out dental care for an obese patient: the maximum lifting weight for modern dental chair is approximately 140kg (23 stones). The position of anatomical landmarks may be less obvious if surrounded by fatty tissue, the landmarks for placement of an inferior alveolar nerve block can be impossible to palpate through excess soft tissue. It may be impossible to palpate cervical lymph nodes in a large neck. Also due to constant secretion of saliva, the mouth and teeth are rinsed and kept free off food debris and foreign particles, thus excess of buccal pad of fat hinders with self-cleansing action in the mouth. Because of lack of saliva present in the mouth, it can lead to dry mouth. Sip water frequently throughout the day and use alcohol-free antiseptic mouthwash daily for cure. A normal sized mirror is unsafe for retraction of soft tissues: a ‘lax’ tongue retractor can be useful.What is the alternative to conventional dental chair? Refer the patient to the hospital environment where the patient may be treated on an operating table or trolley. Another possible solution for obese patients is a custom-made chair such as DIACO dental chair. What is the difficulty faced during conscious sedation? When thinking about conscious sedation for obese patients, the potential difficulties in airway management and intravenous cannulation should be considered. The provision of inhalation sedation with nitrous oxide during which oxygen levels are maintained at or above 30% may be more appropriate. If intravenous sedation with midazolam is proposed, the overall benefit to the patient must be carefully weighed up against the increased likelihood of significant respiratory depression and the difficulties in managing a respiratory complication. New research suggests that the health of your mouth mirrors the condition of your body as a whole. A healthy smile can surely transform our visual appearance and the positivity of our mind-set.</p>

	<p style="text-align: justify !important;">Hemp or Cannabis sativa with limited concentration of an active psychotropic substance tetrahydrocannabinol (THC) &lt;0.3% is a promising industrial crop. It is known for its use in paper industry, textile, biodegradable plastics, paint, biofuel, food and feed. Historical evidence of its use can be traced in ancient literature Atharvaveda (1500 BC); Sushruta Samhita for treatment of phlegm and diarrhea and Persian medical text of 18th century (UnaniTibbi). Cannabis sativa, due to its close relative Cannabis indica (high THC Cannabis), always remained under shadow. It can be a useful source of non-psychotropic cannabidiol (CBD). The recent advances in CBD research and identification of Cannabinoid receptors in human revolutionized its importance in western pharmacology. The use of flowers/buds for cannabidiol (CBD), a non-addictive substance, is extremely useful for pharmacological formulation managing chronic pain, anxiety, inflammation, depression etc.</p>

<p style="text-align: justify !important;">The processing of flower and buds of Cannabis is illegal in India. Flower and bud trichome’s release psychotropic substances formulated as charas, hashish and ganja. Phytocannabinoids accumulate in the secretory cavity of the glandular trichomes, which largely occur in female flowers and in most aerial parts of the plants. The seeds and leaves are permitted for usage depending on state policy though licensing. Ironically, since ages peoplein India have been consuming bhang milkshake (a preparation of C. sativa or Vijaya) as sacred drink during Lord Shiva related rituals, though its cultivation was legally banned in 1985 by Narcotic Drugs and Psychotropic Substances Act. This act came after International Treaty of 1961 regarding Narcotics Substances.</p>

<p style="text-align: justify !important;">Mechoulam (2005) described hemp as “neglected pharmacological treasure trove”. Cannabinoids are now recognized more because of their relation with human endocannabinoid system which includes two G proteincoupled cannabinoid receptors (CB1 and CB2) and two ligands (anandamide and 2-arachidonylglycerol). They can modulate variety of physiological responses such as pain, memory, inflammation, appetite, behavior etc. In contrast to Cannabinoids, THC is partial agonist of above mentioned receptors with higher binding with CB1. This may be associated with its known psychoactive abilities. Even though THC’s are more potent as a drug which can be used for various treatment protocols, the associated side effects makes it inferior candidate of choice in contrast to CBD. CBD can also reduce THC negative effects and help in improving efficacy of Cannabis based products. <strong>CBD is very promising cannabinoid and has been successfully used in preclinical studies of nervous system related diseases (epilepsy, schizophrenia etc.). However, at present, itsuse in therapeutic treatments is still limited.</strong></p>

