<h2 class="wp-block-heading" style="font-size:22px"><strong><strong>What is proactive healthcare?</strong></strong></h2>



<p>In 2017, Stanford Medicine partnered with Apple to launch the digital health study. This study made use of the Apple Watch to screen for irregular heart rhythms in 400,000 participants. Its goal was to learn if the watch could detect atrial fibrillation, a heart condition causing 130,000 deaths and 750,000 hospitalizations in the United States each year. It often remained undiagnosed because many people never experience symptoms; it can increase the risk of stroke and heart failure.</p>



<p>The algorithm in the Apple Watch was revealed to successfully identify atrial fibrillation. Additionally, it also showcased user engagement that can be enhanced with the use of digital health alerts. A survey of participants who received an irregular-pulse notification showed that 76% contacted either the telehealth provider or a non-study provider, suggesting that many actively sought medical attention as a result of an irregularity identified by their Apple Watch</p>



<p>This is a fine example of a person-first healthcare system, also called a proactive healthcare system. It proactively analyzes data to identify people at risk and takes the right steps to intervene and provide support and sustain wellness.</p>



<h2 class="Body" style="text-align: justify; text-justify: inter-ideograph; color: #a5a5a5; font-size: 22px; line-height: 1.7;"><strong><em>Health management devices such as fitness trackers or blood sugar monitors help people become active partners in managing their condition, rather than remain passive recipients of treatment.</em></strong></h2>



<h2 class="wp-block-heading" style="font-size:22px"><strong><strong>How can UX contribute to designing proactive healthcare?</strong></strong></h2>



<p>A people-first healthcare system is one that mindfully engages them to identify the best care based on their personal health status. For a healthcare system to transform into a people-enabled system, the key motivations and expectations of users must be at the forefront of its design. People are now proactively asking for healthcare tools that work best for them, creating an intensive demand for personalized options.</p>



<p>Here are three ways in which UX design can take things forward –</p>



<ol class="wp-block-list" type="1"><li><strong>Designing personalized tools that put users in charge</strong></li></ol>



<p>Most healthcare apps until now were centered on solving an explicit problem.</p>



<p><em>A user complains of symptoms?</em></p>



<p>Help them book a doctor’s appointment.</p>



<p><em>A user’s insurance premium is due?</em></p>



<p>Provide easy payment options.</p>



<p>However preventive healthcare primarily focuses on wellness, dealing with situations wherein the user is yet to encounter a problem. Therefore, proactive healthcare tools have to be designed in a way to get users interested in activities that would prevent them from having the explicit problems mentioned before.&nbsp;</p>



<p>The World Health Organization estimates that 50% of the global burden of disease is chronic illness. Health management devices such as fitness trackers or blood sugar monitors help people become active partners in managing their condition, rather than remain passive recipients of treatment. Fitbit’s achievement badges are among the best examples of technology in healthcare, which rewards the user for having accomplished a set of fitness goal, encouraging them to keep working on their health.</p>



<p>Regular and timely communication between health systems and patients is a standout feature of proactive healthcare design and is prominent among healthcare technology trends. It is a feature that has transformed the patient experience. For instance, cancer patients who completed treatment over a year ago and are overdue for screening can be reminded of the same via a highly targeted outreach. This early intervention plays an important role in identifying any relapses and ensuring timely treatment.</p>



<ul class="wp-block-list"><li><strong>Tracking population and individual health trends</strong></li></ul>



<p>Preventing the onset of illness is the holy grail of sustainable healthcare transformation and UX design plays an important role in its facilitation. There are 2 primary types of healthcare tools –</p>



<p><strong>i.) Proactive solutions</strong>, which identify at-risk people by running known algorithms to track social determinants of health (SDOH) such as tobacco or alcohol consumption, sedentary lifestyle, or lack of nutrition, etc. Preventive action is then applied before the manifestation of symptoms, let alone illness. By integrating non-clinical factors and incorporating SDOH data into workflows, the gaps in patient management can be closed. For example, care providers can are the patients residing in remote food deserts and have the ability to provide them with a nutritional food delivery service.</p>