These bricks absorb carbon monoxide and are durable as cement bricks. The production of hemp bricks can provide economic benefit venture for Indian farmers. This brick-based material was used in preserving Ellora Caves for 1500 years. In India, the major problem in any such venture is ‘middlemen policy’ which makes end product expensive with least benefit to the farmer (licensing cost etc.). Permission for contract farming lacks legal support. India, unlike developed countries, neither has strict regulation nor it is relaxed enough to cater as uniform business opportunity. Furthermore ‘stigma’ attached to hemp cultivation (confusion with marijuana) and lack of industrial linkage to farming units with direct export potential further reduces its industrial potential.</p>

As per understanding of Ayurvedic perspective, the plant resin (especially from wild plants at 2000 or 3000 m height) is charas which is an extremely powerful banned narcotic drug. However, its leaf based formulation bhang is used in Ayurvedic preparation as stimulant, aphrodisiac and also as sedative. The use of CBD in Indian Ayurvedic Formulations (with traditionally known 191 Ayurvedic preparations for 29 different diseases), regulated and approved by AYUSH after clinical testing, are still in a nascent stage. AYUSH medicines still need to bridge gap between their standards with globally accepted norms. The legal aspects of excise duties on medicinal or toilet preparation containing Indian Hemp limits its probable utility. It means when business is exploring its potential crossing excise department, agriculture department, narcotics department, there is no single window approach feasible. <strong>This environment-friendly, pest resistant and sustainable crop is self sustainable after few production cycles and can even be easily regulated by the provision of raw material i.e. seeds only via government sources.</strong></p>

Secondly, the implementation of law requires adequate State machinery with upgradation of Indian Industrial Hemp Association to a forum for awareness, training and feedback platform. The integration of Central Digital Policy for linking of Aadhaar system to transfer benefits directly to farmers by controlling middlemen. Lastly, the development of uniform code throughout the country addressing excise, narcotics, medicinal, research and agriculture/export policy related to Indian Hemp multiple industrial uses with special focus on promising pharmacological relevance.</p>

	<p style="text-align: justify !important;">The US Food and Drug Administration (FDA) approved ketamine on 6th March 2019 —a ketamine like nasal spray, which is to be taken in conjunction with one of the commonly used oral antidepressants —to help in treating a form of depression called treatment resistant depression (TRD). TRD is a condition that typically affects people suffering from major depressive disorders (MDD). The key feature of TRD is the inability to respond adequately to at least two antidepressant treatments.</p>
<p><em><strong><a href="https://innohealthmagazine.comtrends/exercise-in-big-parks-can-reduce-depression/">Exercise in big parks can reduce depression</a></strong></em></p>
<p style="text-align: justify !important;">The drug is administered via the nasal route, where it is absorbed by the nasal lining into the bloodstream. Part of the promise that ketamine holds, lies in the role it plays in swiftly alleviating depressive symptoms. Esketamine is reported to show statistically significant effects as quickly as a matter of days, as against the traditional antidepressant treatments that can take several weeks before showing any results.</p>

<p style="text-align: justify !important;">“There has been a long-standing need for additional effective treatments for treatment-resistant depression, a serious and life-threatening condition,” said Tiffany Farchione, MD, Acting Director of the Division of Psychiatry Products in the FDA’s Center for Drug Evaluation and Research, in a press release announcing its approval. Unsurprisingly, the FDA’s approval has been hailed as a landmark milestoneby its advocates.</p>

<p style="text-align: justify !important;">Given the potential risk of abuse and the risk of serious adverse outcomes, like sedation and dissociation (disconnect in a person’s thoughts, memories and a sense of identity) among others, associated with administering of the drug, the drug will only be available through a restricted distributed system. In addition, the drug will only be administered in a clinical setting where patients are required to stay back and be monitored for a few hours.</p>
<p><em><strong><a href="https://innohealthmagazine.comtheme/ai-can-help-decode-epileptic-brain/">Epilepsy is the Fourth Most Common Neurological Disorder</a></strong></em></p>
<p style="text-align: justify !important;">Ketamine as a drug has been around for many years and has been used off-label to treat depression. Many psychiatrists in India provide ketamine infusion therapy, which is essentially ketamine administered intravenously, as it is felt to be beneficial during emergencies and especially in patients diagnosed with suicidal ideation.</p>
<p><em><strong><a href="https://innohealthmagazine.comresearch/innovative-biomarkers-early-diagnosis/">Innovative Biomarkers for Early Diagnosis and Treatment of Life</a></strong></em></p>
<p style="text-align: justify !important;">An average of about 4.4% of the world and 5.9% of the United States’ population, suffer from varying degrees of depression and the incidence in India is not vastly different at 4.5%. Insuch a scenario, where the incidence rates are similar, treatment options for depression available around the world are required to be made available here in India as well. It is, therefore, incumbent on all the stakeholders to deliberate the efficacy and the potential harm of the drug expeditiously, and if found suitable for the Indian population, fast track its availability.</p>

<p style="text-align: justify !important;">Despite Ketamine’s success in giving the patient a new impetus to recovery, it is important to sustain the gain. All aspects of a patient’s disease, from the physical to the psychological and from the social to the environmental need to be addressed. Ketamine is really a shift in thinking about and acting on depression, which really represents a leap in terms of new ideas for tackling the disease.</p>

Did you know that one student commits suicide every hour in India? Often in disbelief, parents of these victims cannot find reasons for such an ‘extreme step’ in their seemingly normal lives.</p>
<p style="text-align: justify !important;">Have you ever found your mind playing tricks on you? The devil on your shoulder telling you how unworthy you are, how useless your life is? You are not alone. Many people that you see around you might have felt the same way at some point or the other.</p>

	<p><strong>Data Speaks Loudly</strong></p>
<p style="text-align: justify !important;"><a href="https://innohealthmagazine.comnewscope/non-pharmacological-management-copd/">Mental illness</a> contributes to 7% of the global average disease burden. The number of suicides in India is double than the global average of 11 persons per 100,000 population annually. Mental maladies affect about 15% of Indian (and world) population as per latest estimates. One in six people are victims of mental illness. Depression and anxiety disorders alone affect around 8% of us.</p>
<p style="text-align: justify !important;">Among the young, suicide is the third biggest killer, with depression the leading cause of early-age disability globally. In fact, mental disorders are thought to affect younger people than statistics suggest.</p>

	<p><strong>It&#8217;s Complicated</strong></p>
<p style="text-align: justify !important;">People experiencing mental disorders resort to one of several coping strategies: discussing the problem with friends or family, social isolation to escape being noticed, or camouflage true emotions into acceptable actions for fear of being judged or abandoned. Unfortunately, the latter two are more popular, driving forces being social stigma and taboo. There is little or no support to sufferers and their families. Surveys reveal that society even feels fearful of the sufferers!</p>
<p style="text-align: justify !important;">Undoubtedly, behaviors that indicate mental disturbances can sometimes be hard to pinpoint, passed as the simple quirkiness of character than a disorder of the mind. But is the matter really that straightforward?</p>

On the other hand, when farmers drowning in debt kill themselves, the reasons are evident and understandable. Similarly, while some mental illnesses are conspicuous, others are not.</p>

Yet, in this fast-paced life in the 21st century, signs of distress escape the friendly radars of our loved ones and our own selves. Besides the lack of free time, our reluctance to understand these complex phenomena, ignorance and social stigma worsen the situation for sufferers. Depressive bouts are confused with laziness, anxiety simply called lack of focus and attention deficit equated to incompetence. Such labels make a negative impact, especially on the young impressionable minds. In fact, half of all mental illnesses begin even before the age of fourteen!</p>