<p><strong>ii.) Predictive solutions</strong>, which make use of healthcare AI technology or machine learning data algorithms for not only predicting and classifying risks, but also intervene at the right stage. For predictive healthcare solutions to work requires data from across the healthcare system. It also has to be aggregated, centralized, and analyzed to provide care managers with a 360-degree view of the population’s health and suggest actionable insights. In the recent pandemic, predictive solutions were deployed to virtually screen and identify people at risk of COVID-19 to determine the most urgent set of vaccine recipients. Another example of upcoming healthcare technology companies is Benevolent AI, which uses the deep learning to produce a better target selection and provide previously undiscovered insights, thus getting the right treatment to the right patients at the right time.</p>



<h2 class="Body" style="text-align: justify; text-justify: inter-ideograph; color: #a5a5a5; font-size: 22px; line-height: 1.7;"><strong><em>Well-designed apps are those that reinforce the concepts of privacy and security through functionality, navigation, and content. </em></strong></h2>



<ul class="wp-block-list"><li><strong>Giving users confidence with managing their health data</strong></li></ul>



<p>Privacy and security have always been considered as important concerns in digital experiences across any industry, more so in the case of healthcare. According to a<a href="https://www.pwc.com/us/en/services/consulting/cybersecurity-privacy-forensics/cyber-and-privacy-innovation-institute.html"> </a>survey conducted for Cyber crime and privacy breach, only 25% of consumers surveyed believe most companies handle their sensitive personal data responsibly but 85% of them say cyber security and privacy breach are among the biggest risks facing society. Well-designed apps are those that reinforce the concepts of privacy and security through functionality, navigation, and content. Two-factor authentication and end-to-end encryption work well, but users should also be made aware of background details such as cookies and data capturing to reinforce trust in privacy and security.</p>



<p>Conversely, there are also times where users may want to share access with other trusted members among their family, friends, or caregivers. A great user experience around privacy and security is one that enables users to let in the right people along with keeping out the wrong ones; and letting users make that choice themselves.</p>



<p>Today, virtually every sector has gone the digital way, curating well-designed experiences to better engage and be more responsive and responsible towards their customers. The impact of technology on healthcare is undeniable. Flyers get a text in advance informing them of flight delays, and next-day delivery at a preferred time and location is commonplace among eCommerce giants. A common thread running through these strategies is the focus on user-centric design, engaging their consumers proactively, and keeping them at the centre of their decision-making.</p>



<p>While the healthcare technology industry has been lagging on this front, it is now taking steps in the right direction, designing proactive systems to diagnose diseases in early stages to optimize treatment outcomes, helping people stay fit, and deploy digital tools to encourage self-management of health and lifestyle behaviours to prevent disease and illness.</p>

										<content:encoded><![CDATA[<p>In any developing country, there is huge inequality in healthcare distribution due to various reasons such as poor infrastructure, poor communication, inadequate transport facilities, etc. Doctors find it difficult to stay in rural centers due to the above reasons.<br />
It is also well known that 65% to 75% of the population resides in villages that are scattered and not well connected by roads. Hence, the rural population has to travel nearly 8-10 kilometers to access basic healthcare in nearby towns. They spend most of their out of pocket health expenses and at many times borrow money for these expenses.<br />
Keeping this in the background, governments have been working on Sustainable Development Goals launched by the United Nations. Aim of these goals is to ensure healthy lives and promote well-being at all ages. Sustainable health is a huge challenge and it can only be achieved by adopting technology and innovations to improve healthcare delivery.<br />
India now has a robust information technology manpower and the established communication technology is freely available at the village level, mobile phone applications in healthcare and wellness are being used in the rural bridge. <a href="https://innohealthmagazine.comblog/telemedicine-the-journey-so-far/">Telemedicine</a> is the combination of its communication network and healthcare promises to reach the rural-urban divide and it is also the cheapest and fastest way to reach the unreached population.<br />