	<p><strong>Is It All In Our Heads?</strong></p>
<p style="text-align: justify !important;">Brains are complex machines. Scientists have been studying it for years to find neuropsychological causes and genetic bases of mental illnesses, which are many- depression, anxiety disorders, eating disorders, cognitive disability and so on. To understand where the problems lie, researchers dive deep into the workings of the brain. The clockwork includes a huge repertoire of chemical and electrical signals that pass between connected brain cells called neurons. Imbalances in signals between or within different parts of the brain result in myriad mental anomalies.</p>
<p style="text-align: justify !important;">Anxiety is a result of an over-active amygdala- the fear center of the brain, resulting from an imbalance between excitatory and inhibitory signals for this area, making sufferers more nervous and fearful than normal people. Severe anxiety disorders such as panic and post-traumatic stress disorder (PTSD) involve the same parts of the brain, only different disturbances in the ongoing signaling.</p>
<p style="text-align: justify !important;">Depression, on the other hand, appears to be more complex. Firstly, the levels of chemical signals (neurotransmitters) called serotonin and dopamine are abnormal. Serotonin signals well-being and happiness to our brains while dopamine induces motivation. Secondly, the brain tissue is sometimes shrunken. Third, the ability to form new neurons and connections or repair and modify old ones is impaired. Lastly, immunity modulators called cytokines, which induce sickness behavior like staying indoors, sleeping a lot and generally, low energy levels are found in excessively high amounts in depressed individuals. Cytokine signaling is in fact elevated under chronic stress. Though depression manifests as slow behavioral changes that evade our observation, often simply under the cloak of sadness, it is a serious medical issue.</p>
<p style="text-align: justify !important;">These findings imply that maladies of the mind are just like those of the body, say cancer or diabetes- complex diseases that manifest upon an interplay of genetic predispositions and environmental triggers- preventable, manageable, treatable. Basically, balanced neurological circuitry is essential for mental health.</p>
<p style="text-align: justify !important;">Interestingly, we have also witnessed that most people tide over adversities, but some are unable to- not every victim of bullies becomes mentally ill. What determines mental resilience? Environment? Education? Or simply will power? Studies suggest that a feeling of security, purpose in life and positive experiences are the foundations to mental resilience. The sooner one starts building it, the better, but there is never a wrong time to build resilience. These building blocks are present both within and around us.</p>

	<p><strong>Happiness is Health</strong></p>
<p style="text-align: justify !important;">Preventing chronic negative thoughts is the key to a healthy mind. Children especially need to be allowed more leisure time. Physical activities like exercise, sports and dancing are known to release feel-good hormone endorphin which benefits the brain. Pursuing hobbies, activities that make you happy or fulfilled and content can keep you motivated in low phases of life, teaching you to celebrate little victories. A hearty laugh works the same way.</p>
<p style="text-align: justify !important;">Invest time in friends and family. Talking to and confiding in someone trustworthy helps you create a support system that you can summon in times of distress or crises. Give the children around you their due time and attention. Cuddling with a pet and playing with kids generates oxytocin- the bonding hormone. Above all, sleep well, for it helps your body and mind heal itself. Rejuvenate them both time to time using tools like meditation to keep serotonin and dopamine balanced.</p>
<p style="text-align: justify !important;">If circumstances still catch you unaware, know that help is out there, if not in your home, at the touch of a button. Mental health foundations and support groups are at your disposal if only you call them for help. Phone helplines and online counseling portals like “Type a Thought” and “mystruggles.in” – that keep your identity anonymous, exist to resolve your distress. Sometimes simply talking to friends or family is enough to help you tide through. If none of the above helps, tap into professional psychiatrists. Taking medication to relieve yourself of sickness is not a shame. Do you think twice before taking a pill for cold? You might not even need medication. Interventions like Cognitive Behavioural Therapy (CBT) and Neuro-Linguistic Programming (NLP) can help you learn new thinking patterns, overcoming anxiety and fear.</p>

	<p style="text-align: justify !important;">If only we as a community realize that mental illnesses are not chosen by those who are ill, like any other ailment, and fulfill our roles as parents, teachers, and caregivers, healthy and happy youth is not a distant dream. If only we would give children enough time, attention and affection, they would not grow up as broken individuals. If only we learn to ask “How are you?” and really mean it; if only someone were to ask us “Are you alright?” at our most terrible times; if only we accepted the differences among people; if only no one shamed the fat teenager; if only we didn’t burden our children with our ambitions and expectations (and maybe instead followed them ourselves), we could save lakhs of lives. And that would be a life worth living. Wouldn’t it?</p>
<p style="text-align: justify !important;">We must remember, childhood and adolescence are tender, confusing and ruthless, all at the same time. Young minds need a sense of belonging, affection, and appreciation to grow up normally with nurture, cautious prodding and guidance at every step. This journey translates into meaning and purpose around which their entire adult life would revolve. Many mental illness victims report feeling unloved, unnoticed, or worse &#8211; blamed, bullied, beaten and shamed as a youth. So, if we want to save more people from mental disorders, the younger we catch them, the better.</p>

	<p style="text-align: justify !important;">The study involved 1,195 people who had an ischemic stroke or a stroke where blood flow to part of the brain is blocked. They were asked questions about their physical functioning, fatigue, anxiety, sleep problems, thinking skills such as planning and organizing, how much their pain affects other aspects of their life and their satisfaction with their current social roles and activities. Participants took the questionnaires an average of 100 days after their stroke, and about a quarter of the participants needed help from a family member to fill out the questionnaires. Researchers also measured their level of disability.</p>
<p style="text-align: justify !important;">The people with stroke had scores that were considerably worse than those in the general population in every area except sleep and depression. Not surprisingly, the area where the people with stroke were most affected was physical functioning, where 63 percent had scores considered meaningfully worse than those of the general population, with an average score of 59, where a score of 50 is considered the population average.</p>

<p style="text-align: justify !important;">“People may benefit from social support programs and previous studies have shown a benefit from efforts to improve the social participation of people with stroke, especially exercise programs,” said Katzan.</p>
<p style="text-align: justify !important;">The thinking skills of people with stroke in executive functions, or planning and organizing, were also affected, with 46 percent having scores that were meaningfully worse than the population average.</p>

	<p style="text-align: justify !important;">“The social participation and executive functioning skills are areas that have not received a lot of attention in stroke rehabilitation,” Katzan said. “We need to better understand how these areas affect people’s well-being and determine strategies to help optimize their functioning.”</p>
<p style="text-align: justify !important;">Limitations of the study include that the questionnaires did not ask about other problems that can occur after stroke, such as communication issues.</p>
<p style="text-align: justify !important;">Also, the study participants had milder strokes on average than people with stroke overall and the average age of participants was 62, which is lower than the average age of 69 for people with stroke overall.</p>

	<p style="text-align: justify !important;">The noise levels commuters are exposed to while using public transport or while biking, could induce hearing loss if experienced repeatedly and over long periods of time, according to a study published in the open access Journal of Otolaryngology-Head &amp; Neck Surgery. Efforts to control noise should focus on materials and equipment that provide a quieter environment, researchers at the <strong><a href="https://www.utoronto.ca/">University of Toronto</a></strong> suggest. Hearing protection while using public transport should also be promoted.</p>