<strong>Government Initiatives in Telemedicine </strong><br />
Numerous telemedicine projects are now running in the country under the aegis of the National Health Mission. Rajasthan government has initiated a PPP model telemedicine project on BOOT (build own operate &amp; transfer) concept. The Global Health System Kolkata is the vendor who has been operating the Telemedicine project for the past 3 years. Mahatma Gandhi Medical College and Hospital Sitapura Jaipur have signed MOU with the vendor for rendering telemedicine consultations to the periphery (100/ 150 nodes).<br />
<strong>Mahatma Gandhi University of Medical Sciences &amp; Technology (MGUMST) Experience</strong><br />
The experiences gained by providing healthcare through telemedicine is very interesting, it may be noted that the telemedicine consultations have grown with each passing year. It was interesting to note that the teledermatology was increasing on analysis, it was realized that dermatology revolves around photography and visible rules to make a diagnosis.<br />
The numerous efforts made over the years can be analyzed based on the number of consultations: while dermatologists data showed 4011 consultations in 2017, it grew to 31436 in 2019. On the other hand, general medicine was ranging from 4759 consultations in 2017 and dropped down to 2889 consultations in 2019. Cardiologists/CVS/cardiac surgeon consultations were 324 in 2017 and dropped down to 238 in 2019. Obstetrician/gynecologist showed an increase from 854 consultations in 2017 to 1135 in 2019. Oncologist consultations were 33 in 2017 and dropped to 29 in 2019 while orthopedic was at 525 in 2017 and increased to 747<br />
in 2019. Pediatrician/neonatologist was at 6260 in 2017 and drastically reduced to 585 in 2019. Overall consultations during the years 2017-2019  were 105,269.<br />
<strong>Conclusion </strong><br />
Telemedicine is the future healthcare system as it reaches the unreached and it allows the remote health workers to avail consultation with territory care specialists in urban areas. The growing number seeking teleconsultations from the peripheral medical setup is proof that telemedicine delivery of healthcare is now being adopted by the rural population.</p>

	<p><strong>Integration of Artificial Intelligence in Healthcare IT</strong></p>
<p style="text-align: justify !important;">It is no secret that the healthcare sector holds a humongous amount of data; if incorporated with artificial intelligence (AI), it will open innumerable opportunities in the future. Microsoft, one of the largest information technology companies, has been working on several healthcare-related projects. Recently, Microsoft made clear its exact intentions behind developing a Microsoft healthcare team. The company aims to create a cloud-based patients’ profile, push doctors to use more cloud-based services, and eventually develop an AI to analyze the huge data. Microsoft hired two industry past masters: Jim Weinstein and Joshua Mandel to help achieve this goal. Jim Weinstein, the former CEO of Dartmouth-Hitchcock Health System, will help the migration of healthcare data to the cloud. Joshua Mandel, ex-Google employee, will assist in building open cloud architecture for all healthcare service providers.</p>
<p style="text-align: justify !important;">The prime motive behind the project is to find a way to shift healthcare data to the cloud without jeopardizing its confidentiality. This new team will be a part of Microsoft’s AI research division. Peter Lee, the head of Microsoft healthcare, explained, at Microsoft, we’re confident that many aspects of the IT foundations for healthcare will move from on-premise doctors’ offices and clinics to live in the cloud. We are taking concrete steps with an initial blueprint intended to standardize the process for the complaint, privacy-preserving movement of a patient’s personal health information to the cloud and the automated tracking of its exposure to machine learning and data science. The company wants to pave the way to create an open architecture that eventually helps doctors, nurses and healthcare service providers, and to allow more time for patients to spend face-to-face with the doctors. The new platform will open new opportunities for healthcare data to be shared and to protect its privacy. Such innovative projects and huge investments in healthcare AI proves that the incorporation of AI in healthcare IT will be a fruitful investment for the healthcare providers, and no doubt it will be a key to the bright future of healthcare sector.</p>

Hence, this incentive program will eventually increase the quality of care and decrease the cost of healthcare. This incentive program should be initiated first in government hospitals, profit hospitals, and physician group practice. This will further attract more <a href="https://innohealthmagazine.compolicy/medical-tourism-needs-systematic-structure/">medical tourist to India</a> due to the high <a href="https://innohealthmagazine.comblog/healthcare-communication-the-cornerstone-of-quality/">quality of care</a>.</p>

<p style="text-align: justify !important;">The award recognizes and promotes writing that has had a significant impact and advances the body of knowledge regarding operational excellence. The Shingo Institute is most interested in recognizing thought leadership.</p>
<p style="text-align: justify !important;">The book was published in the USA in 2015. Special Indian edition published in 2017.</p>
<p style="text-align: justify !important;">Elated over the achievement, a beaming Dr Singh has told newspersons recently that a team of Shingo examiners judged the book on the various yardsticks, “and then observed Singh and Paul have written a thoughtful and engaging book describing various aspects of healthcare management innovations and the journey organizations have followed to streamline the delivery of healthcare while facing unique challenges. The book was written by the two authors using 34 contributors and compiled into 22 chapters of interesting content covering a wide range of topics. Most of the chapters provide a snapshot of approaches taken to improve healthcare management but do not delve into the roadmap for implementation.</p>
<p style="text-align: justify !important;">The examiners said the book would be a good companion guide to stoke innovative thinking for those involved with a healthcare system.</p>
<p style="text-align: justify !important;">Innovations in Healthcare Management covers a wide range of topics that can be used to create a Lean transformation roadmap. Not only does the book cover the systemic burning platforms shared by various healthcare organizations (i.e. improve quality, reduce cost, improve patient satisfaction), but the book puts a high value on healthcare improvement from the end-user perspective. Additionally, the book identifies opportunities that impacted communities, supply chain, and stakeholders.</p>
<p style="text-align: justify !important;">According to them, the book adds to the existing body of knowledge by providing not only tools and techniques for transforming an organization, but also includes real-world examples of Lean transformation within healthcare organizations such as Tan Tock Seng Hospital in Singapore, Stanford Health Care, and the Mayo Clinic. Also, there are examples of Lean tools such as A3, Just in Time, and Quantitative versus Qualitative impact analysis.</p>
<p style="text-align: justify !important;">The book provides a comprehensive approach to Lean transformation within diverse healthcare environments.</p>
<p style="text-align: justify !important;">It is full of insights and supported with real examples that benefit the reader regardless of Lean knowledge or level of experience.</p>
<p style="text-align: justify !important;">On the impact front, they said the demonstrated impact for Innovations in Healthcare Management on the body of knowledge is difficult to ascertain. The number of books sold is not reported. However, it can be inferred there is demand and there has been sufficient impact to justify a second printing with a special Indian edition.</p>
<p style="text-align: justify !important;">The book has been prescribed in Indian Institutes of Management (IIMs) prestigious institutes established by Government of India. The authors indicate in the application that the book has reached all continents, there has been “very big demand” in the USA and Europe, and demand in India is “huge”.</p>
<p style="text-align: justify !important;">There are 12 endorsements from a very impressive list of healthcare experts from the United Kingdom, United Arab Emirates, India, United States, Tanzania, Brunei, South Africa, Sweden, and the Cayman Islands. This information all indicates there is some level of impact being made. However, sales figures and the number of books sold is not available from the publisher or any other resource. A general online search does not generate any appreciable insight on demonstrated impact.</p>
<p style="text-align: justify !important;">The examiners said the premise of the book is quite simple: “A lot can be done with a little. It is reducing healthcare delivery cost yet maintaining quality through technology, innovation, and various quality models like Lean and others.”</p>
<p style="text-align: justify !important;">The authors’ statement that “trying to do more with more ends up accomplishing less with more” is the reason for any organization to embrace becoming Lean. The examples in this book describe the successful actions taken inpatient treatment goals of better healthcare in less patient time. The authors have done a great job of compiling a wealth of knowledge from across the globe to demonstrate how innovation, technology, and Lean principles can transform the delivery of quality healthcare and reduce costs.</p>
<p style="text-align: justify !important;">Healthcare is a troubled industry all over the world. There are new approaches in this book we can all learn from. At the same time, what is needed in America may not be the same as what is needed or will work in India with over 500 million people and incredible levels of poverty. In essence, the book focuses on a country with lesser developed infrastructure, and therefore the problems encountered are not unique to India, but also not universal.</p>
<p style="text-align: justify !important;">The institute which conferred the award said Innovations in Healthcare Management is a compilation of 22 articles from 34 contributors and two authors. The styles of writing used in the articles flow together quite well and contribute effectively to the authors’ objectives. The stories and examples provided throughout the book offer compelling ideas for understanding how to positively impact the delivery of healthcare from Britain, United States, Singapore, and India.</p>
<p style="text-align: justify !important;">The book contains many simple illustrations and is grounded with facts and visual examples. All of the articles are well referenced. The summary at the end of each chapter provides a linkage back to the overall premise. The book is an easy read and effective at getting the reader to understand the concepts.</p>
<p style="text-align: justify !important;">Examiners agreed that there are many takeaways in the publication. The authors successfully support the premise that more can be done with less. They prove that people can be very creative and innovative when faced with a meaningful challenge that can be embraced. They also effectively show the importance of relationship management between the patient, medical staff, and ancillary stakeholders and dispel the concept that patient care only occurs between the hospital and the patient.</p>
<p style="text-align: justify !important;">Additionally, not all the important healthcare innovations have come from the USA, as India is becoming a leading innovator in healthcare, and others can learn from them.</p>