	<p style="text-align: justify !important;">Dr. Vincent Lin, the corresponding author said: “This study is the first to look at and quantify the number of noise people are exposed to, during their daily commute, specifically on the Toronto Transit System. We are now starting to understand that chronic excessive noise exposure leads to significant systemic pathology, such as depression, anxiety, increased risk of chronic diseases and increased accident risk. Short, intense noise exposure has been demonstrated to be as injurious as longer, less intense noise exposure.”</p>
<p style="text-align: justify !important;">Dr. Lin said: “We were surprised at the overall average noise exposure commuters experience on a daily basis, especially the peak noise intensity not only on trains but also on buses. Planners need to be more considerate of noise exposure in the future planning of public spaces and public transit routes. Toronto in particular, as the transit network expands, needs to consider ways to reduce noise exposure as a preventive measure for future health risks.”</p>
<p style="text-align: justify !important;">According to thresholds recommended by the US Environmental Protection Agency (EPA), exposure to 114 A-weighted decibels (dBA) for longer than four seconds, exposure to 117 dBA for longer than two seconds and exposure to 120 dBA for longer than 20 seconds may put people at risk of noise-induced hearing loss. A-weighted decibels express the relative loudness of sounds experienced by the human ear; taking into account that sensitivity to noise differs depending on noise frequency. Peak noise levels in dBA across both public and personal transport exceeded the EPA recommended thresholds. The average noise levels by bike were greater than any level caused by modes of public transit.</p>
<p style="text-align: justify !important;">To measure noise exposure, the researchers used noise dosimeters, which they carried on their shirt collars about two inches away from their ears. The researchers collected 210 measurements in total, comparing the noise on subways, buses, and streetcars, while driving a car, cycling, and walking. They measured in-vehicle noise and outside or boarding platform noise for all modes of private and public transportation.</p>
<p style="text-align: justify !important;">The authors found that 19.9% of the loudest noises (peak noise) measured on the subway were greater than 114 dBA, while 20% of the loudest noises inside streetcars were greater than 120 dBA. 85% of peak noise measurements from bus platforms were greater than 114 dBA, while 54% were greater than 120 dBA. All peak noise exposures while riding a bike exceeded 117 dBA, with 85% being greater than 120 dBA.</p>
<p style="text-align: justify !important;">When the authors extrapolated the EPA recommended noise thresholds for an average Toronto commuter who uses public transport, the recommended level of noise exposure was exceeded in 9% of the subway, 12% of bus and 14% of biking measurements but not when using streetcars, bikes or when walking.</p>
<p style="text-align: justify !important;">The authors caution that the number of measurements taken for individual modes of transport is relatively low and that the cross-sectional nature of the study does not allow for conclusions about cause and effect. Further studies are needed to investigate other factors that may contribute to noise exposure such as the use of music players and lengthy transit times.</p>

<strong>Difference between Pranayama and Breathing</strong></p>
<p style="text-align: justify !important;">Pranayama is the conscious awareness of breath. The term is derived from Sanskrit, Prana means breath, respiration, vitality or energy. Ayama means expansion or stretching. Thus, Pranayama connotes extension and control of breath. Proper rhythmic, slow and deep breathing, strengthens the respiratory system, soothes the nervous system and increases concentration. According to Yoga, the breath connects the Body, Mind, and Spirit.</p>
<p style="text-align: justify !important;">The rate of breathing changes with circumstances such as it increases due to emotional or physical disturbances whereas, it automatically slows down when one is calm and peaceful. Like, when we get tired while climbing stairs, we get breathless. Try the following to regulate your breathing and feel less exhausted. Climb keeping shoulders straight and take a deep breath in for two stairs (inhale), and exhale for two steps, with a rhythm of two in and two out. Like this, we expel more carbon dioxide and take in great supply of oxygen, thus we feel less fatigue.</p>
<p style="text-align: justify !important;">Mostly we use only a fraction of our lung capacity as we do shallow breathing. We barely expand the ribcage, shoulders are often hunched and have painful tension in the upper part of back and neck due to which we suffer lack of oxygen which makes us breathless and tired. So, keep both shoulder blades as close as possible without strain and exhale gently and fully pause, then inhale with a deep, slow, gentle breath until the lungs are comfortably filled. Then breath out slowly through the nose, without changing the position of shoulder blades. Repeat this cycle many times. By doing this, the brain gets stimulated and ease nerve tension by providing the fuller supply of oxygen.</p>
<p><strong>Types of Breathing </strong></p>
<p style="text-align: justify !important;"><em>Clavicular Breathing (Sectional Breathing):</em> It is shallow breathing in which the abdomen is completely controlled, and breathing is performed by forcing the air into the uppermost region of the lungs. The shoulders and collar bones are raised while the abdomen is contracted during inhalation.</p>
<p style="text-align: justify !important;"><em>Thoracic (Chest Breathing):</em> Breathing takes place by expanding and contracting the chest only, while the abdomen is kept under control. The middle lobes of the lungs are fully activated through this breathing.</p>
<p style="text-align: justify !important;"><em>Abdominal Breathing:</em> Deep abdominal breathing brings air to the lowest and largest part of lungs. Breathing is slow and deep such that diaphragm is used properly.</p>
<p style="text-align: justify !important;">A full yogic breath combines all three, beginning with abdomen and continuing the inhalation through the thoracic and clavicular area. The abdomen should expand outward on inhalation and contract on exhalation. To get the feeling of this motion, while sitting in any meditative posture preferably Vajrasana, place the hands on the abdomen close to each other. Exhale slowly inhale through the nostrils taking the hands away from each other so that the abdomen bulges. Hold the breath for a second. Then slowly exhale so that the abdomen retracts bringing the hands closer. Hold the breath for a second. Repeat the breathing cycle five times, preferably in the ratio 4:2:8:2. Breathing should be deep, slow and rhythmic.</p>
<p style="text-align: justify !important;">The lower sections of the lungs increase with air flow. Rhythmic movement of the diaphragm massages the abdomen gently and enables the organs to function effectively.</p>
<p><strong>Types and Benefits of Pranayama</strong></p>
<p style="text-align: justify !important;">Pranayama is practiced in a specific posture, especially sitting in Padmasana or Ardhpadmasana and should be performed early morning on empty stomach at a well ventilated quite place. Breathing should be slow and rhythmic, eyes should be close to control the mind and body. It has three aspects (1) Poorka (Inhalation) (2) Kumbhaka (Retention) (3) Rechaka (Exhalation).</p>
<p><strong>Types of Pranayama:</strong><br />
• Ujjayi Pranayama (victorious breath)<br />
• Anulom Vilom (alternate breathing)<br />
• Brahamari Pranayama (humming bee breath)<br />
• Bhastrika Pranayama (air is forcibly in and out)<br />
• Shitali Pranayama (Cooling breath)<br />
• Bahya Pranayama (forcibly breath in, breath out and then hold the breath)<br />
• Kapalabhati Pranayama (exhales air from lungs is forcibly but inhalate involuntary)<br />
<strong>Benefits of Pranayama</strong></p>
<p style="text-align: justify !important;">Pranayama purifies the psychic channels (nadis) and enables physical and mental stability. Pranayama is known to purify about 72,000 nadis or channels in the body. It helps to purify the blood and respiratory system. Deep breathing enriches the blood with oxygen. Large amounts of oxygen reach the brain, lungs, heart, and capillaries.</p>
<p>Pranayama techniques are beneficial in treating a range of stress-related disorders:<br />
• Improves the autonomic functions<br />
• Helps relieve the symptoms of asthma<br />
• Aids in reducing unnecessary thoughts which helps in calming down the overactive mind thus reduces anxiety and depression<br />
• Causes change in the cardiorespiratory system and help in lowering the blood pressure<br />
• Improves physical fitness by practicing certain asanas of yoga, it is excellent for weight loss also<br />
• Deep breathing i.e. bringing focus on the breath can be extremely relaxing and rejuvenating. Regular practice improves concentration, memory and overall state of mind.<br />
<strong>Precautions </strong></p>
<p style="text-align: justify !important;">Pranayama should be practiced with care and it is better to do under the guidance of trained yoga teacher, as certain Pranayama is not advisable to be performed by people having certain ailments.</p>