<p style="text-align: justify !important;">Six months ago,we were worried as we were not getting the desired response rates of patient surveys and we were spending lot of time in analyzing the results and the time consuming step of segmenting and sending the results to the concerned department. It was difficult and time consuming to keep a track of the complaints and also patients were not very keen to fill a paper feedback form. Of course, one of major drawbacks in a fledgling but growing organization is that the data was retrospective and helped with system changes but did nothing for the patients while a commitment to excellence in the hospital, our top management and the board decided to digitize this process as a way to manage the patient complaints efficiently.</p>

<p style="text-align: justify !important;">To capture the unbiased feedback we also started sending the survey link to all the patients via emails and the links were directly merged with the kiosk survey responses.</p>
<p style="text-align: justify !important;">This system help edour team to analyze patient feedback instantly and accurately, eliminating the use of paper and long wait times for results to be  analyzed. Furthermore, it has encouraged more participation from patients.</p>

<p style="text-align: justify !important;">Many hospitals have adopted digitized patient feedback systems and continually gauging patient satisfaction and using the digital feedback system to build on quality improvement initiatives will help demonstrate a commitment to patient-centred care and improve the overall healthcare experience.</p>
<p style="text-align: justify !important;">Digital survey data can be captured automatically and downloaded quickly without transcription error thereby saving time, reducing costs, and maximizing data accuracy.</p>
<p style="text-align: justify !important;">For those patients who are not tech savvy, the staff can help the patient by using the paper survey forms and feed the data into the digital system.</p>
<p style="text-align: justify !important;">As with paper surveys, digital surveys need to be designed well to be usable in practice and acting on the feedback and communicating the actions to patients is best service recovery method which every hospitals should strive for.</p>
<p style="text-align: justify !important;">Currently, we are working towards increasing the response rate by creating awareness campaigns with posters informing patients of surveys and by increasing the frequency of emails with the survey links to the patients.</p>

<p style="text-align: justify !important;">Clinical practice is the process by which a clinician is treating a client (patient) independently or in group, with their best of knowledge and experiences considering clinical guidelines, ethics and evidence based treatment. It has been existing in Indian context since beginning of medical sciences as vaid, hakeem or allopathic doctor (angrazi davakhana) . During earlier days even Govt hospital used to allow private practice by their doctors after hospital hours,legally (still existing in few states of India). Doctors were healers and their patient used to treat them equal to God. Faith and trust were the binding forces for doctor patient relationship. Until, the law brought the awareness about the doctor as providers and patient as consumer. Health care services are market driven so as to survive in this competitive world.</p>
<p style="text-align: justify !important;">Data reveals that 70% of health care services are provided by private health care providers, in India. And these private health care providers are actually sharing the burden of Government, who isfalling short in providing these quality services, timely to their people. Vis a Vis, clinical practice is actually the strong pillar support, to the health care system of India where there is poor doctor patient ratio.</p>
<p style="text-align: justify !important;">Keeping in view of present status of medical education with lack of campus placement and limitation of seats for Post- graduation, by default many medical professionals start their own clinical practice. A medical student who comes out fresh aftergraduation has no road map for their future usage. It all begins with a thought or compulsion or by choice or lack of alternative. As the processes of clinical practice are yet to develop, this article attempts to ensure the vision and mission.</p>
